BACKGROUND: The incidence of adenocarcinoma of the oesophagus and gastric cardia is increasing rapidly. Barrett's oesophagus is the major risk factor. Intestinal metaplasia at the squamocolumnar junction in the absence of Barrett's oesophagus is common but its relation to adenocarcinoma and gastro-oesophageal reflux disease is unclear. AIMS: To study the prevalence and clinical, endoscopic, and histological associations of intestinal metaplasia at the squamocolumnar junction. METHODS: Biopsy specimens were taken from 120 randomly selected patients undergoing routine diagnostic endoscopy. Eight biopsy specimens, taken from above and below the squamocolumnar junction, gastric fundus, and gastric antrum, were stained with haematoxylin/eosin, alcian blue/periodic acid-Schiff, and Gimenez, and graded independently by one pathologist. RESULTS: Intestinal metaplasia at the squamocolumnar junction was found in 21 patients (18%). Metaplasia was associated with increasing age (p < 0.01) and antral intestinal metaplasia (p = 0.04). Logistic regression analysis revealed that age was the only independent predictor (p < 0.01). There was no association with symptomatic, endoscopic, or histological markers of gastro-oesophageal reflux disease. CONCLUSIONS: Intestinal metaplasia at the squamocolumnar junction is a common finding. It is associated with increasing age but not gastro-oesophageal reflux disease.

Diseases of the upper gastrointestinal tract, including ulceration, infection, parasitism and the effects of ingested foreign bodies are common in captive dolphins. Using a fibreoptic colonoscope, techniques were devised for examination of the oesophagus and stomach and the removal of foreign bodies.

The results of an endoscopic bioptic study of 16 cases of primary gastric malignant lymphoma are reported. Endoscopic observation suggested a diagnosis of malignant lymphoma in 50% of the cases, while directed biopsy gave a similar diagnosis in 75% of the cases. A correlation of the endoscopic and the histologic results gave a diagnostic reliability or 87.5% (14 of 16 cases). In order to obtain also in this group of neoplasms the diagnostic positivity already obtained in the epithelial forms, both improvement in the bioptic sampling technique and a better knowledge of the endoscopic morphology of the lesions must be achieved.
Midazolam, a water-soluble benzodiazepine, was given intravenously in doses of 0.07 mg/kg as sedation prior to oesophago-gastro-duodenoscopy in 40 patients. This was very effective as a sedative in elderly patients but less so in the young, with a large individual variation in response. Pain on injection and venous thrombosis which are common with diazepam were not seen with midazolam.

Endoscopic ultrasonography (EUS) was performed in 10 patients with gastric non-Hodgkin lymphoma (NHL). EUS visualized hypoechoic intramural lesions with destruction of normal gastric wall architecture together with perigastric lymph node involvement. EUS was more accurate than endoscopy, barium meal, and computerized tomography scan in detecting and staging gastric NHL because of the high resolution. The non-resectability could reliably be diagnosed before surgery based on the detection of deep infiltration of the lesion into the surrounding tissues and/or organs EUS appears to be a sensitive diagnostic modality for detecting, staging, and following up after chemotherapy of gastric NHL. Technical improvements allowing ultrasonically guided biopsy or cytological puncture may further enhance its diagnostic usefulness.
