Post-abortion adjustment of health care professionals in training.

Sixty-three women students of the health care professions were surveyed about their abortion and post-abortion experiences, feelings, and attitudes. Variables predictive of short- and long-term adjustment, belief in the wisdom of the choice, and right to choose were analysed, as were effects on emotional and attitudinal responses of passage of time and pressure to abort. Most reported mild, transient, short-term negative effects, and generally benign long-term effects. Implications of the findings are discussed.

Bacterial vaginosis: current review with indications for asymptomatic therapy.

Bacterial vaginosis is a definable clinical entity whose exact origin is unknown. A shift in normal vaginal flora from aerobic, predominantly but not exclusively lactobacilli, to a predominantly anaerobic flora characterizes the condition. More than one half of all women with bacterial vaginosis have no symptoms. The condition is not entirely benign. The potentially pathogenic bacteria present in the vagina in large numbers place these women at risk for postoperative morbidity and adverse obstetric outcome. Sexual transmission has not been proved, but therapeutic cures sometimes require that patient and partner be treated simultaneously. Recommended therapy is with metronidazole or clindamycin and must be given for 7 days for maximal effectiveness. Recurrence of disease can be a problem.

Safety and tolerability of long-term propafenone therapy for supraventricular tachyarrhythmias. The Propafenone Multicenter Study Group.

An important issue regarding the long-term use of antiarrhythmic drugs concerns the safety of these agents, particularly with regard to cardiac toxicity. Propafenone is an effective drug for preventing supraventricular tachyarrhythmia, but the incidence of side effects during longterm therapy in patients with such arrhythmias has not been adequately reported. A total of 480 patients received oral propafenone as therapy for symptomatic atrial fibrillation, atrial flutter, or supraventricular tachycardia. During the follow-up (mean 14.4 months), 290 patients (60%) discontinued propafenone therapy, but in only 70 patients (15%) was the reason for discontinuation an adverse drug reaction. Overall, 284 patients (59%) experienced at least 1 adverse reaction, and the incidence was related to dose and age >65 years. The overall incidence of side effects was not related to structural heart disease; however, cardiovascular toxicity including arrhythmia aggravation, congestive heart failure, and serious conduction disturbances occurred more often in those with heart disease (20% vs 13%). Sixteen patients died during drug therapy, but in only 1 case was the drug considered contributory. For patients with a supraventricular arrhythmia, propafenone was well tolerated and was infrequently discontinued because of side effects. The incidence of serious cardiac toxicity when propafenone was used to treat supraventricular arrhythmia was low, and these side effects were more frequent in patients with structural heart disease.

[On the demographic and economic effects of migration to the Federal Republic of Germany in the past and in the future].

"The aim of this paper is to provide a survey of the previous and possible future global effects of net immigration on economic growth and the labor market from a macroeconomic point of view....First, some fundamentals relating to the expected economic effects of migration are briefly examined. There follows--in the first main section--a retrospective view of the development of economy and immigration in...[Germany] based on a statistical comparison and existing simulations with macroeconomic models. In the second main section the effects of further migration are discussed on the basis of population and labor market projections." (SUMMARY IN ENG AND FRE AND RUS)

Late effects of induced abortion. Hypothesis or knowledge?

Recent local, state and federal regulations intending to provide "informed consent" for women considering termination of pregnancy have misrepresented the current state of knowledge about certain long-term complications: the alleged risks are presented to the patient as if they were scientific facts rather than still-unproven hypotheses. While it is possible that some late sequelae are associated with previously induced abortions, there is no agreement in the scientific literature on the magnitude of the risks, if any. Moreover, few studies adequately define the particular method of induced abortion whose effects they are investigating. The current data do not support firm conclusions about induced abortion either causing or not causing any of the alleged long-term complications.

Far beyond the Gulf: the implications of warfare for Asian labour migration.

The implications of the Gulf War are assessed for the countries in Asia that send labor migrants to the Middle East. "This paper seeks to examine the effects of...the loss of remittances (and related issues, including return migration), primarily in terms of the long-term implications of the War for the future of contract labour migration in the Asian region...."

Midtrimester abortion induction with intraamniotic prostaglandin F2 alpha in women with previous uterine surgery.

Of 207 women who underwent midtrimester induced abortion with intraamniotic prostaglandin F 2 alpha instillation, 30 (14.5%) had had one or more previous uterine operations. Multivariate analysis showed no correlation between a previous uterine operation and the hospitalization length, procedure complication rate or length of the injection-delivery interval (IDI). The IDI was similar in patients with and without a previous uterine operation. No case of uterine rupture occurred. Intraamniotic prostaglandin instillation for midtrimester abortion induction seems to be an effective procedure without severe complications in women with previous uterine operations.

Long-term risk of hysterectomy among 80,007 sterilized and comparison women at Kaiser Permanente, 1971-1987.

To study the long-term risk of hysterectomy after tubal sterilization, the authors analyzed historical hospital discharge data on 39,502 parous women sterilized during 1971-1984 and 40,505 comparison women matched on age, race, parity, and interval since last birth. Sterilized women were significantly more likely than were comparison women to undergo hysterectomy (relative risk (RR) = 1.35, 95% confidence interval (CI) 1.26-1.44), especially for diagnoses of menstrual dysfunction and pelvic pain (RR = 1.88, 95% CI 1.65-2.13). Higher relative risks were not associated with greater tissue-destructive methods of tubal occlusion. Relative risks were highest for women who were young on the reference date (RR = 2.45, 95% CI 1.79-3.36 for women aged 20-24 years), but declined steadily as age increased (RR = 0.96, 95% CI 0.72-1.28 for women aged 40-49 years). In all age groups, relative risks were significantly above 1.00 after 7 years of follow-up. Reasons for elevated risks may be related to a greater willingness of sterilized women to forgo their uteruses. The emergence of greater risk in all age groups, however, prevents the authors from ruling out a possible latent biologic effect of tubal sterilization.

Outcome of delivery subsequent to vacuum-aspiration abortion in nulliparous women.

Altogether 4719 women who had had a legally induced abortion in Uppsala County during 1970-75 were followed by means of computerized record linkage, and deliveries during 1970-78 after abortion were noted. Out of 1 364 deliveries traced, 670 were to primiparas who had had a previous vacuum-aspiration (VA) abortion. The outcome of the delivery for the 670 women was compared with that of matched controls, 622 primiparas and 626 secundiparas. Confounding variables were checked for by multiple regression analysis using the infant's birthweight and gestational duration as dependent variables. This analysis showed that the abortion did not have a statistically significant effect on the outcome. Low birth weight infants and preterm delivery did not occur more often after a previous VA abortion.

The ups and downs of marriage in Austria.

The author examines reasons for fluctuations in marriage patterns in Austria since 1950. "We propose to investigate here not only the immediate impact of these perturbations in terms of first marriage rates, non-marital fertility and legitimations, but also the possible longer-term effects, by studying proportions never married in birth cohorts and fertility and divorce rates in marriage cohorts; finally, we shall consider total fertility trends for the most recent period." The impact of changes in Austria's fiscal policy with regard to newlyweds is considered.

Effects of Cryptosporidium parvum infection in Peruvian children: growth faltering and subsequent catch-up growth.

The authors conducted a 2-year (1989-1991) community-based longitudinal study in a shantytown in Lima, Peru, to examine the effect of Cryptosporidium parvum infection on child growth during the year following the onset of infection. A cohort of children, aged 0-3 months at recruitment, was followed monthly for anthropometrics, weekly for stool samples, and daily for diarrheal status. Data from 185 children in the cohort permitted a comparison of growth in C. parvum-infected and noninfected children. The analyses fitted smooth, flexible curves with a linear random-effects model to estimate growth differences between C. parvum-infected and noninfected children. Children infected with C. parvum experienced growth faltering, both in weight and in height, for several months after the onset of infection, followed by a period of catch-up growth. Younger children took longer to catch up in weight than did older children. Catch-up growth, however, did not occur in children infected between ages 0 and 5 months. These children did not catch up in height, and one year after infection they exhibited an average deficit of 0.95 cm (95% confidence interval (CI) 0.38-1.53) relative to noninfected children of similar age. Stunted children who became infected also did not catch up in either weight or height, and one year after infection they exhibited a height deficit of 1.05 cm (95% CI 0.46-1.66) relative to noninfected, stunted children of similar age. These results indicate that Cryptosporidium parvum has a lasting adverse effect on linear (height) growth, especially when acquired during infancy and when children are stunted before they become infected.

Longterm therapy in polymyalgia rheumatica: effect of coexistent temporal arteritis.

OBJECTIVE: To analyze the clinical course and duration of therapy in a series of 104 patients with polymyalgia rheumatica (PMR), identifying factors that influence prolonged steroid use and relapses. METHODS: Retrospective study of 104 cases of PMR diagnosed from 1985 to 1995. Patients were followed from time of diagnosis until either their death or December 31, 1995. To assess the effects of the coexistence of temporal arteritis (TA) on outcome in PMR, patients were grouped according to the absence or presence of arteritis. Kaplan-Meier survival analysis was performed to evaluate the duration of therapy, the incidence of prolonged remissions and relapses, and the average time to relapse. The log-rank test was used to test for significant differences between groups. Multivariate Cox proportional hazards regression models were used to identify variables associated with the occurrence of these events. RESULTS: Of 104 patients, 69 had pure PMR and 35 had both PMR and TA. Although some patients had limited disease requiring limited corticosteroid (CS) therapy (usually about 2 years), a significant number of patients had sustained disease requiring longterm treatment. Patients with simultaneous arteritis tended to have a longer duration of therapy, but no increased risk of relapse. By multivariate analysis, increasing age at diagnosis, female sex, higher baseline erythrocyte sedimentation rate, and lower daily CS dose were significant risk factors associated with long duration of therapy. No clinical feature predicted patients who were more likely to relapse. CONCLUSION: Although there was great individual patient variation, we found that typically CS therapy lasted at least 2 years. Our findings allow the identification of patients who are particularly predisposed to need prolonged and higher dose cumulative steroid therapy and merit preventive strategies to decrease the incidence of steroid related adverse events.
