BACKGROUND: We have done a prospective, controlled, randomized study to investigate the role of the "zipper", a new device for skin closure. We have also analysed morbility and advantages with the use of the "zipper" compared with sutures. METHODS: 610 consecutive patients underwent surgery for abdominal thoracic endocrinologic and post traumatic pathologies. In 203 cases we used the zipper a new device for skin closure. RESULTS: 6/203 Morbility: in six cases it was necessary to substitute the zipper with sutures or leave the wound healing by second intention. The patients were operated for inguinal hernioplasty, axillary lymphadenectomy, appendicectomy and cholecystectomy. These patients developed complications after surgery as hematoma, lymphorrhea, wound infection and a reintervention. The correction has been done removing the zipper and positioning sutures or leaving the wound healing by second intention. CONCLUSIONS: The use of the zipper permits to achieve an efficient seal, a simple application, an aesthetic comfort; it can be applied in local anaesthesia and for its painless, application it is indicated in pediatric surgery.

PURPOSE: To investigate a new way in the treatment of serious congenital persistent pupilary membrane. METHODS: In four cases of six eyes, after temporal clear corneal tunnel incision was made, viscoelastic material was injected into the anterior chamber, and the persistent pupilary membrane was cut down at the very point connected to the iris. RESULTS: There is no ocular hypertention and hyphema after operation. After one year's follow-up, the lens remained transparent in 3 cases(4 eyes). There was corneal astigmatism of 0.12 to 0.25 D after operation. And only a few corneal endothelial cells were lost. CONCLUSION: There are few postoperative complications and corneal astigmatism to incise congenital persistent pupilary membrane by temporal clear corneal tunnel incision. It is an ideal way to treat serious persistent pupilary membrane.
BACKGROUND: Robotic mitral valve repair with the da Vinci robotic surgical system has been performed in more than 70 patients at our institution. This procedure reduces the need for blood transfusions, shortens hospital stay, and hastens return to normal activities. However, the robot-assisted repair also requires longer cardiopulmonary bypass and arrested-heart times than conventional open repairs. Because of increased risk of myocardial damage, arrhythmia, and other significant morbidities associated with longer arrested-heart time, a more efficient tissue approximation and adherence technique was evaluated to reduce operating time. METHODS: Twelve Dorset sheep were divided equally into 2 groups. In the control group Cosgrove-Edwards annuloplasty bands were secured to the posterior annulus with conventional 2-0 Ticron mattress sutures placed with robotic assistance. In the experimental group, the band was secured with double-armed nitinol U-clips placed with robotic assistance. Postoperative echocardiography was used to assess mitral valve function, and the animals were sacrificed at 3 or 6 months for histological evaluation. RESULTS: Total U-clip placement time was significantly decreased at 2.6 +/- 0.2 (mean +/- SEM) minutes versus total suture placement time at 4.9 +/- 0.4 minutes (P =.001). The main difference in time occurred between clip deployment at 0.75 +/- 0.1 minutes and suture tying at 2.78 +/- 0.2 minutes (P =.000003). Pathologic review showed excellent band incorporation at 3 and 6 months. Echocardiographic imaging showed no discernible mitral valve stenosis or regurgitation. CONCLUSIONS: With more cardiac procedures progressing toward minimally invasive approaches, novel technology to improve existing techniques must be evaluated. Nitinol U-clips help to reduce arrested-heart time and may improve outcome by decreasing morbidity. U-clip placement is intuitive, easily learned, and effective in securing the annuloplasty band to the mitral annulus.

There have been efforts to develop an ideal method of intestinal anastomosis throughout medical history. In ancient China, Egypt and India living ants were used to approximate the transected edges of bowel. Many other devices and techniques have since been introduced, using organic or synthetic, nonabsorable or absorbable materials. Lately mechanical stapling devices have become exceedingly popular. The recently introduced biofragmentable anastomotic ring (BAR) is composed of an absorbable polyglycolic acid in 12.5% barium sulfate suspension to permit x-ray visualization. Prospective randomized studies have shown it to be both effective and safe as compared to conventional handsutured or stapled anastomosis. Its main advantages are simplicity and ease of handing, short operating time and the absence of foreign bodies at the anastomotic site that might interfere with imaging methods, such as CT or MRI. We present our preliminary, satisfactory experience with BAR in 14 patients with a mean age of 67 years. 8 underwent right colectomy, 2 left colectomy, 1 sigmoidectomy, 2 resection of upper rectum, 1 subtotal colectomy, 1 excision of a giant lymphoma of the cecum, and 1 had had recurrent bouts of volvulus of the sigmoid.
