Community-dwelling seniors with declining self-sufficiency are at risk for premature institutionalization. Early intervention while the senior has capabilities is the solution for guiding the elder to appropriate care along the continuum. This article demonstrates how a gerontological nurse as facilitator of healing helped an elder achieve the desired maximum quality of life.

This article examines transitions between the community and nursing homes among the private pay and the Medicaid eligible older (65+) persons. Discrete-time hazard functions were estimated to determine factors associated with the probability of these transitions. The analysis shows that recent hospitalizations for stroke, dementia, or hip fractures, while strongly predictive of nursing home admissions among the Medicaid elderly, were not significant (except for dementia) predictors for the private pay population. The results are of particular relevance in designing long-term care insurance, and more broadly, long-term care policy.
OBJECTIVE: Women beginning the transition to menopause now have access to complementary and alternative therapies (CAM) that were not universally available in the past. Little is known about the association between CAM use and the use of conventional health care during menopause. We investigated the longitudinal association between use of CAM therapies and utilization of conventional health care in a large, multiethnic sample of midlife women who were enrolled in the cohort phase of the Study of Women's Health Across the Nation (SWAN). DESIGN: We used generalized estimating equations, repeated measures modeling to examine the association between CAM use and the yearly number of conventional health-care contacts, adjusting for relevant covariates, during a two-year period. CAM use was evaluated longitudinally as continuous, incident (no CAM use at baseline), sporadic, or no use during the study period. RESULTS: Relative to nonusers, continuous CAM users had more conventional health-care contacts (beta 0.14, 95% CI 0.06, 0.21). Incident users and sporadic users also showed an excess in conventional health-care contacts (beta 0.14, CI 0.06, 0.22) and (beta 0.09, CI 0.01, 0.16), respectively. Ethnicity was independently associated with health-care contacts. Relative to white women, Japanese women had fewer contacts (beta -0.18, CI -0.31, -0.05). CONCLUSIONS: Despite being a generally healthy, well-educated and high-income population with good access to health care, CAM users were using more of both conventional and alternative health-care resources than women who did not use CAM. These findings have practical implications for conventional health-care practitioners, allied health professionals and CAM practitioners.
School nursing is an interdisciplinary specialty demanding diverse nursing skills. Many of these skills might best be acquired through experience in other settings. The school population presents many nursing challenges, and there may be no supportive circle of specialists and peers readily available to the new school nurse. Further, in some states there is a shortage of educational opportunities and practicums specific to school nursing; therefore, most school nurses enter school nursing with education that is not specific to the school setting and with skills acquired in other settings, often in acute care. The main challenge for many new school nurses is effectively transferring those skills to an educational setting. This article shares observations and ideas on ways that the new school nurse can effectively make the transition.

In geriatrics there results a necessity of rehabilitative treatment in opinion on some important facts and it is always orientating on the individual patients. As basic condition for a successful rehabilitation on appropriate "therapeutic milieu" is required. Rehabilitation and nursing are inseparably connected in geriatrics as two elements of geriatrical intervention. Nursing is firmly included in decisive processes and has diagnostic and therapeutic functions. For the patients common competence it is not so important what sort of illness he suffers from but the effect on functional efficiency.
As the prognosis for children and adolescents with cancer improves, we must reconsider how we will provide care to the adolescents and young adults who have survived these conditions. On the premise that the global needs of adults are best served by health care obtained in the adult health-care system, the case is made for transition from pediatric to adult-oriented health care for adolescents and young adults with cancer. The benefits of transition, barriers to implementation, and models of care are discussed.
The care of patients transitioning from adolescence to adult life always raises challenges for the clinician. This has proven to be especially true in the case of growth hormone (GH) treatment, in part due to the absence of any immediately observable consequences of the cessation of therapy. This paper describes the challenges involved in reestablishing the diagnosis of GH deficiency (GHD), titrating GH dosage, and sustaining patients on long-term GH treatment.
