Fluoride concentrations in fissure and cervical enamel of unerupted human teeth.

Fluoride concentration of fissure enamel from unerupted third molars was higher than that of the cervical-lingual but not cervical-buccal surfaces at a similar depth of the removed enamel. The highest fluoride concentration was found in the fissure enamel due probably to its earlier formation, higher permeability and embedding tissue before eruption.

A comparison of two methods of assessing orthodontic treatment need in the mixed dentition: DAI and IOTN.

This paper assesses the unmet orthodontic treatment need in a random sample of 10-year-old schoolchildren, using two indices: the Dental Aesthetic Index (DAI) and the Index of Orthodontic Treatment Need (IOTN). The DAI scores were adjusted by omitting the missing teeth component of the index because many children were in the mixed dentition with unerupted permanent teeth. Although both indices assessed the same number of children with malocclusions requiring orthodontic treatment, not all were ranked similarly by each index.

"Fatigue on rest" and associated symptoms (headache, vertigo, blurred vision, nausea, tension and irritability) due to locally asymptomatic, unerupted, impacted teeth.

"Fatigue on Rest", headache, vertigo and the feeling of loss of balance, blurred vision, nausea, tension and irritability, were found to be prevalent amongst patients who had locally asymptomatic, unerupted impacted teeth. A comparative pressure sign was developed, which, when positive, confirmed the relationship between the impacted teeth and the medical symptoms. Removal of the impactions resulted in the alleviation of the symptoms. Stress and psychogenic factors are considered as trigger mechanisms, rather than as basic causes of the symptoms.

Idiopathic external coronal resorption. Case report.

External resorption occasionally affects the coronal tissues of unerupted teeth. In young patients these teeth may be interpreted radiographically as being carious. The resorption frequently does not extend to the pulp chamber. Loss of integrity of the reduced enamel epithelium may predispose to these changes. This paper reports the features of an example of idiopathic external coronal resorption.

Study of the frequency and the features of supranumerary teeth found in one Portuguese population.

The present study was performed on a population of subjects assisted at the dental clinic of Instituto Superior de Ciências da Saúde-Norte. The aims of the study were: (1) to analyse the prevalence of supernumerary teeth, (2) to characterise its gender distribution, and (3) to investigate the repercussions of supernumerary teeth over neighbouring teeth and tissues. From the records of all the patients received from 1993 to 2000, 16771 clinical files contained panoramic X-ray plates (8712 females and 8059 males) and were selected. A total of 151 supernumerary teeth were found in 127 patients (72 males and 55 females, aged from 5 to 63 years), corresponding to a prevalence of 0.76%. Other designed important issues were one supernumerary teeth for individual; the mesiodens type (followed by the 4th molar); conical morphology; the maxilla (namely on the maxillary medial line): location as predominant findings. Evolutionary state, 47 (31.1%) were erupted and 104 (68.9%) unerupted. In most cases 108 (71.5%), the radiological images didn't suggest alterations on adjacent teeth and/or tissues. The repercussions found, potentially associated to supernumerary teeth were: 12 (7.9%) displacement of adjacent teeth, nine (6%) delayed eruption of permanent teeth, and eight cases (5.3%) diastema formation.

Three-dimensional replicas of pit and fissure morphology in human teeth.

Nuva Seal replicas of pits and fissures from surgically removed unerupted human third molars and premolars, and from erupted premolars extracted for orthodontic reasons, were examined in the scanning electron microscope. The results confirm that the morphology on occlusal surfaces of third molars is extremely variable with numerous pits of considerable length positioned at various angles relative to the fissures. In contrast, the morphology of unerupted and erupted premolars is simpler with fewer and shorter pits positioned perpendicular to the fissure. It is pointed out that fine details of the morphology are lost when reconstructions of the occlusal morphology are made from serial ground sections. However, such reconstructions seem to be less reliable in third molars than in premolars.

Hard tissue repair in idiopathic external resorption of teeth.

A histopathological study has been made of 18 erupted and eight unerupted teeth which showed signs of idiopathic external resorption. The findings support the generally accepted theory that osteoclasts are actively involved in the resorptive process, there was no evidence that mononuclear cells were active in this process. The concept that metaplastic hard tissue was deposited on resorbed dentine surfaces, possibly as a means to shield the pulp from the external environment could not be supported from the evidence from this study. When measured, the length of resorption on which metaplastic hard tissues had been deposited, expressed as a percentage of the resorbed area ranged from zero to 69.3 per cent in the 18 erupted teeth. The percentage of metaplastic tissue replacing resorbed tissue in the eight unerupted teeth examined ranged from zero to 55.6 per cent. This study has provided some information on the type of cell responsible for idiopathic external resorption and these findings have been discussed in the light of the results of research of others in this field. The role of metaplastic hard tissue and its function have been evaluated.

Cleidocranial dysplasia. Case report.

A 29-year-old Caucasian woman who presented with short stature and multiple unerupted supernumerary teeth is described. Radiological investigations of her cranial and skeletal abnormalities revealed cleidocranial dysplasia. Because of the advanced age of the patient and contraindication for orthodontic treatment, only surgical and prosthetic treatment were performed. The characteristics and treatments of this rare autosomal dominant disorder are discussed.

The use of an autogenous free gingival graft in the combined orthodontic and periodontal management of a buccally ectopic maxillary canine.

A case is reported in which an unerupted maxillary canine, situated buccally above the level of the mucogingival junction, was exposed surgically in conjunction with a free gingival graft. Alignment was achieved using the edgewise appliance. An adequate width of attached gingiva was maintained out of retention, with a good aesthetic result. The problems associated with radical exposure of buccally misplaced teeth are discussed.

Surgical exposure and orthodontic alignment of an unerupted primary maxillary second molar impacted by an odontoma and a dentigerous cyst: a case report.

This report describes the surgical and orthodontic management of a 3-year old girl with an unerupted primary maxillary second molar impacted by an odontoma and a dentigerous cyst. Following surgical removal of the odontoma and dentigerous cyst, further surgical intervention and orthodontic traction were required to bring the primary maxillary second molar into correct occlusion with resultant proper functioning and good periodontal health.

Unerupted and displaced teeth in CT-scan.

The results of clinical examination, orthopantomographic examination and computed tomography were compared in 55 patients with impacted teeth. The results of computed tomography and orthopantomograms were equally good. Both methods were superior to clinical examination. With regard to the detection of displacement, contact and resorption, computed tomography was superior to both clinical examination and orthopantomograms.

Compound composite odontome associated with an unerupted deciduous incisor--a rarity.

Odontomes are considered to be hamartomatous malformation rather than true neoplasm and are generally asymptomatic. Here is a case presentation of compound composite odontome with impacted deciduous teeth in an 11-year-old girl. A calcified mass was revealed in the radiograph and the case was finally diagnosed to be compound composite odontome. The mass was surgically excised.

Molar distalization with pendulum appliances in the mixed dentition: effects on the position of unerupted canines and premolars.

INTRODUCTION: The pendulum appliance allows for rapid molar distalization without the need for patient compliance. Its efficiency has been confirmed in a number of clinical studies. However, the potential interactions and positional changes between the deciduous molars used for dental anchorage and the erupted and unerupted permanent teeth have yet to be clarified when this appliance is used for molar distalization in the mixed dentition. METHODS: Twenty-nine patients in the mixed dentition each received a modified pendulum appliance with a distal screw and a preactivated pendulum spring for bilateral distalization of the maxillary molars. The patients were divided into 4 groups based on dentition stages: patient group 1 (PG 1, n = 10) was in the early mixed dentition; patients had resorption of the distal root areas of the deciduous molars being used for dental anchorage, and the unerupted premolars were located at the distal margin of the deciduous molar root region. Based on radiographs taken before placement of the pendulum appliance, patient group 2 (PG 2, n = 10) was diagnosed as having a central location of the unerupted premolars. In the third group (PG 3, n = 4), the first premolars were already erupted and could be integrated into the dental anchorage, but the canines were not yet erupted. In the fourth group (PG 4, n = 5), the first premolars and both canines were fully erupted. RESULTS: Statistical analysis of the measured results showed significant differences in the side effects between PG 1 and PG 2. In patients being treated with pendulum appliances, the anchorage quality of the deciduous molars that were already partially resorbed in the distal root area was comparatively reduced. Consequently, the mesial drift of the deciduous molars and incisors was increased, without impairing the extent and quality of the molar distalization. Anchorage loss in the supporting area had no direct impact on the sagittal position of the unerupted premolars in the early mixed dentition. CONCLUSIONS: If permanent teeth have already started to erupt in the supporting area, additional space restrictions should be avoided in patients with critical topography, especially if there is little space for the unerupted canines. At this stage of the mixed dentition, premolar extraction or augmentation of the supporting area with extraoral headgear offers a therapeutic alternative to intraoral distalization appliances with exclusively dental anchorage.

Mandibulofacial dysostosis. Case report.

A case of mandibulofacial dysostosis (Treacher Collins syndrome) is presented. Clinical features and skull radiographs revealed typical anomalies associated with the syndrome. Cleft of the soft palate and unerupted multiple supernumerary teeth were present in this case. No haematological, biochemical or immunological abnormalities could be detected in the patient. Pedigree analysis showed an autosomal dominant mode of transmission of the disease. Chromosomal studies did not reveal any structural or numerical discrepancies.

[Eruption disturbances of the second molar in the upper jaw by the third molar].

Eruption disturbances of the upper second permanent molars are quite rare, mostly asymptomatic and usually discovered by coincidence on a panoramic x-ray. The exact cause of eruption disturbances is unknown. A panoramic image related to the chronological age shows whether there is an eruption disturbance or not. Treatment modalities of eruption disturbance of the upper second permanent molar in combination with an unerupted third molar are being discussed in this article. Mostly the third molar is removed and the second one will erupt. Eruption disturbances that are discovered after completion of the root formation usually will not lead to spontaneous eruption of the second molar, even when the obstruction has been removed.
