[Basic and clinical studies on cefminox in obstetrics and gynecology].

Fundamental and clinical studies on cefminox (CMNX, MT-141), a new cephamycin antibiotic, were carried out and the results were as follows. After the intravenous one shot administration of 1 g CMNX to 17 cases, serum concentrations, tissue concentrations in pelvic organs and protein binding capacities in serum were measured. Blood concentrations ranged between 100-200 micrograms/immediately after the injection and declined gradually. Half-life (T1/2) was 75 minutes. Tissue concentrations were 30-70 micrograms/g (tissue weight) 10 minutes after injection and declined. Half-life was 90 minutes. The average serum protein binding rate was 66.3 +/- 8.4%. Correlation between tissue concentration (Y) and free serum concentration (X) was expressed as Y = 0.42 + 7.14X. Correlation coefficient (r) was 0.80 (r = 0.80). Correlation coefficient in serum concentrations between HPLC and bioassay was 0.83 and that in free concentration was 0.97. In clinical studies, CMNX was administered to 19 cases of obstetric and gynecological infections. The patients were constituted of 3 with puerperal endometritis, 3 with pyometra, 3 with pelvic peritonitis, 3 with parametritis, 3 with adnexitis, 2 with BARTHOLIN's abscess and 1 with retroperitoneal abscess and vulva abscess. Overall clinical efficacy was 89.5% (17/19). Bacteriologically 26 strains were isolated, 6 strains of E. coli, 4 strains of B. fragilis, 2 strains of P. anaerobius and P. asaccharolyticus and each one of E. faecalis, S. epidermidis, N. gonorrhoeae, S. marcescens, P. maltophilia, A. calcoaceticus, K. pneumoniae, P. mirabilis, H. influenzae, S. intermedius, E. lentum and F. varium. Twenty of these 24 strains were eliminated after the administration, K. pneumoniae was decreased, E. faecalis, S. marcescens and P. maltophilia remained unaffected. No side effects were observed but 1 case showed mild elevation of transaminases which normalized 1 week after CMNX was stopped.

[Basic and clinical studies on cefminox in the field of obstetrics and gynecology].

Fundamental and clinical studies on cefminox (CMNX, MT-141), a new cephamycin antibiotic, were carried out and the following results were obtained. When CMNX was administered at a dose of 1 g by intravenous drip infusion taking 1 hour, the serum concentrations as high as 20 micrograms/ml and 21 micrograms/ml were detected even at 4 hours after administration for the uterine artery and elbow vein, respectively. From these results, CMNX was supposed to maintain in vivo concentrations high enough to inhibit 80 to 100% of the growth of bacteria such as S. aureus, E. coli, Klebsiella, Proteus and anaerobic bacteria, B. fragilis, which are the commonest clinical isolates in the field of obstetrics and gynecology. Clinically, in 7 cases of female genital infections (4 cases of parametritis, 1 case of intrauterine infection, 1 case of pyometra and 1 case of pelveoperitonitis), CMNX was administered at a daily dosage of 2 g in 2 divided doses as intravenous drip infusions taking 90 minutes each. The clinical results were excellent in 1 case, good in 3 cases and poor in 3 cases, with an efficacy rate of 57.1%. In 6 out of these 7 cases progressive carcinoma was present as the underlying disease. No side effects nor abnormalities in laboratory findings were observed in any of the 7 cases. These results suggest that CMNX is effective in the treatment of obstetrical and gynecological infections.

Efficacy and toxicity of estrogens commonly used to terminate canine pregnancy.

Three groups of bitches were treated with diethylstilbestrol (75 micrograms/kg) orally for 7 days (n = 12), estradiol cypionate intramuscularly once (22 micrograms/kg; n = 12), or estradiol cypionate intramuscularly once (44 micrograms/kg; n = 12). Treatments commenced during late proestrus (n = 4/group), the fourth day of behavioral estrus (n = 4/group), or the second day of diestrus (n = 4/group). All bitches were bred on alternate days throughout estrus to stud dogs of known fertility. Ovariohysterectomies were performed on day 25 of diestrus to diagnose pregnancy and to assess any pathologic changes in the uterus. Eleven bitches treated with diethylstilbestrol, 6 bitches treated with the low dosage of estradiol cypionate, and 4 bitches receiving the high dosage of estradiol cypionate were pregnant at the time of surgery. Ten of the bitches treated with estrogens during proestrus, 6 treated during estrus, and 4 treated during diestrus were pregnant. The serum concentration of progesterone in 2 bitches treated with the high dosage of estradiol cypionate decreased to less than 2 ng/ml by day 25 of diestrus, suggesting premature luteal regression. Diethylstilbestrol appeared to have little efficacy in terminating pregnancy. Estradiol cypionate appeared to have greater efficacy when administered during estrus or early diestrus; however, pyometra developed in 2 bitches treated with this estrogen during diestrus.

[Evaluation of the clinical effect and tissue distribution of cefpimizole in the field of gynecology].

Cefpimizole (AC-1370) was administered to 5 cases with uterine myoma before hysterectomy, and tissue distribution was determined. AC-1370 was also administered to 5 cases with gynecological infections. The following results were obtained. One gram of AC-1370 was administered from 43 to 299 minutes before hysterectomy, tissue distribution of AC-1370, such as ovary, oviduct, myometrium, cervix uteri, and portio vaginalis was showed the highest level (30.0 approximately 49.5 micrograms/g) at 43 minutes after administration, and these were 39.0 approximately 64.4% of the concentration in uterine arterial blood. Tissue concentration of AC-1370 was then gradually decreased following with the decreasing of the concentration in uterine arterial blood. AC-1370 was administered to 3 cases with pyometra, 1 case with Bartholin abscess, 1 case with adnexitis. The clinical efficacy was good in all 5 cases. Bacteriological study revealed that A. faecalis and E. coli were eradicated, but B. fragilis was persisted. No side effect was observed in all cases.

[Clinical use of ketamine-xylazine for anaesthesia in the cat (author's transl)].

Based on experiences from anaesthesia of approximately 7000 cats in a four year period the effects of a combination of 20--25 mg/kg Ketamine and 0.5 mg/kg Xylazine given i/m are described. In the present study the Xylazine has been applied in considerably lower doses, compared with previous reports on these drugs, and this change has reduced the unwanted side effects, without at the same time reducing the effect on the muscular tension and the psychical disturbances induced by the Ketamine. Ketamine and Xylazine were given in one injection after being taken in the named sequence and mixed in the syringe. Indication for anaesthetizing the cats were, besides routine surgery in the out-patient clinic, operations of weakened animals for pyometra, foreign bodies, intestinal invaginations with and without resection, removal of abdominal tumors and urolithiasis. Animals with impared liver function were not anaesthetized with these drugs due to the important role of liver metabolism in their excretion. In spite of the fact that the corneal and laryngeal reflexes normally persist, the combination of the two drugs allowed surgery in these organs after application of local anaesthetics as an extra precaution. Premedication with atropine has not been used routinely, and still only very few cases of increased salivation or vomiting have been observed. Aspiration has not been a complication and in the whole material, only 3 deaths have occurred, none of them with a specific post mortem finding besides shock. In these 3 cases the patient died later than 45 minutes after the injection and after ended surgery. Side effects ascribed to phenomena of interaction have not been observed. One cat was anaesthetized a number of times during pregnancy without any effect on the cat or its kittens. It is concluded, that the Ketamine/Xylazine combination, when mixed as prescribed gives a very safe and pleasant narcosis, and that side effects are minimized, if the corneas are moistened with an ophthalmic ointment and the patient is allowed to recover in dark and quiet surroundings.

Age, breed and seasonal patterns in the occurrence of ten dairy cow diseases: a case control study.

A total of 5,990 occurences of the diseases abomasal displacement, cystic graafian follicles, endometritis, hypocalcemia, ketosis, mastitis, metritis, ovarian hypofuction, pyometra and retained placenta were included in a case control study. The study was based on a hospital population of Holstein, Ayrshire, Guernsey and Jersey cows who were at least two years of age. The data were retrieved from computer storage of material abstracted for the Veterinary Medical Data Program. The transcription of data from original case records to these abstracts was examined as a source of error. An attempt was made to define each of the diseases by reference to random samples of the clinical records. Using the log-odds method, trends were noted for the youngest cows to be at increased risk of the reproductive diseases and for the Guernsey cows to be at increased risk of the uterine diseases. There was a tendency for peaks in disease occurrences in the winter (as opposed to summer) months.

[Fundamental and clinical studies on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

Fundamental and clinical studies on T-1982 (cefbuperazone), a new cephamycin antibiotic, were carried out, and the following results were obtained. When T-1982 was administered at a dose of 1 g by intravenous drip infusion for 30 minutes or 1 hour, the concentration in serum showed as high as 23.0 micrograms/ml or 25.0 micrograms/ml even 2 hours after administration. The concentrations in the genital tissues about 5 hours after administration ranged 1.2-45.6 micrograms/g for 30 minutes drip infusion and 0.9-26.8 micrograms/g for 1 hour drip infusion. From these results, T-1982 was supposed to maintain the in vivo concentration to inhibit 80-100% the growth of bacteria such as S. aureus, E. coli, Klebsiella, Proteus, S. marcescens and Gram-negative anaerobic bacteria, B. fragilis which were often isolated clinically in the field of obstetrics and gynecology. When T-1982 was administered at a dose of 1-2 g twice a day to 14 patients with female genital infection; 2 intrauterine infection, 2 pyometra, 7 pelveoperitonitis, 1 adnexitis, 1 adnexal abscess and 1 vaginal cuff abscess, the clinical results were excellent in 9, effective in 4 and poor in 1. The efficacy rate was 92.9%. No side effects nor abnormalities in laboratory findings were observed in any of the 14 cases. These results suggest that T-1982 has efficacy for the treatment of obstetrical and gynecological infections.
