Intraocular pressure level in glaucomatous and nonglaucomatous eyes after complicated cataract surgery and implantation of an AC-IOL.

The effects of a semiflexible, one-piece, open-loop anterior-chamber intraocular lens (AC-IOL) implanted after intracapsular or extracapsular cataract extraction complicated by posterior-capsular or zonular rupture on intraocular pressure (IOP) level and the control of previous glaucoma were studied in 48 eyes with primary implantation and in 10 eyes with secondary implantation. Fifty-four percent of the eyes with primary implantation had exfoliation syndrome; anterior vitrectomy was performed in 60.4%. An immediate pressure rise (IOP greater than or equal to 30 mm Hg) was observed in 29.3% of the eyes. The long-term IOP level (median follow up, 21.5 months) was less than or equal to 20 mm Hg in 83.7% and between 21 and 29 mm Hg in the remaining 16.3%. In three of the nine glaucomatous eyes, the medication had to be increased; in the remaining six, the IOP could be controlled with the previous or reduced medication or with none at all. In three out of the 49 (6.1%) nonglaucomatous eyes, glaucoma medication was started during the follow up. All of these eyes had exfoliation syndrome and two of the fellow eyes had similar IOPs and were receiving similar medication. The results indicate that the semiflexible, open-loop AC-IOL has little effect on IOP and seems to be a safe alternative, even in glaucomatous eyes, if a posterior chamber lens cannot be used.

Central visual field, visual acuity, and sudden visual loss after glaucoma surgery.

The relationship between Snellen visual acuity and central visual field loss as determined by the Humphrey 10-degree test was examined in 96 glaucomatous eyes of 79 patients. The severity of the field defect was determined by the number of affected quadrants, defined as a sensitivity loss of at least 10 decibels from normal at the most central point of 1.4-degree eccentricity. The decrease in median acuity was gradual, one-half line per quadrant for one and two affected quadrants, and an additional one and one-half line to two lines for three and four quadrants. Loss of acuity was disproportionate when both temporal quadrants were affected. The major source of error was difficulty with fixation. One type, a prolonged fixation shift, was not related to short-term fixation losses. There was a significant correlation between acuity and foveal threshold over a wide range of both variables. Sudden visual loss from additional optic nerve damage was studied retrospectively in 96 eyes with advanced glaucoma and a central field defect. A frequency of 3% for any loss of visual acuity and a frequency of 1% for a loss to 20/200 or worse were noted. There were no apparent predictive factors.

[The immediate results of trabeculoplasty with the YAG-neodymium laser in wide-angle glaucoma].

There are discussed the observations that had been done for a period of 10 weeks about, after the YAG-neodymium laser trabeculoplasty for 79 eyes suffering of simple chronic glaucoma. The choosing of the patients has been done because of the tensional uncompensation under the maximum treatment for 42 eyes having ocular tension constantly over 30 mm Hg, or because of the lack tolerating the drugs, the secondary, effects, allergy. The average decreasing was of 6.31 mm Hg, high semnificative from the point of view of statistic mathematics but the decreasing of the tension researches 15 mm Hg for isolated cases, that permitting to decrease or renounce the drugs.

[Chemiluminescence and activity of acid phosphatase in the drainage zone of the eye in glaucoma patients].

Ocular drainage zone tissue, obtained during surgery from patients with glaucoma, was examined. Chemiluminescence has revealed changed amplitude of the flash and the value of its total light volume. Increased intensity of peroxide chemiluminescence evidences enhanced processes of lipid peroxidation in the tissue and a reduced antioxidant level. Histochemical study of acid phosphatase has discovered destabilization of lysosomal membranes, more marked during an acute attack of glaucoma.

[Characteristics of trabeculectomy in the treatment of some forms of glaucoma in young patients].

The described technique of modified trabeculectomy with irido-retraction according to O. V. Grusha-G. A. Sokolovsky is intended for the treatment of some glaucoma forms in young patients with the anterior fixation of the iris. The basic features of the technique are as follows: (1) besides the sinus, deep scleral layers and abnormal trabecula, the scleral autoimplants include the anterior section of the scleral calcar; (2) during the operation the iris is disconnected from the site of its adhesion to the trabecular zone; (3) to prevent cilio-choroidal detachment, sclerectomy is carried out. A total of 14 eyes in 11 patients aged 17 to 42 were operated on and the results followed up for up to 2.5 years. Surgical and stable hypotensive effect was achieved in 12 cases. Two obligatory conditions of effective surgery are emphasized: punctilious performance of all the particulars of the operation and postoperative cytostatic therapy.

[A variant of glaucoma surgery].

The suggested variant of surgery provides an enhanced outflow of aqueous fluid from the anterior chamber of the eye into the suprachoroidal and subconjunctival spaces, this resulting in a reliable and stable hypotensive effect. In 53 of the 56 eyes with various forms, stages, and compensation degree of primary glaucoma, operated on, ophthalmic tone after surgery was within 18-20 mm Hg, in 3 cases it was within 15-17 mm Hg, and when the patients were discharged from hospital it was 18-21 mm Hg in all the eyes operated on. No cases of ciliochoroidal detachment, hyphema, or other complications were recorded in the postoperative period, that ran a smooth course with the minimal recovery inflammation reaction of the eye and was shorter by 3.5 bed-days, on an average. The technique is simple and available for ophthalmic surgeons, low traumatic and highly effective in patients with various stages of primary glaucoma, this recommending it for wide practice.

Peripheral nasal field defects in glaucoma.

One hundred fifty-one eyes of 101 consecutive patients with chronic open-angle and low tension glaucoma showed typical visual field changes. Sixty of the eyes had a nasal step, either alone or in combination with other defects. In 17 of these eyes, it was possible to demonstrate an isolated scotoma in the nasal periphery. It was concluded that the peripheral nasal step is a nerve fiber bundle defect that in its earliest phase produces a scotoma. In this sense, it behaves similar to the more central defects, rather than simply as a depression of isopters nasally.

Diathermo-trabeculotomy ab externo: indications and long-term results.

The authors present the results of 2 years' observation of 47 eyes operated with diathermo-trabeculotomy ab externo. This technique, based on a diathermic effect in association with trabeculotomy ab externo, allows an ample opening of Schlemm's canal and of the trabecular meshwork, with normalisation of intraocular pressure and outflow facility, for a period of up to 2 years, in open-angle and congenital glaucoma.

Combined cyclodialysis and cataract extraction.

Combined cyclodialysis and cataract extraction was performed on 78 eyes in a consecutive series of 26 years duration. This operation was performed on eyes with severe glaucoma, inadequately controlled on maximum medical therapy, and with a cataract needing extraction. Glaucoma types included open-angle, angle-closure with peripheral anterior synechias, combined open-angle and angle-closure, and secondary to uveitis. Seventy of the 78 eyes had follow-up of six months or more and the results in these 70 eyes were analyzed. In thirteen percent of the eyes in this study the postoperative intraocular pressure was the same or higher than the preoperative level, or reductions of intraocular pressure were not clinically significant. Eighty-seven percent (61 eyes) had a clinically significant lowering of intraocular pressure following the combined procedure. Thirty-nine percent (27 eyes) of the 70 eyes in this study had a tension of 0-14 mm Hg, and 44% (31 eyes) had a tension of 15-20 mm Hg. Intraocular pressure was improved significantly more often in patients over 60 years of age. Resulting intraocular pressures of 14 mm Hg or less were more likely when the cyclodialysis was performed in the lower quadrant. The results of the combined procedure were not significantly influenced by the type of glaucoma involved or by a history of previous glaucoma surgery. Bleeding was a frequent complication, but was not related to resulting intraocular pressure. Other complications were rare. We conclude that combining cyclodialysis with cataract extraction is safe and effective, and is a suitable procedure when there is need for surgery to relieve severe glaucoma, along with a need for cataract extraction.

[Topical oxpernolol in the treatment of various forms of glaucoma (author's transl)].

Oxprenolol is a new beta-adrenergic blocking drug, whose effectiveness in the treatment of various forms of glaucoma has been evaluated. The drug was administered topically as 0.5% -1%-2% solution. The results of the investigation may be summarized as follows: The instillation of the drug produces a transient and slight conjunctival hyperaemia and a not significant miosis. A significnat ocular hypotensive action has been observed in the treatment of chronic open-angle, hemorrhagic and congenital glaumiosis. A significnat ocular hypotensive action has been observed in the treatment of chronic open-angle, hemorrhagic and congenital glaucoma. The concentration of 0.5% Oxprenolol seems to be as effective as topical 1% Propranolol. At the same concentration (0.5%), Oxprenolol produces no significnat change of the systemic blood pressure or of the heart rate. Topical pre-treatment with 1% Oxprenolol inhibits the systemic side-effects (tachycardia) produced by topical Isoproterenol, a wellknown ocular hypotensive drug. The authors conclude the investigation affirming that Oxprenolol may represent a new effective means for the medical treatment of glaucoma.

Pilocarpine therapy in glaucoma: effects on anterior chamber depth and lens thickness in patients receiving long-term therapy.

The effect of pilocarpine hydrochloride on the anterior chamber depth and lens thickness was measured in 20 eyes of patients with chronic open-angle glucoma who were receiving long-term glaucoma therapy with pilocarpine. Measurements were made with high-resolution ultrasonic biometry. This study demonstrated that regardless of their age (58.4 years, average) and the fact that they had been regularly using pilocarpine from two to four times a day, 85% of these patients demonstrated narrowing of the anterior chamber (AC) depth (average, 0.19 mm) and thickening of the lens (average, 0.21 mm) with each instillation of pilocarpine. In approximately 15% of eyes, a repeatable deepening of the AC depth and flattening of the lens was noted.

The histopathology of pigmentary dispersion syndrome with glaucoma.

Iris tissue and trabecular meshwork, obtained at time of trabeculectomy, was studied using the light and electron microscope in a 54-year-old woman with pigmentary dispersion syndrome with glaucoma. The specific defect was a loss of the outer epithelial cells of the iris with marked thinning of the remaining outer layers so that the two-cell architecture of the iris epithelium was maintained. In addition, the radial muscle layer was increased in both number and size of muscle fiber. It would appear that the pigmentary dispersion syndrome may represent a congenital or developmental abnormality of the iris epithelium, or both, and that the glaucoma which occasionally occurs in conjunction with this syndrome is of the usual open-angle type.

[Use of cytostatics as anti-cicatricial drugs after anti-glaucoma surgery in children, adolescents and young adults].

Anti-cicatricial cytostatic therapy with 5-fluorouracil and prospidin was carried out after antiglaucoma surgery in 78 eyes of 67 patients aged 6 to 42 (2/3 of the patients were aged from 21 to 40). Half (34) of the patients suffered from open-angle (pigmented in 3 cases) and 6 from closed-angle glaucoma, the rest patients presented with common hydrophthalmia, the Frank-Kamenetsky, Sturdge-Weber, Riger's syndromes, etc. The cytostatics were injected subconjunctivally. The results of 67 surgeries were followed up for 6 months to 3 years. Stable normalization of the ophthalmic tone and stabilization of the process were achieved in 60 eyes (89.2%). The authors emphasize that cytostatics may be recommended for children and young subjects after surgery involving fistulization, on condition of thorough biomicroscopic monitoring in the postoperative period, because of a possible disadaptation of the edges of the mucosal incision and corneal diseases.
