Two-year outcome data from a study comparing two kinds of treatment given similar groups of young, newly diagnosed, unmarried schizophrenic patients deemed in need of hospitalization are reported. The experimental program, Soteria, is a nonmedical, psychosocial program with minimal use of antipsychotic drugs; it is staffed by nonprofessionals and located in a home in the community. The control program is a short-stay, crisis-oriented inpatient service in a community mental health center where neuroleptic drugs are the principal treatment. The experimental group had significantly longer initial stays, and only 8 per cent received neuroleptics during their initial admission. Over the two-year follow-up period, there were no significant differences between the groups in readmissions or levels of symptomatology. However, experimental subjects significantly less often received medications, used less outpatient care, showed significantly better occupational levels, and were more able to live independently.
A community campaign against the establishment of psychiatric half-way house in Shatin District, Hong Kong was examined in terms of fear toward mental patients and problems in community relations. Failure of the Government to prepare for the community reintegration of mental patients pointed to lessons to be learned in Community Psychology. The apparent conflict between community empowerment and community care for the disabled was discussed. Suggestions for resolving the paradox included early community mental health education, involvement of community leaders in the planning, application of the knowledge and skills of Social Psychology in dealing with public panic, and cooperation with the mass media to reach the public.
This article discusses the importance of carefully monitoring and documenting patient behavior in mental health inpatient programs. A structured behavioral observation system is presented along with data evaluating its reliability. This Unit Coverage system is shown to have acceptable reliability, as reflected in an obtained interobserver agreement of 86.2%, corrected for chance.
This study examines gender similarities and differences in background characteristics, the effectiveness of treatment, and the predictors of post-release outcomes among incarcerated drug-using offenders. The sample of 1,842 male and 473 female treatment and comparison subjects came from a multi-site evaluation of prison-based substance abuse treatment programs. Three-year follow-up data for recidivism and post-release drug use were analyzed using survival analysis methods. Despite the greater number of life problems among women than men, women had lower three-year recidivism rates and rates of post-release drug use than did men. For both men and women, treated subjects had longer survival times than those who were not treated. There were both similarities and differences with respect to gender and the other predictors of the two post-release outcomes. Differences in background characteristics and in factors related to post-release outcomes for men and women suggest the plausibility of gender-specific paths in the recovery process.
Previous studies have shown elevated mortality among psychiatric and substance abusing patients, including veterans with PTSD. Although early studies showed elevated deaths from external causes among Vietnam veterans in the early postwar years, more recent studies have also shown increased health problems among veterans with PTSD. This study compared mortality due to behavioral causes versus other diseases among 1,866 male veterans treated for PTSD. Death certificates obtained for 110 veterans indicated behavioral causes accounted for 62.4% of deaths, standardized mortality ratio = 3.4-5.5, including accidents (29.4%), chronic substance abuse (14.7%), and intentional death by suicide, homicide, or police (13.8%). Results suggest possible opportunities to improve outcomes of this at-risk patient population through harm reduction interventions and improved continuity of care.

The La Posada program, a short-term alternative to hospitalization for acutely disturbed chronic patients, combines crisis intervention techniques with techniques developed in halfway houses to enhance independent functioning and counteract regressive, dependent behavior. Highly structured days involving a variety of approaches including group and individual therapy, therapeutic activities, expectations of appropriate behavior, and responsibility for ongoing treatment enable most clients to leave the program within nine days and to begin outpatient and day treatment before discharge. The program has proved to be a cost-effective and readily reproducible way of reducing hospital utilization by diverting patients directly or allowing early discharge to La Posada.

We evaluated the proficiency of the federal Medicaid program's survey process for evaluating intermediate care facilities for the mentally retarded. In Study 1, an observational analysis of active treatment during leisure times in living units suggested that these surveys did not discriminate between certified and noncertified units. In Study 2, a reactivity analysis of a survey indicated that direct-care staff performed differently during the survey by increasing interactions with clients and decreasing nonwork behavior. Similarly, results of Study 3 showed increases in client access to leisure materials during a survey. In Study 4, questionnaire results indicated considerable variability among service providers' opinions on the consistency, accuracy, and objectivity with which survey teams determine agency standard compliance. Results are discussed regarding effects of the questionable proficiency of survey processes and the potential utility of behavioral assessment methodologies to improve such processes.
