Calcium phosphate crystal-associated proteins: alpha2-HS-glycoprotein, prothrombin F1, and osteopontin.

PURPOSE: To elucidate the process of urinary stone formation, many urinary constituents have been examined as inhibitors or promoters. MATERIALS AND METHODS: We analyzed specific binding proteins of calcium phosphate (CaP) crystals generated in human urine and found that the separated proteins were alpha2-HS-glycoprotein, prothrombin fragment 1 (PTF1), and osteopontin (OPN). We also studied their inhibitory effects on calcium oxalate crystallization and the quantitative differences in urinary excretion of the proteins in healthy individuals and stone formers. RESULTS: Both PTF1 and OPN had strong inhibitory effects on growth of calcium oxalate crystals. No differences in alpha2-HS-glycoprotein were found between healthy subjects and stone formers. Urinary concentrations of PTF1 and OPN were lower in stone formers than in healthy controls. No differences in these proteins were seen between single and recurrent stone formers. CONCLUSION: Low urinary concentrations of PTF1 and OPN might be one reason for stone formation.

Mineral trioxide aggregate vs. formocresol in pulpotomized primary molars: a preliminary report.

PURPOSE: The aim of this study was to compare the effect of mineral trioxide aggregate (MTA) to that of formocresol (FC) as pulp dressing agents in pulpotomized primary molars with carious pulp exposure. METHODS: Forty-five primary molars of 26 children were treated by a conventional pulpotomy technique. The teeth were randomly assigned to the MTA (experimental) or FC (control) group by a toss of a coin. Following removal of the coronal pulp and hemostasis the pulp stumps were covered with an MTA paste in the experimental group. In the control group, FC was placed with a cotton pellet over the pulp stumps for 5 minutes and removed; the pulp stumps were then covered by zinc oxide-eugenol (ZOE) paste. The teeth of both groups were restored with stainless steel crowns. Eighteen children with 32 teeth arrived for clinical and radiographic follow-up evaluation ranging from 6 to 30 months. RESULTS: The follow-up evaluations revealed only one failure (internal resorption detected at a 17 months postoperative evaluation) in a molar treated with formocresol. None of the MTA-treated teeth showed any clinical or radiographic pathology. Pulp canal obliteration was observed in 9 of 32 (28%) evaluated molars. This finding was detected in 2 out of the 15 teeth treated with FC (13%) and in 7 out of the 17 treated with MTA (41%). CONCLUSION: MTA showed clinical and radiographic success as a dressing material following pulpotomy in primary teeth and seems to be a suitable replacement for formocresol in primary teeth.

Treatment of teeth with open apices using mineral trioxide aggregate.

Injuries to the dentition may ultimately result in the interruption of root development. If the pulp remains vital following trauma, this state should be maintained and root-end closure should be induced by apexogenesis. When the pulp is necrotic, the placement of an apical barrier can be utilized as an alternative to establish an environment that facilitates the closure of the apical opening. This article demonstrates the use of mineral trioxide aggregate as an apical barrier material for root-end closure in the permanent teeth of three patients.

Evaluation of crystallization processes in the calculi of the submandibular salivary glands.

Four patients were subjected to sialolithotomy of the submandibular gland in the Department of Otolaryngology, Collegium Medicum, Jagiellonian University in Cracow. The resected calculi were examined under a scanning electron microscope and chemical analysis of their structure was carried out. Results of these studies were compared with clinical observations. It has been found that sialoliths are built up by inorganic salts, mainly calcium compounds. Crystallization processes proceed at variable rates and are related to duration of the disease and intensity of inflammatory reaction.

Root perforation repair: surgical and nonsurgical management.

Root perforation repair has historically been an unpredictable treatment modality with an unacceptably high rate of clinical failure. Recent developments in the techniques and materials utilized in root perforation repair have dramatically enhanced the prognosis of both surgical and nonsurgical procedures. This article presents a review of the literature pertaining to root perforation repair and illustrates, through clinical case presentations, the principles of extraradicular surgical repair and non-surgical internal repair of root perforation using mineral trioxide aggregate (MTA).

Avoiding "the space" by the treatment of compromised teeth.

The advent of implant therapy has given new a dimension to dentistry particularly where teeth have been irretrievably damaged or pathologically involved. There are many situations however, where endodontic or combined therapy may effectively retain compromised teeth and clinicians should carefully evaluate evidence-based treatment. The main areas discussed are: transverse and crown root fractures and invasive cervical resorption. Transverse root fractures have an excellent natural healing capacity and will also respond favourably to treatment if pulp necrosis occurs in the coronal segment. Studies have shown that crown root fractures can be effectively treated by combined endodontic-orthodontic-periodontic and prosthodontic therapy. Invasive cervical resorption has been classified into four classes depending on the degree of infiltration of resorptive tissue into the tooth structure. A study of treatment results following the topical application of 90% aqueous trichloracetic acid, curettage and restoration indicated that this regimen could be successfully applied to Classes 1, 2 and 3 resorption.

Phosphorus retention capacity of iron-ore and blast furnace slag in subsurface flow constructed wetlands.

The suitability of iron-ore and blast furnace slag for subsurface flow (SSF) constructed wetlands was studied over a period of four months. Dairy farm wastewater (TP 45 mg l(-1)) was percolated through buckets planted with reed (volume 9.1 l; hydraulic load 151 m(-2) d(-1)). One group of buckets was kept under aerobic conditions and the other group under anaerobic conditions, monitored by continuous redox potential measurements. Even at high mass loading rates of 0.65 g P m(-1) d(-1) the slag provided 98% removal efficiency and showed no decrease in performance with time. However, phosphorus fractionation data indicate that the high phosphorus retention capacity under aerobic conditions is to a great extent attributable to unstable sorption onto calcium compounds (NH4Cl-P). Phosphorus sorption of both the slag (200 microg P g(-1)) and the iron-ore (140 microg P g(-1)) was promoted by predominantly anaerobic conditions due to continuous formation of amorphous ferrous hydroxides. None of the substrates had adverse affects on reed growth.

Management of traumatically injured pulps in immature teeth using MTA.

Maintaining pulpal health during tooth development is an important goal in managing traumatic injuries in the immature dentition. The introduction of mineral trioxide aggregate has offered an improved method for pulp protection in such cases. This article describes a technique using MTA for vital pulp therapy on teeth with crown fractures that have resulted in pulp exposure.

[The role of additives in bio-mass coal briquette on sulfur retention enhancement].

The research first conducted the sulfur-fixing experiment of bio-mass coal briquette in a tubular furnace. The impacts of three additives Al2O3, Fe2O3 and MnO2 on the sulfur retention by calcium-based sorbent in briquette were investigated, and only Al2O3 displayed the enhancement of sulfur retention. The TGA experiment was further carried out, and proved that the high-temperature decomposition of CaSO4 in the deoxidization atmosphere was effectively inhibited with the addition of Al2O3. The XPS and XRD analyses of briquette ash showed that due to the interaction among Al2O3, CaSO4 and CaO, the composite CaSO4.3CaO.3Al2O3 which has more thermal stability was formed. With its wrapping or binding onto the surface of CaSO4 crystal, the decomposition of CaSO4 was mitigated.

[Biochemical and pharmacological check-up in emergency orientation].

Biochemical and pharmacological tests usually prescribed in casualty department were reviewed taking into account their physiological significance and predictive value : ions, total proteins, carbohydrate and nitrogenous metabolites, enzymes, tissue markers, pharmacological drugs. Few blood components were kept with the first intention, ideally with a turn around time below one hour: sodium, potassium, chloride, bicarbonate, total proteins, pCO2 and pO2, creatinine, glucose, ketone compounds, calcium, bilirubin, transaminases, lipase, C-reactive protein, myoglobin, troponin, chorionic gonadotropin hormone. Those tests do not have to be systematically performed but prescribed only after the evaluation of pre-test probabilities by the clinician.

The use of mineral trioxide aggregate to create an apical stop in previously traumatized adult tooth with blunderbuss canal. Case report.

The case presented here demonstrates an alternative to conventional treatments in managing a non-vital blunderbuss canal. The use of MTA (mineral trioxide aggregate) to establish an apical stop allows for expedient completion of root canal therapy with predictable results. Following root canal therapy, non-vital bleaching techniques may be employed, offering a means of restoring the tooth to its natural shade without the need for additional tooth preparation. Surgery is avoided, and the technique can easily be followed by the general practitioner.

[Vascular and tissue calcifications of hemodialysis patients].

The present paper aims to assess radiographic vascular and soft parts calcifications occurrence and its correlation with biochemical profiles. The study was performed in 47 patients (ten diabetic patients), 49 years old, who had been on dialysis for a period of 51 months. Vascular calcifications (VCs) were classified as proximal, distal and soft tissues. In addition, Ca, P, CaxP values in the six months prior to the recruitment period, PTH, FAL and calcium carbonate, calcium acetate, and vitamin D3 intake were determined. A higher frequency of VCs was observed in diabetics, yielding a significant association with proximal 60% (p = 0.05) and almost significant with distal calcifications 70% (p = 0.07). Likewise, a lower CaxP was noted for diabetic VCs in comparison to that seen in non-diabetic VCs (p < 0.05). Proximal and distal VCs in the non-diabetics population were 25% and 20%, respectively; and tissue calcifications were 24%. Age was correlated with proximal and distal VCs (p < 0.01). A higher CaxP was observed in patients with VCs and it yielded an even higher value for tissue calcifications. Lastly, calcium acetate and overall calcium intake was higher in patients with tissue calcifications (p = 0.05). VCs were more frequent in diabetics and they also showed a lower CaxP. VCs in non-diabetics were correlated with CaxP values, whereas tissue calcifications were associated with calcium intake. Therefore, the management of renal osteodystrophy should be changed in order to prevent calcifications as well as to decrease morbidity in hemodialysis patients.

Phosphate recovery from sewage sludge in combination with supercritical water oxidation.

Supercritical Water Oxidation (SCWO) is an innovative and effective destruction method for organics in sewage sludge. The SCWO process leaves a slurry of inorganic ash in a pure water phase free from organic contaminants, which opens possibilities for a simple process to recover components like phosphates from the sewage sludge. In a continuous pilot plant for the SCWO process digested sludge has been treated. The ash has been extracted in lab scale with both caustic and acids in order to recover phosphates. By leaching the ash with caustic, 90% of the phosphorus could be separated as a sodium phosphate solution. By treating the sodium phosphate solution with lime, calcium phosphate was precipitated and caustic recovered and circulated back to the leaching process.
