[Expert assessment of "traumatic" lumbar intervertebral disk displacement].

A prolapse of a lumbar intervertebral disc as a consequence of trauma is rather often the subject of medical litigation. This paper shows the different aspects of federal and private insurance companies that insure accident consequences. Items hinting at a causal connection between trauma and prolapse are shown. A prolapse of a lumbar intervertebral disc can only very seldom be certified as being traumatic.

Correlation between nucleus zone migration within scoliotic intervertebral discs and mechanical properties distribution within scoliotic vertebrae.

Correlations between intervertebral disc degeneration and bone mass were investigated previously, but never on scoliotic patients. Using MRI measurements of intervertebral discs behavior and vertebral bone tomodensitometry, correlations between nucleus zone displacement within intervertebral discs and mechanical center migration within vertebral bodies were investigated in vivo on scoliotic patients. The protocol, performed on eleven scoliotic girls, was composed of a CT scan acquisition of apical and adjacent vertebrae followed by a MRI acquisition of the thoracolumbar spine. The displacement between the vertebral body centroid and inertia center was computed from the CT images and called the mechanical migration. The displacement between nucleus zones and vertebral body centroids was quantified from MRI and called the nucleus zone migration. For apical vertebrae, a significant correlation was found in the coronal plane (r = 0.766, p < 0.01), but not in the sagittal plane (r = -0.349, p > 0.05). For adjacent vertebrae, significant correlations were found in both coronal (r = -0.633, p < 0.05) and sagittal (r = -0.797, p < 0.01) planes. The nucleus zone migration occurred in the convexity of the curvature whereas the mechanical migration occurred in the concavity.Known secondary mechanical phenomenon of scoliosis was quantified using new parameters describing intervertebral discs and vertebral bodies. Further investigations should be performed to explain the mechanical evolution of scoliosis and to use these parameters in predictive criteria of scoliosis.

[Lumbar and cervical "traumatic intervertebral disk displacement"].

25 cases with traumatic lumbar and cervical disc herniation are described and criteria for a causal connection between trauma and prolapse are discussed: 1. The accident must be adequate, e.g. jumps from considerable height, falls, car collisions and lifting heavy weights (only in private insurance companies). 2. Typical discopathic complaints must occur soon after the accident. 3. Major degenerative disease before trauma of the vertebral column must be excluded. The medical expert has to assess in each case of possible traumatic disc herniation the cause of trauma and the resulting reduction of earning capacity.

[Etiology of the recurrence of lumbar intervertebral disk displacement].

In a review for reasons of 100 disc prolapses reoperations we found in 62% a recurrence of disc prolapse at the same level, in 24% a recurrence at a different level, whereas in 14% we found that the nerve route was closely connected. We reoperated 44% in the first two years and 69% in the first five years. The time between the operations was prolonged if there was a short period of time for relief of pain after operation and a long period of preoperative symptoms. The variables of risk of a disk prolapse recurrence are recorded in a risk score, in which 64% of the patients belonged retrospectively to a risk group. The correlation between operation interval and score points showed that patients with short interval had high score ratings.
