[Prevention of infections in chronic bronchitis using vaccination and/or immunomodulation].

Polyvalent antimicrobial vaccines have been widely used in chronic bronchitis for nearly fifty years without their effect or mechanism of action always being well understood. The polysaccharide vaccines, in particular pneumococcal, are better defined chemically and in their mode of action. However, it does not seem that ordinary chronic bronchitis is a good indication for these vaccines. The immunomodulators, extracts of the cell wall of certain bacteria, are aimed at increasing the host's defence capacity in a non-specific manner. The addition of ribosomes may favour specific and non-specific immunomodulation. Certain vaccines have shown their efficacy in reduction of infective episodes in chronic bronchitis. However, other studies ought to be undertaken to obtain purified products which are not toxic and are yet more effective. Finally, it is necessary to have better knowledge of the mode of administration and of the doses administered corresponding to the optimal efficacy of the vaccines.

[Methodological aspects of an experimental multifactorial study of the combined use of antibiotics and immunomodulators].

Methodological approaches to investigation of the protective effect of natural immunomodulators are described. The use of multifactorial experiment design, a model of infectious processes and immunomodulators alone or in combination with antibiotics is implied. Theoretical preconditions for rational choosing of the experimental factors and their levels, as well as specific designs are discussed. Potentialities of polynomial statistic model analysis in development of optimal regimens for combined chemoimmunotherapy are shown.

Preliminary experimental and clinical results with inactivated allergens conjugated to the Corynebacterium granulosum-derived immunomodulator P40.

Allergoids have been used successfully for immunotherapy of allergic disorders. It has appeared to us that the effect of allergoids could be potentiated by their coupling to an immunomodulator. In the present study we show that a conjugate made up of the coupling of ovalbumin through glutaraldehyde action to the C. granulosum-derived immunomodulator P40 is completely devoid of antigenicity and of cross-reactivity with ovalbumin. This conjugate was found to significantly inhibit mast cell degranulation. It also proved to be capable of protecting against the lethal systemic anaphylactic shock sensitized mice. Immunotherapy was performed in patients hypersensitive to either the pollen of Dactylis glomerata or to the house dustmite allergens using the conjugates made of the specific allergens and of the P40. Clinical improvement was observed in a significant percentage of the patients subjected to immunotherapy. Administration of the conjugates did not result in untoward reactions in any of the patients.

[Immunomodulating action of ceruloplasmin in tumor growth and the participation of cyclic nucleotides].

The study was concerned with the effect of human ceruloplasmin on blastogenic reaction of lymphocytes (BRL) and cyclic nucleotide level in murine organs in the course of Lewis lung carcinoma development. Both ceruloplasmin and an immunomodulator--zymosan--were found to raise the level of cAMP and to lower that of cGMP in the immunocompetent organs (thymus and spleen) of tumor-bearing mice at all stages of the investigation. The results point to an immunomodulating action of human ceruloplasmin involved in tumor growth and the role played by cyclic nucleotides in the process.

[Importance of the cyclophosphane administration regimen for its immunomodulating and antitumor effect in experimental chemotherapy].

Cytological examinations and evaluation of the proliferative response of lymph node lymphocytes to T- and B-cell mitogens in vivo were carried out to identify the immunomodulating effect of cyclophosphamide (CP) versus the scheme of treatment. Using the system of adoptive transfer of splenic lymphocytes from experimental animals to intact syngeneic recipients, it was found that the scheme-dependent differentiated effects of CP on the antigen-specific tumor-associated and nonspecific suppressive activity of spleen cells provides an important pathway of CP action on the organism. A relationship between CP immunomodulating effects and lymphocyte antitumor activity was demonstrated in experiments using diffusion chambers.

Immunomodulation by recombinant interferon-alpha 2 in a phase I trial in patients with lymphoproliferative malignancies.

A single rising dose tolerance trial of rDNA interferon-alpha 2 (IFN-alpha 2) was conducted in eight patients with the diagnoses of non-Hodgkin's lymphoma (NHL), multiple myeloma, and chronic lymphocytic leukemia (CLL). Patients received a total of six i.m. doses at weekly intervals as follows: 1, 3, 10, 30, 60, and 100 x 10(6) IU. Patients were monitored at each dose level for serum IFN activity, anti-IFN antibodies, immunomodulation, clinical toxicity, and response. All patients exhibited clinical toxicity, including fever, chills, fatigue, headache, anorexia, mild-to-moderate leukopenia, nausea, and vomiting. Toxicity was dose-related, with significant side effects occurring in all patients at levels of 10 x 10(6) IU and above and some evidence of tachyphylaxis at higher doses. All side effects, including leukopenia and thrombocytopenia, were of short duration and were resolved within 3-5 days. Fevers, rigors, myalgias, and fatigue were partially alleviated by premedication with acetaminophen or hydrocortisone. Pharmacokinetic data indicated mean peak serum IFN titers greater than 90 at a dose of 10 x 10(6) IU and greater than or equal to 200 at doses greater than or equal to 30 x 10(6) IU 8 h after injection. No anti-IFN antibodies were detected. However, the serum levels achieved at higher doses were not linear, possibly indicating in vivo degradation. Total T cells, B cells, monocytes, and T subsets monitored by flow cytometry with monoclonal antibodies remained essentially constant throughout the trial. Although some patients demonstrated minor augmentations of antibody-dependent cellular cytotoxicity (ADCC) and natural killing (NK) activity at the lowest IFN-alpha 2 doses, the majority of patients demonstrated decreases in NK activity after higher IFN doses. No correlation between immunomodulation and clinical response to IFN was observed. At higher dose levels, the predominant immunomodulatory effect of IFN-alpha 2 was suppression of NK, ADCC, and blastogenic responses to T-cell mitogens and recall antigens. B-cell functional deficits as well as radioresistant T-helper and radiosensitive T-suppressor function assessed in a pokeweed mitogen-driven immunoglobulin secretion assay appeared unaffected by IFN administration. One myeloma patient showed progression and was discontinued after 60 x 10(6) IU. There were four patients (3 NHL, 1 myeloma) who achieved partial remission (greater than or equal to 50% tumor reduction) and three (1 CLL, 2 NHL) who showed objective tumor responses of less than 50%. These data suggest that rDNA IFN-alpha 2 is well-tolerated and may have significant antitumor activity against lymphoproliferative malignancies. Clin

[Isolation, purification and identification of an immunomodulating polypeptide from human and calf thymus].

A comparison of human and calf thymus extracts was carried out. The thymus extracts and the fractions isolated from them differ in their physico-chemical properties, amino acid composition and biological activity. The extracts tested contain different amounts of the thymus factor, which stimulates the reactions of cellular and humoral immunity. The results obtained suggests that calf and human thymus contain only one immunomodulating factor, namely thymarin, a component of thymus extracts from various sources, in particular of thymosin. In some of its physico-chemical properties thymarin is similar to histone H1 from calf thymus, thus suggesting an essential role of this histone in the formation and differentiation of T-lymphocytes. A possibility of quantitative estimation and identification of the immunomodulating polypeptide in thymus extracts obtained by various methods is postulated.

Typing of immunomodulators in terms of their effects on the electrophoretic pattern of serum proteins and antitumor combination therapy based on this typing.

Changes of serum proteins have recently received much attention in studies of immunomodulators. In this work, changes of serum proteins, especially LB, were studied by gel electrophoresis of sera after administration of 23 immunomodulators or antitumor agents. Fourteen of the 23 compounds increased the concentration of LB in the serum of normal ddY mice when injected once ip. Six compounds caused a very rapid (day 1) increase of LB, and 8 agents caused a slow increase (day 4 approximately day 10). On the basis of the results, these compounds were classified into type I (causing a rapid increase in LB; i.e., lipopolysaccharide, dextran sulfate and poly (I)-poly(C), type II (causing a slow increase in LB; i.g., lentinan, TAK and PS-K), and type O (causing no increase in LB; e.g., levamisole and bestatin). The antitumor activities of these three types of compounds in combination with lipopolysaccharide (type I) or lentinan (type II) were studied in an Ehrlich carcinoma-ddY mouse system. The results suggested that different types of compounds frequently showed synergistic antitumor activities. Typing of immuno-modulators and the antitumor activities of combinations of these compounds are discussed.

[Antibacterial and immunomodulating activity of fucoidan].

The experimental data on the study of the antibacterial and immunomodulating activity of phucoidan, a sulfatated polymer from brown algae (phucan sulfate), were generalized and it was shown that phucoidan was able to markedly inhibit the growth of gramnegative and grampositive organisms and to stimulate immunoreactions of the humoral and cellular types, as well as phagocytosis. The possible use of phucoidan as a drug with associated antibacterial and immunomodulating activity is discussed.

A method to assess the immunomodulating effects of the Tołpa Torf Preparation (TTP) by measuring the hyporeactivity to interferon induction and tumor necrosis factor response.

Tołpa Torf Preparation (TTP) is an immunomodulator which was found to be interferon (IFN) and tumor necrosis factor (TNF) inducer in human peripheral blood leukocytes (PBL). PBL cultures prepared from the blood of healthy volunteers taking 5 mg TTP tablet daily loose ability to respond to IFN induction by TTP, phytohemagglutinin (PHA) or lipopolysaccharide (LPS). The phenomenon of hyporeactivity or tolerance is characteristic of every IFN inducer described so far. In the TTP treated volunteers the tolerance developed during three weeks and it lasted one to two weeks. In comparison with the tolerance to the IFN induction the hyporeactivity to TNF induction developed slowly and it was only partial. Because the detection of the tolerance to IFN induction is a very sensitive indicator of TTP activity in vivo in patients taking the drug orally, we suggest that it may be applied to assess the mode of action, dosage and schedule of administration of the IFN-inducing immunomodulator.

The cytoprotective effect of the immunomodulator AS101 against hydrochloride induced gastric lesions.

AS101 as a new immunomodulator has been shown to induce production of a variety of cytokines such as interleukin-1, interleukin-2, colony-stimulating factor, interferon-gamma and tumor necrosis factor, and demonstrate a potential chemo-protection from chemotherapy induced immunosuppression and hematopoietic toxicity in tumor bearing mice and cancer patients on phase I and II clinical trials. This study was designed to verify whether AS101 exerts a cytoprotective effect in rats and mice with gastric lesions induced by intragastric (i.g.) instilling of 0.6N hydrochloride (HCl). AS101 given intraperitoniously (i.p) 2h before a HCl administration markedly prevented HCl-induced gastric lesions both in rats and mice. It also accelerated the ulcer repair when given i.p. 1h after a HCl treatment. Indomethacin (IND), a cyclo-oxygenase inhibitor, given i.p. at a non-ulcerogenic dose of 5mg/kg 1h before AS101 administration, abolished its protective effect. Mechanistic analysis showed that the gastric cytoprotective property of AS101 appears to be mediated through the induction of prostaglandin E2 (PGE2) and epidermal growth factor (EGF), of which, both prevent the gastric mucosa from HCl ulceration while EGF also contributes to the promotion of ulcer repair. This study adds another cytoprotective property to the known immunomodulating role of AS101.

[The role of proteins in the immunomodulating effect of bioginseng products].

Immunomodulating properties of the bioginseng preparations (Panax ginseng), isolated from the cell culture of ginseng calluses, were studied in model experiments of acquired immunodeficiency developed as a result of long-term protein starvation or after vinblastin administration. The following preparations were studied: 1) preparation obtained by means of the cell culture cryoconcentration, 2) ethanol extract of the cell culture, 3) high-molecular protein containing fraction of the bioginseng I. Under conditions of acquired immunodeficiency all the preparations studied were shown to increase the content of antibody producing cells developed in response to sheep erythrocytes administration, while the highest effect exhibited the bioginseng preparation III. These data suggest that proteins are of great importance in the immunomodulating effects of bioginseng preparations.

Some biochemical characteristics of the early phase of immunomodulation.

The study estimates some biochemical changes of chosen immunological and biochemical parameters after intraperitoneal application of a) immunomodulators of microbial origin--Propionibacterium acnes and Geotrichum candidum, b) chemical substances--indomethacin and phenobarbital. The estimation was concentrated above all to the changes of chemiluminiscence, phagocytic activities, the amount of cAMP in peritoneal macrophages, the amount of liver cytochrome P-450 and cAMP in the liver, spleen and thymus. The experiments also included histological examination of the spleen, thymus, lungs and diaphragm. The effect of Propionibacterium acnes was evident as soon as 24 hrs after application. In the frame of studied parameters, there was manifested the nonspecific activation of the immune system by the mechanisms independent to oxygen. The early changes were accompanied by the increase of cAMP in macrophages. In contrary, the intraperitoneal application of Geotrichum candidum activated the release of oxygen radicals from peritoneal macrophages. The amount of the cytochrome P-450 correlated to the intensity of immunomodulation. On the other hand, the induction of cytochrome P-450 by phenobarbital decreased the value of several immunity indices.

[The effect of immunomodulators on the functional activity of polymorphonuclear leukocytes from the peripheral blood of healthy subjects in vitro].

The effect of such immunomodulators as glucoaminylmuramyldipeptide, gamma-interferon and polyoxydonium on the functional activity of phagocytic cells of peripheral blood taken from 33 healthy donors was studied. The adhesion of these cells on the plastic surface, their spontaneous and induced luminol-dependent chemiluminescence, the activity of myeloperoxidase and acidic phosphatase and changes in the activity of enzymes after the in vitro stimulation of the cells, the results of the spontaneous and induced nitro blue tetrazolium test, chemotaxis of neutrophils and the content of nonenzymatic cation proteins in neutrophils were determined. The study revealed considerable differences in the effect produced by the above-mentioned immunomodulating preparations on some manifestations of the phagocyte reactiveness in the examined subjects. Only in 30.0% of cases similar in vitro sensitivity of cells to two or all three preparations was registered, and in 15.2% of cases no stimulating effect was noted. Peripheral blood neutrophils are supposed to have individual sensitivity to certain immunostimulators.

Synthesis, toxicity and immunomodulating activity of Co(II), Ni(II), Cu(II) and Zn(II) complexes of L(-)-2,3,5,6-tetrahydro-6-phenylimidazo[2,1-b]thiazole (levamisole).

Co(II), Ni(II), Cu(II) and Zn(II) complexes of levamisole (LMS) were prepared and characterized by elemental analyses, IR spectroscopy, 1H and 13C NMR and mass spectrometry. The following general formula was derived: M(LMS)2Cl2, where M = Co, Ni, Cu, Zn. It was established that LMS behaved as a monodentate ligand and the coordination was accomplished through the N-7 atom. The toxicity and the immunomodulating activity of the complexes on mice and rats in comparison with uncomplexed LMS was assayed. The metals in the complexes exerted different changes in the toxicity of LMS. The complex containing Zn(II) was less toxic and manifested higher immunomodulating activity than LMS.

[The experimental efficacy of vaccination with a recombinant yeast vaccine against hepatitis B in combination with immunomodulators].

Experiments in BALB/c mice were carried out to study a number of immunomodulators in combination with vaccination using a recombinant yeast hepatitis B vaccine. The use of poludan, ridostine, amixine, larifan, myekonide significantly enhanced specific antibody production and at least doubled cell-mediated immune response by 14 and 28 days postvaccination. The above-mentioned immunomodulators are planned to be used in vaccination with the recombinant yeast vaccine against hepatitis B.
