Morphological changes in the terminal oesophagus with varices, following sclerosis of the wall.

Autopsies were performed on 53 patients with oesophageal varices treated with ethoxysclerol to effect sclerosis of the terminal oesophageal wall. The morphological changes after perivascular injection, disturbance of wound healing and complications are described. In 20 cases it was possible to follow the various stages of the normal resorptive inflammatory reaction with transition to perivascular sclerosis of the oesophageal wall. Fatal complications tended to arise in cases with an existing oesophageal lesion produced by the mechanical action of an oesophageal tube. Deeply penetrating mucosal ulcers, perforation of the wall and phlegmonous inflammation leading to mediastinitis were observed in almost 75% of the cases. The results of this morphological analysis might be used as a basis for a prospective morphological study of patients with varices treated with oil-containing solutions injected into the oesophageal wall.

Tetracycline sclerotherapy for hydroceles and epididymal cysts.

Tetracycline (Achromycin, Lederle) solution was used to produce sclerosis of 37 hydroceles and 18 epididymal cysts in patients over the age of 50 years. All but one of the epididymal cysts were sclerosed satisfactorily at 9 months; 35 hydroceles sclerosed satisfactorily, although four patients required re-treatment at 3 months. One patient required orchiectomy for an infected haematocele. Pain after instillation of tetracycline was an occasional complication which could be prevented by prophylactic spermatic cord block. Tetracycline sclerotherapy is a safe and effective alternative to surgery for hydroceles and epididymal cysts in older patients.

Treatment of essential telangiectasia: effects of increasing concentrations of polidocanol.

A double-blind, double-paired comparison study was performed to evaluate the effects of increasing concentrations of polidocanol in the sclerotherapy of essential telangiectasias of the legs. Polidocanol 0.25%, 0.50%, 0.75%, and 1.0% were compared with regard to clinical effectiveness, safety, and patient acceptance. All dosages were well tolerated by the patients. There were no allergic reactions to polidocanol and no cases of superficial ulceration nor necrosis. Among those whose veins cleared, there was little difference in time to clearing for the four concentrations, which averaged three to four treatment sessions. No statistically significant differences existed among the four dosages with respect to level of improvement, itching, or neovascularization. Polidocanol 0.75% and 1.0%, however, caused more side effects noted by patients and induced more hyperpigmentation than did the lower concentrations. Polidocanol 0.25% yielded the lowest percentage of patients whose veins cleared. The 0.50% solution was the most effective concentration for total overall clearing of the types of vessels treated in this study. From this information it appears that 0.50% polidocanol may be the sclerosing agent of choice.

Absence of disseminated intravascular coagulation with endoscopic sclerosis of esophageal varices.

Esophageal varices were injected with 5% sodium morrhuate and a solution containing thrombin, concentrated dextrose, and cephalothin sodium using the flexible gastroscope with balloon cuff modification. Hematologic and coagulating parameters were checked before and after the procedure to look for evidence of disseminated intravascular coagulation. No effect was noted on hematocrit, hemoglobin, platelet count, haptoglobin, prothrombin time, partial thromboplastin time, fibrinogen, fibrin split products, factor V, or factor VIII. Injection sclerotherapy with currently available solutions appears to have no effect on the systemic coagulation system.

New technique for endoscopic treatment of bleeding gastric ulcer.

In two patients with bleeding gastric ulcer hemorrhage could be stopped by submucous infiltration around the lesion without any complication. This simple method using a selfconstructed needle seems to guarantee good results especially in bleeding little arteries. Infiltration was performed with a 1.5% solution of Aethoxysklerol (Kreussler, Wiesbaden-Biebrich) using 3-5 ml totally for treatment.

Antibiotic sclerotherapy for renal cysts.

Seven patients with a large symptomatic renal cyst were treated by aspiration under US guidance and injection of a tetracycline solution (5-8 ml; 1%). All patients were relieved of complaints directly after treatment and US follow-up revealed complete remission or only small residual cysts after one week to twenty-one months. The combination of a strong sclerosing action with a long-acting local antibiotic prophylaxis gives tetracycline an advantage over other sclerosing agents, especially in patients with impaired renal function.

[State of the coagulation system of the blood following treatment of varicose enlargment of the veins of the lower extremity with phlebosclerosing preparations].

The present work is concerned with the study of the coagulation blood system in 50 patients showing varicous veins of the lower limbs in phlebosclerosing therapy. It was found that coagulative properties of blood depend on a stage of the disease. The most pronounced changes in blood coagulation toward hypercoagulation were observed in patients with decompensated varicous veins of the lower limbs 24 hours following the instituted therapy. It is recommended that injection of phlebosclerosing solutions in varicous veins be followed by a strict bed rest during a day.

[Experimental testicular fibrosis and atrophy induced by intratesticular propylene glycol injection].

Fifty-four 15 day old male Wistar rats were given single intratesticular injection of experimental preparations which contained formaldehyde, xylocaine and epinephrine diluted in propylene glycol (FXEP); xylocaine in propylene glycol (XP): epinephrine in propylene glycol (EP) propylene glycol (P); formaldehyde in 0.1 M phosphate buffer solution (F); an one control group. The group of rats that were given FXEP underwent testicular weight reduction; body weight and size were not affected. Also the treatment with P produced atrophy and fibrosis in testis and a more severe testicular weight reduction. The sclerosing effect of P treatment was more satisfactory than treatment with FXEP, and apparently no one affected body weight and size, thus, this could be a safe, easy and inexpensive method for non surgical castration.

Chemical ablation of the gallbladder.

BACKGROUND: Chemical ablation of the gallbladder might avoid the need for surgery in elderly, unfit patients. This study examined the efficacy of various chemicals in destroying gallbladder mucosa. METHODS: Ninety-five per cent ethanol, 3 per cent sodium tetradecyl sulphate (STD), trifluoroacetic acid (TFA) 2 mol/l, tetracycline 50 mg/ml, 30 and 50 per cent phenol, and a mucosal exfoliant solution (compound ethylene diamine tetra-acetic acid) were tested for gallbladder ablation in rabbits. Histology was obtained 8 weeks after exposure to these chemicals. RESULTS: Thirty per cent phenol, tetracycline, TFA and ethanol when used as single agents were moderately effective in causing complete gallbladder mucosal obliteration, 50 per cent phenol caused a macroscopic burn of the entire gallbladder. The mucosal exfoliant solution and STD on their own did not cause mucosal destruction but had significantly enhanced efficacy when combined with 95 per cent ethanol, allowing reliable mucosal destruction with a 5-min contact duration. CONCLUSION: Ninety-five per cent ethanol and STD after pretreatment with a mucosal exfoliant solution may be the combination of choice for in situ gallbladder mucosal ablation.

[Basal cell nevus].

The main features of the nevoid basal cell syndrome, also called Gorlin-Goltz-syndrome, are basal cell nevi, jaw cysts, anomalies of the vertebras and ribs, and intracerebral calcifications. The diagnosis is often incidentally made by discovery of bilateral jaw cysts, as it was in the case of our patient and her family. The therapy is the complete enucleation of the cyst. The cavity is filled with homologous cancellous bone graft. Recurrence and little cysts are treated with the solution of Carnoy eliminating remaining microcysts.

Embolia cutis medicamentosa of the foot after sclerotherapy.

Typically, embolia cutis medicamentosa is reported after intramuscular injections of drugs. We describe a case of embolia cutis medicamentosa after sclerotherapy of intracutaneous veins of the foot with a polidocanol solution of 1%. Under therapy with intravenous alprostadil, pentoxifyllin, internal steroids and anticoagulation with heparin, the lesions healed completely without necrosis.

Therapy for essential cutaneous telangiectasia.

A method of treatment of essential cutaneous telangiectasia is described which has been used successfully in more than 150 cases. Small amounts of a weak solution of sodium chloride are injected slowly into the dilated vessels. The injections are repeated, and stronger solution used, if necessary. Before any physician attempts to treat telangiectasia by this method, he or she should observe its performanace by an experienced operator.

Sclerotherapy with tetracycline solution for hydrocele.

A study of sclerotherapy for hydrocele using different concentrations (10%, 5%, 2.5%) for tetracycline solution was done on 24 patients, 23 patients were cured. The effectiveness of sclerotherapy was the same for the three groups of patients with use of each different concentration of the solution. Pain was the only adverse effect. Nonspecific cellular foreign body reaction and fibrin strand proliferation were observed in the hydrocele fluid after this procedure. We consider sclerotherapy for hydrocele with tetracycline solution safe and the procedure of choice for patients in whom surgery or anesthesia is contraindicated, for patients who refuse surgery, and for economic reasons.

[Combined injection/endoscopic sclerotherapy of upper gastrointestinal peptic ulcer hemorrhage].

In the last 8 years period 369 patients with acute upper gastrointestinal bleeding were admitted to the Department of Internal Medicine, District Hospital Siófok. All patients were treated by combined injection sclerotherapy during the urgent endoscopy. The sclerotizing solution contained at the first step ethamsylate, calcium gluconate, epinephrin, hypertonic saline solution, and the second step 1% aethoxysklerol. At he time of urgent endoscopy 77% of the patients had acute bleeding (Forrest I.a., I.b.) and 23% of them showed stigmata of fresh bleeding (Forrest II.a., II.b.). Primary endoscopic hemostasis was achieved in all patients. In 7.9% of patients (n = 29) developed recurrent bleeding during the observation period, who were resclerotized. Twenty of them (5.4%) were treated by elective surgery. No hemorrhagic or sclerotizing therapy associated mortality occurred. Five out of 20 operated patients (1.4% of all sclerotized cases) had died due to the serious complications of their chronic liver disease. Patients with succesful initial endoscopic hemostasis were treated conservatively (proton pump inhibitors, H-2 blockers, antacids, sucralfate, lactulose) and underwent control endoscopy after 24 hours. Using this sclerotizing method in the treatment of acute upper gastrointestinal bleedings the number of acute surgery and mortality decreased significantly. It is supposed, that the locally administered ethamsylate, calcium gluconate, epinephrin, hypertonic saline solution and aethoxysklerol containing sklerotizing solution might play a favorable role in the successful control of acute bleeding ulcers.
