Angiographic transcatheter embolization in gynaecological practice.

Intractable uterine haemorrhage, unresponsive to medical therapy or curettage, has been traditionally treated by hysterectomy and/or internal iliac artery ligation. Percutaneous angiographic localization of the bleeding vessel with subsequent embolization, may be a genuine alternative approach that enables preservation of reproductive potential with minimal morbidity. We present 3 such cases here, all subsequent to curettage in the first trimester of pregnancy, 2 after a therapeutic termination of pregnancy and the other after an incomplete abortion.

Spontaneous abortion: short-term complications following either conservative or surgical management.

Spontaneous abortion is a common gynaecological condition. It is a commonly held belief that medical morbidity associated with this condition is low and that routine treatment should be surgical evacuation of the uterus. This study was performed to study the short-term complications of spontaneous abortion and its management. Transvaginal sonography (TVS) was used to determine whether retained products of conception (POCs) were visible inside the uterus in women presenting with spontaneous abortion. If tissue was present, surgical evacuation of retained products of conception (ERPC) was performed. If the uterus was empty, the patients were managed expectantly. Four hundred and seventy women were treated with ERPC and 297 were managed expectantly. The complication rate was 3.0% in those managed expectantly compared with 5.8% for those treated by ERPC. Subjects with no POCs on TVS can therefore be managed expectantly without increasing the risk of morbidity associated with this condition.

Estrogen and progesterone receptors and telomerase enzyme immunohistochemical detection in gestational trophoblastic tumors.

The purpose of this study is to evaluate the immunohistochemical detection of telomerase enzyme and estrogen receptor (ER) and progesterone receptor (PGR) in gestational trophoblastic neoplasia (GTN) and its clinical significance. Formalin-fixed paraffin blocks for 30 patients (24 with molar pregnancy, 3 with choriocarcinoma, and 3 with placental site trophoblastic tumor) as cases and six products of conception samples from patients with incomplete abortion as controls were included in the study. Immunohistochemical detection of the telomerase catalytic protein and ER and PGR was carried out using streptavidin-biotin-peroxidase method. All control tissues were negative for telomerase and ER expression, while five of six were PGR positive. Significant positive telomerase expression was detected in all gestational trophoblastic tumors (three of six partial moles, 12 of 18 complete moles, three of three choriocarcinomas, and two of three placental site trophoblastic tumors). Nine of 24 molar pregnancies were followed by GTN. Molar pregnancies followed by GTN were associated with higher serum beta-hCG (human chorionic gonadotrophic hormone), larger uterine size for gestational age, negative ER expression, negative PGR expression, and positive telomerase expression. All patients with molar pregnancy with negative telomerase expression (9 of 24) showed spontaneous regression after evacuation. Positive telomerase expression and its immunohistochemical detection are associated with the development of GTN. Negative telomerase expression is highly predictive of postmolar spontaneous regression. Patients with molar pregnancies with negative telomerase expression can be saved the long-term follow-up. ER and PGR expression do not show a significantly different pattern in molar tissues, while negative expression is associated with developing GTN. Cautions on the use of postmolar hormonal contraception may be unjustified.

Holoprosencephaly in a long-tailed macaque (Macaca fascicularis): a case report.

A long-tailed macaque fetus with ethmocephaly, alobar holoprosencephaly, and arhinia is reported. This fetus was exposed to an antiprogestational agent, RU 486 (2.5 mg/kg intramuscular, once daily), during gestational days 15-18. Three hypotheses are proposed to explain these malformations: (1) they are a direct result of drug exposure, (2) they are a secondary effect of treatment and the result of decreased blood supply to the developing embryo due to an incomplete abortion, and (3) they represent a spontaneous occurrence.

Effect of influenza infection on the phagocytic and bactericidal activities of pulmonary macrophages.

The effect of mouse-adapted influenza A/PR/8/34 virus on pulmonary macrophage function was evaluated by using an in vitro system which allowed direct virus interaction with macrophages and then separate analysis of the steps required for bacterial clearance by macrophages. Infection of macrophages with this virus resulted in the appearance of a hemagglutinating activity on the macrophage surface; expression of this activity was inhibited by amantadine, 2-deoxyglucose, and cycloheximide and by pretreatment of the virus inoculum with ultraviolet light and specific antiserum. Since there was no release of extracellular virus, this growth cycle appeared to be incomplete (abortive). After influenza infection, net ingestion of viable Staphylococcus aureus by macrophage monolayers was unaltered and there was no change in the fraction of the monolayer which ingested cocci over a wide range of bacterial inputs. Influenza-infected macrophages also inactivated intracellular S. aureus at a rate indistinguishable from controls. Therefore, these in vitro studies do not support the hypothesis that the defect in pulmonary antibacterial mechanisms associated with influenza infections results from a direct effect of virus infection on either the phagocytic or bactericidal activity of resident pulmonary macrophages.

Evidence for adverse reproductive outcomes among women microelectronic assembly workers.

Microelectronics assembly entails complex processes where several potentially fetotoxic chemical compounds are used extensively. This study was undertaken to assess the potential adverse reproductive outcomes among former women workers in a microelectronics assembly plant in New Mexico with respect to a comparable population from the same geographical region and to examine the relation between these outcomes and employment history in this plant. After matching a pool of 143 former microelectronic female workers and 105 referents, 90 former microelectronic female worker-referent pairs were constituted (representing 302 and 324 pregnancies in former workers and referents respectively). The odds ratio (for pair matching design) of spontaneous abortion among women workers, before beginning to assemble microelectronic components, was 0.9 (chi 2 = 0.04; NS). After the beginning of employment this odds ratio became 5.6 (chi 2 = 9.8; p less than 1%). This estimated odds ratio decreased to 4.0, taking into account the increased risk for spontaneous abortion in previous pregnancies before employment (chi 2 = 5.4; p less than 5%). It was not possible to determine if this effect was reversible owing to the small number of pairs available after employment. The findings of this study corroborate the results of former studies that suggest a potential association between electronic manufacturing activity and risk of spontaneous abortion. Although the organic solvents were suspected of being the potential risk factor, this study was inconclusive from this point of view. Nevertheless, these investigations may provide some insight into reproductive outcomes among female workers exposed to solvents.

Early pregnancy complications: endovaginal sonographic findings correlated with human chorionic gonadotropin levels.

Endovaginal sonography results were compared with quantitatively determined human chorionic gonadotropin (hCG) levels in 84 women referred for early pregnancy complications. Of the 27 with normal intrauterine pregnancies, an intrauterine gestational sac was prospectively identified in one of five cases (20%) in which hCG levels were below 500 IU/L (Second International Standard), four of five (80%) with hCG levels of 500-1,000 IU/L, and all 17 with hCG levels above 1,000 IU/L. In comparison, 17 of the 26 women with ectopic pregnancies (65%) had hCG levels greater than 1,000 IU/L, and none of the 26 had an intrauterine gestational sac. Endovaginal sonography demonstrated an adnexal mass and/or a gestational sac-like structure in 16 of the 17 cases (94%) in which hCG levels were above 1,000 IU/L, compared with only three of the nine (33%) with lower hCG levels (P less than .01). These findings indicate that an intrauterine gestational sac should be normally visualized with endovaginal sonography when the hCG level exceeds 1,000 IU/L, and that visualization of an extrauterine gestational sac and/or adnexal mass is significantly more likely in ectopic pregnancies when the hCG level exceeds 1,000 IU/L.

Evaluation of the accuracy of transvaginal sonography for the assessment of retained products of conception after spontaneous abortion.

Transvaginal sonography (TVS) was used to assess uterine contents in 100 consecutive subjects presenting with a diagnosis of spontaneous abortion. TVS assessments were correlated with findings at subsequent evacuation of retained products of conception (ERPC). There was a strong correlation between the weight of the surgical specimen at ERPC with both the sagittal (r = 0.76; p < 0.05) and transverse (r = 0.73; p < 0.05) plane area measurements of the uterine cavity. Combining the 2 sonographic measurements increased the correlation to r = 0.81 (p < 0.05). Fifteen of 25 subjects who had been categorised to have an 'empty' uterus had less than 5 g of tissue removed and the other 10 subjects less than 10 g of tissue. In 20 of these 25 cases, the surgeon described the amount of curettings as 'small or non-significant'. TVS can accurately identify those women who do not have a significant amount of residual uterine tissue following spontaneous abortion.

Unusual urethral duplication.

Urethral duplication is a rare congenital anomaly occurring almost exclusively in males. The described varieties include the sagital (Y-duplication), spindle, epispadiac, hypospadiac, complete, incomplete, abortive and collateral duplications. Whereas over 150 cases of this anomaly have been reported in the world literature, the associated findings of an adenocarcinoma of the urinary bladder and a urethral calculus have not been previously encountered. This case report describes the clinical presentation, the extraordinary intraoperative finding of an adenocarcinoma of the fundus of the urinary bladder, the investigative procedures, the management and follow-up of this most unusual complete urethral duplication.

Bromocriptine treatment during early human pregnancy: effect on the levels of prolactin, sex steroids and placental lactogen.

To elucidate the role of prolactin (Prl) in the endocrinology of early human pregnancy, 28 healthy women were given 5.0-7.5 mg of bromocriptine daily for two weeks between weeks 6-9 of normal gestation. Plasma Prl, oestradiol-17 beta (Oe2), progesterone (P), testosterone (T) and human placental lactogen (hPL) were measured before, and one and two weeks after the start of bromocriptine, and they were compared with those in 22 control women who were followed similarly but without bromocriptine treatment. Bromocriptine treatment induced a significant Prl depression at one week (7.3 +/- 4.3 ng/ml vs 23.7 +/- 11.4 ng/ml) and two weeks (5.3 +/- 2.5 ng/ml vs 31.9 +/- 16.4 ng/ml). Oe2, P, T and hPL levels, however, showed no significant differences between the groups. Two women undergoing bromocriptine treatment (7.1%) and one control women (4.5%) experienced spontaneous incomplete abortion during the study period, but these three already had a low Oe2 level and a low/or undetectable hPL level at the beginning of the study. It is obvious that neither maternal 'hypoprolactinaemia' nor bromocriptine during early human pregnancy interfere with the normal progress of pregnancy or with the normal synthesis of sex steroids and hPL at this time.

HVJ infection in alveolar macrophages of various laboratory animals.

Alveolar macrophages (AM) in bronchoalveolar lavage derived 6 species of laboratory animals (mouse, rat, Syrian hamster, Mongolian gerbil, guinea pig, and rabbit) were cultivated in vitro and were infected with HVJ, parainfluenza virus type 1, for the purpose of studying virus reproduction in these cells. Using infectivity assay, fluorescent antibodies and hemadsorption techniques, three categories were proposed depending on the potential of virus replication and antigen production. Category 1 (mouse, Syrian hamster, and guinea pig AM) was the most susceptible group to HVJ infection. All AM became antigen positive, and more than 10(6) PFU/ml of virus infectivity in the culture fluids was noted at 4 days after the virus infection. Rat and Mongolian gerbil AM were grouped into Category 2. Their AM showed a moderate virus yield (average 10(5) PFU/ml of infectivity) and antigen production. On the other hand, Category 3 (rabbit AM), exhibited no production of an infectious virus. It was likely that the virus penetrated into the AM but did not replicate complete virus. However, antigen positive cells ranging from 10 to 50% depended on the multiplicity of the infection to the AM. These findings suggest that HVJ infection in the rabbit AM was abortive (incomplete virus replication), since trypsin treatment of the culture fluids, which may contain incomplete HVJ particles, did not induce infectivity.

[Clinical diagnosis and anatomic-pathologic diagnosis: disagreements].

UNLABELLED: Many pathologic entities in the clinical practice generate disagreements regarding its identification, not only by its likeness with other lesions but also by its semantics. BACKGROUND: The goal of this work is to clarify which disagreements are more frequent in the clinical practice, supply new knowledges to facilitate the identification of the larger pathologies controversies and to enlarge its differentials diagnosis. METHODS: We revised 1825 reports of referring biopsies to the period of 1992 up to 1999, belonging to the Laboratory of Pathological Anatomy of Unoeste, being excluded 439 lauds that did not introduce diagnostic hypothesis, or that had as signals hypothesis and clinical indications or "for clearing". We confronted the clinical hypothesis with the diagnosis Anatomic-pathological, obtaining itself 444 (32.05%) of discordant cases. RESULTS: Note that the biggest disagreements were between Hansen's disease clinical diagnosis, which in 65.7% cases were chronicles Unspecific Dermatitises, between Sebaceous Cyst, which in 80% cases were cyst of Epidermical Inclusion and Incomplete Abortion, which in 68.2% cases were complete Abortion. CONCLUSION: We conclude that an adequate concepts definition discerning an anamnese and narrow correlation of the clinical characteristics of the lesions promote a minor disagreements number among clinical diagnosis and Anatomic-pathological.

Comparison of complication rates in first trimester abortions performed by physician assistants and physicians.

The outcomes of 2,458 first trimester abortions performed in a freestanding clinic in Vermont were studied. Procedures were performed by physician assistants and by physicians. Demographic information, medical history, and data relevant to the abortion were recorded. Both immediate and delayed (up to four weeks post-abortion) complications were noted. Direct follow-up four weeks after the procedure by clinic visit, letter, or telephone contact was achieved for 96 per cent all patients. An overall rate of 29.1 complications per 1,000 procedures was observed, with a rate of 27.4 for abortions performed by physician assistants and 30.8 for physicians. The incidence of immediate complications was 6.1 per 1,000 procedures; delayed complication incidence was 23.2 per 1,000 procedures. Overall complication rates varied according to operative procedure used.
