Voluntary ingestion of Cortinarius mushrooms leading to chronic interstitial nephritis.

'Magic mushrooms' ingestion among the drug-using population has become a popular cheap way to get hallucinogenic effects which is not free of complications. One of these is acute renal failure related to Cortinarius genus intake. This one greatly resembles 'magic mushrooms' and confusion is possible for inexperienced collectors. We report the case of a young male ex-drug addict who developed acute tubulointerstitial nephritis after voluntary ingestion of Cortinarius orellanus. The clinical picture was preceded by a long latency period, had an insidious course without any data of hepatoxicity and evolved to a chronic state. Renal biopsy showed nonspecific histopathological findings. In summary, it is important to bear this possibility in mind when facing an acute tubulointerstitial nephritis of unknown origin in a drug-taking patient.

TGF-beta and regulation of interstitial nephritis.

TGF-beta1 has been implicated as a profibrotic growth factor in the bulk of published experimental work regarding the actions of this cytokine in kidney disease. Such investigations have spanned a methodologic spectrum from in vivo analyses to cell culture work with purified growth factors and analyses of gene expression. Important correlative work using clinical specimens has established the presence of augmented TGF-beta expression in renal diseases characterized by excessive sclerosis or fibrosis. While in the aggregate this information supports a compelling argument in favor of TGF-beta having a predominant effect to accelerate progressive renal failure, the cytokine clearly also demonstrates effects which would tend to abrogate renal injury. We provide a summary of published and new experimental data outlining immunosuppressive and 'renal-protective' actions of TGF-beta1.

Acute interstitial nephritis associated with the use of mesalazine in inflammatory bowel disease.

BACKGROUND: In the last 10 years several reports have linked oral 5-aminosalicylic acid (5-ASA) therapy to acute and chronic tubulointerstitial nephritis, but to our knowledge only 2 patients have been reported with terminal end-stage renal disease due to mesalazine (5-ASA). After 1 year of treatment with 5-ASA annual monitoring of serum creatinine is recommended. RESULTS: We report the development of end-stage renal disease in a patient 10 months after the last assessment of serum creatinine, which was normal at 71 micromol/l. CONCLUSION: Because of this dangerous side effect, we would like to recommend 6-monthly assessment of renal function.
