Protection from ototoxicity of intraperitoneal gentamicin in guinea pig.

BACKGROUND: Aminoglycoside antibiotics are common to treat peritonitis and exit-site infections in patients on peritoneal dialysis. Ototoxicity (loss of hearing or balance) is a well-documented adverse effect of aminoglycosides, and severe ototoxic reactions have been noted in patients receiving these drugs by intraperitoneal lavage. We have proposed a free-radical hypothesis for the mechanism of aminoglycoside ototoxicity and suggested a therapeutic prevention by the concomitant administration of antioxidants or iron chelators. Here we investigate whether 2, 3-dihydroxybenzoate can prevent the ototoxicity of intraperitoneal gentamicin. METHODS: Two strains of pigmented guinea pigs received daily intraperitoneal injections of gentamicin. Both strains developed ototoxicity, although different dosages were needed to produce similar auditory deficits (120 mg gentamicin base/kg body weight daily for 19 days vs. 135 mg/kg for 14 days). Dihydroxybenzoate was administered intraperitoneally once or twice daily. Auditory thresholds were measured by evoked brain stem response. Pathology was assessed as a loss of sensory cells in surface preparations of the organ of Corti. RESULTS: The auditory threshold shifts and hair cell loss were similar to the pathology observed following subcutaneous injections of gentamicin. Animals sustained almost complete loss of outer hair cells in the basal cochlea and a progressive hearing loss with threshold shifts of 60 dB at 18 kHz. The concomitant administration of dihydroxybenzoate significantly attenuated the threshold shift to less than 30 dB and reduced the loss of hair cells. The treatment with dihydroxybenzoate did not affect serum gentamicin levels. CONCLUSIONS: Antioxidant therapy is a promising approach to prevent aminoglycoside-induced hearing loss following intraperitoneal application.

Impulse noise of different durations.

This paper describes a comparison between auditory threshold elevations and morphological cochlear changes evaluated with scanning electron microscopy in order to identify cochlear defects in guinea pigs exposed to impulse noise of varying durations. There was a significant difference in the auditory threshold shifts between animals exposed for 3 and 12 hours. Hair cell loss is generally used as a morphological correlate to noise-induced hearing loss. Minor stereocilia changes, quantified with non-standard statistical methods have been used as a complement. Morphologically, it was not possible to separate cochleas exposed to impulse noise for 3 and 12 hours when only lost hair cells were evaluated. Quantitative evaluation of subtle morphological changes are valuable in the correlation of noise-induced hearing loss with morphologic changes.

Prevention of gentamicin induced ototoxicity by trimetazidine in animal model.

OBJECTIVE: To show the efficacy of intra-peritoneally administered trimetazidine to prevent gentamicin ototoxicity, which is still an important cause of profound deafness among children in different parts of the world. METHODS: Two groups of Swiss albino mice received daily intra-muscular injections of gentamicin for 30 days. One of the groups received trimetazidine intra peritoneally in addition to the gentamicin. Auditory thresholds of the animals were measured by evoked brain stem response at the beginning and the end of the study. Results were compared to the results of the control group, which received intra peritoneal saline injections. RESULTS: Both groups receiving gentamicin injections had significant auditory threshold shifts, but in the group receiving additional trimetazidine, the threshold shift was not statistically significant when compared to control group. Threshold shift in gentamicin group significantly differed from that of the control group (p=0.0001) and gentamicin+trimetazidine group (p=0.0001), on the other hand there was no statistically significant difference between control group and trimetazidine+gentamicin group (p=0.102). CONCLUSION: Gentamicin ototoxicity can be prevented by intra peritoneal trimetazidine injections in animal model. This treatment modality may be a mode of protection from gentamicin ototoxicity in children.

Effect of noise on auditory evoked responses in awake guinea pigs.

Changes in the auditory nerve action potential (AP), evoked responses from the inferior colliculus (IC-ER) and auditory cortex (AC-ER) were assessed after exposure to white noise of 120 dB SPL for 1 h in awake guinea pigs. Auditory thresholds were estimated with the aid of averaged AP, IC-ER and AC-ER, besides the threshold shifts also the changes in amplitude-intensity functions were evaluated. Auditory thresholds for tone pips and clicks increased by 20-30 dB 1 h after exposure and were similar in all the three investigated structures. The maximum threshold shifts for tone pips were observed at 8 kHz and were 33.2 +/- 12.9 dB for AP, 30.4 +/- 12.7 dB for IC-ER and 30.8 +/- 13.0 dB for AC-ER (n = 20). The thresholds recovered to preexposure levels within one week. Reduction in AP and IC-ER amplitudes 1 h after exposure was similar, the amplitude-intensity functions were shifted by 20-40 dB. In contrast, the amplitude-intensity functions in the auditory cortex 1 h after exposure were steeper than before exposure and this amplitude enhancement was present for 24 h after exposure. The enhancement of the AC-ER which resembles recruitment and which may be a sign of hypersensitivity of the animal to auditory stimuli was present only when the animals exposed to noise were awake. The noise exposure in animals anaesthetized with urethane reduced the amplitude-intensity functions of all three recorded potentials.

Low-frequency hearing loss following perinatal exposure to 3,3',4,4',5-pentachlorobiphenyl (PCB 126) in rats.

Previous research has demonstrated the sensitivity of the developing rat to the ototoxic effects of exposure to Aroclor 1254. In this study we assessed the effects of developmental exposure to an individual PCB congener (3,3',4,4',5-pentachlorobiphenyl; PCB 126) on auditory function. Nulliparous Long Evans rats received either 0, 0.25, or 1.0 microg/kg/day (5 days/week) for 35 days prior to breeding and throughout gestation and lactation. Auditory thresholds for 0.5-, 1-, 4-, 8-, 16-, 32-, and 40-kHz tones were assessed in offspring on postnatal days (PND) 76-90. Perinatal maternal PCB 126 exposure caused low-frequency hearing deficits. Elevated auditory thresholds occurred in the 1.0 microg/kg/day treated group for 0.5- and 1-kHz tones, whereas thresholds were not significantly affected at any higher frequencies. These results are important in that the data implicate, at least partially, the coplanar PCBs in the developmental ototoxicity induced by Aroclor 1254.

TNFalpha blockers do not improve the hearing recovery obtained with glucocorticoid therapy in an autoimmune experimental labyrinthitis.

The effectiveness of etanercept [tumour necrosis factor-alpha (TNFalpha) blocker] and corticoids in treating immuno-mediated inner ear disease (IMIED) was compared in an animal model of autoimmune labyrinthitis. IMIED is one of the few forms of sensorineural hearing loss that is reversible with proper medical treatment. While the effectiveness and usefulness of immunomodulating agents (corticosteroids) in treating IMIED have been demonstrated, TNFalpha antagonists, which inhibit granuloma formation in rheumatoid arthritis and other autoimmune diseases, have been considered as an alternative therapy. The efficacy of etanercept (anti-TNFalpha) was evaluated in a guinea pig model of experimental autoimmune labyrinthitis in which 25 guinea pigs were divided in a control group, which was used to document the rise in hearing thresholds following immunisation, and two experimental groups, which were treated with steroids (6-methylprednisolone) and anti-TNFalpha (etanercept), respectively, after the immunisation. Comparison of the auditory thresholds obtained by means of auditory brainstem response (ABR) revealed that the auditory thresholds of the two experimental groups were not statistically different (6-methylprednisolone: 41.5 dB, SD: 8.51; etanercept: 37.5 dB, SD: 7.91) and that both compared favourably with that of the control group (60 dB, SD: 7.91) at p=0.001. We therefore conclude that etanercept is as effective as glucocorticoids in an animal model of autoimmune labyrinthitis; however, the potential adverse effects and high price of the former advise against its use as an initial therapy for IMIED.

Frequency specificity of human distortion product otoacoustic emissions.

The amplitudes and growth functions of distortion product otoacoustic emissions (DPOE) at 2f1-f2, elicited by two primary tones f1 and f2 with a constant frequency ratio f2/f1 = 1.23 and varying geometric mean values 1, 1.5, 2, 4, 6 and 8 kHz, were measured for 25 normal subjects and 50 patients with sensorineural hearing loss. Partial correlations between DPOE amplitudes and auditory thresholds (0.25 to 8 kHz, half-octave steps) were examined. The amplitude of DPOE evoked by low-intensity primary tones (at or below 62 dB SPL) was strongly correlated only with the auditory threshold at their mean frequency, and DPOE disappeared for local hearing losses larger than about 30 dB. Moreover, DPOE amplitudes did not depend on the basal cochlear state. Confounding effects of middle ear transmission and aging were not significant in this set of experiments. When elicited by higher intensities of primary tones (72 dB SPL), DPOE exhibited a more complex and non-local behavior, and their sensitivity to hearing loss decreased. These results suggest that when low-intensity primaries are used, DPOE patterns provide frequency-specific information on the local cochlear state.

Carboplatin is ototoxic.

For assessment of the ototoxic potential of carboplatin [cis-diammine-1,1-cyclobutane dicarboxylate platinum(II); CBDCA], pure-tone audiograms were evaluated in 27 patients receiving a total of 119 doses of carboplatin in the range of 300-400 mg/m2. Pure-tone audiometry (PTA) was done immediately prior to and 4 weeks after the administration of 80 doses (67%). Defining carboplatin ototoxicity as an increase of greater than or equal to 30 dB in auditory thresholds that was unexplainable by other causes, we identified 5 examples (19%). Hearing loss tended to be cumulative with increasing dose and was always maximal at 8,000 Hz. Two patients had an increase in auditory thresholds at 1,000 Hz, but this only amounted to 10 dB in each case. Patients developing ototoxicity tended to be older. Sex, the pre-treatment creatinine clearance, the pretreatment audiogram, the number of doses, and the cumulative dose did not emerge as being reliable predictors of subsequent ototoxicity. We conclude that although carboplatin is ototoxic, clinically significant deafness does not occur with conventional dosing and routine audiometric monitoring is therefore unnecessary. However, we suggest that caution should be exercised when carboplatin is given either at higher doses or for longer periods when there is concomitant use of other potentially ototoxic agents or when there is significant pre-existing auditory impairment.

Augmenting-reducing and sensation seeking: a test of Sales' hypothesis.

Physiologically, augmenters amplify incoming stimulation, while reducers attenuate such stimulation. Sales has theorized that this characteristic is a determinant of an individual's need for stimulation and that reducers require more actual stimulation to reach this perceived optimal level than do augmenters. The theory predicts that measures of these characteristics would be correlated. The absolute auditory threshold was used as a measure of augmenting-reducing and the Sensation-Seeking Scale as a measure of need for stimulation in a study that tested 42 college students. The theoretical prediction was not confirmed. The bearing of these negative findings upon Sales' theory and the possibility that the auditory threshold is not a valid measure of augmenting-reducing were discussed.

Limitations in the use of distortion product otoacoustic emissions in objective audiometry as the result of fine structure.

There is strong evidence for a link between intact cochlear function and otoacoustic emissions (OAE). However, all attempts to find a close correlation between auditory thresholds and amplitudes of distortion product otoacoustic emissions (DPOAE) have failed. As an explanation for these findings, we have studied DPOAE fine structure and its dependence on increasing primary sound levels. Errors due to different calibrations of equipment for measuring DPOAE and auditory thresholds were also investigated. DPOAE were measured in 16 subjects using a frequency range of 500-1000 Hz. Frequencies were changed in 12.5 Hz steps at primary levels of 55, 60, 65 and 70 dB SPL. DPOAE amplitudes were found to vary by up to 20 dB for a frequency step of 50 Hz. Some fine structures showed narrow dips that shifted in frequency and diminished in amplitude with increasing primary levels. These findings demonstrated that sampling DPOAE amplitudes at widely spaced frequencies gave incomplete information about true course. DPOAE growth functions measured close to a dip in the DPOAE fine structure were rendered useless by interference with either the frequency shift or amplitude variations of the dip at different primary levels.

[Asymptotic points of functional and morphological damages produced by acoustic trauma in guinea pig cochleas].

Experimental animals were exposed to 3 kHz pure tone with an intensity of 110 dB (A) for from 10 minutes to 5 days. Structural damages were assessed by surface specimen technique and functional damages estimated by the N1 threshold audiogram. The asymptote of auditory threshold shift was reached after 2 hours of exposure. Corresponding to it, there was an asymptote of morphological damage as well. The existence of asymptote doesn't mean free from further damage in spite of continuous exposure. The maximum N1 threshold occurred at a frequency higher than the exposed frequency. However, the maximum morphological damage was observed in a region of the organ of Corti corresponding to the exposed frequency.

Acute human trial of the floating mass transducer.

Five patients underwent acute implantation of the Floating Mass Transducer (FMT) to evaluate the ability of the FMT to produce measurable auditory thresholds when temporarily placed in the middle ears of patients undergoing routine stapedotomy procedures. The FMT was placed on the long process of the incus in an inferior position, as is planned for clinical application, and in two additional positions. The ability of the FMT to produce measurable auditory thresholds was demonstrated in all patients. Results were affected by the lack of a healing period, the presence of a fixed stapes and an open middle ear space, and patient sedation.

On objective criteria for hearing impairment and hearing disability.

The relationship between subjective symptoms of hearing disorder and objective criteria of hearing impairment and hearing disability was examined in a group of 77 male subjects (median age 57 years, range 49-68) suffering from noise-induced permanent hearing loss. Among all subjects, 52% (40/77) reported having a general impression of subjective hearing disability. The average of pure-tone auditory thresholds at 0.5 + 1 + 2 + 3 + 4 kHz greater than or equal to 20 dB HL used as an objective criterion for hearing impairment and hearing disability appeared to be the more sensitive and reliable of the various criteria used. Based on the definition of hearing handicap, it is suggested that more sophisticated testing than pure-tone auditory thresholds should be taken into account when approving or disapproving hearing disability as a hearing handicap.

[Acoustic variation after surgery for severe Bell's palsy].

Auditory thresholds were studied before and after surgical decompression of the facial nerve in 35 patients following either transmastoidal (20 cases) or transmastoidal and suprapetrosal (15 cases) operations. Seventeen of the patients were under 40 years of age at the time of surgery, which did not affect hearing in the opposite ear. However, auditory threshold levels fell in the operated ear. These were of the bone conduction type, and possessed characteristics of an acute hearing lesion (maximal in the 4000 frequency range and improving progressively) of 0 to 35 dB (mean 5 dB). Possible causes of the hearing loss are discussed.

The effects of hearing loss on the contribution of high- and low-frequency speech information to speech understanding.

The speech understanding of persons with "flat" hearing loss (HI) was compared to a normal-hearing (NH) control group to examine how hearing loss affects the contribution of speech information in various frequency regions. Speech understanding in noise was assessed at multiple low- and high-pass filter cutoff frequencies. Noise levels were chosen to ensure that the noise, rather than quiet thresholds, determined audibility. The performance of HI subjects was compared to a NH group listening at the same signal-to-noise ratio and a comparable presentation level. Although absolute speech scores for the HI group were reduced, performance improvements as the speech and noise bandwidth increased were comparable between groups. These data suggest that the presence of hearing loss results in a uniform, rather than frequency-specific, deficit in the contribution of speech information. Measures of auditory thresholds in noise and speech intelligibility index (SII) calculations were also performed. These data suggest that differences in performance between the HI and NH groups are due primarily to audibility differences between groups. Measures of auditory thresholds in noise showed the "effective masking spectrum" of the noise was greater for the HI than the NH subjects.

Protection from noise-induced lipid peroxidation and hair cell loss in the cochlea.

In order to delineate mechanisms of noise-induced hearing loss, we assessed noise trauma and its pharmacological modulation in the guinea pig. Auditory threshold shifts (measured by auditory brainstem responses), hair cell loss and lipid peroxidation (8-isoprostane formation) were determined in the absence or presence of agents known to influence the formation or action of reactive oxygen species (ROS): the non-specific N-methyl-D-aspartate (NMDA) receptor antagonist (+)-MK-801, its inactive isomer (-)-MK-801, the selective NR1/2B NMDA receptor antagonist PD 174494, the nitric oxide synthase (NOS) inhibitor L-N(omega)-Nitroarginine methyl ester (L-NAME) and the anti-oxidant N-acetylcysteine (NAC). (+)-MK-801 and NAC attenuated threshold shifts and hair cell loss effectively while PD 174494 did so partially. L-NAME attenuated threshold shifts at 2 kHz but increased them at 20 kHz, and (-)-MK-801 was ineffective. Noise-induced elevation in 8-isoprostane in the cochlea was significantly attenuated by (+)-MK-801 and PD 174494 in the organ of Corti and modiolar core, by L-NAME in the lateral wall and modiolar core, and by NAC in all three regions. (-)-MK-801 did not influence noise-induced 8-isoprostane formation. There was a significant correlation between threshold shifts at 4 kHz, hair cell loss and the level of 8-isoprostane formed in the organ of Corti, but not in the lateral wall tissues. This finding suggests a causal relationship between ROS formation and functional and morphological damage. NMDA receptors and, to some extent, NOS may be involved in noise-induced ROS formation. The data also indicate that lipid peroxidation in the lateral wall tissues does not influence permanent threshold shifts.

[Effect and its mechanism of gentamicin on melanin in stria vascularis of guinea pig].

OBJECTIVE: To investigate the effect and its mechanism of gentamicin(GM) on melanin in stria vascularis of guinea pig. METHODS: The differences of auditory thresholds between pigmented and albino guinea pigs, given GM of 150 mg/kg for 7 days, were studied. Moreover, the content of melanosomes, activity of tyrosinase and expression of proliferating cell nuclear antigen(PCNA) in intermediate cells of stria vascularis in gentamicin-treated pigmented guinea pigs were compared with those in control animals by electron microscope and immunohistochemistry, respectively. RESULTS: After gentamicin exposure, the auditory thresholds of all animals increased significantly (P < 0.001), whereas threshold shifts averaged across all frequencies of pigmented animals were much less than those of the albinos(P < 0.001). The number of melanosomes of each examined area (300 microns 2) in intermediate cells was obviously increased from 19.83 +/- 2.74 to 58.33 +/- 16.22. The ratio of tyrosinase reaction products area to the total measured area was significantly increased from 1.65% +/- 0.40% to 3.45% +/- 0.41% after gentamicin exposure. However, the numbers of positive intermediate cells expressing PCNA were 14.08 +/- 2.76 and 13.58 +/- 2.09 before and after gentamicin treatment, respectively. But there was no statistically significant difference between them (P > 0.05). CONCLUSION: The increase of content of melanin in stria vascularis after GM exposure does not result from the change of proliferating activity of melanocytes, but from the enhanced tyrosinase activity. Melanins in stria vascularis may possess the ability to protect the inner ear from ototoxicity of gentamicin.
