Racial differences in contraceptive choice: complexity and implications.

Previous research has failed to generate consensus about why black fertility has persistently exceeded that of whites in the United States. In an effort to shed light on this question, this article examines black/white differences in sociodemographic factors affecting contraceptive choice. Using data from the 1976 and 1982 National Surveys of Family Growth, we find a complex pattern of black/white differences. Not only does contraceptive choice vary by race, but the effects of such variables as age, marital status, and education also differ between blacks and whites. For example, compared with whites, black married women avoid coital methods, and compared with blacks, white women shift contraceptive behavior more as they change marital status. The complex nature of the racial differences in contraceptive choice are interpreted as reflecting differences in marriage patterns and trends.

The role of pregnancy-specific beta-1 glycoprotein (SP1) in assessing the human blastocyst quality in vitro.

The advent of new culture techniques resulting in more than 60% of embryos developing in vitro to the blastocyst stage suggests that blastocyst transfer in humans with its potential to increase in-vitro fertilization success rates could be a feasible option. Blastocyst quality markers, however, are required to ensure that an increase in numbers is not achieved at the expense of lowering quality. We have previously reported a morphology based method for grading blastocysts. The current study sought to determine whether the secretion of pregnancy-specific beta-1-glycoprotein (SP1) (a trophoblast product known to be associated with fetal well-being) by blastocysts of differing quality was reflected in the morphological grading. SP1 concentrations were measured in the culture medium of grade 1 (n = 19), grade 2 (n = 4) and grade 3 (n = 4) blastocysts as well as vacuolated morulae (n = 6) daily from day 1 to day 14. Cumulative SP1 concentrations secreted by blastocysts were significantly higher than those secreted by vacuolated morulae. However, SP1 levels could not distinguish between blastocysts of differing morphological grades. The inconsistent pattern of secretion suggests that at this early stage of development, SP1 secretion is probably not sufficiently defined to allow any differences in levels to be reflected by the morphological grading.

Evolving patterns of iatrogenic multifetal pregnancy generation: implications for aggressiveness of infertility treatments.

OBJECTIVE: Our purpose was to assess the changes in multifetal pregnancy generation as a function of utilization of ovulation stimulation and assisted reproductive techniques and to document the relative control of hyperstimulation between the methods. STUDY DESIGN: The methods of infertility treatment, number of fetuses, and outcomes of 220 patients referred for multifetal pregnancy reduction were compared over three time periods (1986 to 1989, 1991 to 1992, and 1992 to 1993). Clomiphene, human menopausal gonadotropin, and ovulation stimulation with urofollitropin were compared against gamete intrafallopian transfer, zygote intrafallopian transfer, and in vitro fertilization (assisted reproductive techniques). RESULTS: The proportion of multifetal pregnancies generated by assisted reproductive techniques has steadily risen from 26% in the first two time periods to nearly half in the last 2 years. However, the number and proportion of quintuplet and greater pregnancies from assisted reproductive techniques have steadily fallen while for ovulation stimulation the proportion has remained about one third. CONCLUSION: Despite considerably increased use and proportionate generation of triplet and quadruplet pregnancies, the incidence of quintuplets has fallen for assisted reproductive techniques while there has been no improvement for ovulation stimulation. Continued, greater vigilance, particularly in the use of human menopausal gonadotropin and urofollitropin, must be emphasized.

Field methods for the assessment of health and disease in pre-agricultural societies.

The few surviving pre-agricultural societies preserve the best available indication of human adaptation during more than 99% of the species' history. The field methods described allow collection of data that may explain why hunters and gatherers are physically small in the face of apparent plenty, what causes their death and how they control population growth. Observations of daily life, especially family, food and work, provide the context within which biological data can be interpreted. Computer-compatible event-recording systems make possible the collection and encoding of quantitative behavioural observations. A careful census permits characterization of fertility and mortality. Physical assessment, by medical history, physical examination and anthropometry, establishes the patterns of growth and development in the population and the prevalence of clinically recognizable diseases and nutritional disorders. If blood, urine or faeces can be collected, lightweight portable field-proven equipment and techniques are available for the collection, analysis and preservation of specimens for biochemical, nutritional, haematological and genetic determinations.

No advantage of laser-assisted over conventional intracytoplasmic sperm injection: a randomized controlled trial [NCT00114725].

BACKGROUND: Intracytoplasmic sperm injection (ICSI) is a component of infertility treatment often employed when conventional in vitro fertilization is unlikely to be successful. Despite good clinical results with ICSI, the procedure is typically associated with degeneration of a significant percentage (approximately 10%) of the treated oocytes. The cause of this degeneration remains unclear. Speculation that damage caused by oocyte compression during the injection procedure may be responsible has led to the development of a novel technique known as laser-assisted ICSI. This procedure involves drilling a small hole through the zona pellucida with a laser prior to sperm injection. Preliminary studies have suggested that laser-assisted ICSI may dramatically reduce oocyte degeneration rates. The objective of this study was to examine whether the reported benefits of laser-assisted ICSI could be verified on a larger, less-selected group of patients. METHODS: Oocytes retrieved from 59 patients scheduled for ICSI were randomly divided into equal treatment and control groups. Oocytes in the treatment group were inseminated by laser-assisted ICSI, while oocytes in the control group were inseminated by conventional ICSI. Outcome variables (oocyte fertilization and degeneration, embryo cell numbers and fragmentation on days 2 and 3, and compaction and blastocyst formation rates) were compared between treatment and control groups by paired-sample t-test. Subgroup analysis was performed according to zona pellucida and oolemma breakage patterns. RESULTS: No significant differences between treatment and control groups were observed for any of the measured outcome variables. However, fragile zonae pellucidae were associated with significantly poorer embryo quality, and fragile oolemmas that broke easily upon insertion of the injection needle were associated with a significantly higher oocyte degeneration rate. Nevertheless, there were also no between-treatment differences in clinical outcomes within these patient subpopulations. CONCLUSION: Contrary to previous reports based on smaller sample sizes, the results of this study suggest that there is no benefit of laser-assisted ICSI, either for the general population of ICSI patients, or for patients prone to zona pellucida or oolemma fragility.

Generalized linear mixed models in dairy cattle breeding.

Fitness and fertility traits of dairy cattle are of increasing importance and are often measured on a discrete scale. The development and application of generalized linear mixed models to the genetic analysis of these traits are reviewed. Because current genetic evaluation systems are predominantly based on animal models, the inferential challenges of highly parameterized generalized linear mixed models are discussed. Development and adoption of new methods for drawing appropriate inferences on dispersion parameters are essential. Recent hierarchical extensions have been proposed for generalized linear mixed models, allowing for complex dispersion patterns that accommodate heteroscedasticity and outlier robustness. Steady advances in available computing power have facilitated multiple-trait analyses involving continuous and discrete measures. Full Bayesian inference via the development of Markov Chain Monte Carlo methods will continue to allow even greater generality and dimensions in the genetic model.

Accurate identification of abnormal ploidy using an artificial intelligence model in preimplantation genetic testing.

STUDY QUESTION: Can ultra-low-coverage whole-genome sequencing (ulc-WGS) accurately identify abnormal ploidy during preimplantation genetic testing (PGT)? SUMMARY ANSWER: The artificial intelligence (AI)-based PGT-Plus model demonstrates high accuracy in ploidy detection, offering a cost-effective solution that enhances clinical utility of PGT. WHAT IS KNOWN ALREADY: The predominant PGT for aneuploidy can identify chromosomal aneuploidies but cannot determine ploidy status. Transferring embryos with ploidy abnormalities can result in miscarriage and molar pregnancy. On the other hand, in ART, fertilization is assessed by morphological pronuclear assessment at the zygote stage. However, it has a low specificity in the prediction of abnormal ploidy status and embryos deemed abnormally fertilized can yield healthy pregnancies. Accurately identified abnormal ploidy in PGT-A can resolve current limitations and expand the utility range of PGT-A. Several studies have identified ploidy abnormalities; however, they were mainly based on single-nucleotide polymorphism (SNP) arrays or needed to combine additional targeted-next-generation sequencing (NGS) information. Studies based on ulc-WGS remain scarce. STUDY DESIGN SIZE DURATION: The study consisted of two stages: methodology establishment and validation. An AI model, named PGT-Plus, was developed using 653 samples with known ploidy status, which was further validated using 792 different ploidy status samples. In the clinical application stage, the approach was used to analyse the ploidy status of 19 103 normally fertilized PGT blastocysts and 140 single pronucleus (1PN)-derived blastocysts collected between May 2022 and December 2023. All blastocysts were tested using trophectoderm biopsy and NGS. PARTICIPANTS/MATERIALS SETTING METHODS: The methodology is based on the ulc-WGS data. First, based on samples with known ploidy status: the heterozygosity rate of high-frequency biallelic SNPs, the likelihood ratio (LLR) of alleles was calculated under different assumptions ('both parental homologs' [BPH] from a single parent, 'single parental homolog' [SPH] from each parent, disomy, and monosomy) by leveraging allele frequencies and linkage disequilibrium (LD) measured in the 1000 genomes project database. Twenty-three continuous candidate features derived from heterozygosity rates and LLRs of chromosomes or selected windows were included to establish the ploidy prediction AI model. Gini importance analysis and multicollinearity mitigation was performed for feature selection, then the performance of Random Forest (RF), Support Vector Machine (SVM), and Logistic Regression for modelling was compared. Subsequently, the parameter optimization was performed based on the RF model. Ploidy constitution concordance was evaluated in known ploidy status samples. The frequency of abnormal ploidy in normal fertilized PGT blastocysts and 1PN-derived blastocysts (including conventional IVF and ICSI) was evaluated. MAIN RESULTS AND THE ROLE OF CHANCE: Eleven features were collected for model architecture compared to SVM and Logistic Regression; RF achieved superior performance for ploidy detection. The AI model achieved an AUC of 1 for genome-wide-uniparental diploidy (GW-UPD), 1 for triploidy, and 0.99 for diploidy. For the 792 validation samples, 99.5% of samples were successfully detected using the AI model, and the model showed 100% accuracy for ploidy classification. In the clinical application stage, out of 19 103 PGT samples, 19 069 were successfully analysed using the model, with 110 (0.57%) identified as having abnormal ploidy embryos. Among these, 12.7% (14/110) were identified as GW-UPD, and 87.3% (96/110) were triploid. Among 5563 diploid blastocysts transferred, 3478 clinical pregnancies were achieved. Subsequent ploidy analysis was performed for 217 spontaneous abortion and 935 prenatal diagnostic samples, and no abnormal ploidy was identified. Furthermore, of the 140 1PN embryos tested, 40 (28.6%) exhibited GW-UPD, 3 (2.1%) exhibited triploidy, and 97 (69.3%) were determined to be biparental and normally fertilized. Among the 97 biparental embryos, 46 were diploid, 11 were mosaic, and 40 were aneuploid. In terms of the insemination pattern, the percentage of abnormal ploidy in ICSI was significantly higher than in conventional IVF (P < 0.01, 37.1% vs. 2.9%, respectively). With full informed consent, 20 patients without euploidy from normal fertilization chose 1PN-derived biparental and diploid blastocysts to transfer, resulting in 10 clinical pregnancies and 9 ongoing pregnancies. LARGE-SCALE DATA: N/A. LIMITATIONS REASONS FOR CAUTION: Some rare ploidy abnormalities, such as polyploidy with an equal number of identical sets of chromosomes and ploidy mosaicism cannot be accurately identified. Moreover, the origin of abnormal ploidy was not identified due to the unavailability of DNA from both parents. WIDER IMPLICATIONS OF THE FINDINGS: The PGT-Plus AI model provides a ploidy evaluation method based on the conventional PGT-A data and integrates directly into standard PGT-A workflows. Clinical utility results suggest that the model is a valuable tool for identifying embryos with abnormal ploidy in PGT-A and rescuing normal diploid embryos from abnormally fertilized embryos. These findings demonstrate that PGT-Plus significantly enhances the diagnostic accuracy of PGT. PCR detection of Coxiella burnetii from different clinical specimens, especially bovine milk, on the basis of DNA preparation with a silica matrix.

For PCR detection of Coxiella burnetii in various clinical specimens we developed a sample preparation method in which silica binding of DNA was used. This method was found to be fast, easily performed with large numbers of samples, and equally sensitive for all of the specimens tested (livers, spleens, placentas, heart valves, milk, blood). The DNA preparation method described here can also be used as an initial step in any PCR-based examination of specimens. The procedure was tested with more than 600 milk samples, which were taken from 21 cows that were seropositive for C. burnetii and reportedly had fertility problems (and therefore were suspected of shedding the agent through milk intermittently or continuously). Of the 21 cows tested, 6 were shedding C. burnetii through milk. Altogether, C. burnetii DNA was detected in 6% of the samples. There was no correlation between the shedding pattern and the serological results.

Canine surveillance system for Lyme borreliosis in Wisconsin and northern Illinois: geographic distribution and risk factor analysis.

A seroprevalence survey for Borrelia burgdorferi was conducted among the healthy canine pet population in selected counties of Wisconsin and northern Illinois to determine the distribution of Lyme disease and associated risk factors. Information obtained for each dog included place of residence, Lyme disease vaccination status, history of travel and tick exposure, signalment, and medical history. Serum samples were screened by enzyme-linked immunosorbent assay and confirmed by an immunoblot procedure. Seroprevalence by county ranged 0-40%, with the highest estimates from west-central Wisconsin. The spatial pattern was significantly correlated with human incidence of Lyme disease and with abundance of the tick vector, Ixodes scapularis. A geographic information system (GIS) was used to integrate environmental data with the location of the residences of the dogs to determine environmental risk factors. Seropositivity among dogs was positively associated with increased tick exposure and time spent outdoors and negatively associated with vaccination against Lyme disease. Seropositivity was also associated with living in forested and urban areas, and on sandy, fertile soils. A canine surveillance system is a useful method for assessing the geographic distribution of Lyme disease, and in combination with a GIS, it can be effective in determining environmental factors associated with I. scapularis endemicity.

Surgical sterilization of free-ranging wolves.

The objective of the study was to determine whether surgical sterilization of both males and females in wolf pairs alters basic wolf social and territorial behaviors. Wolves were located from the air by snow-tracking methods and were tranquilizer-darted from a helicopter. Surgeries were performed either in a tent at the capture site or in a heated building in a nearby village. Six vasectomies and seven uterine horn ligations were performed in January and February of 1996 and 1997. Two females died: one likely related to the capture procedure, the other of a peritonitis unrelated to the surgery. One wolf had a litter. None of the wolves have shown changes in behavioral patterns. Surgical sterilization can be effective, but other, less invasive, fertility control techniques should be investigated.

A method of estimating adult mortality trends from widowhood and death distribution data.

"This paper is an attempt to find a procedure for estimating past trends in the rate of decline of adult mortality [using] data collected during a census or a single sample survey. It indirectly focuses on trends in adult death rates by looking at proportions not-widowed by age-groups and changes in these proportions caused by diminishing death rates. An equation has been developed which expresses the proportion not-widowed of the ever-married population in an age-group as a function of the current level of mortality in the population, a first marriage frequency distribution, and the past rate of mortality decline." The technique is applied to data from the 1974 Bangladesh Retrospective Survey of Fertility and Mortality.

Developmental competence of oocytes showing increased cytoplasmic viscosity.

BACKGROUND: The objective of the study was to investigate the developmental fate of oocytes with increased cytoplasmic viscosity as assessed by the persistence of the injection funnel after withdrawal of the ICSI pipette. METHODS: For this purpose, 1008 oocytes showing a characteristic injection funnel during ICSI were subdivided into two groups according to the oocyte's ability to restore its spherical shape within 2-3 min after ICSI. Fertilization and further development was evaluated in both groups. In addition, implantation and pregnancy rates were analysed. RESULTS: In the funnel positive cohort (group 1) significantly fewer oocytes degenerated after injection (P < 0.01) compared with oocytes without persistent funnel (group 2). However, at zygote stage, presence of a halo (P < 0.05) and a optimal pronuclear pattern 0 (P < 0.01) was increased in group 2. In addition, significantly fewer poor quality embryos were found in this group (P < 0.01). The number of good quality blastocysts but not blastocyst formation was increased in group 2 (P < 0.05). This resulted in an increased clinical pregnancy rate if embryos which derived exclusively from funnel negative oocytes were transferred (P < 0.05). CONCLUSIONS: Our data suggest that cytoplasm of higher viscosity delays development up to cleavage stage and impairs optimal development. Injection funnel persistence was found to be a negative prognostic marker of preimplantation development.

Fictive swimming motor patterns in wild type and mutant larval zebrafish.

Larval zebrafish provide a unique model for investigating the mechanisms involved in generating rhythmic patterns of behavior, such as swimming, due to the array of techniques available including genetics, optical imaging, and conventional electrophysiology. Because electrophysiological and imaging studies of rhythmic motor behaviors in paralyzed preparations depend on the ability to monitor the central motor pattern, we developed a fictive preparation in which the activity of axial motor neurons was monitored using extracellular recordings from peripheral nerves. We examined spontaneous and light induced fictive motor patterns in wild type and mutant larval zebrafish (4-6 days post-fertilization) paralyzed with curare. All spontaneous and light-induced preparations produced alternation of motor activity from side-to-side (mean contralateral phase = 50.7 +/- 7.0%; mean burst frequency = 35.6 +/- 4.7 Hz) and a progression of activity from head-to-tail (mean ipsilateral rostrocaudal delay = 0.8 +/- 0.5 ms per segment), consistent with lateral undulation and forward propulsion during swimming, respectively. The basic properties of the motor pattern were similar in spontaneous and light-induced swimming. This fictive preparation can be used in combination with conventional electrophysiological and imaging methods to investigate normal circuit function as well as to elucidate functional deficits in mutant lines. Toward this end, we show that two accordion class mutants, accordion and bandoneon, have alternating activity on opposite sides of the body, contradicting the hypothesis that their deficit results from the absence of the reciprocal glycinergic inhibition that is typically found in the spinal cord of swimming vertebrates.

Human sperm hyperactivation in whole semen and its association with low superoxide scavenging capacity in seminal plasma.

OBJECTIVES: To characterize the very vigorous type of motility observed in the semen of some infertile men and to compare the superoxide anion scavenging capacity of the seminal plasma from these men and that from normal men. SUBJECTS: Patients consulting for infertility related to sperm motility problems and men presenting as sperm donors. METHODS: Motility patterns and measurements of sperm motility parameters were evaluated by computer-assisted digital image analysis system. The superoxide anion scavenging capacity of seminal plasma was measured by inhibition of nitroblue tetrazolium reduction due to the superoxide anion generated by the combination xanthine plus xanthine oxidase. RESULTS: Spermatozoa from 9 of 68 semen samples with normal sperm concentration, morphology, and percentage of motility showed the typical motility patterns observed during hyperactivation (HA) and a significant level of HA (16% +/- 3%) as compared with those in semen (2.3% +/- 0.3%) from fertile volunteers. The superoxide anion scavenging capacity of the seminal plasma from men with sperm HA was 37% lower than that of control seminal plasma. CONCLUSIONS: Sperm HA can occur in whole semen and is associated with a low superoxide anion scavenging capacity in seminal plasma. It could be one of the causes for idiopathic infertility.
