[Treatment of psoriasis by UVB and PUVA phototherapy].

PUVA has definitely proved effective in outbursts of extensive psoriasis vulgaris. Efficacy is enhanced by combination with retinoids. The pharmacokinetics of psoralen, which lead to adjustment of dosage to each case, is a concrete improvement which facilitates treatment. The side-effects of PUVA, and notably the risk of carcinogenesis, are well known and can be alleviated. UVB phototherapy, successfully used for a long time against psoriasis, has benefited from technical advances in the field of artificial sources of UV. In its conventional modality (wide UVB band), the adjunction of greasy excipients is necessary for efficacy. SUP and narrow-band phototherapy (311 nm lamp), more recently developed, are of considerable interest but ought to be further evaluated.

Chronic eczematous dermatitis of the hands: a comparison of PUVA and UVB treatment.

The efficacy of PUVA and UVB treatment in chronic eczematous dermatitis of the hands was compared in a randomised controlled study including 35 patients who were randomly allocated to PUVA or UVB treatment. One hand was exposed to light and the other served as an untreated control. The dermatitis cleared on the treated hand in all PUVA patients, but in 9 out of 14 there was a relapse within three months. In the UVB group clearing of the skin lesions was not achieved, but compared with the "untreated" hands a statistically significant improvement was found at 12 weeks of treatment. A statistically significant improvement of "untreated" hands was found in both groups. PUVA and UVB treatments are alternative treatment modalities in patients with recalcitrant chronic eczematous dermatitis of the hands. PUVA is superior to UVB.

Epidermal Langerhans' cells in chronic eczematous dermatitis of the palms treated with PUVA and UVB.

Epidermal Langerhans' cells (LC) were studied in patients with chronic eczematous dermatitis of the palms. The monoclonal antibodies anti-Leu 6 and anti-HLA-DR were used, and the cells visualized with an immunoperoxidase technique. Increased numbers of LC were found in allergic contact dermatitis as well as in irritant contact dermatitis and hyperkeratotic dermatitis of the palms. The increased number of epidermal LC in lesional skin may facilitate presentation of exogenous or endogenous antigens to activated T-cells. One hand was treated with PUVA or UVB and the other hand served as a non-exposed control. PUVA treatment cleared the dermatitis and the LC number decreased markedly. With UVB treatment clinical improvement was achieved, and a less pronounced decrease in epidermal LC was noticed. There seems to be a crude relationship between the extent of clinical improvement and the reduction in epidermal LC numbers. The change in LC might be a primary event or secondary to a reduction of the inflammatory process.

[Psoriasis and day hospital. Clinical and therapeutic considerations concerning 104 cases].

One hundred and four patients with psoriasis, treated in regimen of Day Hospital, have been examined. The diagnosis was widespread plaque type psoriasis (67.3%), guttate psoriasis (18.3%) and nail psoriasis (1.0%). Furthermore psoriatic erythroderma has been observed in 2.9% and pustular psoriasis of Barber in 1.9%. 10% of the patients showed psoriatic arthritis. 71.2% of the patients have been successfully treated with UVB and PUVA irradiation therapy (respectively 86.6% and 70.0% of complete clearing or marked improvement). Etretinate and topical therapy were successful (respectively 67.0% and 70.0% of complete clearing or marked improvement). Day Hospital has proved to be the ideal regimen for the treatment and control of psoriatic patients.

[UV-B phototherapy in psoriasis].

67 patients, 41 with chronic plaque-psoriasis and 26 with acute eruptive or seborrhoic psoriasis, were treated in a stand-up unit equipped with UV-B fluorescent bulbs. 92 per cent of the latter group of patients showed satisfactory results (13 exposures, total UV-B dose: 464 mJ/cm2), whereas only 46 per cent of the patients with plaque psoriasis responded comparably (19 exposures, total UV-B dose: 994 mJ/cm2). A split comparison study of PUVA and UV-B on plaque-psoriasis revealed the superiority of PUVA in both initial treatment phase and duration of the remission. For certain types of psoriasis, UV-B phototherapy represents an alternative option. With regard to long-term hazards, however, as carcinogenicity in particular, UV-B has to be judged with the same scrutiny as PUVA.

Comparison of phototherapy (UV-B) and photochemotherapy (PUVA) for clearing and maintenance therapy of psoriasis.

One hundred eighty-three patients with psoriasis were treated with UV-B irradiation or oral methoxsalen plus longwave UV light (PUVA). Patients treated with PUVA, in the initial and maintenance period, achieved in general a higher therapeutic score (95% to 100% clearance) than those receiving UV-B therapy. However, taking 80% to 100% improvement as criterion, no difference was found between initial UV-B and PUVA therapy, if less than 50% of the skin surface was affected by psoriasis. If more than 50% of the skin was involved, PUVA was better than UV-B therapy. The maintenance treatment frequency for the UV-B-treated patients for more than a year seemed to be higher than for PUVA-treated patients. A positive correlation was found between response to sunbathing (questionnaire survey) and the response to UV-B phototherapy. An extra UV-B treatment to the leg lesions appeared useless.
