Possibilities of preperitoneal approach methods in the treatment of groin hernias. Preperitoneal approach methods.

A series of 93 patients operated on for groin hernias by applying the preperitoneal approach is reported (89 bilateral and four unilateral). The patients were divided into two groups. Group one included 24 patients without prosthetic repair. Group two included 65 patients who had undergone repair with Ampoxen mesh. Some untraditional methods are reported, such as interparietal repair, rotative transposition of external oblique muscle aponeurosis, etc. As far recurrences are concerned, no differences were recorded between the two groups (Pt greater than 0.05). Of all 193 groin hernias 81% were followed up and the total percentage of recurrences was 4.5%. Slight testicular and neuralgic postoperative complications were reported (three and two patients respectively). The advantages of biomaterials were confirmed by ultrasound examinations of ten patients from both groups. A conclusion is made that hernia repair strategy should be based on natural groin tissue resources.

[Treatment of varicose veins].

The authors analyze the results of treatment of 1500 patients suffering from varicose conditions. They have developed original methods for the treatment of varicose dilatation of subcutaneous veins: endovascular and ++perivascular electrocoagulation of subcutaneous veins, removal and crossing of subcutaneous and communicant ++ veins with vein extractor and capron fish line; liquidation of venous conglomerates by means of a submerged removable suture; ++supra-fascial crossing of subcutaneous veins. Employment of these methods in combination with the traditional treatment modalities yields a good therapeutic and cosmetic effect. The methods are simple and available, this recommending them for any surgical hospital.

[The results of the formation of a new anal sphincter by using the mm. graciles in acquired anal incontinence].

The operative method of Pickrell for shaping a new anal sphincter from mm.graciles is described, emphasizing the refinements which the authors have introduced in it. In mobilizing the muscle, fascial fibers are left on both sides of the vascular-nervous bundle. They prevent the latter from overdistension when the muscle turns around the anal canal. Two arcuate lateral sections are made in the perianal area; this is followed by driving a subcutaneous tunnel, but under the perineal raphe and the anococcygeal ligaments. The two muscles are perianally wound each on 360 degrees, whereby m.gracilis dex. is fixed to tub. ossis ischii sin. and m.gracilis sin. is fixed to tub. ossis ischii dex. Before being fixed, the tendon end is inserted in the subperiosteal bone tunnel. When the tendon of the muscle is shorter, it may successfully be elongated by the explantoplast Ampoxen (applied in 3 patients). A total of 16 patients were treated. The result was good in 15 (93.8 per cent) and unsatisfactory in 1 (6.2 per cent).

[Internal ligatures of the pyloro-duodenal region after surgical treatment of bleeding ulcers].

1886 fibrogastroduodenoscopic examinations were performed in 820 patients within 7 years after operative treatment of bleeding pyloroduodenal ulcers. Catgut was used in 589 patients for internal sutures in pyloroplasty, the unresolving suture material--capron--was used in 221 patients. Clinico-endoscopic investigations have shown that the application of catgut is followed in the early postoperative period by the development of inflammatory reactions in the tissues along the suture line, while the use of the unresolving suture material is followed by inflammations of less degree, the rejection of internal ligatures was slow with episodes of bleedings, ulcerations, formation of granulomas. The article gives grounds for expedience of removal of internal ligatures during the endoscopic examination of organs in the zone of the operative intervention.

[The use of a new plastic method for consolidating a preternatural anus as prevention in paracolostomy hernias].

Once the stage of extraperitoneal evasion of the sigmoid has been achieved for definitive preternatural anus, transsection is made of the aponeurosis of m. obliqu. ext. abdominis along its tendon fibers at a length of 5-6 cm. Then an aperture is made along the muscle fibers with the same length on m. obliqu. int. abdominis and m. transversus abdominis as well and finally f. transversalis is transsected. The peritoneum is intra-abdominally detached from the fascia beneath and alongside the aperture. An "Ampoxen" layer is placed around the internal aperture of the abdominal wall. The explant is sutured to the fascia and the evaded sigmoid. The latter is sutured to the aponeurosis of m. obliqu. ext. abdominis. Another Ampoxen layer is placed over this aponeurosis and sutured to the evaded intestine and in a chess-board way to the fascia. The method was applied in 22 patients with very good postoperative result.

[Use of Nichrome suture material].

The suture material and the method of its application play an important role in the prevention of wound complications. With the use of Nichrome wire thread as the suture material in surgical interventions, the number of postoperative complications significantly was decreased. The Nichrome thread is also used expediently in operative interventions on patients with purulent diseases of the soft tissues, including cases after primary surgical treatment of purulent foci by carbon dioxide laser.

[A stabilizing plastic repair method for reconstruction in extensive complete defects of the chest wall].

Experience is recorded with the application of a new operative method for combined plastic-stabilizing reconstruction of the thoracic wall in complete defects larger than 8-10/10 cm (greater than 80 cm2). The method consists of covering in layers the defect with Ampoxen, auto-rib and skin-muscle flap. Nine patients were operated: 2 with recurrent tumors of the mammary gland, 2 with metastatic thoracic wall tumors, 2 with osteoradionecrosis. 2 with osteomyelitis of 3-4 ribs and 1 with wide communicating bronchopleurocutaneous fistulas. The advantages of the method are pointed out: permanent stabilization of the thoracic wall, provision of optimal biological terrain for postoperative radiotherapy, applicability in purulent-necrotic defects. The method is described in detail, as well as the results in the operated patients for a period from 1 to 4 years.

[Main results of experimental studies on the toxicology of inhibitors of atmospheric corrosion of metals].

Basing on experimental toxicity research it was established that, out of 50 atmosphere metal corrosion inhibitors, some 14 per cent were found extremely hazardous, 42 per cent--of high level hazardous, 33 percent--of moderate and 11 per cent--of low hazardous. Relationships were identified between the structure of polymethylene amine salts, azole compounds and carbonic acid, and the way they influence human organism. It was also found that inhibitors exercise a polytropic action in man, the toxicity action being concentrated on oxidation processes, and the inhibitors specifically influence protein, carbohydrate and phosphoric metabolisms, as well as the red blood system. This causes functional and structural disorders of CNS and in the parenchymal organs. Inhibitors are characterized by local and skin-resorption actions, their degree and specific features depending on their chemical structures. 22 MACs of working zone inhibitors are proposed, along with early diagnostic tests and preventive measures.

[A new plasmid pBS1001 with broad range of hosts].

A new broad host-range plasmid capable of conjugative transfer has been isolated and characterized. The plasmid has the high frequency of conjugation transfer, is capable of conjugative transfer mobilization of nonconjugative plasmids, carries no known phenotypic markers. The plasmid demonstrates the specific interaction with the plasmids of P incompatibility group. The comparatively small size of the plasmid permits one to use it efficiently for comparative study of organization of the broad host range plasmids.

[A new method for the surgical treatment of urinary incontinence following surgery of the prostate].

A new method is reported for operative treatment of urine incontinence, following transversal adenomectomy and transurethral resection of the prostate gland. The operative technique is described in detail. Its basic point is placing of a small "Ampoxen" ball over the urethral bulbus. Disappearance of the incontinence after such an operation is due both to the compression on the bulbar part of the urethra and to the formation of connective tissue, which proliferates into the sheet being placed. Twelve patients from 62 to 75 years of age were treated in this way. In 10 of them the immediate and the late results are considered very good. Incontinence disappeared until the 3. day after removing the indwelling catheter. In the other two patients the result is considered satisfactory. The encouraging results make the authors prefer the method which they suggest for postoperative urine incontinence.
