Hydronephrosis due to ureteral endometriosis treated by transperitoneal laparoscopic ureterolysis.

Ureteral obstruction secondary to endometriosis is relatively uncommon. We present a 43-year-old multiparous woman who suffered from periodic left loin pain in the terminal period of her menstruation. Excretory urogram demonstrated left hydronephrosis and hydroureter and obstruction of the lower left ureter just inferior to the left sacroiliac joint without urolithiasis. An enhanced computed tomography scan showed soft tissue density mass around the left ureter at the level of the stenosis. She underwent transperitoneal laparoscopic ureterolysis and adhesiotomy of the left ureter under the diagnosis of ureteral endometriosis. Because blueberry spots were clearly observed on the pelvic brim, the fibrous tissue surrounded the ureter was removed with peritoneal bleeding spots. Histological examination of the surrounding tissue confirmed the ectopic endometriosis. Even though retroperitoneoscopy is frequently used for ureteral lesion, transperitoneal laparoscopy has an advantage for resection of ectopic endometriosis surrounding the ureter.

Enchondroma versus chondrosarcoma in the appendicular skeleton: differentiating features.

Distinction of enchondroma versus intramedullary chondrosarcoma affecting the appendicular skeleton (proximal to the metacarpals and metatarsals) is a frequent diagnostic dilemma. The authors studied a large series of patients with these lesions (92 with enchondromas, 95 with chondrosarcomas) using statistical assessment of both clinical parameters and numerous radiologic manifestations on images from multiple modalities to identify differentiating features. Multiple clinical and imaging parameters demonstrated statistically significant differences between enchondroma and chondrosarcoma, particularly pain related to the lesion, deep endosteal scalloping (greater than two-thirds of cortical thickness), cortical destruction and soft-tissue mass (at computed tomography or magnetic resonance imaging), periosteal reaction (at radiography), and marked uptake of radionuclide (greater than the anterior iliac crest) at bone scintigraphy. All of these features strongly suggested the diagnosis of chondrosarcoma. These criteria allow distinction of appendicular enchondroma and chondrosarcoma in at least 90% of cases.

Hilar lymph nodes: identification with spiral CT and histologic correlation.

PURPOSE: To determine the appearance of normal hilar lymph nodes and associated soft tissue at spiral computed tomography (CT). MATERIALS AND METHODS: At spiral CT in 50 healthy patients, peribronchovascular hypoattenuated areas of 42 right and 45 left hila were systematically evaluated. Histologic results were correlated in five additional cases. RESULTS: Hilar lymph nodes were depicted (a) in the right hilum (lateral to A2 [n = 33 (79%)], medial [n = 29 (69%)] or lateral [n = 11 (26%)] to the interlobar pulmonary artery, and medial to the lower-lobe pulmonary artery [n = 30 (71%)], A7 [n = 9 (21%)], and A8-10 [n = 8 (19%)]; and (b) in the left hilum (lateral to A2 [n = 22 (49%)], medial to the interlobar pulmonary artery [n = 45 (100%)], and the lower-lobe pulmonary artery [n = 27 (60%)], and in the angles of bifurcation of A7 + 8 and A9 + 10 [n = 18 (40%)]). Hilar lymph nodes were seen to be triangular or linear and to be less than 3 mm wide except around the left-lower-lobe pulmonary artery. CONCLUSION: Spiral CT accurately depicts normal hilar lymph nodes and their major anatomic relationships.

[Non-invasive follow-up control of tumor therapy following chemotherapy using 31P-MR spectroscopy in whole body tomography].

The 31P Magnetic Resonance Spectroscopy allows to monitor noninvasively and in vivo the energy and phospholipid metabolism of tissue. At present the high technical demand prevents the application of this new modality in the clinical routine. By means of some cases the advantages and limitations of the 31P-MRS are demonstrated. Recent results show that 31P-MRS monitors effects on the metabolism of tumor cells as early as two days after onset of chemotherapy. Further clinical studies and technical developments are necessary to render 31P-MRS a clinic routine examination.

The in vivo elastic properties of the plantar fascia during the contact phase of walking.

The in vivo elastic properties of the plantar fascia during the contact phase of walking were determined experimentally by integrating a pressure-sensitive optical gait platform with a radiographic fluoroscopy system for recording skeletal motion. In order to calculate the fascia's tension-deformation relation, lateral images of the foot's skeleton that allowed evaluation of the fascia's transient length from the arch-contact to toe-off stages of walking were obtained simultaneously with the vertical foot-ground contact forces. The plantar fascia was shown to undergo continuous elongation from arch-contact to toe-off, reaching a deformation of 9 to 12% between these positions. Rapid elongation of the fascia, at a strain rate of about 0.9 +/- 0.1 Sec-1, was observed before and immediately after midstance, while a significantly slower elongation occurred at a strain rate of approximately 0.2 +/- 0.1 Sec-1 around push-off and toe-off. The average stiffness of the fascia at the slow-to-moderate walking velocities was 170 +/- 45 N/mm, which is similar to reported stiffness values for cadaver fascia specimens. The present technique may be useful for validation of computational models of the soft tissues of the foot as well as for testing the effectiveness of orthoses and shoe types for relieving excessive strain of the fascia in the treatment of plantar fasciitis.

Primary hydatid disease in femoral muscles.

Usually, intramuscular hydatid cysts are secondary, resulting from the spread cysts from other areas either spontaneously or after operations for hydatidosis in other regions. We present an unusual case of a primary hydatid cyst found in the left thigh of a 35-year-old woman, presenting as an enlarging soft-tissue tumour. Ultrasound, computed tomography and magnetic resonance imaging examinations revealed a multilocular intramuscular cyst in the anterior aspect of her left thigh, and no disease at any other location. We removed the entire cyst surgically, and macroscopic and microscopic histopathological examinations confirmed the diagnosis of muscular hydatidosis. Three years after the operation there had been no recurrence. In regions where hydatidosis is endemic, a tumour in any part of the body should be considered a hydatid cyst until proven otherwise.

Mastoid canal and migrated bone wax in the sigmoid sinus: technical report.

A study of the migration of bone wax into the sigmoid sinus through the mastoid canal is reported here. In 7 of 161 patients who underwent retromastoid craniectomy, the postoperative soft tissue window image computed tomographic scans demonstrated a hypodense mass in the ipsilateral sigmoid sinus. The density value of the hypodense mass ranged from -34 to -79 Hounsfield units, which was neither as low as that of air nor as high as that of cerebrospinal fluid, but was comparable to that of fat tissue or bone wax. The continued presence of all of these masses in the sigmoid sinus was confirmed 1 month to 2 years after surgery. These computed tomographic findings suggested that this abnormal hypodense mass might be a migrated fragment of the bone wax that had been used for the control of venous bleeding from the mastoid emissary vein, because each of the seven affected patients had a large mastoid foramen and a large quantity of bone wax had been needed to control the bleeding during retromastoid craniectomy. No other material with the potential to migrate into the sigmoid sinus had been applied as a packing material. In two of the seven patients, venous magnetic resonance angiography after surgery demonstrated that the ipsilateral sigmoid sinus was not patent and the computed tomographic scans also revealed that the hypodense masses occupied the sigmoid sinus. It is concluded that the intrasurgical application of a large quantity of bone wax to control the bleeding from the large emissary veins carries a risk of the migration of bone wax into the sigmoid sinus.(ABSTRACT TRUNCATED AT 250 WORDS)

Inflammatory pseudotumor of the mesentery causing portal venous thrombosis and cavernomatous transformation.

A 32-year-old man was admitted to our hospital complaining of abdominal pain in the left upper quadrant. A mass was palpable on the left side of the umbilicus. Laboratory data revealed anemia, elevated erythrocyte sedimentation rate, hypergammaglobulinemia, and prolonged prothrombin time. Computed tomography demonstrated a soft tissue mass in the mesentery of the jejunum, portal venous thrombosis, and cavernomatous transformation in the porta hepatis. The patient was eventually diagnosed by laparoscopic partial resection as having inflammatory pseudotumor of the mesentery. Four months later, all of his symptoms and abnormal laboratory findings completely disappeared without any therapy. Inflammatory pseudotumor should be kept in mind as a cause of portal venous thrombosis, and/or cavernomatous transformation although it is rare.

[Determining in the circumstances of head injuries].

Due to the common character of head injuries and the risk involved, the litigation offices are often interested in reconstructing the circumstances of such injuries (their number, localization, time, differentiating active and passive traumas). The paper presents the possibilities and limitations of the methods evaluating the circumstances of head injuries used so far and the chances created by new techniques of computer imaging of head soft tissue structures. Moreover, the author discusses the evident value of external injuries to the head, various cranial fractures (in relation to deducing on the basis of single fractures) and foci of brain contusion. The usefulness of modern imaging methods (CT and MR) demonstrating the extracranial injuries to soft tissues was emphasized.

[Research of dynamic measurements of perioral soft tissue]

OBJECTIVE:This article introduced a concept of dynamic image measurement for calculate the quantity of cirumoral soft tissue movements. METHODS: A developed computer image analyzing system was used to calculate the quantity of circumoral soft tissue movement under the three conditions. RESULTS: (1)When/mo/is pronounced,lip protrudes,the upper lip landmarks move upwards,imwards and forwards,and the lower lip landmarks downwards and backwards.The variation of upper lip triangular area will be M2 M2>M1. (2)When/si/is pronounced,as a result of vowel,the amount of lower lip moving down backwards is more than that when/mo/is pronounced,which makes the lower lip triangular area comes to be M3>M2>M1. CONCLUSION: Pronunciation is one of the important oral functins. It could be used as a method of research to measure the facial soft tissue change dynamically and effectively.

Bounds for damping that guarantee stability in mass-spring systems.

Mass-spring systems are often used to model anatomical structures in medical simulation. They can produce plausible deformations in soft tissue, and are computationally efficient. Determining damping values for a stable mass-spring system can be difficult. Previously stable models can become unstable with topology changes, such as during cutting. In this paper, we derive bounds for the damping coefficient in a mass-spring system. Our formulation can be used to evaluate the stability for user specified damping values, or to compute values that are unconditionally stable.

[Matrix stone: a case report].

Here, we report a case of matrix stone in a 32-year-old man with diabetes, gout, and chronic renal failure. The patient complained of pain in the left flank. He had undergone an operation for bilateral vesicoureteral reflex at the age of 17 and matrix stone discharge was repeated. Computed tomography revealed a soft tissue mass in the right hydroureter. Percutaneous ureteral lithotripsy was performed successfully. Analysis of the stone components revealed the stone to be composed entirely of protein. Radiological imaging of matrix stones may be difficult to separate from urothelial cancers.

Is this a variant of Lemierre's syndrome?

Lemierre's syndrome is an oropharyngeal infection leading to secondary septic thrombophlebitis of the internal jugular vein (IJV). It is classically anaerobic in origin and Fusobacterium necrophorum is the commonest pathogen. We report an unusual variant of this condition in a 68-year-man with uncontrolled diabetes mellitus who presented with extensive cellulitis of the left neck. No primary oropharyngeal sepsis was detected. Neck radiographs revealed soft tissue gas and computed tomography confirmed neck space infection with gas formation and complete thrombosis of the left IJV. Klebsiella pneumoniae was isolated from the fluid, tissue and blood samples. Repeated debridement was required until complete healing. Computed tomography of the neck at six months showed resolution of infection, although the vein remained thrombosed. Although the radiological features resemble Lemierre's syndrome, this case differs from it in the absence of oropharyngeal sepsis and presence of background diabetes mellitus. The isolation of Klebsiella pneumoniae as a cause has not been previously described.

Adenoid cystic carcinoma of the external auditory canal.

Adenoid cystic carcinoma of the external auditory canal is much less common than squamous cell carcinoma and basal cell carcinoma. Signs and symptoms of the case presented here consisted of a longstanding discharging ear, a progressive hearing loss, and the presence of a circumferential subcutaneous induration in the distal third of the external auditory canal. No dizziness, tinnitus, or facial weakness was present. Computed tomography showed a soft tissue mass in the external auditory canal and middle ear with no lytic areas of the mastoid or the temporal region. A biopsy specimen was interpreted as an adenoid cystic carcinoma. Treatment consisted of radical excision of the external auditory canal via a modified temporal bone resection. This radical approach resulted in the local eradication of the lesion.

[Multicenter study of obstetrico-perinatologic quality control in the Rostock district. I. Comparative medical statistics].

Report about outcome-quality assessment in obstetrics and perinatology using an uniform check list for data collection in the district of Rostock. Various possibilities of outprint of the results in 1985 are demonstrated. For daily data collection in obstetrics a special book has been developed. The investigation concerns a data pool of more than 55,000 deliveries. Considering a trend of the results the following optimal criteria in obstetrics and perinatology are postulated: (table; see text) The check list of the Society of Perinatal Medicine of the GDR for obstetrical and neonatal data collection should be generally used. The soft ware for the computed data analyses has been developed by the "Rechenzentrum der Ernst-Moritz-Arndt-Universität Greifswald". Analysis and output of obstetrical and perinatological datas by means of a personal computer are prepared.

Anatomic variations of the accessory soleus muscle.

Four cases of accessory soleus muscle are presented. By means of soft tissue radiography and computed tomography the course of the accessory muscle as well as its exact site and mode of insertion could be determined. It was found that the accessory muscle in these four cases showed four different varieties of insertion, i.e. along the tendon of Achilles, fleshy to the upper surface of the calcaneus, by a separate tendon to the upper surface of the calcaneus and fleshy to the medial surface of the calcaneus. These variations may be of importance in the planning of operative treatment. Therefore, meticulous radiologic examinations as depicted in this study are recommended before surgery.

[The matter of computerized medical records in psychiatry].

To determine the choice of an adequate automatic data processing system in psychiatry, as in medicine, requires a serious set of specifications: one must take into account the aims, and distinguish between everyday applications of file processing, and other ones, more demanding, of research and statistical applications, often little compatible with each other. The financial means have to be compared with the expected benefits (output): often in psychiatry, the patient turn-over is too low to justify the purchase of an expensive hardware. The objectives of filing and classification have to break out, in order to supply the best answer (soft ware). However a computer system alone will not be sufficient to settle, anyway, the question of diagnosis in psychiatry.

[The value of computerized tomography for the diagnosis of diabetic osteoarthropathy].

The diagnostic possibilities of computed tomography as compared to conventional radiology are presented. Morphological changes, which can be identified by normal x-rays, can be shown by computed tomography without any disturbing overshadowing. The computed tomography is showing especially well necrotic lesions, pathological fractures, and joint changes. The high resolution of computed tomography reveals good visibility of the adjacent soft tissues and para-articular structures. As computed tomography gives additional and important informations, it should be used for solution of diagnostic problems in diabetic osteoarthropathy.
