Natural Nashers: a programme of dental health education for adolescents in schools.

This research set out to recognize and overcome the difficulties and doubts in designing, testing and disseminating a dental health education programme for adolescents, as part of their school curriculum. After extensive pilot work, large-scale field trials were mounted of Natural Nashers for 13-14-year-old pupils in school. These trials ran among 6700 pupils in 45 largely urban, mixed sex, state comprehensive schools in England and Scotland. A quasi-experimental control group design was used and evaluation was carried out at the levels of process (teachers' questionnaires, pupils worksheets, field-workers' reports) and outcome (pupils' pre- and post-questionnaires, worksheets, plaque and gingival scores), among a random subsample (n = 1900) divided between study (n = 1300) and control (n = 600). Eighty-three teachers completed a questionnaire and 330 field workers' reports were analysed. Two hundred and fifty pupils were further examined after a minimum of 6 months. Significant improvements in plaque and gingival scores, knowledge and attitudes compared to control pupils were noted. Gains in the areas of knowledge and attitudes were the largest of any recorded in similar trials in Europe. Process data showed that in the experimental schools, improved oral hygiene was correlated with teachers who were enthusiastic and used a cooperative style. Results from the teachers' questionnaire showed that the programme was very well received and educationally suitable.

Osteonecrosis of the femoral head treated by pulsed electromagnetic fields (PEMFs): a preliminary report.

This has been a preliminary report with a short-term follow-up of a small number of observations (28 hips of 24 patients). The follow-ups ranged from 6 to 36 months, with an average of 17.8 months. Only eleven hips (in eleven patients) were followed an average of 8 months after cessation of the treatment. It should be emphasized that this was a "pilot" study, in which no control series was used to determine the natural course of the disease in a comparable clinical setting. Of note was the pain relief, in 19 of 23 patients with moderate to severe pretreatment pain. Also there was an improved function, which suggests that at least in approximately two thirds of the patients there was some clinical benefit from this mode of treatment. In eight hips, clinical conditions did not change; and in two they worsened, requiring further treatment. Eighteen remaining hips were thought to have been benefited by the treatment. Six femoral heads that had already developed varying degrees of collapse (Ficat Type III) collapsed further (1 to 2 mm), and two round heads (Ficat II) progressed to off-round (Ficat III). This preliminary study suggests that further exploration of pulsed electromagnetic fields (PEMFs) is warranted in the treatment of osteonecrosis of the femoral head.

Absence of the relative afferent pupillary defect with monocular temporal visual field loss.

We report five patients with monocular temporal visual field abnormalities who did not have clinically detectable relative afferent pupillary defects. The causes for the field defects were posterior ischemic optic neuropathy, craniopharyngioma, pituitary adenoma, pseudotumor cerebri, and traumatic optic neuropathy. We discuss the possible explanations for our observations, considering the known anatomy of the pregeniculate visual pathways and the afferent pupillary pathways.

[Diseases in animals associated with exposure to electric and magnetic fields of 50/60 Hz: report of a case].

Symptoms of diseases and mortality observed in animals bred near two high-voltage electric transmission lines in the district of Aosta are described. Preliminarily the international scientific literature on this subject is considered. The goal of the present paper is to determine the value of the animals with spontaneously occurring diseases as comparative model for risk assessment associated with 50 Hz magnetic fields.

Psychological sequelae of female sterilization: short-term outcome in a prospective controlled study. A report from the UK Field Research Centre of a WHO collaborative project.

Healthy multiparous women having elective interval (N = 69) or postpartum (N = 69) sterilization were interviewed pre-operatively and 6 weeks and 6 months post-operatively, using standardized instruments. They did not differ significantly from control samples of comparable non-sterilized women with respect to mental state (Present State Examination) or subjectively-assessed mental or physical health or abdominal pain. More sterilization subjects than control subjects reported subjectively experienced improvement in sexual satisfaction at the later follow-up. Reports of poor physical health and abdominal pain increased over time within both the sterilization and the control groups. Reports of adverse effects at follow-up were often associated with higher PSE scores initially. Regrets and wish for reversal were rare and were also associated with higher initial PSE scores. Since the adverse effects were more common among postpartum subjects, it is suggested that subjectively experienced sequelae of sterilization may sometimes be attributable to 'normal' postnatal events.

Kaposi's sarcoma. An update on the results of extended field radiotherapy.

This report reviews the results of extended field radiotherapy (EFR) in the treatment of Kaposi's sarcoma. During the 22-year period that is under study, 54 patients with Kaposi's sarcoma were seen and 38 were treated with EFR. Thirty patients had spontaneous Kaposi's sarcoma and eight had received immunosuppressive therapy. Extended field radiotherapy is compared with both local methods of radiation and chemotherapy in the treatment of Kaposi's sarcoma. It was concluded that EFR is the treatment of choice because of its simplicity, high disease-control rate, and lack of complications in patients without systemic disease manifestations.

Concurrent periostalgia and chronic proximal deep posterior compartment syndrome in a collegiate track and field athlete: a case report.

OBJECTIVE: Exercise-induced leg pain may be triggered by abnormally high compartment pressure. In addition to the more widely publicized anterior compartment syndrome, the deep posterior compartment syndrome can just as frequently occur, resulting in severe pain and disability due to muscle and nerve ischemia. BACKGROUND: Obtaining a thorough history and compartmental pressure measurements are the usual components in the accurate diagnosis of compartment syndromes. While few other disorders mimic compartment syndromes, differential diagnoses must be considered. Surgical management of deep compartment syndrome, consisting of fasciotomy or fasciectomy, or both, is successful for most patients. DIFFERENTIAL DIAGNOSIS: Tibial stress fracture or microfracture, tibial periostitis, tibial periostalgia, distal deep posterior chronic compartment syndrome, proximal deep chronic compartment syndrome, superficial lateral compartment syndrome, deep venous thrombosis, popliteal artery entrapment, or chronic compartment syndrome. UNIQUENESS: Chronic deep compartment syndrome is one of the most common causes of exercise-induced leg pain in aerobic athletes. Therefore, the athletic trainer must be able to recognize the condition. Signs, symptoms, diagnosis, and surgical management of chronic deep compartment syndrome, chronic periostalgia, and superficial lateral compartment syndrame in a 21-year-old Division IA track and field athlete are presented. CONCLUSIONS: With the correct diagnosis, persistent and methodical reevaluation, and appropriate management, the athlete can expect a successful treatment outcome.

Homonymous hemianopia in multiple sclerosis. With report of bilateral case.

A patient with multiple sclerosis and bilateral retrochiasmal visual field defects is reported. Homonymous field defects are rare in multiple sclerosis despite the frequency of pathological involvement of the retrochiasmal visual pathways. A higher incidence might be found with a higher index of suspicion and careful visual field testing with qualitative confrontation technique. Other reasons for the infrequent clinical detection of retrochiasmal lesions are considered. Such lesions may exist without demonstrable defect. This may be explained by anatomical factors, for example, fibre arrangement, or physiological factors, such as, geniculate or retrogeniculate integration. Lesions producing demonstrable defects may be asymptomatic because they: affect only the peripheral field, are small scotomas that do not impair visual acuity, affect only one eye, or occur late in the course of disease when masked by optic nerve involvement.

Report of an epidemiological field study about neuroses and psychosomatic disorders.

Our field study deals with a cohort investigation and a follow-up study of the age groups 1935, 1945 and 1955. The sample consists of 600 randomly selected Germans living in an industrial city (Mannheim/FRG). 200 subjects of each of the age groups are to be examined personally with regard to ICD diagnoses 300-307 (WHO, 8th rev.); these are: psychoneuroses, personality disorders, sexual deviations, alcohol and drug addiction, functional and other psychosomatic diseases. Preliminary countings from the first research check-up of the sample revealed 26% of cases with a clear predominance of females and statistically significant connections between actual disease and impairing or disturbing influences during early childhood.

Prevalence of self-reported hypersensitivity to electric or magnetic fields in a population-based questionnaire survey.

OBJECTIVES: The prevalence of medically unexplained symptoms attributed to exposure to electromagnetic fields is still largely unknown. Previous studies have investigated reported hypersensitivity to electricity in selected groups recruited from workplaces or outpatient clinics. The aim of this study was to estimate the prevalence of self-reported hypersensitivity to electric or magnetic fields in the general population and to describe characteristics of the group reporting such hypersensitivity with regard to demographics, other complaints, hypersensitivities, and traditional allergies. METHODS: A cross-sectional questionnaire survey was conducted in 1997 among 15,000 men and women between 19 and 80 years of age in Stockholm County. The response rate was 73%. RESULTS: One and a half percent of the respondents reported hypersensitivity to electric or magnetic fields. Prevalence was highest among women and in the 60- to 69-year age group. The hypersensitive group reported all symptoms, allergies, and other types of hypersensitivities included in the survey (as well as being disturbed by various factors in the home) to a significantly greater extent than the rest of the respondents. No specific symptom profile set off the hypersensitive group from the rest of the respondents. CONCLUSIONS: The results should be interpreted with caution. But they suggest that there is widespread concern among the general population about risks to health posed by electric and magnetic fields. More research is warranted to explore ill health among people reporting hypersensitivity to electric or magnetic fields.

Training for an agricultural discrimination task.

Skeleton weed, centaurea juncea, is a declared weed in Western Australia because it competes with grain crops for nutrients and moisture. When it is found during harvesting, mechanised teams search and eradicate it. In an earlier report of field trials (Hartley et al, 1989) it was reported that search teams' detection rate was poor and since search teams had usually never seen skeleton weeds, visual discrimination learning was to be expected and observed during searches. The present study investigated the nature of this discrimination learning in a laboratory by developing a training programme of colour photographic slides of weeds in stubble. Subjects receiving specific training with feedback on their performance compared with those receiving pseudo-training showed a significant improvement in detections. Subsequently the benefit of the programme was validated in a field trial.

Visual field loss following vitrectomy for stage 2 and 3 macular holes.

AIM: To describe the phenomenon of peripheral field loss following routine pars plana vitrectomy for stage 2 and 3 full thickness macular hole and to investigate the underlying mechanism. METHODS: Five patients, who reported peripheral field defects after apparently uncomplicated vitrectomy, posterior cortical vitreous peeling, and perfluoropropane (C3F8) gas tamponade, were studied retrospectively with slit-lamp biomicroscopy, automated and kinetic perimetry, fundal fluorescein angiography, focal electroretinography (ERG), and colour contrast sensitivity (CCS) testing. RESULTS: All five patients, who were between 50 and 73 years of age, reported an inferotemporal field defect following resolution of the intraocular gas bubble. In all eyes, the scotomata encroached to within 20 degrees to 30 degrees of fixation and to within 5 degrees to 15 degrees of the blind spot. In one eye, a partial altitudinal component was evident. All scotomata subsequently remained stable and three eyes developed subtle segmental nasal disc pallor and nerve fibre loss corresponding to the field defect. CCS testing revealed absent colour contrast in the scotomatous area, in the presence of a preserved focal quadrantic flash ERG, compared with normal CCS protan thresholds and focal ERGs in unaffected quadrants, indicating preserved outer retinal function in the area of the scotoma. CONCLUSIONS: These observations support the hypothesis that field defects occur as a result of retinal nerve fibre layer damage. It is proposed, on the basis of intraoperative observations and other evidence, that the most likely site of nerve fibre damage is at the nasal portion of the optic nerve rim or peripapillary retina, probably due to traction during cortical vitreous peeling.

MR-guided aspiration cytology in the head and neck at high field strength.

Published reports of magnetic resonance (MR)-guided needle biopsy have involved low- and mid-field-strength systems because of the concern of needle artifact and risk of increased needle torque at higher field strengths. To the authors' knowledge, this is the first report of a high-field-strength (1.5-T) system used to guide aspiration cytology in the head and neck in a patient. The procedure was successfully accomplished without additional needle torque or substantial increase in image artifact relative to lower-field-strength imaging.

Skin metastases confined to a field of previous irradiation. Report of two cases and review of the literature.

Two cases of tumor recurrence within a previously irradiated skin area are reported. Typical lesions evolve from an erythematous base and progress to induration and tumor papules. A review of clinical literature indicates that irradiation does not increase the frequency of metastases. Experimental data suggest that irradiation does increase metastases as a result of both local mechanical factors and increased tumor-cell survival.
