20 patients with advanced carcinomas of the oral cavity (T3NxMo) were treated at the same time with the cytostatic drugs bleomycin and methotrexate as well as with a cobalt-60-irradiation. 14 of them are tumor free after an observation period of one half to two and a half years. Among them were 4 recurrences that had to be treated surgically. 1 patient is living with a residual tumor which cannot be further treated. 5 patients died of the tumor or of other causes. The survival rate, which is 80% after 1 year, is, as a tentative result, higher than could be attained by other therapeutic measures.

One hundred and twenty-six patients underwent a mandibular resection (segmental or marginal) as a part in the treatment of a malignant lesion. The surgical morbidity is increased by the preoperative high-dose irradiation. Complications such as osteitis and/or fracture of the mandible were noted in 31 cases (a total of 80) who underwent a preoperative external irradiation with a tumor-dose of 5,500 rads or higher, whereas only in 8 cases (a total of 46) without such preoperative radiotherapy. Prophylactic antibiotics, the use of flaps to replace the buccal mucosa without any suture-tension, replacement of the mandible by a low profile stainless steel prosthesis are advocated to reduce these postoperative complications.
The symphyseal region of the mandible is the commonest site of location of ameloblastoma in Nigerian patients. The hazards of resection of the lesion together with the aesthetic defects and problems of reconstruction are discussed in light of the authors' experience.
A series of 14 mandibular reconstruction in 13 patients with chips of autogenous, cancellous bone and marrow supported by a titanium mesh tray is presented. The mandibular defects were the results either of operation for carcinoma of the tongue and the floor of the mouth or trauma. Satisfactory rehabilitation was accomplished in 11 patients. The advantages of this technique are ease of accomplishment, ability to replace long and awkardly shaped segments, less shrinkage due to bone resorption, and shorter intermaxillary fixation.
An experience with 69 osteoplastic operations with formalinized homografts in 66 patients having various affections and injuries of the lower jaw is summarized. The grafts were preserved in 0.5% formalin solution or gel containing 0.25% formalin and 0.25% betapropiolactone as sterilizing agents. Positive issues during the follow-up from 4 months to 4 years 7 months were noted in 60 operative procedures, negative--in 9. It is concluded that the formalinized homobone shows good osteoplastic properties and may be used as a plastic material in reparative operations on the lower jaw bone.
The aim of this study was to assess the use of titanium mandibular reconstruction plates after cancer surgery. The medical records of 36 patients were reviewed. The mean follow-up was 22 months. All patients had primary reconstruction of the mandible with Leibinger titanium plates. Thirty patients underwent postoperative radiotherapy with the plate in the field. The mean dose was 60 Grays over 6 weeks and the overdosage into tissues was reduced to 5%. Thirty patients (83%) had retained the plate at 6 months and no sign of osteomyelitis was found in any patient. Quality of speech and deglutition were satisfactory in 63% and 94% of the patients respectively; sixty-six p. cent of them found their cosmetic appearance good or excellent. The overall success rate of the technique was 74%. This technique did not excessively prolong operating time unlike microvascular transfer. Plates are an effective method of primary reconstruction in patients with advanced cancer and uncertain long-term survival. Tolerance of plates after radiotherapy is outstanding. Speech, deglutition and facial contour are immediately restored and further bone reconstruction is feasible.
