Metabolic control through L calcium channel, PKC and opioid receptors modulation by an association of naloxone and calcium salts.

beta-endorphins (beta-ends) are released from the anterior pituitary and from lymphocytes directly into inflamed tissue in response to stress and pain. At the site of inflammation and trauma, the link of beta- ends to opioid receptors hyperpolarizes nerve terminal, by blocking L-calcium gated channels, induces modifications of receptor stereoisomerism and alters the bond-energy. Opioids increase potassium and decrease calcium and sodium currents through interactions with G-protein. In some pathologies, it has been found a loss of desensitization and down regulation of opioid receptors by means of Ca++ blocking that, in turn, inhibits PKC-activation. The physiopathological mechanism dependent on the high concentration of linked opioids affects cellular level of Ca++, ATP and NADH. This biochemical reaction exerts deep influence on energetic cell status and metabolism. In gram negative bacteria, expression of mu-receptors on cell surface has been observed, with a possibility to interfere with host cell metabolism. There are many human and veterinary pathologies in which the reported mechanisms are well known: polycystic ovary syndrome, gross cystic breast disease, milk fever, ruminal tympanites, pyometra, equine colic syndrome, ovarian follicular cyst in dairy cows, calcium deficit in post-partum cows, uterine involution in cows. Also incoming pathologies such as Electro-Magnetic-Field exposure may induce alteration of calcium channel activity through the same mechanism. On clinical bases, it has been pointed out that the therapeutic administration of an association of calcium salts and naloxone controls calcium turnover, pain and functional activity of endocrine glands, via down regulation/desensitization of opioid receptors, PKC stimulation and energy restoration.

Serum amylase values in callitrichids.

We determined the amylase levels in serum samples from six callitrichid species. The normal serum amylase values for all of these species was within or higher than the normal human range. Amylase values higher than the normal range occurred not only in association with pancreatitis but also pyometra, bone fracture, abscesses, diabetes mellitus and gastrointestinal conditions leading to diarrhea. We concluded that although serum amylase activity may be helpful in diagnosing pancreatitis, it is, as in humans, not specific for this condition in callitrichids.

The clinical implication of sodium-potassium ratios in dogs.

Although there have been substantial evidences on the usefulness of electrolytes for the diagnosis of disease, the evidences for a direct link between serum sodium and serum potassium in relation to a specific disease are very limited. This study was performed to investigate an association between diseases and Na:K ratios in dogs. From January 1997 to December 1999, a total of 39 cases with an Na:K ratio less than 27 were retrieved from the medical records of Veterinary Medical Teaching Hospital, Seoul National University. Ten dogs (25.6%) had a renal or urinary disease, and six (15.4%) had a parasitism. Other miscellaneous diseases included deep pyoderma, grade III patellar luxation, bacterial pneumonia, diabetes, pancreatitis, and pyometra. The Na:K ratio was significantly lower in dogs with renal failures than those with parasitic diseases (p=0.0735). With the criterion of the Na:K ratio < 27, twenty seven dogs (69.2%) had hyperkalemia, whereas thirteen dogs (33.3%) had hyponatremia. Of 13 dogs with Na:K ratios between 20 and 24, six were diagnosed as a renal or urinary tract disease, two as diabetes, and two as a parasitism. The Na:K ratios of 9 dogs were < 20, being with the most prevalent with the disease of renal failures (55.6%). The serum Na:K ratios were more closely related to serum potassium concentrations ( gamma= -0.8710) than serum sodium concentrations ( gamma=0.4703). Two dogs with diabetes had an electrolyte pattern of hyperkalemia with normonatremia. Further studies are needed to determine the usefulness of Na:K ratio for diagnosis of hypoadrenocorticism, and to establish a relationship between patellar luxation and electrolyte unbalance.

Zearalenone applied per os provides adverse effects in structure of chosen parts of bitch reproductive system.

Both earlier investigations and complete formulas of pet food for dogs show that professionally prepared pet food is based in 30% on the material of the plant origin. Our own investigations prove that animal feeding stuffs of the plant origin are very often vectors of many undesirable substances (including micotoxins) that are unfavourable for carnivores. Zearalenone is a micotoxin, which frequently occurs among them. It was revealed in pet food in quantities from 5.0-299.5 microg/kg. It is a very dangerous xenobiotic because of its quick and easy absorption in the organism after per os application in monogastric animals. It is also a sterid and despite its plant origin it binds estrogen receptors in the reproductive tract and causes its hypo- and dysfunction. It is clearly visible in young, growing organisms and those during the menopause. In bitches, dysfunctions of the reproduction system, such as: endometritis-pyometra syndrome, prolonged oestrus, ovarian cysts and others are often found. It is supposed that specific for this species hormonal regulation of the reproduction processes based on the long progesterone and prolactin cycles, and on high sensibility to estrogens plays an important role in the etiopathogenesis of these dysfunctions. The application of hormones in bitches in therapeutic and biotechnical (contraception) purposes are the other factors, which should be taken into account in this aspect. There are some suggestions that therapeutic management, unfortunately connected with mistakes in the medical art, is often a cause of pathological condition in a reproductive organ in bitches. However, another reason of these disorders may be that bitches take zearalenone with the commercial pet food. The long-lasting intake of the feed with a low dose of this micotoxin may be the factor, which complicates hormonal regulation of the reproduction processes and is the cause of many disorders. Our own pilotage investigations showed that application of zearalenone at a dose of 200 microg/kg b.w. for 7 days leads to apoptotic-like changes in granule cells in the bitch reproductive system, atresia of follicular cells and oedema and hyperplasia of the oviductal and uterine cells. They could also indicate a follicular phase of the oestrus cycle in bitch. However, it did not take place.

Dietary origin of mycotoxins with estrogenic potential and possible health implications to female dogs.

In Poland, occurrence of toxigenic fungi in cereals, foods, feeds and their components as well as mycotoxins accumulation in such material has been studied by numerous teams including our research group for over thirty years since 1969. Mostly cereal kernels and feeds have been examined for presence of toxigenic fungi, their toxigenic potential and natural contamination with mycotoxins. Ochratoxin A, deoxynivalenol, nivalenol and moniliformin were found to be significant contaminants of agricultural products in high percentage of cereal grain samples. The profile of toxic metabolites was similar but the concentration levels of the toxins were lower when compared to already published data of the same climate zone. Zearalenone (ZEA), a nonsteroidal mycotoxin with estrogen-like activity, is synthesized by molds (Fusarium) commonly contaminating poorly stored agricultural products and foodstuffs. Since in the course of examinations and during surgical procedures performed in dogs, ovarian cysts were detected and because frequently this is the first stage of the endometrica pyometra complex (EPC) found in approximately 30% of the females we assume that both factors, mycotoxins (ZEA) and pathological aberrations are possibly related in these animals. Similar activity of the toxin (possibly present in pelleted dog feed) and effects including infertility of female dogs is speculated with indication and suggestion on necessity of additional studies on the problem.

Endometrial cancer: asymptomatic endometrial findings. Characteristics of postmenopausal endometrial cancer.

Endometrial cancer affects patients at every age, however it occurs more frequently in menopause (> 50) and in postmenopause (> 70). The most frequent symptoms are bleeding and vaginal discharge. When hematometra or pyometra is present the patient may feel pain. In some cases endometrial adenocarcinoma is asymptomatic and the diagnosis is casually made during ultrasound examination or by histological examination of a uterus surgically removed for other indications. In these cases the most frequent findings are polyps and abnormally increased thickness of the endometrial mucosa. In postmenopause polyps and abnormal endometrial thickness are usually limited to a small area and surrounded by atrophic mucosa. Higher incidence rates of endometrial cancer were correlated with polyps and an increased number of serous type tumors were identified in the > 65-year age group. Endometrial carcinoma may be estrogen correlated or non-estrogen associated. Patients in postmenopause are often affected by non-estrogen correlated endometrial carcinoma. According to Kurman and other authors the first type of endometrial adenocarcinoma (estrogen correlated) is characterized by low-grade malignancy. On the contrary, non-estrogen correlated neoplasia is more aggressive. In our case series including 102 women aged > 70 years with endometrial carcinoma we found that survival was correlated with stage and grading - early stages were the most frequent and the grade increased with stage. In fact all the patients with relapses had grade 2 or 3 adenocarcinomas. Thirty-one patients > 70 years (30.69%) had a non-endometrioid type of cancer.

Gynecologic abscess: CT-guided percutaneous drainage.

A 42-year-old woman with recurrent bilateral endometrial ovarian cystoma presented with fever and pelvic pain caused by a tubo-ovarian abscess (TOA), which was resistant to several varieties of intravenous and oral antibiotics for 2 weeks (Case 1). Computed tomography (CT)-guided diagnostic aspiration for a rapid enlarged right ovarian cystoma through a transabdominal route confirmed that it had developed into a TOA. Subsequent percutaneous abscess drainage (PAD) and irrigation for 3 days were successful. One-year follow-up revealed no recurrence of TOA. A 58-year-old woman with recurrent cervical cancer after external radiation therapy (RT) presented with fever, confusion and tremor caused by pyometra (Case 2). Since transvaginal drainage was impossible due to cervical os obstruction, the patient had undergone CT-guided transabdominal PAD and irrigation for a month. Thereafter, the clinical findings improved and a tracheloplasty was performed to prevent recurrence. CT-guided PAD may be a useful treatment option for gynecologic abscess as a diagnostic aspiration, a temporizing procedure until surgery, or an alternative surgery.

Clinical and pathologic correlation of endometrial cavity fluid detected by ultrasound in the postmenopausal patient.

A registry of ultrasound procedures spanning nearly 5 years was searched retrospectively to discover cases of endometrial cavity fluid collections in postmenopausal women. Twenty cases were identified; all medical records were available for review. One patient was lost to follow-up. Seventeen patients had surgical procedures: 11 had only a D&C, and six had a primary evaluation of laparotomy with removal of the uterus and adnexa. Five women had cancer (two ovarian, one tubal, one endometrial, and one cervical); eight women had benign gynecologic conditions, including uterine fibroids (five), ovarian serous cystadenoma (two), and cervical dysplasia (one). There were two cases of apparent subclinical pyometra. Five women had endometrial pathology consistent with prescribed hormone therapy for breast cancer (four) or endometrial hyperplasia (one).

Surgical treatment of perforating duodenal ulcer in a dog by use of modified choledochoduodenostomy and gastrojejunostomy.

Modified choledochoduodenostomy and gastrojejunostomy were used successfully to treat extensive duodenal ulceration and perforation in a dog. Concomitant pyometra, simultaneously managed by ovariohysterectomy, may have predisposed the dog to stress ulceration. Choledochoduodenostomy was performed by transplantation of the entire common bile duct and associated major duodenal papilla to a more distal duodenal location. An indwelling choledochostomy tube was used to stent the anastomotic site, externally divert bile flow, and allow postoperative cholangiography. Early postoperative complications included presumed metronidazole-induced neurotoxicosis, choledochostomy tube obstruction, and pancreatitis, all of which resolved within 2 weeks. The dog was clinically normal and medication (ranitidine and sucralfate) was discontinued 35 months later.

[Role of X-ray computed tomography in the evaluation of prognosis of cervical carcinoma].

In 129 patients with cervical cancer, CT findings before treatment were analysed on the basis of the outcome during a 5 year follow-up period (75 survivals, and 54 deaths). 1) The incidence of each CT finding in the two groups was; (1) enlargement of the uterine cervix: 40.0% in survivals, 64.8% in deaths, (2) low density area in the cervix: 18.7%, 37.0%, (3) necrotic cavity: 4.0%, 33.3%, (4) pyometra: 8.0%, 31.5%, (5) irregular and indistinct margin of the cervix: 2.7%, 51.9%, (6) abnormal image of parametrium: 25.3%, 87.0%, (7) extension to the vagina: 2.7%, 37.0%, (8) extension toward the bladder: 8.0%, 57.4%, and (9) enlarged lymph node: 0%, 27.8%. 2) CT scores obtained by multivariate analysis for prognostic discrimination of cervical cancer were (1) + 1, (2) - 1, (3) + 1, (4) - 3, (5) + 25, (6) type B + 29, C + 25, D + 10, (7) + 2, (8) grade I + 12, II + 7, III + 20, and (9) + 18, respectively. The discrimination ratio for the prognosis of cervical cancer was 82.9%. 3) The score obtained by multivariate analysis showed significant increases according with the stage. Even in the same stage, there were significant differences between the scores for survivals and deaths (stage II: p less than 0.01, stage III: p less than 0.05). The survival period in the poor prognosis group was correlated with the score, especially in the cases treated with radiotherapy (r = -0.54, p less than 0.01).

Magnetic resonance imaging (MRI) of abnormal uterine masses.

Sixteen women with clinically diagnosed uterine masses were studied by magnetic resonance imaging (MRI). Pelvic study was carried out in the coronal, sagittal and axial planes. Uterine leiomyomas were detected in 12 cases, while the remaining cases were one each of uterine sarcoma, invasive molar pregnancy, cervical malignancy with pyometra and haematometra with congenital cervical stenosis. The uterine origin of the masses could be clearly detected in all patients, as well as the nature of the masses, the presence of degenerative or malignant changes and the nature of the intrauterine fluid. MRI characteristic findings of the studied masses are presented and discussed.

[Rare pelvic tumors. A case report].

Two cases of rare pelvic masses of obscure origin are presented. The first was a giant pyometra, simulating an ovarian cyst, detected accidentally 13 years after conisation. The second was hematoma of lower parts of rectus muscles, also detected accidentally. The woman had a history of long-lasting anticoagulation, but couldn't remember any trauma. The difficulties of differential diagnosis are discussed.

Effect of megestrol acetate on the endometrium of the prepubertally ovariectomised kitten.

Five prepubertally ovariectomised kittens served as controls. Ten prepubertally ovariectomised kittens were given megestrol acetate (MA) orally at the rate of either 5 or 15 mg twice weekly for 12 to 13 weeks. The uterine horns of treated cats increased in length and diameter. The endometrium became deeply folded with marked hypertrophy of glandular epithelium and production of glycogen by the glandular cells. One cat developed pyometra. Once weekly administration of 2.5 mg MA for a further 12 weeks maintained the hypertrophic changes. The proliferated endometrium regressed considerably but not completely after 12 weeks without MA administration. In two other kittens 2.5 mg MA once weekly for six weeks was sufficient to induce similar changes. MA is a very potent progestational agent in cats, even if they have been ovariectomised prepubertally.

Release of immunoreactive arachidonate metabolites by equine endometrium in vitro.

The ability of equine endometrium to release prostaglandin (PG) F, PGE2, and leukotriene (LT) B4 was studied in vitro, using endometrial tissue from diestrous mares. Because of the high cross-reactivity of the PGF antiserum with PGF1 alpha and with PGF2 alpha, results were quoted as total immunoreactive PGF. Significant concentrations of these arachidonate metabolites were released into tissue culture medium between 1 and 24 hours of incubation. Significantly higher concentrations of PGE, but not of PGE2 or LTB4, were released from endometria of mares with chronic endometritis than from genitally normal mares. Prostaglandin F was released only in low concentrations from the endometrium of a mare with pyometra, but concentrations of PGE2 and LTB4 were similar to those of genitally normal mares.

[Pharmacokinetic and clinical study on cefodizime in obstetrics and gynecological field].

Clinical investigation of cefodizime (CDZM, THR-221), a newly developed cephem antibiotic, was carried out with regard to its distributions to genital organs, and the drug was evaluated clinically against infections in obstetric and gynecological fields. 1. Distributions to genital organs One gram of CDZM was administered to each patient who received simple total hysterectomy by 1 hour intravenous drip infusion and concentrations of CDZM in genital organs such as antecubital vein, uterine artery, ovary, oviduct, endometrium, myometrium, cervix uteri and portio vaginalis were examined. Serum concentrations were elevated to an average of 90.63 micrograms/ml in 15 minutes after administrations of CDZM and decreased gradually at fixed times. CDZM was distributed in concentration ranges of 45.32-10.96 micrograms/g in ovary, 26.58-10.20 micrograms/g in oviduct, 42.20-9.80 micrograms/g in endometrium, 31.28-11.72 micrograms/g in myometrium, 42.20-12.52 micrograms/g in cervix uteri and 45.32-9.40 micrograms/g in portio vaginalis, and these high concentrations lasted more than 3 hours after administrations. 2. Clinical evaluations CDZM was given to 10 patients, including 7 cases with pelvioperitonitis and 1 case each with pyometra, Bartholin's abscess and puerperal fever, at a dose level of 1 or 2 gram 2 times daily by 30-60 minutes intravenous drip infusion. Overall clinical efficacies were excellent in 3 cases and good in 7 and the efficacy rate was very high, at 100%. Bacteriological efficacies were eradicated in 6 cases, and unknown in 2, and the eradication rate was 100%.(ABSTRACT TRUNCATED AT 250 WORDS)
