[Heart transplantation in patients with coronary disease. Preoperative period].

The authors report on the current heart transplantation (= HTx) frequency in international as well as domestic--Slovak conditions. The exact criteria for selection of patients who will gain maximum benefit from HTx are given. The alternative procedures namely in patients with progressive coronary artery (ischemic heart) disease, which could substitute or at least postpone HTx until the time when definitive solution of chronic and terminal congestive heart failure will be available are discussed. Apart from that, medical, as well as, psychological problems of pre-operative period and the possibilities of their solution are analysed.

[People's way to fight the epidemic diseases in the early Choson period].

Epidemic diseases in the early Choson period were big problems to the people and society. However, the weapons to the epidemic diseases were very limited at that time. There were few drugs and well-trained practitioners for people. Most people could only depend on religious means and simple folklore medicine including inhalation of specific flavor. People tried to overcome or prevent the epidemics by praying, sorcery, ornaments, runaway and isolation. In most cases, the epidemic diseases came with or broke out from the famine, severe labour (especially in civil engineering) imposed by the ruling class. The epidemics of that time are thought to be typhoid fever, epidemic typhus, or similar febrile diseases.

[Importance of determination of circulating immune complexes in systemic lupus erythematosus].

UNLABELLED: The concentration of circulating immune complexes (CIC) was investigated in the group of 9 patients with revealed systemic lupus crythematosus in active disease period and remission. In the same time it was estimated the blood level of complement C3 and C4, anti-dsDNA antibody and qualitative estimation of antinuclear antibody (ANF). It was estimated the significance of their changes in the remission and in activity period. CIC were performed with N-Latex (Clq) CIC nephelometry, C3 and C4 k.k. with nephelometry, and anti-dsDNA antibodies with ELISA test, specific for dsDNA antigen, IgG type, and ANF was performed with direct immunofluorescence microscopy. In the results, we found increase the concentration of CIC with all patients in active disease period, but in the remission period it was decreased (therapy effect). The blood level of ANF, C3, C4 were not significantly different between activity and remission period. The anti-dsDNA level was significantly elevated (90%) in the period of high SLE activity, while decrease was confirmed during remission. IN CONCLUSION: the CIC level in patients with SLE is adequate parameter for disease activity estimation, together with measuring other immunological parameters (C3, C4, anti-dsDNA antibody), is useful in follow up of SLE activity.

[The dynamic change in the content of total lipids in myocardial infarct patients].

The dynamics of the content of total lipids (cholesterol and cholesterol esters) was studied in patients with acute large-focal uncomplicated myocardial infarction in different disease periods. Rapid quantitative changes that occur in the content of cholesterol and its esters in the test groups were established to be characterized by marked specificity and by the form of its own in different disease periods (in the acute period--days 2-5 and on day 15--the health status stabilization). It is advisable that the characteristics of the dynamics of rapid reversible changes in the content of total lipids may be used for estimating the status gravity of patients with uncomplicated large-focal myocardial infarction.

Prolonged disease-free period in patients with advanced epithelial ovarian cancer after adoptive transfer of tumor-infiltrating lymphocytes.

Thirteen patients with epithelial ovarian cancer, who did not show any detectable lesion after cisplatin-containing chemotherapy following primary operation, were treated with adoptive transfer of tumor-infiltrating lymphocytes (TIL group). Eleven patients with almost equivalent conditions of disease, who were treated with only chemotherapy following primary operation, served as a control group. The median time of follow-up was 36 (range, 23-44) months in the TIL group and 33 (range, 14-48) months in the control group. The estimated 3-year overall survival rate of disease-free patients in the TIL group and in the control group was 100% and 67.5%, respectively. A significant difference was noticed between the overall survival rate of the TIL group and the control group (P < 0.01). Furthermore, the estimated 3-year disease-free survival rate of the patients in the TIL group and in the control group was 82.1% and 54.5%, respectively. The disease-free survival rate of patients in the TIL group and in the control group was significantly different (P < 0.05). These results suggest that the adoptive transfer of TILs after all chemotherapy has been finished might be one promising method to achieve complete cure of advanced epithelial ovarian cancer.

A case of diffuse Lewy body and Alzheimer's diseases with periodic synchronous discharges.

Periodic synchronous discharges were consistently observed in a 68-year-old man with a four-year history of progressive dementia. Pathological examination revealed diffuse atrophy of the brain with remarkable dilatation of the temporal horns. Histologically there were severe changes of Alzheimer's disease and diffuse Lewy bodies (LB), either typical or less distinctive ones without clear halos. Cerebellar LB were also observed in the cerebellum. Four types of abnormal intraneuronal filamentous structures were observed: neurofibrillary tangles, accumulated neurofilaments, thick linear structures studded with ribosome-like dense granules, and fuzzy, thin filaments similar to, but generally wider than, a neurofilament.

Significant prolongation of disease-free period gained by oral polysaccharide K (PSK) administration after curative surgical operation of colorectal cancer.

To examine the clinical efficacy and the mechanism of action of polysaccharide K (PSK), a protein-bound polysaccharide extracted from a Basidiomycetes fungus, a randomized double-blind trial was performed by administering PSK to 56 patients and a placebo to another group of 55 patients after surgical operations on their colorectal cancers. The rate of patients in remission (or disease-free) was significantly higher in the PSK group than in the placebo group; the difference between both groups was statistically significant at P less than 0.05 by the log-rank test. The survival rate of patients was also significantly (P less than 0.05) higher in the PSK group than in the control group. The most significant laboratory finding was that polymorphonuclear leukocytes from PSK-treated patients showed remarkable enhancement in their activities, such as random and/or chemotactic locomotion, and phagocytic activity, when compared with those in the control group. In conclusion, PSK was useful as a maintenance therapy for patients after their curative surgical operations for colorectal cancer. The beneficial effects were probably due to the activation of leukocyte functions as one of the many biological-response-modifying (activities induced by PSK).

The dynamics of microfilaraemia and its relation with development of disease in periodic Brugia malayi infection in south India.

Rates of acquisition and loss of Brugia malayi microfilaraemia were estimated using the parasitological data of a cohort of population in Shertallai, South India. The rate of acquisition of microfilaraemia was found to be dependent on age but not gender. The decline in the rate of acquisition of microfilaraemia in adults above 35 years could be due to the development of acquired immunity. The mean reproductive lifespan for the periodic Brugia malayi adult female worm was estimated to be 3.4 years and it was independent of host age and gender. The age-specific estimated proportion of population at risk (microfilaria carriers who lost their microfilaria in course of time) of developing lymphoedema approximately mirrored the observed age specific prevalence of lymphoedema in the study population. On an average, 99% of population at risk developed disease in different endemic areas is compared and its epidemiological significance is discussed.

[Assessment of the inhalation technique using diskhaler in adult patients with asthma].

This study evaluated the proper use of Diskhaler in 104 adult outpatients with asthma. Logistic regression analysis was performed to determine whether age, gender, the experience with metered dose inhaler (MDI), and the disease period were predictive of incorrect use of the Diskhaler. Frequencies of correct performance of "all actions" (10 items) and "all essential actions" (2 items) were 47% and 69% of patients respectively. The most frequent errors were "not holding breath for five seconds" (37%) and "not inhaling forcefully and deeply" (31%). Relative risks of incorrect inhaler use were as follows: age 1.000, female gender 1.341, no experience with MDI 1.707, and disease period 0.991, all of which were statistically insignificant. We should instruct patients especially to breathe in forcefully and deeply, and to hold breath for five seconds.

[Familial paroxysmal polyserositis. Previously unpublished peritoneal complications. A case].

A young unmarried Lebanese woman presenting with periodic disease (familial paroxysmal polyserositis) since she was 3 months old developed recurrent abundant ascites at the age of 21 years. Several hundred millilitres of strongly eosinophilic fluid were evacuated. Exploratory laparotomy unexpectedly disclosed an encapsulating peritonitis with adhesions involving the small bowel and the ascending colon; there were masses of lipid-laden cells, clusters of cholesterol/crystals and marked mesoepithelial reaction. In view of the patient's dramatic response to colchicine 2 mg/day, these findings were regarded as being related to the periodic disease.

Detection of Japanese encephalitis (JE) virus-specific IgM in cerebrospinal fluid and serum samples from JE patients.

Detection of Japanese encephalitis virus (JEV)-specific IgM by IgM-capture enzymed-linked immunosorbent assay (IgM-capture ELISA) has been accepted as the standard for serological diagnosis. In the present study, we analyzed the time course of the positive rate of JEV-specific IgM in serum and cerebrospinal fluid (CSF) specimens from confirmed JE patients. Serum and CSF samples were obtained from 155 JE cases for diagnostic purposes at hospitals in Thailand from 2002 to 2004. The levels of specific IgM were assessed by IgM-capture ELISA in the 171 serum and 156 CSF samples. Anti-JEV IgM was detected in 26 of 44 serum samples collected on days 1-4 of the disease period, in 31 of 44 samples collected on days 5-8, in 23 of 26 samples collected on days 9-12, and in all the samples collected on day 13 or later. Specific IgM was detected in 60 of 66 CSF samples collected on days 1-4 of illness, and in all the CSF samples but one collected on day 7 or later. The results indicate that the detection of JEV-specific IgM in CSF by IgM-capture ELISA is a reliable laboratory diagnostic method for confirmation of JE throughout the disease period, while the detection of IgM in serum samples is a reliable method on day 9 or later.

Objective histopathologic grading of cutaneous malignant melanomas by stereologic estimation of nuclear volume. Prediction of survival and disease-free period.

Modern stereologic techniques enable unbiased and shape-independent estimation of the three-dimensional nuclear volume (Vv). This study investigates the prognostic impact of Vv in 47 patients with malignant melanomas (10 years of follow-up) and compares Vv to traditional prognostic parameters and two-dimensional morphometric estimates. The averaged Vv was 226 microns3 and 457 microns3 in Stage I and II melanomas, respectively. The Vv was significantly increased in the case of ulceration, nodular melanoma, and Clark's level greater than III. The Vv showed only poor correlation to two-dimensional morphometric estimates. Cox regression analysis indicated Vv to possess excellent prognostic information, only rivaled by tumor ulceration, the latter being a 100% predictor of metastatic spread. Histologic type, Clark's level of invasion, tumor thickness (according to Breslow), and patient sex were without independent prognostic significance, which may be due to attributes of the small data base. It is concluded that Vv may be a powerful prognostic indicator in cutaneous melanomas, suitable for objective malignancy grading. The clinical and prognostic value of nuclear Vv needs further investigation in a larger and contemporary series of patients with malignant melanomas.

Modifications in the sarcoplasmic reticulum and subcellular calcium distribution in skeletal muscle in a case of Westphal's disease (hypokalemic periodic paralysis).

In a case of hypokalemic periodic paralysis with characteristic alterations of the sarcoplasmic reticulum (SR) in the skeletal muscle, subcellular calcium re-partition, as revealed with the pyroantimonate technique, appears disturbed during paralysis. Pyroantimonate precipitates, normally concentrated in the terminal cisternae of the SR, were localized in the T tubules, whereas the terminal cisternae appeared empty. The increase (about 14%) in muscular calcium during paralysis may result from the accumulation of calcium in the extracellular compartment (T tubules). Defects in calcium uptake and storage by the SR may be involved in the pathogenesis of the periodic paralysis syndrome.
