[Effect of immunization with K1-antigen of Escherichia coli on the course of experimental urinary tract infection in the rat].

The influence of the vaccination with an immunogenic K1-antigen of Escherichia coli on the course of an experimental pyelonephritis in rats due to Escherichia coli O2:K1:H3 was investigated. K1-antigen is a poor immunogen. Therefore a conjugate of K1-antigen with bovine serum albumin (BSA) was used. This K1-BSA-conjugate was immunogenic in preliminary experiments. The influence of a vaccination on the course of pyelonephritis may be dependent on the route of infection. Rats which are infected by a retrograde route shows significant less inflammated kidneys when they received a vaccination by K1-BSA-conjugate within the first six weeks of infection.

[Cystic pyelo-ureteritis apropos of 3 cases].

This anatomo-radiologic entity is rare. It presents diagnostic, pathogenic, therapeutic and prognostic problems which are discussed about three personal cases and the review of the literature.

[Cystic pyeloureteritis. Apropos of 40 cases].

A series of 40 cases of cystic pyeloureteritis (CPU) is reported, this affection being considered as rare since only about 150 cases have been described in the literature. The lesion is probably more prevalent than was once believed but many cases probably remain unrecognized because of the paucity of radiologic signs. Based on findings in these 40 patients, results of radiologic imaging are described, the technical precautions needed for its detection emphasized and differential diagnoses discussed, none of which presents insurmountable obstacles since these are more theoretical than real. Pathogenesis is thought to be an acquired particular form of urinary mucosa metaplasia associated with urinary tract lesions responsible for stasis or chronic inflammation. Treatment should be conservative because of the usual benign nature of the lesion, although exceptional but possible occlusion by the lesions necessitates a certain surveillance.

[The use of ceftazidime in infectious pathology].

The clinical applications of chemotherapy and antibiotic treatment are reviewed with specific reference to ceftazidime. The encouraging results obtained with this cephalosporin on a group of patients suffering from sepsis, higher and lower respiratory tract, biliary, bone and urinary tract infections are reported. The doctrinaire and practical use of this drug in single or multiple antibiotic treatment programmes is discussed.

[Laboratory and clinical studies on SM-4300 in the pediatric field].

The authors have carried out the laboratory and clinical studies of SM-4300 and obtained the following results. Serum level of IgG before and after administration of SM-4300 was observed in a case with agammaglobulinemia. The serum level reached the peak at 30 minutes after administration of SM-4300 and the half-life was about 25 days. SM-4300 enhanced the opsonic activity of human neutrophils to Pseudomonas aeruginosa in vitro. In substitution therapy against agammaglobulinemia, the clinical effect of SM-4300 was observed. Four patients with severe infections were treated with SM-4300 and antibiotics and evaluated. Clinical effects of SM-4300 in combination therapy with antibiotics were excellent in 2 cases, good in 1 case and poor in 1 case. The subjective and objective clinical side effects and abnormal laboratory findings were not noted.

Hematogenous Streptococcus faecalis pyelonephritis in the rat. A histologic, immunopathologic and bacteriologic study.

The demonstration of bacterial antigens in active pyelonephritis in man has been inconsistent. In this paper we have studied 110 rats with experimental pyelonephritis induced by a single intravenous injection of Streptococcus faecalis. The animals were studied at intervals up to 1 year; bacteremia, urine and renal parenchymal bacterial counts were monitored. In these animals it was observed that bacteriuria and positive bacterial cultures of renal tissue persisted up to 1 year in some rats. Bacteria and their antigenic products in small foci were detected by immunofluorescence in the pyelonephritic lesions. The highly focal distribution of the bacterial antigens in bacteriologically positive tissues of this model suggest the need for careful fixation technics, availability of large tissue specimens and careful control of immunologic factors. These specific requirements to detect bacterial antigens may preclude the practical study of human pyelonephritic kidneys.
