Cartilage growth inhibition and necrosis in vitro caused by prostaglandin A1.

This paper details experiments using the Fell technique of organ culture of 8-day chick embryo femoral and tibial rudiments to test the effects of prostaglandin A1 (PGA1) on cartilage growth. Growth was studied during 8 days in vitro by measurement of rudiment length and wet and dry weight, and by histology. PGA1 inhibited explant growth in a dose-related manner. Linear growth was significantly decreased by 20 and 25 microgram/ml PGA1 at 2, 4, 6 and 8 days, and by 15 microgram/ml at 6 and 8 days. Linear growth was unaffected by 1 and 10 microgram/ml doses. Weight measurements were significantly reduced by 25 microgram/ml PGA1 (2, 4 and 8 days) and by 20 microgram/ml (8 days). Chondroblast degeneration was caused by doses of 15, 20 and 25 microgram/ml PGA1. Progressive degeneration was seen at the 25 microgram/ml concentration after 2 days in vitro. Cellular changes as early as 27 h in vitro were seen using electron microscopy. Tritiated thymidine autoradiography confirmed reduced chondroblast proliferation in the presence of PGA1 (25 microgram/ml). The mechanisms of prostaglandin-induced changes in embryonic cartilage remain uncertain. The possible role of intracellular cyclic nucleotides in the reaction is discussed.

Rapid evaluation of the efficacy of pharmacologic agents and their analogs in enhancing bladder capacity and reducing the voiding frequency.

We report here the results of a simplified screening method to rapidly compare the pharmacologic action of drugs relevant to the urinary tract. This method avoids the use of anesthesia and of external infusion into the bladder relying on the physiological stimulation of the volume-evoked micturition reflex (VEMR) by diuresis. Mature female rats weighing 240-310 g were placed in restrainer cages that afforded access to food and water but limited movement. Under the rear of the rat, a collecting funnel and weight measuring device was secured. For each VEMR, the weight of the volume voided was recorded on a polygraph which also provided a record of the time of voiding. To evaluate the relative pharmacologic efficacy of the drug under study, 1 mg/kg furosemide along with the drug to be evaluated was diluted in 5 ml of saline and injected subcutaneously. Rats received approximately equimolar concentrations of thiphenamil and 15 other analogs, Ca2+ channel blockers, and K+ channel openers. The furosemide was given to obtain a controlled level of diuresis and avoid the effects of circadian variations in urine flow. Parameters considered were 1) mean volume voided per VEMR, 2) frequency, 3) output, and 4) latency. This model allows the rapid evaluation of drugs designed to increase bladder capacity and decrease the frequency of voiding, and it is particularly useful in evaluating the relative efficacy of drugs that are chemical analogs.

Mechanism of preferential enrichment, an unusual enantiomeric resolution phenomenon caused by polymorphic transition during crystallization of mixed crystals composed of two enantiomers.

The mechanism of Preferential Enrichment, an unusual enantiomeric resolution phenomenon observed upon recrystallization of a series of racemic crystals which are classified as a racemic mixed crystal with fairly ordered arrangement of the two enantiomers, has been studied. On the basis of the existence of polymorphs and the occurrence of the resulting polymorphic transition during crystallization from solution, the mechanism has been accounted for in terms of (1) a preferential homochiral molecular association to form one-dimensional chain structures in the supersaturated solution of the racemate or nonracemic sample with a low ee value, (2) a kinetic formation of a metastable crystalline phase retaining the homochiral chain structures in a process of nucleation, (3) a polymorphic transition from the metastable phase to a stable one followed by enantioselective liberation of the excess R (or S) enantiomers from the transformed crystal into solution at the beginning of crystal growth to result in a slight enrichment (up to 10% ee) of the opposite S (or R) enantiomer in the deposited crystals, together with an enantiomeric enrichment of the R (or S) enantiomer in the mother liquor, and (4) a chiral discrimination by the once formed S (or R)-rich stable crystalline phase in a process of the subsequent crystal growth, leading to a considerable enantiomeric enrichment of the R (or S) enantiomer up to 100% ee in the mother liquor. The processes (3) and (4) are considered to be directly responsible for an enrichment of one enantiomer in the mother liquor. The association mode of the two enantiomers in solution has been investigated by means of (i) the solubility measurement and (ii) the number-averaged molecular weight measurement in solution by vapor pressure osmometry, together with (iii) the molecular dynamics simulation of oligomer models. The polymorphic transition during crystallization has been observed visually and by means of the in situ FTIR technique and DSC measurement. Both metastable and stable crystals have been obtained, and their crystal structures have been elucidated by X-ray crystallographic analysis of their single crystals.

Polymerizable bis(2-ethylhexyl)sulfosuccinate: application in microemulsion polymerization.

A hygroscopic and polymerizable salt ([2-methacryloyloxy]ethyl trimethylammonium chloride) is used to ion exchange the sodium ion in AOT (bis[2-ethylhexyl]sulfosuccinate, sodium salt) to produce a polymerizable form of AOT, MDOS ([2-methacryloyloxy]ethyl trimethylammonium bis[2-ethylhexyl]sulfosuccinate). A partial ternary phase diagram of water, MDOS, and methyl methacrylate (MMA) was determined at room temperature (22 +/- 1 degrees C). A relatively large L2 domain is obtained, but this domain is smaller than that obtained with AOT. Microemulsion polymerization in this domain at 70 degrees C, using AIBN (azoisobutyronitrile) as an initiator, produces an optically clear copolymer solid domain nearly as large as the L2 domain. This interesting behavior contrasts with similar studies of Pavel and Mackay [Langmuir 2000, 16, 8528] using a polymerizable surfactant DDAMA (didecyldimethylammonium methacrylate) that produced a much larger L2 domain than MDOS but yielded a much smaller optically clear domain after thermally initiated polymerization. Thermogravimetric analysis indicates that optically clear composites obtained at an MDOS/MMA weight ratio of 1:4 and containing 5% water (w/w; weight % water in microemulsion) released the water in a transition commencing around 160 degrees C and continuing to 250 degrees C. Thereafter, the thermal decomposition was substantially impeded relative to poly(methyl methacrylate) as a control, which was due to the fire-resistant nature of the MDOS monomer. Molecular weight measurements indicate MDOS/MMA copolymers form substantially higher molecular weights as the proportion of MDOS increases. At a given radius of gyration, higher MDOS-containing copolymers exhibit higher molecular weights, suggesting a more compact structure with increasing MDOS.

Sex hormones and sexual function in obese men losing weight.

OBJECTIVE: To study the impact of a weight-loss program on sex hormones and sexual function among 38 middle-aged obese men (BMI >or=35 kg/m(2)). RESEARCH METHODS AND PROCEDURES: A randomized controlled clinical trial was conducted. The treatment group (n = 19) participated in a 4-month weight-loss program including 10 weeks on a very-low-energy diet (VLED) and 17 behavior modification visits. There was no intervention in the control group (n = 19). Both groups were followed for 8 months, i.e., 22 weeks after the active weight loss in the treatment group. The outcome measures (weight, sex hormones, sexual function, leptin, and metabolic variables) were obtained at baseline and at three time-points during follow-up. RESULTS: The mean weight loss in the treatment group was 21 kg at the end of the 10-week VLED. At the end of follow-up, the maintained weight loss was 17 kg of baseline weight. The control group was weight stable throughout the study. In the treatment group, increases in sex hormone-binding globulin, testosterone, and high-density lipoprotein-cholesterol, as well as decreases in insulin and leptin, were maintained until the end of follow-up, although with VLED, the level of several hormones and metabolic variables improved transiently during the rapid weight loss. There were no significant changes in the questionnaire scores on sexual function in either group. DISCUSSION: We conclude that obese men lose weight and increase their serum testosterone level on a weight-loss program with VLED and behavior modification. However, they do not change their sexual function scores.

Predictive value of early weight loss in obesity management with orlistat: an evidence-based assessment of prescribing guidelines.

OBJECTIVE: To assess the clinical usefulness of published guidelines for the use of orlistat, by studying whether weight loss >/=2.5 kg during a 4 week dietary lead-in period, and weight losses of >/=5% after 12 weeks and >/=10% after 6 months of drug therapy predict weight loss and risk factor changes after 2 years. DESIGN: A retrospective analysis of pooled data from 2 multicentre, randomised, placebo-controlled clinical trials with similar design. SETTING: Twenty-nine centres throughout Europe. PARTICIPANTS: Two hundred and twenty men and women (BMI 28-43 kg/m(2)) who completed 2 years of treatment. INTERVENTION: After a 4 week hypocaloric diet plus placebo, 2 years of treatment with orlistat 120 mg tid, plus a hypocaloric diet for the first year and a weight maintenance diet in year two. MAIN OUTCOME MEASURES: Weight loss and obesity-related risk factor changes. RESULTS: Weight loss >/=5% body weight after 12 weeks of diet plus orlistat therapy was a good indicator of 2 year weight loss, whereas weight loss of >/=2.5 kg during the 4 week lead-in and >/=10% after 6 months did not add significantly to the prediction of 2 year outcomes. Patients who lost >/=5% of their weight at 12 weeks (n=104, 47.3%) lost significantly more weight after 2 years than others: -11.9% (95% confidence interval (CI) -13.4% to -10.3%) vs -4.7% (-5.7% to -3.7%) (P=0.0001), and had significantly greater reductions in total cholesterol, LDL-cholesterol, triglycerides, glucose, insulin, and blood pressure. Among those who achieved >/=5% weight loss at 12 weeks, the overall health benefits were not significantly greater in patients who went on to lose >/=10% body weight at 6 months compared with those who did not achieve >/=10% weight loss by month 6. CONCLUSIONS: Of the criteria currently suggested for assessing response to orlistat treatment, weight loss of >/=5% at 12 weeks accurately predicts sustained improvements in weight and major risk factors at 2 years, while other suggested criteria are less useful.

Effects of AE0047 on cerebral ischaemia and oedema after middle cerebral artery occlusion in cats.

1. AE0047 is a new dihydropyridine calcium antagonist with protective effects against cerebral ischaemia and the occurrence of stroke in several animal models. 2. In the present study we investigated whether AE0047 would improve the reduced cerebral blood flow (CBF) and oedema formation in cats subjected to middle cerebral artery (MCA) occlusion and compared it for efficacy with other dihydropyridine calcium antagonists with different moieties, such as nilvadipine and nicardipine. 3. Middle cerebral artery occlusion reduced local CBF (ICBF), as measured by the hydrogen clearance method, while dry weight measurement showed that water content in the cortical tissues surrounding each ICBF measurement electrode had increased after 4 h ischaemia. 4. Both AE0047 (10 micrograms/kg) and nilvadipine (30 micrograms/kg), given intravenously 20 min after MCA occlusion, produced an approximate 10% hypotensive response and significantly increased ICBF in severely and moderately ischaemic regions, grouped according to the initial reduced flow values. However, nicardipine (5 micrograms/kg bolus followed by infusion of 3 micrograms/kg per min for 60 min) failed to mitigate the reduction in ICBF despite an increase in the ICBF of the contralateral cortex. In addition, AE0047 tended to prevent an increase in cortical water content in severely ischaemic regions, whereas water content in both nilvadipine- and nicardipine-treated groups tended to increase. 5. These results suggest that dihydropyridine calcium antagonists act differently on cerebral ischaemia and oedema formation in a manner dependent on their side-chain structures and that AE0047 effectively attenuates ischaemic brain damage without aggravating oedema.

[A 13-week oral repeated dose toxicity study of a new antineoplastic agent S-1 in rats].

S-1, an antineoplastic formulation of a fluorinated pyrimidine derivative containing tegafur (FT), CDHP, and potassium oxonate (Oxo) in a molar ratio of 1:0.4:1, was recently developed by Taiho Pharmaceutical Co., Ltd., with the aim of prolonging the effective plasma concentration of 5-fluorouracil (5-FU) over that produced by FT alone and reducing its dose-limiting gastrointestinal toxicity. As a part of the S-1 toxicity study, a 13-week oral repeated dose toxicity study and a recovery study using male and female rats was conducted. Doses of S-1 were adjusted to deliver 1.5, 5, and 15 mg/kg/day as doses of FT, and FT was given at 15 mg/kg/day. The following results were obtained. 1. In clinical observation, edema of the limbs and face or swelling of the auricle of the ear and an anemic appearance were observed in both sexes in the 15 mg/kg/day group as dose of FT. Subsequently, males in this group developed severe anemia, decreased spontaneous motor activity, emaciation, and subnormal skin temperature, and many males died. In the survivors, keratosis of the palm, sole, or tail was observed, with necrosis and loss of the tail tip in the severe cases. 2. Body weight gain was suppressed from about week 2 of treatment in both sexes in the 15 mg/kg/day group as dose of FT, and there was almost no weight gain after week 4-5. Food consumption was consistently less than the control value for males in the 15 mg/kg/day group as dose of FT throughout the treatment period. 3. No marked changes were observed in water intake and on opthalmologic examination. 4. In the fecal test for occult blood, a positive tendency was observed in both sexes in the 15 mg/kg/day group as dose of FT. 5. Urinalysis disclosed a slight increase in protein and decrease in sodium, potassium, and chloride in males, and an increase in protein in females in the 15 mg/kg/day group as dose of FT. 6. Hematologically, both sexes in the 15 mg/kg/day group as dose of FT showed decreases in red blood cell count, hemoglobin, and hematocrit, and increases in platelet count and fibrinogen, with a slight decrease in white blood cell count in males. 7. In the blood biochemical test, abnormal findings included increases in total cholesterol and free cholesterol, and decreases in non-esterified fatty acid and albumin in both sexes in the 15 mg/kg/day group as dose of FT. 8. In organ weight measurement, abnormal changes included a decrease in thymus weight in both sexes in the 5 mg/kg/day or higher dosage groups and a decrease in the testis weight in males and an increase in the liver weight in females in the 15 mg/kg/day group as dose of FT. 9. Histopathologically, both sexes in the 15 mg/kg/day group as dose of FT showed a decrease in the red pulp of the bone marrow, atrophy of the thymus, white pulp of the spleen, and testes. degeneration of the renal tubules, and ulcerative changes of the skin or oral mucosa. 10. The findings were unremarkable in the FT group. 11. During the recovery study, all the toxic effects tended to reverse. 12. The NOAEL of S-1 was estimated to be 1.5 mg/kg/day as dose of FT for both sexes.

A stylized computational model of the rat for organ dosimetry in support of preclinical evaluations of peptide receptor radionuclide therapy with (90)Y, (111)In, or (177)Lu.

UNLABELLED: The therapeutic effects of peptide receptor-based radionuclide therapy are extensively being investigated in rats bearing tumors. Both the dose to the tumor and the therapy-limiting dose to normal tissues, such as kidneys and bone marrow, are of interest for these preclinical studies. The aim of this work was to develop a generalized computational model for internal dosimetry in rats. METHODS: Mature rats were dissected and the relative positions, dimensions, and weights of all of their major organs were measured. A mathematic model was set up for the rat body and its internal organs to enable Monte Carlo radiation transport calculations to determine estimates for both tumor and organ self-doses as cross-organ doses for (90)Y, (111)In, and (177)Lu. The organs and body were mostly of ellipsoid shape with the axes given as the measured length, width, and height normalized to values that, together with the measured weights, are consistent with the recommended soft-tissue and bone densities. A spheric tumor of 0.25 g was positioned on the right femur. Calculations were performed with the Monte Carlo neutral particle transport code MCNP for the beta-emitters (maximum energy, 2.28 MeV) and (177)Lu (maximum energy, 0.497 MeV) and for the gamma-emissions from (177)Lu and from (111)In. The presented absorbed dose S values are used to calculate the absorbed dose estimates for the rat organs in a study on the biodistribution of (177)Lu-DOTA-Tyr(3)-octreotate (DOTA is 1,4,7,10-tetraazadodecane-N,N',N",N"'-tetraacetic acid). Three activity distributions were considered in the kidney: uniform in the whole kidney, in the cortex, or in the outer 1-mm-thick rim of the cortex. Isodose curves and dose volume histograms were calculated for the dose distribution to the kidneys. RESULTS: Depending on the activity distribution in the kidneys, the renal dose for (177)Lu-DOTA-Tyr(3)-octreotate is 0.13-0.17 mGy/MBq. CONCLUSION: The renal dose of 70-95 Gy for an injected activity of 555 MBq will likely cause radiation damage, although the higher amount of peptide with this activity may influence the dosimetry by partial receptor saturation. Dose volume histograms show that (111)In and (177)Lu are likely to have a higher threshold for renal damage than (90)Y.

[Cardiovascular risk factors. An audit of their detection in primary care].

After having found that the prevalences of hypertension, hypercholesterolemia, obesity and smoking were low in our adult population on care, we developed a study to investigate the causes of the problem and to introduce measures for its solution following the methodology of the medical audit. The indexes showed that the performance of preventive cardiovascular activities (PCVA) was low: blood pressure measurement 45%, blood cholesterol determination 23%, height and weight measurement 8% and questioning on smoking habits 40%. After the implementation of corrective measures, indexes of 79%, 76%, 71% and 56%, respectively, have been achieved. In addition, a defective recording of the detected risk factors was found: 87% in the clinical history record, 72% in the age and sex card; after the corrective measures, the rates were 98% and 91% respectively. The present study demonstrates the effectiveness of quality control measures carried out by the professionals responsible for care to improve its quality.

Carrying the burden of cardiovascular risk in old age: associations of weight and weight change with prevalent cardiovascular disease, risk factors, and health status in the Cardiovascular Health Study.

Measured weight in old age, reported weight at age 50 y, and weight change from age 50 y to old age were studied in association with prevalent cardiovascular disease (CVD), CVD risk factors, and health status in a population of 4954 men and women aged > or = 65 y in the Cardiovascular Health Study (CHS). Heavier weight (i.e., generally weight in the fourth quartile for the cohort) at age 50 y was more closely associated with prevalent CVD than was current weight, with these associations stronger in women than in men. Heavier current weight and heavier weight at age 50 y were associated with cardiovascular risk factors, including higher blood pressure, lower high-density-lipoprotein cholesterol, and higher fasting insulin. Heavier weight at both time points was related to mobility problems in both men and women and to lower current physical activity levels; among women, strong associations were also seen with lower education and current income. Remaining within 10% of reported weight at age 50 y was associated with better health status as measured by reported health, mobility difficulty, number of medications, and prevalent CVD in men. Paradoxically, most cardiovascular risk factors were lowest for weight losers despite an association of weight loss with poorer health. In this cohort of persons aged > or = 65 y, heavier weight was associated with CVD and CVD risk factors, suggesting that prevention of overweight may prove beneficial in improving cardiovascular risk in older persons. Weight stability from age 50 y to old age was associated with better health status than was weight gain or loss.

The epidemiology of recent involuntary weight loss in the United States population.

BACKGROUND: Although recent involuntary weight loss (RIWL) has been associated with mortality, no national studies described the prevalence among the general population, characteristics and long-term outcomes of people with RIWL. METHODS: The authors analyzed data from the NHANES II Mortality Study of 5838 individuals 50-74.9 years old who between 1976-1980 underwent a physical examination that included height and weight measurements, biochemical tests and responded to questions about involuntary weight loss within the past six months. Vital status was determined through 1992. Logistic regression was used to examine characteristics associated with RIWL and Cox proportional hazard modeling was used to measure associations between RIWL and mortality. RESULTS: 13.3% of the population reported RIWL with 6.9% reporting > or = 5% RIWL. Obese individuals were at significantly higher risk of RIWL of > or = 5% compared to those with BMI 19-24.9 (OR=1.57. 95% CI: 1.13, 2.18). Other significant risk factors for RIWL included; poor self-reported health, cancer, high white blood cell count, low albumin and low hemoglobin levels, age and current smoking status. RIWL of > or = 5% was significantly associated with mortality (RR=1.24, 95% CI: 1.01, 1.53). CONCLUSION: In summary, RIWL is fairly common among community-dwelling older adults, occurs disproportionately among obese individuals, is associated with characteristics of poor health and independently associated with mortality. These results indicate that RIWL needs to be considered an adverse health indicator even among obese individuals and despite the absence of several clinical indicators of disease.

Mandible and mandibular first molar tooth measurements in dogs: relationship of radiographic height to body weight.

Height measurements of the mandible and the mandibular first molar tooth were obtained from 234 dogs. Statistical analysis was performed to determine possible associations between these measurements and body weight, sex, age, and body condition. There was a statistically significant association between the mandible: mandibular first molar tooth height measurement ratio and body weight (p < 0.0001) in all body weight groups. Small dogs had proportionally larger mandibular first molar teeth relative to mandibular height compared with larger dogs. This relationship may contribute to an increased susceptibility in small dogs for periodontitis and subsequent tooth loss. There was no statistically significant association between the mandible: mandibular first molar tooth height measurement ratio and other parameters evaluated in this study.

Body mass index: a comparison between self-reported and measured height and weight.

BACKGROUND: Body mass index is used to measure obesity in individuals and to monitor trends in population obesity. Some observers use self-reported height and weight to assess body mass index; others use measured parameters. This paper reports on a study to compare body mass index when calculated from self-reported and measured heights and weights. METHODS: A randomized postal questionnaire survey and follow-up clinical measurement study were carried out in a geographically defined population in the rural South West of England; subjects were 6000 residents of Somerset health district aged 16-64 years selected from the Family Health Services Authority register. RESULTS: The response rate for the postal questionnaire was 57.6 per cent. A total of 73.3 per cent of responders agreed to clinical measurements; 84 per cent of measured volunteers had over-reported their height and 74 per cent under-reported their weight. The difference between body mass indices based on self-reported and measured values of height and weight is highly statistically significant for the whole population and for most age-sex and body mass groups. CONCLUSIONS: Self-reported heights and weights are unreliable and if used for monitoring health targets should be treated with caution.

Radiation sterilization and the wear rate of polyethylene.

The resistance to sliding wear was measured in bovine serum for two polyethylenes with molecular weights of about 10(5) and 10(6) that had been irradiated in air and nitrogen with gamma-ray dosages up to 20 Mrad. Molecular weight measurements were performed after irradiation as well. Wear generally increased with dosage and contact stress, becoming measurable in many cases only after a critical dose (or stress) was exceeded. The most significant effect noted was that the irradiation changed the pressure dependence of the wear rate. Thus, whether or not a sterilizing (or resterilizing) dose will measurably increase the wear depends on the contact stress and, therefore, on the specific application. The increase in wear rate appeared to be due to a combination of scission and oxidation, suggesting the practical advisability of radiation sterilization under an inert atmosphere, as confirmed by comparative measurements at the higher dosages.

Temporal integration and multiple looks, revisited: weights as a function of time.

This study tests the hypothesis that temporal integration for detection of tone bursts with various durations can be explained by optimally combining multiple looks of brief signal segments whose contribution to detection increases over time. Detectability was measured for signals consisting of six consecutive 25-ms, 1-kHz tone pulses presented in a 50-Hz-wide masker or in maskers consisting of seven 50-Hz-wide noises, one critical band apart, with either coherent or incoherent envelopes. The level of each signal pulse varied randomly around masked threshold according to a Gaussian distribution. The slopes of conditional psychometric functions--plotted in terms of d'2 as a function of the squared signal-pulse intensity for pulses in a particular temporal position--yielded estimates of the contribution to detection provided by each pulse. Results for three normal listeners showed a small, but significant, effect of the temporal location of the pulse. Multiple-looks predictions of temporal-integration functions based on the measured weights and on measured psychometric functions were compared to measured temporal-integration functions. For the single-band and incoherent maskers, the predicted temporal-integration slopes were reasonably consistent with those measured, but for the coherent masker it was not. Whereas no current theory can explain the very steep temporal-integration functions obtained in the coherent masker, the present results are not inconsistent with the multiple-looks hypothesis as an explanation for the decrease in threshold with increasing duration of signals presented in random maskers.
