[Cystic pyeloureteritis. Our approach].

We report a case of massive cystic pyeloureteritis that had been diagnosed by ureterorenoscopy in a patient with recurrent urinary infection and episodes of nephritic colic. The reports published in the literature indicate there is no specific treatment for this disease whose etiology is unknown. Its pathogenesis has not been well-established and it is difficult to distinguish from other urothelial filling defects. Although it has also been reported that it may progress to malignancy, we believe that the therapeutic approach should be conservative.

[Clinical experience of cefoxitin in the field of obstetrics and gynecology (author's transl)].

Cefoxitin was given to the 7 patients of infections in the field of obstetrics and gynecology, and the following results were obtained: 1) The clinical response was excellent in 2 patients, good in 4 and poor in 1 patient with the efficacy rate of 85.7%. Out of the 4 patients resistant to the previous therapy with other antibiotics, 3 patients responded to cefoxitin, and all the 3 patients of anaerobic infections responded satisfactorily to cefoxitin. 2) Microorganisms isolated were 2 strains each of E. coli and Staphylococcus aureus, 3 strains of Peptococcus and 1 strain of Eubacterium lentum. All the 8 strains isolated were sensitive to cefoxitin. As to bacteriological response, all the strains isolated were eradicated except 1 strain of Staphylococcus aureus which recurred on the 9th day after completion of the therapy with the eradication rate of 87.5%. 3) No subjective nor objective side effects were noted. Especially, the elevated GOT and GPT observed on a patient complicated with hepatitis prior to the initiation of cefoxitin treatment were found to be normal upon completion of the treatment.

[Pyeloureteritis cystica diagnosed by using a ureteroscope: a case report].

We report a case diagnosed as pyeloureteritis cystica by ureteroscopic examination. A 70-year-old female was admitted to our hospital with microhematuria. Intravenous pyelography and retrograde pyelography demonstrated multiple filling defects in bilateral renal pelvis and ureters. Urinary cytology findings indicated class III on both side. Ureteroscope and cold cup biopsy were performed, and histological examination revealed chronic ureteritis. Forty-four Japanese cases including our case are herein reviewed.

[Netilmicin in the treatment of infections of the lower urinary tract].

The authors treated 30 patients (11 females and 19 males) with cystitis or cystopyelitis. Patients were randomly assigned to one of the following treatments: a) netilmicin 200 mg daily, b) netilmicin 200 mg + ampicillin 1 g daily. Clinical and bacteriologic results were positive in all cases with resolution of clinical signs of infection and negative cultures at the end of treatment. Netilmicin alone yielded the same results as its combination with ampicillin. The safety, monitored also with netilmicin serum levels, was always good.

Dosage regimen of ranitidine in patients with renal impairment.

The validity of the Giusti-Hayton method for dosage regimen adjustment in patients with renal impairment was investigated using ranitidine. Five normal healthy volunteers received a single ranitidine recommended dose of 150 mg, and 20 patients with renal disease received single administration in doses individually calculated by the Giusti-Hayton method from each patient's creatinine clearance level, and the various pharmacokinetic parameters were compared. As a result, the area under the time-concentration curve 0----infinity (AUC0----infinity) in the renal disease patients was almost comparable to that in the normal healthy volunteers. The Giusti-Hayton method was considered useful to keep both the AUC0----infinity constant and the mean blood level of the drug in a steady state, in any degree of renal impairment.

[Therapeutic effect of ceftizoxime on infection in patients with lung cancer].

Ceftizoxime (CZX) was given in daily doses of 4 approximately 6 g by intravenous drip infusion to 30 patients with infection accompanying lung cancer to investigate the usefulness of the drug for infectious disease: The rate of effectiveness (marked and moderate) was 73.3% (22/30 patients). Of the 30 patients, 2 had drug fever; 1, arthralgia; and 1, eosinophilia. These side effects improved after the drug was withdrawn. CZX is a very useful antibiotic with high effectiveness and safety in immunocompromised patients with infection accompanying advanced lung cancer.

[Pyelocystitis in breeding sows].

Pyelocystitis results from ascending infection of the urinary tract with Corynebacterium suis in sows. Infection occurs regularly during coitus by healthy boars, which are often contaminated with Corynebacterium suis. Characteristic clinical signs of pyelocystitis are haematuria, anemia and emaciation. The whole urinary tract shows severe purulent-necrotic inflammation with progressive vascular lesions. Diagnosis is based on clinical, pathological and bacteriological findings. Distinct changes in urine samples are present. Treatment is only successful in sows with intact function of kidneys. In early detected cases treatment can be effective with repeated application of appropriate antibiotics, e.g. penicillin or ampicillin. There are no proven methods of prevention.
