[The achievements and problems in controlling human echinococcosis in Bulgaria].

Progress in and problems of combatting human echinococcosis in Bulgaria are reviewed. It is emphasized that due to complex measures of the disease control started in 1960 considerable progress has been made in decreasing the morbidity and mortality rate of the population and animals. Along with the obvious progress there are also drawbacks and problems in the work performed. However, it is noteworthy that in Bulgaria there are all the necessary conditions for reducing the echinococcosis morbidity rate to just sporadic cases.

[Interleukin-1 production in patients with unilocular echinococcosis].

The production of interleukin-1 (IL-1) by peripheral blood monocytes was estimated by the H. R. Recalde method (1984) in 45 patients with echinococcosis and 10 virtually healthy controls. In echinococcosis patients interleukin-1 turned to be significantly lower (p less than 0.05) than in the controls (Kact 1.2 +/- 0.1 and 2.2 +/- 0.1 respectively). In patients with a malignant course of the disease (mostly in those with disseminated lung damages) Kact of Il-1 was 0.93 +/- 0.1. In patients who underwent surgical interventions 1.5-6 years before Kact of Il-1 reached 1.43 +/- 0.1 but was still lower (p less than 0.05) than in the controls. The lowest production of IL-1 was identified in patients with lung disseminated lesions and bone damages (Kact 0.9 +/- 0.1 and 0.79 +/- 0.1, respectively).

[Strategy in sonographic suspicion of a parasitic disease of the liver (echinococcosis and amebic abscess)].

Ultrasonography is a widely used, early available and well established method in the examination of abdominal symptoms. The strategy for investigation of focal liver lesions, suspected to be parasitoses of the liver (echinococcus cysts or amebic abscess) is shown. The ultrasonographic morphology of echinococcosis and amebic abscess is characteristic, although it is imperative to distinguish these lesions from other tumorous or cystic lesions. The diagnosis suspected through case history and clinical appearance is confirmed by modern serological tests in 90 to 95% of echinococcosis and in 95 to 100% of amebiasis. Fine-needle aspiration guided by ultrasound is indicated in cases of a negative serological result in order to exclude a pyogenic abscess. This evaluation allows a fast diagnosis and efficient therapy.

[Diagnosis and treatment of pulmonary echinococcosis in young patients].

Of the 55 young patients with echinococcosis of the lungs and pleura, in half the disease was revealed at prophylactic roentgenofluorography, in 11.5%--at examination for other disease, in 38.1%--when taking medical advice. The retrospective analysis of the roentgenofluorograms performed earlier has shown that in 18.2% of the patients no small, localized in the basal pulmonary segments echinococcal cysts were revealed. The complicated forms were diagnosed in 11%, multipole echinococcosis--in 7.3% of the patients. In complex examination, the correct preoperative diagnosis was established in 91% of the patients. The reaction of latex-agglutination with echinococcal diagnosticum was positive in the titer 1:8 and higher in 57.9% of the cases. All the patients were operated on. The outcomes of surgical treatment are good. The complications which didn't not require reintervention occurred in 9.1% of the patients.

[Epidemiology and prevention of echinococcosis in France].

Echinococcosis is a cosmopolitan parasitic disease caused by the larvae of two tapeworms: Echinococcus granulosus responsible for unilocular hydatidosis, and Echinococcus multilocularis responsible for alveolar hydatid disease. In both cases man is infected by ingesting eggs of these tapeworms belonging to the genus Cestoda. In France, the only animal that acts as definitive host and transmits the unilocular type of hydatid disease is the dog, while the alveolar type is transmitted by dogs, foxes and cats. Carnivorous pet animals must be taken into account in the prevention of echinococcosis, whether their role is direct or indirect (fouling plants).

[Echinococcosis in the Nenets Autonomous Okrug].

The paper presents results of an immunological screening of the population from the Nenets Autonomous Okrug of the Arkhangelsk region for echinococcosis. A high level (8.4 +/- 1.0%) of seropositivity of the population has been found to Echinococcus antigens. Occupational groups, such as reindeer-breeders, domestic servants in the tent of skins, hunters, fur-farmers, furriers) have been identified to be mostly at high risk for Echinococcus infection. Some animal species have been demonstrated to be implicated in the epidemiology of echinococcosis. The most likely source of human granulosus Echinococcus invasion is a dog whose mature Echinococcus infection rate is 5.8 +/- 3.2%, that of multilocularis Echinococcus is a polar fox whose infection rate is 40.3 +/- 4.3%.

[Avidin-biotin-peroxidase complex enzyme-linked immunosorbent assay in the diagnosis of human multilocular echinococcosis].

The Avidin-Biotin-Peroxidase Complex enzyme-linked immunosorbent assay (ABC-ELISA) with antigen from gerbils infected with alveolar hydatid (GAH) was performed on the sera of 54 patients with multilocular echinococcosis, 107 patients with other diseases and 102 healthy adults. All the sera were diluted 1:200 and an S/N value greater than 2.2 was taken as positive. Both the sensitivity and specificity were 98.1%. The S/N value (mean +/- SD) of sera of the patients with multilocular echinococcosis, other parasitic diseases, tuberculosis, other diseases and healthy adults were: 5.9 +/- 3.0, 1.1 +/- 0.4, 1.4 +/- 0.8, 1.1 +/- 0.5 and 1.0 +/- 0.4. The geometric mean titre (GMT) of antibody measured by ABC-ELISA was 13.7 times that of ELISA. The quantity of serum used in ELISA was 8 times that of ABC-ELISA. ABC-ELISA might be particularly helpful to identify cases with low immune responsiveness. In the detection of alveolar hydatidosis and cystic disease with GAH antigens, the positivity was 98.1 and 67.1% with 87 and 25.6% having S/N greater than 5, respectively. It is suggested that in the diagnosis of hydatid diseases by ABC-ELISA, homologous antigen should be used. The temperature and the incubation time needed for the present assay are practical, for the total time requires only 55 minutes.

[Drainage of the pericystic cavity in echinococcosis of the liver].

The authors emphasize that echinococcosis is cosmopolitan parasitosis and liver is the most often target organ. The most common complications are summarized. At any slightest doubt of the liver infection, the authors recommend modern diagnostic tools (ultrasonography CT) in liver examination. They analyze their lo years experience (20 hepatal). The conclusion is that basic therapeutic procedure is removing of pericyst content with hydatid membrane and residual cavity is treated later. The authors accept updated biac of liver echinococcosis treatment (radical removing of pericyst performing typical or atypical resection, or total lobectomy in case of lobar destruction). However they point out that drainage operations (marsupialization, external drainage with rubber drain, irrigation-aspiration drainage, T-drainage and internal drainage omentopeksia, anastomosis of the cavity with jejunal lumen) still have their place and value in Surgery.

[Echinococcosis of the liver--10 years' experience].

According to the world medical literature echinococcus is mostly located in the liver (60%). In the period 1978-1987 42 patients, 22 male and 20 female, were operated upon at the Titov Veles Medical Center. In 27 of these patients echinococcosis on right lobe was found. There were no patients operated of solitary cyst in left lobe. In 6 persons echinococcosis has relapsed and in 4 it was found in other organs. In 5 bi-lobular, patients the most common age group was 30-60 years. In addition to the classical methods computerized tomography (CT), scintigraphy and ultrasound were also used for making diagnosis. The appropriate method of operation was selected depending on the status, size and location of cysts as well as on the changes in surrounding organs and accompanying complications.

Multicentre clinical trials of benzimidazole-carbamates in human cystic echinococcosis (phase 2).

A multicentre study which constituted the second phase of trials of the efficacy of albendazole and mebendazole in human cystic echinococcosis was coordinated by WHO. A total of 112 patients from four clinical centres in Beirut, Paris, Rome and Sofia completed standardized dosage of regimens of each drug and 68 patients were followed up for at least 12 months after treatment. Albendazole was more effective than mebendazole and adverse reactions were comparable with both treatment regimens. At least 12 months is needed after treatment for an objective evaluation of the efficacy of benzimidazoles. At present, treatment with albendazole or mebendazole should be reserved for inoperable cases of cystic echinococcosis (under strict medical supervision) and individualized according to the patient's response and the occurrence and severity of adverse reactions.

[Parasitic liver cyst. Indications and choice of procedure in cystic echinococcosis].

About 60-90% of clinically diagnosed cases of echinococcosis are found in the liver. Half of these patients show episodes of acute abdominal or back pain. The others complain about discrete and unspecific symptoms. Diagnosis is confirmed by combination of imaging techniques and serologic tests. Operation still is the only way for eradication of the parasite. This paper presents a summary of the literature and clinical data of 27 patients treated in our hospital from 1966 to March 1988. Cystectomy with omentoplasty is confirmed to be a simple and effective surgical method in operative treatment of echinococcosis.

An unusual pulmonary and renal presentation of echinococcosis. Report of a case.

Echinococcosis, although relatively rare in North America, is becoming more prevalent. The rapid diagnosis of hydatid disease, using a noninvasive approach, is highly desirable. This paper describes a rare case of simultaneous pulmonary and renal echinococcosis, without demonstrable liver involvement, diagnosed by cytologic examination of a bronchial aspirate and sputum. This case emphasizes the importance of cytologic evaluation of a variety of body fluids to diagnose multiorgan infection by Echinococcus granulosus.

Circulating antibodies in human echinococcosis before and after surgical treatment.

Antibody levels in 89 patients who had been operated on for hydatid disease were studied over a period of 4 years by complement fixation, latex agglutination, bentonite flocculation, and passive haemagglutination tests. The geometric mean titres were much higher in patients with liver echinococcosis than in those whose lungs were affected. In the first month after operation, an increase in antibody level was observed more often in the former than in the latter. The raised level usually persisted for 3-6 months. In one group of 77 patients, the serological tests either became negative 6 months to 2 years after operation or remained positive during the entire postoperative period. In the other 12 patients, who had recurrent echinococcosis, the tests did not become negative, although there was a reduction in antibody levels during the first and second postoperative years in 6 patients, followed by a rise when the disease recurred. Antibody levels remained high during the entire observation period in the other 6 patients. The prognosis can be considered favourable in patients with low pre-operative titres, or negative tests, provided the tests remain negative up to the end of the first year after operation or become negative within one and a half years. Where there is only a small reduction in antibody titre, the prognosis may still be favourable if the decrease continues to fall up to the end of the second year. Since a drop in antibody titres occurred in certain cases with recurrent infection, it is advisable to defer an opinion on the prognosis until the end of the second year. If the fall in antibody titres is followed by a steady rise, or if they remain high or show slight fluctuations, a recurrence is almost certain.

[Surgical treatment of echinococcosis of the lung based on 575 personal cases and a study of the literature].

Echinococcosis has continued to be a worrying problem in endemic zones. An account is given in this paper of 25 years of experience in echinococcosis treatment. Reported are 575 patients on whom operations had been performed at the Surgical Clinic of Malaga University under the supervision of Professor Vara López. Fifty-six per cent of these patients were males and the remainder females. The disease was found to occur with regularity to all age groups. Cough, pain, lung bleeding, and geographic origin were characteristic phenomena of the disease and its anamnestic background. Diagnosis was confirmed by these parameters together with eosinophilia, complement fixation reaction according to Weinberg, intracutaneous testing according to Casoni, and radiography. Surgical approaches were pericystectomy in 415 cases, cystectomy in 57 cases, segmentectomy in 64, lobectomy in 34, and pneumectomy in five cases. Postoperative complications occurred to seven per cent of the patients, and the lethality rate amounted to 1.5 per cent. The absence of clinical information in publications is discussed together with lack of analyses of clinical situations, modes of treatment, and complications.

[Experimental therapy in Chinese hamsters and rats infected with larval Echinococcus multilocularis by using mebendazole, albendazole and ivermectin with brief review of chemotherapy of human multilocular echinococcosis].

The effects of the mebendazole, albendazole and ivermectin on secondary multilocular echinococcosis in Chinese hamsters infected with intraperitoneal inoculation of protoscolices and in rats infected with transportal inoculation of protoscolices were investigated. A reduction in weight of the hydatids greater than 95% was recorded in Chinese hamsters intraperitoneally injected with mebendazole suspension. Oral administration of mebendazole moderately inhibited the development of the hydatids. Albendazole was less effective than mebendazole. Ivermectin was ineffective. The treatment with mebendazole of larval E. multilocularis inhibited the growth of the hydatids but it could not completely kill the parasite tissues. The present status of chemotherapy of the human multilocular echinococcosis was briefly discussed.

[Incidence of echinococcosis in animals].

Studies were carried out on echinococcosis in animals, making use of the results obtained with investigations carried out throughout this country. A great number of definitive and intermediate hosts were studied. A considerable drop of the percent of infected animals was recorded over the 1966-1984 period both with the definitive and the intermediate hosts. Data is presented on the territorial spread of hydatid echinococcosis. Enumerated are the factors that bear an influence on this process. The demonstration of the high incidence of the disease as well as of the rising number of dogs substantiates the view that the epizootic status is still unfavourable, which is also due to some other factors.

[Intravital diagnosis of intestinal echinococcosis in dogs under field conditions].

Studies were carried out with regard to the diagnosis of intestinal echinococcosis in dogs as the definitive host when still alive, under field conditions. Effective worming and macrohelminthoscopy in such conditions resulted in 52.36 per cent effect. The value found for the average percent of the overall effect of treatment was close to the real values found for the helminthologic status of intestinal echinococcosis. The method employed minimized the epidemiologic and epizootiologic hazards. It was shown to be economically effective and readily applicable.

Pulmonary embolism from a parasitic cardiac clot secondary to hepatic alveolar echinococcosis.

An exceptional form of hepatic alveolar echinococcosis with metastasis of the right atrium is reported. This cardiac location of the parasitosis was revealed by attacks of pulmonary embolism which produced secondary pulmonary lesions. This case suggests that pulmonary metastases of alveolar echinococcosis of the liver might be due to the migration of parasitic clots from the hepatic veins.
