Reproducibility of repeated mountings of a noninvasive CT/MRI stereoadapter.

The reproducibility of a noninvasive computed tomography (CT)/magnetic resonance imaging stereoadapter was tested on 12 healthy volunteers. The adapter was easy to mount and detach without discomfort. Two repeated mountings of the adapter showed a good reproducibility in relation to the scalp. When extrapolated to a hypothetical target in the right thalamus, the mean differences between the two mountings were 0.07 mm in the lateral, 0.16 mm in the anteroposterior and 0.17 mm in the height coordinates. This high degree of reproducibility seems to permit a safe CT-guided functional stereotaxis, where the preoperative CT scanning can take place on one day and the surgery may be performed on any other suitable day.

Imaging for right lobe living donor liver transplantation.

The authors describe their experience using multidetector, multiphase computed tomography (CT) for the preoperative evaluation of donors undergoing adult right lobe liver transplantation. The marked improvement in image resolution has been extremely valuable in delineating small hepatic arterial structures, including accessory and replaced vessels. Assessment of liver parenchyma is valuable in determining the presence of steatosis or other unexpected abnormalities. Measurement of total liver volume is highly accurate and reproducible. Virtual hepatectomy can be performed in an avascular plane between the right and left lobes, to the right of the middle hepatic vein. Knowledge of the resultant right and left lobe volumes is critical in patient selection. Multidetector, multiphase CT, as a single imaging modality, provides a minimally invasive comprehensive pre-operative evaluation that can be expected to have a major impact on patient selection and surgical planning.

Comparison between maximal early diastolic velocity in long-axis direction obtained by M-mode echocardiography and by tissue Doppler in the assessment of right ventricular diastolic function.

Recently the maximal early diastolic velocity in long-axis direction of the right ventricle (RV) obtained by tissue Doppler imaging (MDV TDI) has been introduced in the assessment of RV diastolic function (RVDF). There are reasons to think that also the maximal early diastolic velocity in long-axis direction of the RV obtained using M-mode echocardiography (MDV TAM) could be used to assess RVDF. Therefore, 29 patients were examined with echocardiography and MDV TAM and MDV TDI were measured and compared. A good correlation (r = 0.76, P < 0.001) was found between MDV TAM and MDV TDI indicating that MDV TAM might be used in the assessment of RVDF. However, the velocities obtained by MDV TDI (126.7 +/- 38.9 mm s(-1)) were significantly (P < 0.001) higher than the velocities obtained by MDV TAM (78.3 +/- 27.8 mm s(-1)) and the agreement between MDV TAM and MDV TDI was rather poor probably mainly due to differences in the measuring technique. This means that reference values cannot be used interchangeably between MDV TAM and MDV TDI. If MDV TAM is going to be used in the assessment of RVDF new reference values have to be produced if today's technique and recommendations to measure MDV TAM and MDV TDI are used. However, as most new echocardiographs are equipped with PW-TDI technology it seems preferable to use this technique and compare obtained values with already established reference values.

The loss of pacing-induced preconditioning in atherosclerotic rabbits: role of hypercholesterolaemia.

A brief rapid pacing has been shown to protect rabbit heart against global myocardial ischaemia induced by subsequent longer pacing. We studied whether pacing-induced preconditioning was reproducible in experimental hypercholesterolaemia. In conscious rabbits with an implanted right ventricular electrode and left ventricular polyethylene catheters, pacing of 500 bpm over 20 min induced an intracavitary ST-segment elevation of 3.2 +/- 0.41 mV, shortened ventricular effective refractory period and increased left ventricular end-diastolic pressure from prepacing 105 +/- 3.9 ms and 4.0 +/- 0.93 mmHg to post-pacing 62 +/- 6.4 ms and 27.9 +/- 7.2 mmHg, respectively. A 10-min preconditioning pacing followed by a 5-min interval markedly attenuated these test pacing-induced ischaemic changes. Rabbits were fed a cholesterol-enriched diet over 4, 8 and 12 weeks, responded to a 5- or 10-min pacing with ischaemic changes of the same degree as did controls to a 10- or 20-min pacing, respectively. A 4-week diet elevated total serum cholesterol from 1.7 +/- 0.4 to 24.1 +/- 2.9 mmol/l without apparent atherosclerotic lesions in the thoracic aorta assessed by Oil-Red O staining and planimetry, but it abolished protection induced by a 5-min preconditioning pacing. A 12-week diet increased serum cholesterol and lesion surface area to 26.9 +/- 3.2 mmol/l and 89.6 +/- 6.4%, respectively, and continued to block preconditioning. When these animals were refed normal chow over additional 6 weeks, serum cholesterol level dropped to 2.6 +/- 0.80 mmol/l with no change in atherosclerotic lesions, the preconditioning effect, however, recovered. We conclude that hypercholesterolaemia blocks preconditioning irrespective of the development of atherosclerosis.

Problems raised by the simultaneous reproducibility of positive allergic patch test reactions in man.

The dose of petrolatum patch test material applied with the Leukotest or the Finn Chamber method varies significantly and is one important factor in the lack of reproducibility with present patch test methods. Ready-to-apply methods such as Epiquick and TRUE Test avoid this problem. In a right-side versus left-side comparative study of 19 Epiquick patches on 100 consecutive patients, the reproducibility was 95%. This is a much higher correlation than with previous comparative tests with both identical and different patch test systems.

Reproducibility study of magnetic resonance image-based computational fluid dynamics prediction of carotid bifurcation flow.

The importance of shear stress in the initiation and progression of atherosclerosis has been recognized for some time. A novel way to quantify wall shear stress under physiologically realistic conditions is to combine magnetic resonance imaging (MRI) and computational fluid dynamics. The present study aims to investigate the reproducibility of the simulated flow by using this combined approach. The right carotid bifurcations of eight healthy subjects were scanned twice with MRI within a few weeks. Three-dimensional geometries of the vessels were reconstructed for each scan and each subject. Pulsatile flows through these models were calculated to assess errors associated with the predicted flow parameters. This was done by comparing various wall shear stress indices, including the time-averaged wall shear stress (WSS), oscillating shear index (OSI), WSS Gradients (WSSG) and WSS Angle Deviation (WSSAD). Qualitatively, all the wall shear parameters proved to be highly reproducible. Quantitatively, the reproducibility was over 90% for OSI and WSSAD, but less impressive (60%) for other parameters. Our results indicated that WSS and WSSG values were extremely sensitive to subtle variations in local geometry and mesh design, particularly in regions around the bifurcation apex where WSS values were high and least reproducible.

Relationship of hand preference to accuracy on a thumb-positioning task.

This study was designed to examine the relationship of hand preference to performance on a thumb-positioning task. Two hypotheses were proposed on the basis of hemispheric lateralization. The first stated that all subjects would perform the task more accurately using the left thumb, regardless of hand preference. The second hypothesis tested was that left-handed subjects would perform more accurately than right-handed subjects. Each of 20 neurologically intact adults grouped by hand preference reproduced a criterion thumb position with both thumbs simultaneously. Absolute error, recorded for each movement and analyzed by mixed analysis of variance, did support the first hypothesis. However, the left-handed group did not perform statistically differently from right-handed subjects. Factors influencing the data are discussed and recommendations for further studies are made.

Long-term reproducibility analysis of fMRI using hand motor task.

This article reports test-retest reproducibility of functional MRI (fMRI) measurement on brain activation elicited by auditory-cued sequential finger tapping. Eight right-handed volunteers participated in nine fMRI sessions, approximately eight weeks apart, for the duration of more than a year. The first scan session was repeated within a day to examine the intra-session reproducibility. The frequency of activation for neural substrates relevant to the task was constructed across the subjects and sessions. The spatial reproducibility was measured as the ratio of the size of the volume as well as its overlaps with respect to the first scan session from regions-of-interest in the selected motor circuitry. Consistent activation patterns between sessions and across subjects were observed in the sensorimotor areas such as the left primary, supplementary, and premotor areas, as well as in the right cerebellar areas without evidence of session-dependent trends. Quantitative analysis showed that the reproducibility measures varied within the range obtained from studies on fMRI reproducibility covering much shorter terms. Intra-session fMRI scans yielded slightly better reproducibility measures compared to the results obtained from other scan sessions. The findings suggest that the reproducible fMRI measurement can be obtained for long-term monitoring of brain function.

Reproducibility of image interpretation in MRI of the prostate: application of the sextant framework by two different radiologists.

The aim of this study was to reproduce prostate cancer (PCA) localization by MRI based on prostatic sextants (right and left base, middle, and apex) with minimal systematic error. Combined endorectal/body-phased-array-coil MRI of the prostate at 1.5 T was retrospectively evaluated twice, with an interval of more than 1 month, by each of two independent radiologists (R1 readings R11 and R12, and R2 readings R21 and R22) in 23 patients (age 51-75 years) who had radical prostatectomy within 1 month of MRI. PCA stage was pT2 in 14 patients, and pT3 in nine. Median Gleason score was 7 (range 5-9). Histopathology showed 83 sextants with PCA and 55 without. Reproducibility of sextant positions was within one MRI slice (3 mm) in over 80% of cases. For PCA localization, ROC analysis (AUC=0.584+/-0.048-0.724+/-0.043) yielded no significant intra-reader differences. R11 and R21 differed slightly (P=0.035). Intra-observer agreement (kappa=0.52-0.58) exceeded inter-observer agreement (kappa=0.35-0.45). Intra-observer Spearman correlation (r=0.72-0.74) exceeded inter-observer correlation (r=0.43-0.51) for sextants with PCA, but not for sextants without (r=0.69-0.74). Per-sextant localization and reporting provides a highly reliable framework in MRI of the prostate. MRI of the prostate should be followed up by the same radiologists to minimize systematic error of interpretation.

Relative 99mTc-MAG3 renal uptake: reproducibility and accuracy.

UNLABELLED: The aim of this study was to estimate the reproducibility and accuracy of 99mTc-mercaptoacetyltriglycine (MAG3) relative percentage uptake. METHODS: Reproducibility was evaluated on healthy volunteers who were submitted twice to a 99mTc-MAG3 renographic study, which used different uptake algorithms, different background corrections and different time intervals. Accuracy was evaluated in a group of patients with symmetrical or asymmetrical relative renal function, who underwent both 99mTc-dimercaptosuccinic acid (DMSA) and 99mTc-MAG3 studies, using the DMSA relative percentage uptake as a reference. RESULTS AND CONCLUSION: The methods that combined the best reproducibility and accuracy for estimating 99mTc-MAG3 left-to-right uptake ratio were the integral method, with subrenal or perirenal background correction, and the Patlak-Rutland plot. The use of the integral method without background correction introduced a systematic bias, whereas the slope method resulted in high variability. Therefore these methods cannot be recommended.

Recording of blood flow velocity waveforms in the uteroplacental and umbilical circulation: reproducibility study and comparison of pulsed and continuous wave Doppler ultrasonography.

Reproducibility of the blood flow velocity waveforms (FVW) recorded from the umbilical artery and the arcuate arteries on the right and left side of the placenta was examined in 8 women with normal pregnancies and anterior placentas. The FVW were recorded using a 2-MHz pulsed Doppler ultrasound system in combination with a real-time linear-array scanner. The FVW were characterized by the pulsatility index (PI) and systolic/diastolic (S/D) ratio. For the PI, the mean coefficients of variation between six operator pairs in the umbilical artery and the left and right arcuate arteries were 8.4%, 26%, and 21.5%, respectively. These findings suggest that, in clinical practice, the umbilical artery FVW is reproducible, but the usefulness of arcuate artery FVW is limited by the wide variation of Doppler signals. Paired recordings of FVW were obtained from the umbilical, arcuate, and uterine arteries using pulsed wave and continuous wave Doppler ultrasonography in a randomized order in another 21 pregnant women with anterior placentas. There was no difference between the two Doppler modes for any of the measured waveform indices.

[Histopathological changes of lung and right ventricle induced by high concentration of carbon dioxide in rats].

OBJECTIVE: To evaluate if high concentration of carbon dioxide causes any histopathological changes of the lung and right ventricle in rats. METHODS: Wistar rats were divided into control group (Group A, n = 21) and hypercapnic group (Group B, n = 30). Group B received mixed gas exposure (8% CO(2), 21% O(2), 71% N(2)), 7 hours daily for 4 weeks. Arterial blood gas, superoxide dismutase (SOD), lipid peroxide (LPO) in the tissue of the lung and right ventricle were determined. Histopathological changes of the lung and right ventricle were investigated by light microscope, transmission and scanning electron microscope. RESULTS: Decompensated respiratory acidosis [pH 7.21 +/- 0.04, PaCO(2) (56 +/- 3) mm Hg] developed in Group B. SOD in the lung [(463 +/- 174) U/g protein] and ventricle tissue [(235 +/- 94) U/g protein] in Group B were significantly lower than those in Group A (P < 0.01), while LPOs [(8.1 +/- 3.2) nmol/ml and (17 +/- 5) nmol/ml] in group B were significantly higher than those in Group A (P < 0.01). The index of pulmonary capillary permeability in Group B (37 +/- 4) was also higher than that in Group A (28 +/- 4, P < 0.001). Tissue edema in the lung and heart, endothelial cell damage of the small blood vessels and severe myofibril damage of the right ventricle, pulmonary microthrombus formation and increased pulmonary capillary permeability were found in Group B. CONCLUSION: Hypercapnic rat model is successfully reproduced by exposure of 8% CO(2); high concentration of CO(2) causes severe damage of the lung and right ventricle as well as the whole body.

Reproducibility of cortex-muscle coherence.

Cortex-muscle coherence is a frequency-analysis technique that has been increasingly applied in the investigation of movement disorders. To study the intra- and inter-session stability of the cortex-muscle coherence, we recorded from 12 healthy subjects magnetoencephalographic (MEG) and surface electromyographic (EMG) signals during unilateral isometric contractions of the left- and right-hand muscles. Two identical measurements were performed during one session, and the session was repeated once after about 1 year. In one experienced subject, the recordings were repeated seven times within 20 months. The MEG-EMG coherence exceeded the noise level in 10 out of 12 subjects. Both the frequency (correlation coefficient r = 0.77-0.93, P < 0.01) and strength (r = 0.78-0.91, P < 0.01) of coherence were well reproducible within each session for both left- and right-sided contractions. The inter-session reproducibility was high for the mean of cumulative coherence frequency (r = 0.90-0.95, P < 0.01), but relatively low for coherence strength (r = 0.43-0.59, P > 0.05). The results for one subject participating in 8 repeated sessions strongly supported the results of the whole group. Thus, intra-session reproducibility of both strength and frequency of the cortex-muscle coherence is good and studies comparing different conditions at the group level within one session are feasible. However, caution is needed when interpreting absolute levels or changes in the strength of coherence in single subjects between the sessions.

A combined behavioral-physiological method for the assessment of thermal sensibility in the rat.

A behavioral and physiological method has been developed to study thermal detection in the rat. Threshold temperatures signaling (1) disruption of a trained behavior (forelimb bar pressing) and (2) increased heart rate were determined using a gradually increasing thermal stimulus delivered to the hindlimb. Response thresholds for disruption of bar press activity (44.8 degrees C) and heart rate elevation (44.4 degrees C) were statistically equivalent. No differences in thresholds for either response end point were found between the left and right hindlimbs, thus validating the consistency of the technique and reproducibility of the stimulus delivery device during test periods of 3-4 weeks. Advantages of the method include the use of two independent end points that are determined objectively, performance criteria that do not rely upon motor responses from the stimulated limb and the use of quantitative descriptors that enables comparisons to be made between different test groups. The method combines behavior with cardiovascular and somatosensory function in a way that can be used for the assessment of spinal and supraspinal pathways involved in thermal detection in the unanesthetized, behaviorally active rat.

Restoration of functional gait in paraplegic patients with the RGO-II hybrid orthosis. A multicenter controlled study. II: Physiological evaluation.

We have investigated physiological changes in 21 patients with a spinal cord injury who were fitted with the RGO-II hybrid orthosis. All parameters were measured before and after a training programme in order to evaluate the benefit of gait rehabilitation, cardiovascular adaptation, constipation, spasticity and osteoporosis. A tendency for the improvement in cardiovascular function was noticed, and a segmental decrease in right colonic transit time was observed. However there was no reproducible change in spasticity, and no gain in bone mineral density. These data suggest that the physiological benefits which occur when patients walk with the aid of a hybrid orthosis only correct the effects of immobility. In addition, we did not find any physiological improvement regarding the neurological lesion (spasticity or osteoporosis).

Formation of oiliness and sebum output--comparison of a lipid-absorbant and occlusive-tape method with photometry.

Sebum output and the development of oiliness formation were studied in 24 acne-prone volunteers. Sebutape and photometric measurement by the Sebumeter were compared. Tapes were taken 1, 2 and 3 h after degreasing. Tapes were scored, and light transmission was measured by a special densitometric device. Sebumeter recordings were performed after 3 h. Densitometric evaluation of tapes was accurate with a high correlation to scoring. Sebutape and Sebumeter assessments correlated. However, in some individuals the tape method overestimated the sebum output. Right-left comparison indicated that the tape method was less reproducible, particularly after longer sampling periods. It is suggested that Sebutape, due to water occlusion and temperature insulation during the sampling period, interferes with sebum droplet formation and spreading. However, such systematic interference may be advantageous since sweating and heat are important clinical prerequisites in the formation of oiliness. Thus, it is suggested that the Sebutape is a specialized method for the determination of 'oiliness', the very last phase of sebum output, in which sebum droplets spread over the skin surface. The tape is not automatically comparable to other methods for determining sebum output from the follicular reservoir.

A method for simultaneous epicardial monophasic action potential recordings from the dog heart in situ.

In order to record epicardial monophasic action potentials (MAP) simultaneously from different regions of intact beating hearts, we developed a tripodal suction electrode device (total weight 1.5 g, distance between the flexible silicone legs 25 mm) which we tested in pentobarbital anaesthetized open chest dogs. The device was easy to apply and gave stable and reproducible recordings. Repolarization times for epicardial left ventricular and endocardial right ventricular MAPs correlated well (r = 0.97, P less than 0.001). There was no correlation between MAP amplitude and repolarization times. The beta 1-adrenergic agonist prenalterol decreased MAP duration, while the new class III antiarrhythmic drug melperone increased MAP duration. Mild ischaemia effected MAP prolongation and severe ischaemia MAP shortening, compared to simultaneous recordings from non-ischaemic ventricular regions. We conclude that the new tripodal suction electrode is a simple device for simultaneous recording of multiple MAPs. The method should be suitable for studies of electrophysiological effects of drugs and other interventions in intact beating hearts.
