The development of tubal obstruction and subsequent infertility is a major sequelum of upper genital tract infection with Chlamydia trachomatis; however, little is known about the pathogenesis of the infection. In this investigation, the authors present a detailed study of the progression of ascending chlamydial infection in female guinea pigs resulting from intravaginal inoculation of the Chlamydia psittaci agent of guinea pig inclusion conjunctivitis (GPIC). Isolation of chlamydiae from different tissues of the genital tract revealed definitive evidence for ascending infection that was not dose-related. By 7 days after infection, GPIC was isolated from the endometrium and oviducts of 78% of the animals. Pathologic changes analogous to those seen in human chlamydial disease, including polymorphonuclear, mononuclear, and plasma cell infiltration, were seen in the endometrium and oviducts, although not all isolation positive animals developed overt tubal disease. Long-term fibrosis, often in combination with hydrosalpinx, was noted in the mesosalpingeal tissue in 20% of the animals. Thus, the guinea pig:GPIC system represents a model for ascending chlamydial infection resulting from vaginal inoculation of normal guinea pigs that closely approximates the disease as seen in humans and can be used to study the pathogenesis of chlamydial genital infection.
In a randomised second look laparoscopic study, concomitant antibiotic--anti-inflammatory therapy for the treatment of pelvic inflammatory disease (P.I.D.) was studied. Six weeks after medical treatment results were evaluated during second look laparoscopy based on three parameters: residual inflammation, pelvic adhesions and tubal patency. The combination treatment resulted in decreased pelvic adhesion and tubal occlusion in severe forms of P.I.D. Anti-inflammatory agents combined with anti-biotics are not more beneficial than anti-biotics alone in mild and moderate forms of P.I.D. so combination therapy must be reserved for severe forms of P.I.D.

The diagnosis as well as the severity of acute pelvic inflammatory disease (APID) can be difficult to assess clinically. A study was undertaken to compare the accuracy of clinical findings (Gainesville classification) with laparoscopic findings in 28 patients with APID in terms of: (i) establishing the correct diagnosis; (ii) determining the severity of the disease; and (iii) obtaining specimens to establish the causative organisms. The results were as follows: (i) (a) clinical diagnosis of APID was confirmed by laparoscopic evaluation in 78.6% of cases; (b) clinical staging of APID correlated with laparoscopic findings in 82.6% of cases; (ii) severity of the clinical staging of APID correlated with that of laparoscopic staging; and (iii) polymicrobial cause, as reported by other investigators, was confirmed. Laparoscopic evaluation was of importance in eliminating the inaccuracy of clinical diagnosis in 21.4% of cases and in establishing the causative organisms so that optimal treatment could be given and the long-term after-effects of APID reduced.
