The development of TRIC organisms in cell cultures during multiple infection.

The relationship between multiplicity of infection and the yield of organisms and of polysaccharide was studied in BHK 21 cells infected with TRIC organisms. Although an increase in the multiplicity of infection resulted in an increase in the absolute number of organisms per culture, there was a decrease in relation to the number of infecting organisms. The amount of polysaccharide produced was independent of multiplicity of infection; it was not limited by the concentration of glucose in the medium. Polysaccharide leaked from cells and was slowly broken down in the culture medium.

Neutralization of TRIC organisms by antibody: enhancement by antisera prepared against immunoglobulins.

The neutralizing activities of fowl, rabbit and mouse antisera prepared against TRIC agents were enhanced, in some cases over a 100-fold, by the addition of antiserum against the appropriate IgG. Significant non-specific inactivation of the organisms was observed in the absence of antiserum against TRIC agents, in reaction mixtures containing normal serum and antiserum against IgG. Since the extent of this inactivation showed a prozone, control tests with dilutions of normal serum equivalent to the full range of dilutions of the antiserum tested are necessary to show that enhancement of neutralization is specific.

Interactions of TRIC agents with macrophages: effects on lysosomal enzymes of the cell.

TWO CHANGES WERE OBSERVED IN THE ACID PHOSPHATASE OF MACROPHAGES THAT HAD INGESTED INFECTIVE TRIC ORGANISMS: the proportion of extralysosomal enzyme rose, while the total amount in the cells fell. Both effects were directly related to the number of organisms ingested and increased with time. When macrophages were inoculated with more than 50 organisms per cell, changes were obvious within a few hours; with 2-10 organisms per cell changes were detectable only after 18 hr. or more. Enzyme appeared in the culture medium as the amount in the cells decreased. Ingestion of organisms killed by heat or treated with antibody did not induce such changes. In infected BHK-21 cells, no changes in acid phosphatase were detected at any stage of the developmental cycle of the organism.

Serotypes of trachoma agent isolated in the Gambia: with an observation on the relation between serotype and morphology.

Of 60 TRIC agents isolated from Gambian children with trachoma, 25 were serotype 1 and the remainder type 2. There was a pronounced difference in the proportions of these types in the two villages studied. In the village with a predominance of type 2 strains, TRIC agents remained confined to 2 adjacent compounds over a 14 month observation period. All 19 type 1 strains examined were characterized by the appearance in yolk sac smears of compact aggregates of elementary bodies; such aggregates were seen in only 2 of 35 type 2 strains, and may reflect a chemical difference in the surface of the elementary bodies or in a substance elaborated during their replication.

Interactions of TRIC agents with macrophages and BHK-21 cells observed by electron microscopy.

TRIC agents do not multiply in mouse peritoneal macrophages in culture but have a toxic effect on them, whereas they multiply readily in BHK-21 cells. Sections of macrophages and of BHK-21 cells fixed during the first 90 min after inoculation were examined by electron microscopy. Macrophages ingested all forms of the organism, which were eventually degraded in lysosomes. However, elementary bodies were distinguished from other TRIC particles by the delay in their transfer to lysosomes. BHK-21 cells ingested elementary bodies selectively, and in these cells the organisms were neither found in lysosomes nor degraded. Instead they showed morphological changes that probably represented an early stage of development.

The presentation and incidence of paratrachoma in adults.

Thirteen definite and 3 probable cases of chlamydial eye infection were diagnosed in young adults attending the Bristol Eye Hospital between June 1978 and May 1980, and incidence of about 1 case per 44000 per year in the 15 to 44-year-old community served by this hospital, and 1 per 100000 in the total population of this community. These patients presented with a sub-acute follicular conjunctivitis or kerato-conjunctivitis, which had usually been present for several weeks and had often failed to respond to topical chloramphenicol treatment before presentation. Sera obtained from 14 patients, all had chlamydial antibody titres of 64 or more. Over the same period of time, an estimated 2500 patients per year from the same community attended the Venereology Department at the Bristol Royal Infirmary with genital chlamydial infections. These figures suggest that chlamydial infection of the eye complicates no more than 1 in 300 chlamydial infections of the genital tract in adults.

Long-term follow-up of lid surgery for trichiasis in the Gambia: surgical success and patient perceptions.

BACKGROUND: Surgery is an important component of the SAFE strategy which has been shown to improve visual acuity. There are limited data on long-term surgical success and patient perceptions and satisfaction. A sample of patients from the Gambia who had undergone previous lid surgery for trichiasis were therefore examined for recurrence of trichiasis and interviewed for patient satisfaction. METHODS: Health centre surgical records and community screening were used to identify patients who had undergone previous lid surgery. Consenting patients were examined and a questionnaire administered. Kaplan-Meier survival plots were constructed for recurrence of trichiasis. RESULTS: Sixty-five subjects were recruited. Median age at surgery was 50 years. Median time since surgery was 7 years. Fifty-two of 115 (45%) operated eyes were free of trichiasis at follow-up and 23 of 65 (35%) patients had not suffered recurrent trichiasis in an operated eye. Median time from surgery to recurrence of trichiasis was estimated as 10.0 years (95% CI 3.7-16.3). Recurrent trichiasis was not significantly associated with visual impairment or blindness at follow-up. The following patient perceptions were reported: satisfaction with surgery (88%), less discomfort than before surgery (93%), improved vision (83%), work easier (38%), worth the expenditure (94%), would recommend it to others (93%), had recommended it to others (38%), experienced intra-operative pain (26%) and experienced post-operative pain (26%). CONCLUSION: Factors affecting surgical success, including surgical technique and re-exposure to infection, are discussed. This study provides important preliminary data for programme planners but larger prospective studies are required.

[Diagnosis of ocular Chlamydia infections. Experiences with 2 test systems].

It is well known that both direct and indirect test systems are available for identifying chlamydial infections. In the author's laboratory two of these systems were compared by examining conjunctival scrapings, sera, and tears from more than 150 patients with chronic, therapy-resistant conjunctivitis. The principal criteria considered were the time and technical effort involved and the sensitivity and specificity of these tests in ophthalmology. According to the authors' findings, direct identification of the pathogens with FITC-labeled monoclonal antibodies is useful for diagnosing fresh infections. In cases of chronic disease, however, the indirect serologic test is of greater value, because it is usually impossible to isolate the viruses. This test shows specific IgA and IgG antibodies in serum and tears that do not occur before the tenth (IgA) or 30th (IgG) day after infection. The sensitivity and specificity of both tests seem to be on a par with the culture system, which is still the "gold standard" for diagnosis. For a secure diagnosis both test systems should be used, though of course always taking the clinical picture into consideration.

[Several years' course of an oculogenital Chlamydia infection].

A 46-year-old woman had been suffering from recurrent bilateral keratoconjunctivitis, vascularized corneal scars, and incipient pannus vasculosus for five years. Oculogenital chlamydial disease was diagnosed by Giemsa stains, McCoy cell culture, and serum IgG immunofluorescent techniques. Isolates from cervical smears were positive for C. trachomatis in cell cultures. Systemic therapy with doxycycline 200 mg per day for 14 days resulted in resolution of the acute symptoms and no further recurrences. The patient's partner was also treated with tetracyclines. Because of reduction of vision, a lamellar corneal grafting was performed.

[Diagnosis of ocular Chlamydia infections by direct detection].

Conjunctival smears of 75 patients with a presumptive diagnosis of oculogenital chlamydial infection were evaluated with the monoclonal antibody direct test (SyvaMerck and Orion Research). 69 isolates were correlated with McCoy cell cultures and serum IgG immunofluorescent tests. Sensitivity (100%) and specificity (56-76%) of the monoclonal antibody direct test was comparable to data from recently published studies on genital smears.

[Phlyctenular keratoconjunctivitis in bacterial epibulbar infections].

Phlyctenular keratoconjunctivitis is now a rare clinical entity. Histologic staging has been updated by infection-immunological findings: phlyctenules have been identified as delayed-type cellmediated immune responses involving bacterial, mycobacterial, chlamydial, fungal or parasitic pathogens. Establishment of a correct diagnosis should include conjunctival and systemic work-ups for pathogenic agents. Case reports of three patients are presented.

[Ophthalmia neonatorum].

Silver nitrate prophylaxis against ophthalmia neonatorum due to Neissera Gonorrhoeae was introduced by Crédé in 1884, when he instilled a drop of 2% silver nitrate into the cul-de-sac of a newborn. Today, the prime cause of ophthalmia neonatorum is infection by Chlamydia trachomatis. The high incidence of this type of conjunctivitis is due to the frequent exposure of the newborn during delivery and to the ineffectiveness of silver nitrate as a prophylaxis against Chlamydia. In the study reported here, 198 infants less than one month old were examined because of severe bilateral conjunctivitis. In the first week of life a chemical irritation caused by Crédé's prophylaxis was found in the majority (59.7%), but in the second and third weeks infections with Chlamydia trachomatis were found in 27% of all cases. Bacteria were responsible for the infection in 52% of the cases. No gonococci were found in any of the patients examined in the course of the study.

[Clinical aspects, diagnosis and therapy of ocular Chlamydia infection].

As reported in the recent literature, eye infections caused by Chlamydia trachomatis are increasing in number. Modern laboratory techniques were used in this study to scan a total of 114 patients with unilateral follicular conjunctivitis for these organisms. The authors describe the clinical picture, course, and results of therapy in a total of 6 (5.3%) young patients with chlamydial infections diagnosed by scraping and cell culture.
