A double-blind, controlled trial to ascertain the preventive effect of human interferon-alpha (Hu IFN-alpha) on upper respiratory viral infections was performed on children in a closed community. Drops of Hu IFN-alpha were instilled into the nasal cavity of 13 healthy children aged one to three years. Fourteen children were given placebos as controls. Administration of the interferon and clinical observations were carried out in the winter of 1980. Serological examination revealed that this was the period of outbreaks of influenza type A epidemics in the community. Clinical manifestations referable to influenza virus infection were milder in the interferon-treated group than in the controls. However, there was no significant difference in the serological responses of the two groups after infection with influenza virus type A.
The 14 outpatients transferred for surgery on the hips and legs also made significant gains, primarily in hygiene and sitting tolerance. Unfortunately, they required two to four times the period of hospitalization of similarly involved nonretarded children. This is an important consideration in the outpatient group, perhaps less so in the previously institutionalized child. In the entire group of patients, including those followed for less than five years, we have had three deaths. Two were due to pneumonia, two months and four months after the hip procedure. One was due to osteosarcoma of the operated femur four years postoperatively.

The various aspects of the placement of young children on medical indications are considered on the basis of a pediatric and psychiatric study carried out in the Indre et Loire French department. The aim of the study is not to criticize the personnel in charge of the placements or the quality of the institutions, but to show how a generally satisfactory practice may also entail unrecognized risks. At present, placements solely for medical indications are unusual, and the reasons for such placements are largely psychological and/or social. Supervision of placement decision should be entrusted to competent services, and all decisions should be taken according to the best interest of the children.

A survey was carried out in three orphanages in Jakarta for intestinal helminthic infections. Stool samples and anal swabs of 158 children were examined. The prevalences of intestinal helminthic infections in the Putra Utama, Muslimin and Van der Steur orphanages were respectively as follows: a lumbricoides 70.0%, 76.6% and 50.8%, T. trichiura 78.0% 93.6% and 70.5%, hookworm 20.0%, 12.7% and 3.2% and E. vermicularis 34.0% 29.8%, 59.0%. Treatment with Trivexan (100 mg of pyrantel pamoate and 150 mg of mebendazole), one tablet as a single daily dose for 3 consecutive days resulted in cure rates for A. lumbricoides 96.0%, 100% for T. trichiura 78.0%, 80.9% and 86.9% for hookworm 98.0%, 100% and 100% and for E. vermicularis 82.3% 92.9% and 97.2%. No side effects were observed, except in one child who complained of nausea.

A case of severely symptomatic giardiasis in a school for the mentally retarded prompted an epidemiologic survey of the institution. The rate of parasitic infection in the children were just under 50%. Multiple infections were common and one child harboured five different protozoa. The yield included known pathogens (Giardia lamblia, Metorchis conjunctus and Diphyllobothrium sp.), protozoa of potential pathogenicity (Dientamoeba fragilis) and other protozoa, the significance of which has yet to be determined. The prevalence of G. lamblia in the index ward was significantly higher than in a control ward matched for age and mobility of the children. The epidemiologic data suggested person-to-person transmission of G. lamblia within the institution. Recommendations for the control of protozoal intestinal infections in custodial institutions are presented.

BACKGROUND: To estimate the prevalence of serologic markers of hepatitis B (SMHB) among residents of an institution for social or mental handicapped children in Castellón, and its association with risk factors. METHODS: A cross-sectional study with determination of SMHB by radioimmunoassay. 98 of the 100 residents participate in the study. RESULTS: Prevalence of SMHB was 24.5% (95% confidence interval: 16.4%-34.1%). Seven residents were hepatitis B surface antigen positive with three hepatitis B e antigen positive. SMHB prevalence was 0 in infants, 18.8% in toddlers, 24.5% in schoolchildren, 11.1% in special education, and 76.9% in permanent residents. Logistic regression analysis found associations of SMHB prevalence with age (Odds ratio [OR] = 5.8, 95% CI: 1.1-29.0), old age at admission with protective character (OR = 0.6, 95% CI: 0.4-0.8), and duration of stay (OR = 1.2 95% CI: 1.0-1.3), adjusting by sex, mental deficiency, and hepatitis B (HB) determinants like surgery, blood transfusion, odontology, bites, and injections. CONCLUSIONS: A predominant transmission of HB in young ages is suggested. Vaccination against HB, hygienic measures, and periodic serologic studies of HB are essential to prevent this disease.
