[Epidemiological immunological studies on leprosy in Okinawa. 6. Lepromin, lymphocyte transformation, FLA-ABS and leproagglutination tests in the in-and out-patients with leprosy and the relationships among these tests and personal or family history or clinical findings of the patients].

In- and out-patients with leprosy in Okinawa were surveyed by using the following immunological tests: cutaneous reaction to Mitsuda's lepromin (40 million bacilli/ml) (LPR), lymphocyte transformation test with Dharmendara's antigen (DL-LTT), that with phytohemagglutinin (PHA-LTT), fluorescent leprosy antibody absorption test (FLA-ABS) and Leproagglutination test with cardiolipin-lecithin antigen (LAT). A correlation between two tests and a relationship between the test and personal or family history or clinical finding of the patients were evaluated by non-parametric statistics. Spearman's rank correlation coefficient was significant between LPR and DL-LTT, while a reverse correlation was found between LPR and FLA-ABS, DL-LTT and FLA-ABS, and DL-LTT and LAT, respectively. High reactivity in PHA-LTT was found in almost all of patients, irrespective of type and stage of leprosy, suggesting general immune responsiveness of leprosy patients in Okinawa. The other tests showed a significant difference in the mean rank of reaction values according to clinical findings such as type and stage of leprosy, property of skin lesion, loss of eyebrow, and bacteriological test. The use of LPR and DL-LTT for the study of cell-mediated immunity in leprosy was therefore confirmed. FLA-ABS was found useful for early serodiagnosis, while LAT for the study of autoimmunity in leprosy. None of these immunological tests showed significant difference among the subgroups of patients classified by the following items: presence or absence of consanguineous patient with leprosy, living place, place of onset, presence or absence of neural symptoms such as the enlargement of peripheral nerve, claw hand and facial palsy, ocular and nasal involvement, ENL in lepromatous and borderline leprosy and conditions in the treatment. On the other hand, LPR showed higher reactivity in female than in male, in the patients at home than those discharged, and in the patients with drop foot than those without. Higher reactivity of DL-LTT was also found in male than in female and in the patients with plantar ulcer than those without. FLA-ABS reactivity was significantly higher in the discharged patients than the inpatients, in the period of 5 to 9 years after onset than that of 20 or more years, but the reactivity was lower in the patients with drop foot than those without. LAT reactivity was significantly higher in the 30-39 years old patients than 0-19 years old, and in the patients with physical work than those with mental.(ABSTRACT TRUNCATED AT 400 WORDS)

A comparison of the analgesic effects of methoxyflurane-nitrous oxide and nitrous oxide alone during labour related to the Eysenck personality inventory test.

One hundred and thirty-three paturients who had received either methoxyflurane-nitrous oxide or nitrous oxide analgesia with or without pudendal block, underwent the Eysenck Personality Inventory Test on the second postpartum day and evaluated their memory of the pain (Subjectively Evaluated Pain Suffering Scores) during labor. Parturients who had received methoxyflurance-nitrous oxide analgesia reported significantly lower pain suffering than parturients who had had nitrous oxide analgesia. Subdivision according to Personality Inventory factors showed that at the introvert end of the Extroversion-Introversion scale, methoxyflurance-nitrous oxide analgesia with or without additional pudendal block resulted in significantly lower pain suffering than did not nitrous oxide analgesia. On the other hand, nitrous oxide analgesia without additional pudendal block gave significantly lower pain suffering at the extrovert end of the scale. Among the extroverts there was a tendency, though not statistically significant, towards non-approval of the pudendal block.

Quantitative neurologic and neurobehavioral testing of persons infected with human immunodeficiency virus type 1.

Assessment of neurobehavioral and peripheral neurologic performance among homosexual men was made on two occasions, using a computer-administered neurobehavioral instrument and quantitative vibration threshold testing, respectively. Persons studied included high-risk human immunodeficiency virus (HIV)-negative men (n = 13), asymptomatic HIV-positive men (n = 30), and patients with acquired immunodeficiency syndrome (AIDS)-related complex of AIDS (n = 17). In addition, subjects were characterized immunologically at the time of neurologic and neuropsychologic assessment via determination of circulating lymphocyte counts (total lymphocytes, helper T cells, suppressor T cells, total T cells, activated T cells) and markers of HIV type 1 (HIV-1) infection. At the first cycle of testing, the results of asymptomatic HIV-positive men were indistinguishable from those of HIV-negative men, while persons with AIDS-related complex or AIDS tended to have lower mean performance. These differences did not achieve statistical significance on any single test, but the group with AIDS-related complex or AIDS had the worst mean performance on 12 of 13 individual performance tests. Thirty-seven men underwent repeated testing after a mean interval of approximately 4.5 months. There was little change in mean neurobehavioral performance and vibratory thresholds among all three groups. Measures of neurobehavioral performance and vibrotactile thresholds were not correlated with measures of immunological status. These results are consistent with the concept that asymptomatic infection with HIV-1 does not imply the presence of measurable or significant neurologic or neurobehavioral impairment.

The prospect of predictive testing for personal risk: attitudes and decision making.

As predictive tests for medical problems such as genetic disorders become more widely available, it becomes increasingly important to understand the processes involved in the decision whether or not to seek testing. This study investigates the decision to pursue the possibility of testing. Individuals (one group who had already contemplated the possibility of predictive testing and one group who had not) were asked to consider predictive testing for several diseases. They rated the likelihood of opting for testing and specified the reasons which they believed had affected their decision. The ratio of the numbers of reasons stated for testing and the numbers of reasons stated against testing was a good predictor of the stated likelihood of testing, particularly when the reasons were weighted by utility (importance). Those who had previously contemplated testing specified more emotional reasons. It is proposed that the decision process is internally logical although it may seem illogical to others due to there being idiosyncratic premises (or reasons) upon which the decision is based. It is concluded that the Utility Theory is a useful basis for describing how people make decisions related to predictive testing; modifications of the theory are proposed.

Binocular quantification and characterization of microsaccades.

BACKGROUND: The significance of microsaccades in the visual process has been discussed for more than 50 years. However, only a few studies have measured microsaccades binocularly, and detailed quantification and characterization of these small movements are needed in order to further understand their nature. METHOD: The amplitude, velocity, acceleration and direction of microsaccades were quantified binocularly in 10 normal test persons during a 40-s fixation task, using an infrared recording technique. RESULTS: All microsaccades for all test persons were performed simultaneously and individually with an almost identical amplitude in the right and left eye (a range of 0.003-0.042 deg between right and left eye mean values). The mean microsaccadic amplitude for the test persons was within a range of 0.223-1.079 deg. The directional difference between simultaneously-performed right and left eye microsaccades was less than 22.5 deg for 84.8% of the saccades, indicating that the majority of microsaccades are conjugated. Three different fixation patterns were identified and characterized: (1) a classic interplay between easily identified drifts and medium-sized microsaccades (mean amplitude range 0.328-0.413 deg); (2) long intersaccadic intervals (4-5 s) with almost absent drifts, followed by three or four large microsaccades (mean amplitude range 0.755-1.079 deg); and (3) low-amplitude drift movements interrupted by low-amplitude microsaccades (mean amplitude range 0.231-0.265 deg). CONCLUSION: Microsaccades are involuntary, predominantly conjugated, simultaneously performed, and of almost identical amplitude in the right and left eye, suggesting a central control mechanism for microsaccades at subcortical level.

[An interesting case of defective HLA phenotype].

In a 27 years old man whose parents had HLA phenotypes A 9/B 7, Bw 16 and Al/B 17, B 18 only HLA-B 18 could be reliably identified in lymphocytes by means of microlymphocytotoxic tests. It is probable that HLA-Bw 16 was also present in the lymphocytes of this test person. Even by applying the neutralisation test, the HLA antigens A 1 and a 9 could not be detected in the test person's serum. The cause for the existence of the defective HLA phenotype is discussed.

Stress testing in persons above the age of 65 years: applicability and diagnostic value of a standardized maximal symptom-limited testing protocol.

A standardized symptom-limited stress test on a bicycle ergometer in semisupine position is examined in regard to its applicability in patients over the age of 65. The test protocol allows for exercise at full capacity in the elderly. Its value for diagnosis of coronary heart disease is equivalent with its value in the younger patients' group. In 167 patients above 65 years, there occurred only one case of pulmonary rales with exercise, no other complication could be observed (0.568%). Standardized, symptom-limited stress testing appears to be a simple, highly diagnostic and safe method for testing exercise capacity or diagnosing coronary heart disease in the elderly as in younger persons.

Psychoneurotic and psychopathic personalities of polydrug abusers and effects of didactic instruction.

An investigation into the use of psychometric assessment instruments to produce psychographic profiles which proved superior to the use of individual measures on drug accepting versus drug rejecting age generational gaps is appropriate when considering drug and alcohol education programs. Clinical studies show that polydrug abusers have many psychoneurotic and psychopathic traits which can be measured through valid personality tests. Cuevas (1989), employed the Western Personality Inventory (Manson 1963) to identify individuals whose behavior and personality structure indicated they were polydrug users or had serious polydrug problems. The instrument revealed that a combinative process utilizing comparison groups from known polydrug abusers as a discriminant analysis technique with controls provided a high degree of accuracy. A high degree of precision in prediction was maintained in validation trials on independent samples. The personality characteristics and traits were extracted from a combination of commonly ordered personality tests. Results of the investigation indicate that clinical usage of the screening procedure is readily available without sophisticated computer support or the need for trained psychometricians. Additional results identified the process as interventive, therefore providing a simple way to produce early didactic preventive instruction.

Influence of low power cm-/mm-microwaves on cardiovascular function.

The present study has been designed to investigate physiological effects of short (15 min) exposure to low level energy microwaves (< 10 microW cm(-2)). To this end heart rate, PQ, QS and ST (electrocardiography), respiration (conductive stretch band around the thorax), skin temperature (thermocouple), skin conductance (Ag/AgCl electrodes), systolic and diastolic blood pressure (automatic cuff) were continuously recorded in a group of 50 test persons before, during and after a 15 min exposure to 3 microW cm(-2) high frequency (77 GHz) microwaves. After an equilibration period of 30 min the first group of test persons and after additional 30 min the second group of test persons were exposed. The study has been performed in a strict double blind design. While significant effects on the measured parameters were observed depending on time ('calming' effect), no significant difference was observed between exposure and sham exposure to microwaves. In view of the small scatter of the data the present study rules out physiologically relevant effects of low level energy on the autonomic nervous system and cardiovascular function.

[Evaluation of computer-aided learning illustrated by the multimedia program, "Hereditary diseases of the dog--joints, bones, musculature"].

Results are presented on the evaluation of the interactive tutorial and information system "Hereditary diseases of the dog-joints, bones, musculature" which was developed at the Institute of Animal Breeding and Genetics and the Clinic for Small Animals at the School of Veterinary Medicine Hannover, aiming to improve the problem consciousness for the genetic background of most diseases of the musculoskeletal system of the dog. It addresses veterinarians, students of veterinary medicine and also dog breeders and owners. The programme offers extensive basic information about all known hereditary diseases of the joints, bones and muscles of the dog and gives also information on symptoms, diagnosis, therapy, distribution and genetics of the diseases. The user can apply it as a reference book, as a tutorial to complete or control his knowledge or to gain a fast overview of the up-to-date knowledge. The tutorial was evaluated by students in the second and third academic year, veterinary surgeons and dog breeders. Computer assisted learning was judged as very positive by all test persons. The programme received from all test persons an average score of 2.11 +/- 0.77 on a scale from 1 (very good) to 5 (unsatisfactory). The results show that computer-aided learning can be an effective and stimulating substitution to traditional learning and learning materials.
