Effects of quinolone analog CI-960 in a monkey model of Chlamydia trachomatis salpingitis.

Several quinolones have been shown to have antichlamydial activity in vitro and in vivo. We evaluated the effects of the quinolone CI-960 (Parke-Davis) on primary or repeated chlamydial infection in the monkey salpinx pocket model. The antichlamydial effect was evaluated in the tissues, and we tested for the presence of the organism by culture, immunocytochemical stains, in situ hybridization, and histopathology. An intravenous dosage of 5 mg/kg of body weight produced therapeutic concentrations in plasma (blood sera and pocket fluids) of at least 0.25 microgram/ml at 2 h posttreatment. In monkeys with primary infections, treatment was started 2 days after inoculation and was continued for 7 days. After CI-960 treatment, all animals became culture negative. One of two control animals was culture positive through day 10 postinoculation. In monkeys with repeated infections five inoculations were given within 2 weeks. A 7-day intravenous treatment was started on day 2 postinoculation following the last inoculation. Isolation of Chlamydia trachomatis prior to treatment was positive intermittently for all monkeys. After treatment, isolation of C. trachomatis was negative for all monkeys. In monkeys with both primary and repeated infections, no significant differences were noted in the inflammatory responses in the tissues of treated and untreated animals. All tissues tested were positive by immunoperoxidase staining and/or in situ hybridization. After CI-960 therapy, C. trachomatis organisms were no longer recoverable by cell culture. The persistent finding of chlamydial DNA throughout the observation periods following drug therapy may indicate the presence of dead organisms or viable organisms in an unculturable state.

Protection against infertility in a BALB/c mouse salpingitis model by intranasal immunization with the mouse pneumonitis biovar of Chlamydia trachomatis.

Female BALB/c mice were immunized intranasally with the mouse pneumonitis biovar of Chlamydia trachomatis and subsequently challenged in the ovarian bursa (C. trachomatis immunized, C. trachomatis challenged). Two groups of mice served as controls. One group was sham immunized intranasally with mock-infected HeLa 229 cell extracts and was challenged in the ovarian bursa with C. trachomatis MoPn (sham immunized, C. trachomatis challenged). The second control group was sham immunized and not challenged (sham immunized, nonchallenged). Before challenge, the C. trachomatis-immunized, C. trachomatis-challenged animals mounted a significant humoral response as shown by high immunoglobulin G (IgG), IgM, and IgA levels and high levels of neutralizing antibodies in serum and moderate IgG and IgA titers in vaginal secretions. Reactivity by Western blot (immunoblot) to the lipopolysaccharide, 30-, 40- (major outer membrane protein), and 60-kDa cysteine-rich proteins and 75- and 100-kDa chlamydial components could be demonstrated. However, reactivity to the 60-kDa heat shock protein was only observed 22 days after challenge. In addition, this group of animals mounted a significant immune response to chlamydial antigens, as shown by a lymphocyte proliferation assay, compared with the sham-immunized nonchallenged mice. After intrabursal challenge, there was no C. trachomatis shedding from the vagina in the C. trachomatis-immunized, C. trachomatis-challenged animals, while 63% of the sham-immunized, C. trachomatis-challenged mice had a positive C. trachomatis culture. In addition, histological sections from the genital tract showed, at 2 weeks postchallenge, a marked acute inflammatory reaction in the sham-immunized, C. trachomatis-challenged animals while in the C. trachomatis-immunized, C. trachomatis-challenged mice there was minimal inflammatory reaction. When the animals were mated, only 12% of the mice from the sham-immunized, C. trachomatis-challenged mice were fertile. In contrast, 94 and 80% of the sham-immunized, nonchallenged and C. trachomatis-immunized, C. trachomatis-challenged mice, respectively, were fertile.

Salpingitis due to Entamoeba histolytica.

We describe the pathology of a unique case of Fallopian tube amebiasis, associated with hydrosalpinx, in a 21-year-old woman. She complained of lower abdominal pain, had a foul-smelling green vaginal discharge and fever during one week. There was a discrete increase in body temperature and a painful abdominal palpation at the lower right side, with signs of local peritoneal irritation. Pathological examination showed a marked dilatation of the fallopian tube and hydrosalpinx. Microscopic examination showed a poorly formed granuloma composed of large macrophages with many Entamoeba histolytica trophozoites inside the fallopian tube. Even though it is a rare disease the correct diagnosis of female genital tract amebiasis is of great importance for the indication of proper therapy.
