Effects of a bovine herpesvirus-1 isolate on reproductive function in heifers: classification as a type-2 (infectious pustular vulvovaginitis) virus by restriction endonuclease analysis of viral DNA.

A bovine herpesvirus-1 (BHV-1) isolate (FI) from an aborted fetus was used to infect 9 heifers at various stages of gestation. Two heifers were inoculated IV on postbreeding day (PBD) 1, 7, or 14, and 3 heifers were inoculated in the sixth month of pregnancy. Plasma progesterone assays were used to monitor corpus luteum function in heifers inoculated during early pregnancy. Low progesterone values and infertility were seen in the 2 heifers inoculated on PBD 1. Luteal function remained normal in heifers inoculated on PBD 7 or 14. These 4 heifers inoculated on PBD 7 or 14 carried their fetuses to term, and their calves were free of BHV-1 infection at birth. Three heifers inoculated during the sixth month of pregnancy also carried their fetuses to term. Two calves were born alive, and BHV-1 was not isolated from nasal swab samples of either calf; the third calf was stillborn. Virus was not isolated from the stillborn calf's tissues, but BHV-1 was isolated from the placenta. Lesions were not detected in several tissues examined by light microscopy, and BHV-1 antigen was not detected by immunohistochemical examination of paraffin sections. Restriction endonuclease analysis of viral DNA was used to compare the FI virus to other BHV-1 isolates (Colorado-1, Iowa, and K22). On the basis of restriction endonuclease analysis, the FI isolate should be classified as a type-2 (infectious pustular vulvovaginitis) virus, specifically subtype a.

Pelvic abscess. A sequela of first trimester abortion.

First trimester therapeutic abortions were performed in 6790 patients from January 1, 1972, through December 31, 1973. The infectious morbidity during an 18-month concurrent time period which required readmission to the hospital was 0.52 per 100 women. Four of these women were ultimately diagnosed as having a pelvic abscess thought to be related to the procedure. The cause was probably related to undiagnosed or inadequately managed uterine perforation. Initial management should consist of systemic antibiotics and, if possible, surgical drainage via colpotomy. Two special precautions which should be taken are the use of antibiotics specific for Bacteroides fragilis and the investigation of the possibility of bowel injury. Laparotomy should be considered in unresponsive cases because of the possibility of bowel injury and the necessity of adequate surgical drainage even with appropriate antibiotic coverage.

Association between HIV-1 infection and miscarriage: a retrospective study. DIANAIDS Collaborative Study Group. Diagnosi Iniziale Anomalie Neoplastiche AIDS.

OBJECTIVE: To determine the role of HIV-1 infection on miscarriage, we compared the obstetric histories of a cohort of HIV-1-infected and uninfected Italian women. DESIGN: Retrospective study. METHODS: The study participants were women (with at least one reproductive event) with HIV-1 infection or HIV-1-negative sharing the same exposure modalities; all women were attending a network of 16 infectious disease units in 12 Italian cities. Trained interviewers used a standard questionnaire to collect information on obstetric history [i.e., number of pregnancies, pregnancy outcome (live birth, spontaneous or induced abortion) and time of occurrence of these events (i.e., year of birth)]. The association between spontaneous abortion and HIV-1 status at the time of pregnancy was evaluated. RESULTS: The analysis included 272 women and accounted for 480 pregnancies (217 in HIV-infected women, 132 in uninfected women and 131 in women with undefined HIV status) and 60 miscarriages (23 in HIV-infected women, 22 in uninfected women and 15 in women with undefined HIV status). We estimated an adjusted odds ratio of 1.67 between spontaneous abortion and HIV-1 infection. CONCLUSIONS: We observed a 67% increase in risk of spontaneous abortion among HIV-1-infected women compared with HIV-1-negative women. This result should be considered in the counselling and management of women with HIV-1 infection who are of reproductive age.

Syphilis intervention in pregnancy: Zambian demonstration project.

Despite availability of simpler serologic tests for syphilis and near cure with penicillin, unacceptably high prevalence of infectious maternal syphilis exist in many developing countries, including Zambia. It is the foremost risk factor for mid-trimester abortions, stillbirths, prematurity and morbidity and mortality among infants born with congenital syphilis in Zambia. An intervention project was conducted in Lusaka aimed at demonstrating the effectiveness of new health education methods and prenatal screening for syphilis in reducing the adverse outcomes during pregnancy. During pre-intervention phase, approximately 150 consecutive pregnant women from each of the three study and the three control centres were recruited when they presented in labour at the University Teaching Hospital. The intervention phase lasted for one year at the three study centres during which new methods of health education were introduced to improve early attendances during pregnancy. Also, on-site syphilis screening was performed twice during pregnancy and seroreactive women, and in many cases their sexual partners, were treated by the existing prenatal clinic staff. During the post-intervention phase the steps of pre-intervention phase were repeated to evaluate the impact of intervention. Overall, 8.0% of women were confirmed seroreactive for syphilis; there was no difference between the study and the control centres (p greater than 0.05). Fifty seven percent (132/230) of syphilitic pregnancies ended with an adverse outcome, that is, abortion (RR 5.0), stillbirth (RR 3.6), prematurity (RR 2.6) and low birth weight (RR 7.8). The overall risk of adverse outcomes due to syphilis was 8.29 (95% confidence interval 6.53, 10.53). The new methods of health education were effective and the percentage of women who had their first prenatal visit under 16 weeks of gestation improved from 9.4 to 42.5. Although screening and treatment during intervention was suboptimal, the adverse outcomes attributable to syphilis were reduced to 28.3%; this is almost a two-third reduction when compared with 72.4% of adverse outcomes at control centres (p < less than 0.001). The intervention is culturally and politically acceptable in Zambia. The cost of each prenatal screening is US$0.60 and of averting each adverse outcome US$12. In countries with high rates of syphilis, there is an urgent need for STD control and Maternal and Child Health (MCH) programmes to pool their resources together to revitalise the prenatal care.

High levels of congenital transmission of Toxoplasma gondii in longitudinal and cross-sectional studies on sheep farms provides evidence of vertical transmission in ovine hosts.

Recent research suggests that vertical transmission may play an important role in sustaining Toxoplasma gondii infection in some species. We report here that congenital transmission occurs at consistently high levels in pedigree Charollais and outbred sheep flocks sampled over a 3-year period. Overall rates of transmission per pregnancy determined by PCR based diagnosis, were consistent over time in a commercial sheep flock (69%) and in sympatric (60%) and allopatric (41%) populations of Charollais sheep. The result of this was that 53.7 % of lambs were acquiring an infection prior to birth: 46.4% of live lambs and 90.0% of dead lambs (in agreement with the association made between T. gondii and abortion). No significant differences were observed between lamb sexes. Although we cannot distinguish between congenital transmission occurring due to primary infection at pregnancy or reactivation of chronic infection during pregnancy, our observations of consistently high levels of congenital transmission over successive lambings favour the latter.

Fertility and reproductive choice in women with HIV-1 infection.

OBJECTIVE: To measure fertility and birth rates and to describe the reproductive histories of women diagnosed with HIV-1 infection in Australia. METHODS: The medical records of 294 women with HIV-1 infection in four states of Australia were reviewed. Expected fertility and birth rates were calculated using national statistics. RESULTS: In the study population, 152 (52%) women had at least one pregnancy prior or subsequent to HIV-1 diagnosis. At maternal HIV-1 diagnosis, 71 (24%) women had a total of 106 children aged under 15 years. During the study period, 246 women were aged 15, 44 years and 58 (23%) of these became pregnant after HIV-1 diagnosis. Women whose exposure to HIV-1 was injecting drug use were twice as likely to become pregnant and more likely to have multiple pregnancies than women who did not report injecting drug use. The annual general fertility rate was 30 per 10,000 compared with 63 per 10,000 for the Australian female population aged 15-44 years, and the birth rate in women with HIV-1 infection was one-half that of the general female population. Of pregnancies confirmed after HIV-1 diagnosis, 47% were voluntarily terminated, a rate more than double that of the general population. All multiple terminations were among women whose exposure to HIV-1 was injecting drug use. CONCLUSIONS: Fertility and birth rates among women with HIV-1 infection are lower than the general population and the rate of termination higher. The results of this study provide a basis for the management of women with HIV-1 infection who are considering pregnancy.

Memory B cells producing somatically mutated antiphospholipid antibodies are present in healthy individuals.

Antiphospholipid antibodies (aPLs) are associated with thrombosis and recurrent abortions during autoimmune pathologies, but they are also produced in healthy individuals and during infectious diseases. To analyze the possible links between physiologic and pathologic aPLs, it is of importance to characterize normal aPL production. We took advantage of the known tropism of Epstein-Barr virus (EBV) for B cells in general, and memory B cells in particular, during primary infectious mononucleosis (IMN) in 3 patients to get access to anticardiolipin (aCL)-producing B cells. Flow cytometry analysis of these cells showed that, depending on the patient, 10% to 60% of immunoglobulin M (IgM) aCL-producing B cells express the CD27 marker of memory B cells. Single cell sorting of aCL B cells, followed by single cell reverse transcription-polymerase chain reaction (RT-PCR) amplification of their immunoglobulin variable region genes, showed that some of these cells produce mutated forms of aCL antibodies, confirming their memory B-cell origin. Considering that, during primary IMN, EBV infects and expands already pre-existing memory B cells, we conclude that healthy individuals have a discrete pool of aCL memory cells able to produce mutated forms of antibodies. The implications of this new information are discussed in light of different hypotheses regarding the origin of aCL.

[Mycoplasmas and pregnancy. Preliminary study].

The prevalence of genital mycoplasmas was studied among 191 pregnant women followed up at Diaconesses Hospital, Paris. Ureaplasma urealyticum was recovered from 65% of patients, alone (54%) or in association with Mycoplasma hominis (11%). The relationship to pregnancy outcome, the effect of erythromycin treatment, the interest of biological markers of infection (mycoplasmal quantitation and serologic tests, C-reactive protein) and the role of other known genital pathogens were investigated. Vaginal infection with Ureaplasma urealyticum according to biological criteria (i.e. vaginal concentration greater than 10(3) CCU/ml or coisolation of Mycoplasma hominis) was significantly associated with an increased risk of: 1. premature rupture of the fetal membranes; 2. prematurity in cases of preterm labor. Erythromycin treatment was able to prevent only the risk of prematurity. On the other hand, we didn't show any influence of mycoplasmal colonization on birth weight, if expressed with regard to gestational age. An immune response to Ureaplasma urealyticum was demonstrated in 15% of patients; rare fourfold antibody rises were detected specially in the case of a second trimester spontaneous abortion and of an intrauterine death (both were untreated patients) and probably attest an infectious process. A randomized trial with a larger study sample must be undertaken to corroborate these preliminary data.

Early embryonic death in heifers after inoculation with bovine herpesvirus-1 and reactivation of latent virus in reproductive tissues.

Thirteen crossbred heifers seronegative for bovine herpesvirus-1 (BHV-1) were bred naturally to a seronegative bull. Eight heifers were inoculated with BHV-1, IV, on postbreeding day (PBD) 7 or 14. Viremia was detected in heifers 1 through 7, and virus also was isolated from nasal and vaginal secretions of heifers 2, 3, 4, 6, and 7. The pregnancy status of all heifers was monitored from PBD 14 to PBD 35 by determining plasma progesterone concentrations at 1- to 3-day intervals. Decreased progesterone values indicated that pregnancy was not maintained in BHV-1-inoculated heifers 2, 3, 4, 6, 7, and 8. The postbreeding interestrual period of these 6 heifers was normal or only slightly longer than would be expected in the absence of conception. All 5 noninoculated heifers were pregnant on PBD 35. Three to 4 months after acute infection, all BHV-1 inoculated heifers were treated with dexamethasone for 5 days and were euthanatized. Nasal and vaginal swab specimens were tested daily during dexamethasone treatment for excreted BHV-1, and reproductive tissues and adrenal glands were collected at necropsy for virologic tests and histopathologic examination. Virus reactivation was demonstrated in heifers 2 through 8. The BHV-1 isolations were made from adrenal glands of heifers 2, 3, 5, 6, 7, and 8, vaginal swab specimens of heifers 2, 3, 4, 6, and 7, and nasal swab specimens of heifers 2, 3, and 6. Only heifer 3 had virus in reproductive tissues; these isolations were made from ovary, infundibulum, and uterine tube, but not from endometrium.(ABSTRACT TRUNCATED AT 250 WORDS)
