[Fundamental and clinical studies on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

Fundamental and clinical studies on T-1982 (cefbuperazone), a new cephamycin antibiotic, were carried out in the field of obstetrics and gynecology, and the following results were obtained. In a fundamental study, T-1982 was given to 7 patients with postpanhysterectomy at a dose of 1 g by intravenous drip infusion, and T-1982 concentrations in uterine artery and elbow vein serum and each uterine tissue (fallopian tube, ovary, endometrium, corpus uteri, cervix uteri and portio) were serially determined. The concentration in the endometrium (though the range of values was wide) was found higher than that in the other tissues. In a clinical study, T-1982 was given to 5 patients; 2 patients with parametritis and each one patient with right salpingitis, pyometra and metroendometritis. The efficacy rate was 80%. No side effects nor remarkable abnormalities in hematology, urinalysis and renal and hepatic function were observed during the therapy.

[Therapeutic effect of T-1982 (cefbuperazone) on the gynecologic infectious diseases].

T-1982 (cefbuperazone), a new cephamycin antibiotic with broad spectrum against Gram-positive, negative aerobic and anaerobic organisms, was clinically and bacteriologically evaluated on the gynecologic infectious diseases. Fourteen cases hospitalized at Kanazawa Medical University Hospital and the affiliated hospitals from October 1981 to July 1982 were treated with T-1982. By clinical symptoms, signs and bacteriological examinations the patients were diagnosed as pelveoperitonitis (1), intrauterine infection (3), adnexitis (3), infectious diseases of external genitalia (4), infectious abortion (1), vulvar hematoma (1), and rectovaginal fistula (1). T-1982 was administered intravenously or by drip infusion at a dose of 0.5-2.0 g twice a day after dissolved in a saline solution or a 5% glucose solution. Based on the improvement of clinical findings and antibacterial effect of T-1982, results were evaluated as excellent, good, poor and unknown. Clinical effects more than good were shown in 9 of 11 cases which could be followed up exactly on the therapeutic of T-1982. Two cases showing poor response were pelveoperitonitis and pyometra under carcinoma colli uteri stage IIIb, respectively. In 8 of 11 cases, antibacterial effect of T-1982 could be evaluated. Thirteen strains of aerobic and anaerobic bacteria were disappeared by T-1982 therapy, while no effect was seen on 5 strains. On the side effect of T-1982, exanthema was observed in 1 case but disappeared soon after the cessation of administration.

Estrogen-induced bone marrow depression in ferrets.

The administration of estrogen-induced severe bone marrow depression in 9 of 12 ferrets, independent of sex (male, female) or ovariohysterectomy. Resultant pancytopenia was manifested by subcutaneous petechiae, melena, hematomyelia, pale mucous membranes, pale bone marrow, centrilobular hepatic degeneration, hydrometra, and pyometra. These findings are compatible with the naturally occurring estrus-associated anemia seen in female ferrets.

[Fundamental and clinical studies on T-1982 (cefbuperazone) in the field of obstetrics and gynecology].

Fundamental and clinical studies were made on T-1982 (cefbuperazone) and the results were obtained as follows. Serum and uterine tissue concentrations of T-1982 were determined at 26 minutes before and at 20 to 80 minutes after the completion of intravenous drip infusion of 1 g. The levels of T-1982 in the fallopian tube, ovarium, endometrium, myometrium and uterine cervix were 23.7, 19.5, 50.5, 10.9 micrograms/g and 17.5 micrograms/g at 20 minutes after the completion of infusion. The levels were sufficiently effective against major pathogens (Gram negative bacteria and anaerobic bacilli) isolated in the field of obstetrics and gynecology. T-1982 was administered to 11 patients, including 7 of acute adnexitis, each one of pelveoperitonitis, pyometra, puerperal fever, and 1 of puerperal fever with sepsis, at a dose of 1-2 g twice a day for a period of 5 to 9 days by intravenous injection or intravenous drip infusion. Clinical responses were excellent in 5, good in 3 and poor in 3. No adverse reactions nor marked changes in laboratory findings were observed in any of the cases treated with T-1982.

[An experimental production of suppurative otitis media in dog, and a trial to evaluate the therapeutic effect of cefmetazole on this otitis media].

An experimental suppurative otitis media in dog was produced to evaluate the therapeutic effects of cefmetazole (CMZ) in this study. In order to induce a definite case of otitis media, 1.0 mg of dexamethasone was administered intramuscularly to dog, 12 hours before a bacterial inoculation into the middle ear of it. For inoculation, 5 x 10(8) live bacteria of S. aureus, strain 571 (UOP) was prepared from its 18 hour-bouillon culture and added 0.9 ml of 5% mucin solution for a total inoculation fluid of 1.0 ml. This fluid was injected into the middle ear on one side by inserting a 'mouse syringe' with a swollen tip down the external auditory meatus to pierce the tympanic membrane. On the opposite side, only 1.0 ml of micin solution was injected. In this way, suppurative otitis media which definitely lasted until the 15th day following the bacterial inoculation was realized. Since in each positive controls, the onset of suppurative otitis media had been confirmed on the 4th day after inoculation, the intravenous administration of CMZ (three times a day; 9 : 30, 15 : 30, 21 : 30) was started at the end of the 4th day and continued through the 7th day. The dogs thus treated were sacrificed on the 10th day after inoculation and the live bacteria count within the middle ear, signs of inflammation on the tympanic cavity, and accumulation of pus were examined. The therapeutic effect of CMZ was rated as 'effective' in cases where S. aureus-571 was found to be negative. All the animals administered a daily dose of 30 mg/kg or above it, were found to be S. aureus-571 free, thus the satisfactory therapeutic effect of CMZ was demonstrated. Especially, at the daily dose of 120 mg/kg the membrane of the tympanic cavity in all cases exhibited a picture of a radical cure to almost the same degree as the control side membranes, and any signs of inflammation were hardly observed. The last mentioned phenomena had been observed too by the authors on the treatment of the experimentally induced pyometra in bitches, with CMZ. Thus it is assumed that in treating some suppurative diseases with CMZ, it may be advisable to initiate the administration at relatively large doses in order to achieve generally better and quick effect.

Bitches, queens and prostaglandins.

Luteolysis can be obtained in bitches by prolonged administration of PGF2 alpha resulting in abortion in pregnant bitches. A prostaglandin analogue (TPT) can induce fetal resorption or abortion depending upon the time of administration. In an investigation comprising a few bitches, PGF2 alpha has given encouraging results for treatment of uterine disorders. In the queen prostaglandin can induce abortion and parturition, and it has, when administered during a long period, been used for treatment of pyometra. Irrespective of the preparation and dosage the administration of prostaglandin is accompanied by side effects lasting for up to 3 hours, and therefore further investigations are needed before prostaglandins can be recommended for use in bitches and queens.

[Endometrial cytology by endocyte and by Masubuchi's aspiration technic --a comparative study--].

Lately there is a tendency for an increase in the incidence of endometrial cancer both in this country and other industrialized countries such as the United States. The routine cervicovaginal smear method is generally poor for the detection of endometrial carcinoma as compared to that used for the diagnosis of early cervical neoplasms. The principal reason for the poor detection rate is that the cells are not obtained directly from the lesion as is the case with disease of the cervix. Various techniques for obtaining endometrial cells directly from the lesions have been described, but none has enjoyed wide acceptance by the clinicians. The patients for this study consisted of 138 women who were referred to us for diagnostic curettage. We employed the new disposable device, the Endocyte, and Masubuchi's endometrial aspiration and compared the results. After obtaining specimens with both methods curettage was performed on all patients. In addition, the Endocyte was employed on 8 patients with endometrial carcinoma in another hospital, and the following results were obtained. 1. With the Endocyte it is possible to obtain a larger volume of cells than by aspiration. 2. Although a certain amount of difficulty is encountered in the insertion of the Endocyte in some cases, it is much easier than the brush method used heretofore. 3. No significant difference in the amount of bleeding after insertion could be observed between the two methods. 4. In patients with bleeding in the endometrium and pyometra it is easier to obtain specimens from the endometrium with the Endocyte than by aspiration. 5. With the Endocyte contamination of the cells of the cervix was less than after aspiration. 6. Although pain during collection of cells with the Endocyte was somewhat stronger than during aspiration, analgesics were not necessary in any one of the cases. 7. The Endocyte was used on 13 patients with endometrial carcinoma, and all specimens were positive for carcinoma. From the results of our study we conclude that for endometrial cytology the Endocyte is as useful as the aspiration method. Furthermore, it should pose no difficulty to the gynecologists to use the Endocyte because the Endocyte resembles IUD structurally.

[A gonadoblastoma in a true hermaphrodite. A case history (author's transl)].

The gonadoblastoma has been found in patients with gonadodysgenesis of a pure or mixed variety or who have male pseudo-hermaphroditism. This gonadic tumour is rarely found in true hermaphroditism. This case history would seem to be the first that has been reported in France. The patient had been followed since birth because of having an intersex state with doubtful external genital organs and male internal genital organs consisting of a testis on the left side and feminine ones consisting of an ovary on the right. The caryotype was 46 XY. The gonadoblastoma was discovered at the age of 18 following a hysterectomy with castration which had become necessary because of pyometra and a tubal abscess. The discovery was a chance one, as there had been no clinical sign suggestive of it. Since a gonadablastoma is potentially malignant this finding highlights the problem of management of the gonads in such patients when the caryotype includes the Y chromosome.

[Postmenopausal endometrial tuberculosis, A report of 2 cases and a literature survey].

Postmenopausal endometrial tuberculosis is an uncommon condition. Two patients with this disease, 1 of whom also presented with a pyometra, are described. They were the only 2 patients with this disease out of 21742 patients admitted to the Department of Obstetrics and Gynaecology, Tygerberg Hospital. Both were treated with standard antituberculosis drugs for 9 months. No further episodes of postmenopausal bleeding occurred during or directly after treatment, and a follow-up hysteroscopic examination showed a completely atrophic endometrial lining.

Evaluation of the use of intrauterine infusions of oxytetracycline, subcutaneous injections of fenprostalene, or a combination of both, for the treatment of retained fetal membranes in dairy cows.

Dairy cows (n = 207) that had retained fetal membranes for more than 8 hours after parturition were randomly assigned to 1 of 4 treatment groups (group 1, daily intrauterine infusion of oxytetracycline; group 2, daily intrauterine infusion of oxytetracycline and a single SC injection of fenprostalene; group 3, a single SC injection of fenprostalene; and group 4, untreated controls). Cows were monitored daily. Subsequent disease conditions were recorded, and appropriate treatments were initiated. Interval from parturition to expulsion of fetal membranes was unaffected by treatment regimen. The frequency of cows with high rectal temperatures (> or = 39.5 C) was significantly (P < 0.05) less in cows treated with oxytetracycline infusions; however, the frequency of displaced abomasum, ketosis, and mastitis was unaffected by treatment method. Treatment method appeared to influence the development of postpartum uterine pathologic conditions. Treatment with oxytetracycline and fenprostalene (group 2) increased (P < 0.05) the frequency of pyometra. Treatment with fenprostalene (groups 2 and 3) decreased the number of cows with a palpable uterine lumen detectable during per rectal palpation at 28 to 42 days after parturition. Reproductive efficiency, as determined on the basis of the interval from parturition to conception, was unaffected by treatment protocol.

Tuberculous endometritis in hills of Darjeeling: a clinicopathological and bacteriological study.

Combined histological and bacteriological investigations of 800 specimens of nonpregnant endometrial curettings of 15 to 60 years age group of hill women of Darjeeling District were carried out for detection of tuberculous endometritis. The principal complaints were infertility (47.5 per cent), abnormal uterine bleeding (30.75 per cent), amenorrhoea (11.25 per cent), leucorrhoea (6.25 per cent), and miscellaneous conditions (pelvic pain and pyometra) (4.25 per cent) cases. By histological examination alone, only 10.9 per cent cases could be diagnosed while by combined study the incidence rate was 11.8 percent, an increase in the diagnostic acumen by more than 10.3 per cent. Bacteriological study was of greater value in doubtful cases where there was absence of tuberculous granuloma or epithelioid cell but presence of nonspecific inflammatory cells along with variable degree of necrosis of glandular epithelia. The incidence of M.tuberculosis was 97.7 percent while that of atypical mycobacteria was 2.3 per cent. Thus simultaneous use of culture and biopsy yielded better results. Our prevalence is a little higher than other reports from India. In cold weather at a high altitude, the tubercle bacilli survive longer in fomites which serve as important sources of infection in Darjeeling. Women of third decade are more frequently affected (43.2 per cent).

[Pseudomonas aeruginosa. II. Occurrence in blue foxes (Alopex lagopus) in a Danish fur farm].

Ps. aeruginosa were demonstrated fairly often in samples from the rectum (9,1%) and external genitalia (3,7%) of clinically healthy blue foxes. These animals are considered carriers and thus constitute a risk of infecting other animals, especially those already debilitated. The same organism was isolated as pure cultures from the intrauterine contents in a case of pyometra with septicaemia and fatal outcome and in two cases of puerperal endometritis. In these 3 cases there may have been a question of "superinfection" with this latent pathogen.

Ovulation without cervical stimulation in domestic cats.

Progesterone was measured by radioimmunoassay of serum collected at monthly intervals for 9 months (April-December) in 24 adult female American short-hair cats (age 2.5-11 years, mean 7.4 years); 20 cats were intact, and four were ovariohysterectomized controls. One of the 20 intact queens was ovariohysterectomized after 7 months, when pyometra was diagnosed. Cats could see and hear one another, and could see and hear male cats housed individually in the same room. Direct contact with other cats was prevented. Tactile stimulation of the cats' hindquarters and perineal regions by handlers was avoided. Serum progesterone concentration > or = 4.8 nmol l-1 was defined as evidence of ovulation. This concentration was exceeded in seven of 20 intact queens (35%) at one or more occurrences of non-coital ovulation; there were 13 such occurrences in all (1-3 per queen). Serum progesterone concentration ranged from 0.2 to 103.4 (mean 14.09 +/- 2.0) nmol l-1 in these seven cats, and was significantly greater than concentrations in the other intact and neutered cats. In the remaining 13 intact and four ovariohysterectomized cats, serum progesterone concentrations ranged from 0.2 to 3.2 (mean 1.24 +/- 1.46) nmol l-1. These data suggest that, at least as far as cats housed in proximity to one another are concerned, intact female cats do not always require cervical stimulation to induce ovulation.

[Clinical significance of magnetic resonance imaging (MRI) in evaluation of the extension of uterine cervical cancer].

Magnetic resonance imaging (MRI) was performed in 62 patients with uterine cervical cancer and the preoperative MRI findings were compared with the pathological findings following surgery. The surgical stages of 62 patients were 2 at stage 0, 18 at stage Ia, 19 at stage Ib, 9 at stage IIa, 11 at stage IIb, 2 at stage IIIa, and 1 at stage IIIb. The MRI findings in the present study included: (1) the existence of a high intensity area (HIA) in the uterine cervix or minimum thickness of residual normal cervical tissue, (2) necrotic cavity, (3) pyometra, (4) irregular margin of the cervix, (5) parametrial invasion, (6) vaginal invasion, (7) bladder invasion, and (8) lymphnode enlargement. No HIA was observed in any patient with stage 0 or stage Ia, in 8 patients with stage Ib, and in 2 patients with stage IIa, while all patients with cancer tissues greater than 10mm in diameter had a HIA. In 39 patients in whom hysterectomy was undergone without conization, the findings of MRI correlated significantly (r = 0.929, p < 0.001) with the minimum thickness of residual normal cervical tissue by the pathological measurement. When the irregular margin of the cervix was regarded as a disruption of the cervical myometrium by cancer tissue, accuracy was 87%. Furthermore, the degrees of accuracy for parametrial invasion, vaginal invasion and bladder invasion were 92%, 90% and 94%, respectively.(ABSTRACT TRUNCATED AT 250 WORDS)

[Pharmacokinetic and clinical studies on DQ-2556 in the gynecological field].

Pharmacokinetic and clinical studies on DQ-2556, a new cephem antibiotic, in obstetrics and gynecology were performed and following results were obtained. Concentrations of DQ-2556 were determined in serum, internal genital organs and retroperitoneal exudate after single intravenous administration (i.v.) or drip infusion (d.i.v.) of 1.0 g. Serum levels following i.v. were approximately 30 micrograms/ml at 1 hour, 14 micrograms/ml at 3 hours 30 minutes, and concentrations in internal genital organs including oviduct, ovary, endometrium, myometrium, cervix uteri and portio vaginalis reached approximately 50% to 70% levels of serum concentration. The mean concentration (n = 6) in the retroperitoneal exudate after d.i.v. of 1.0 g following radical hysterectomy were about 20 micrograms/ml, 23 micrograms/ml, 14 micrograms/ml and 8 micrograms/ml at 1 hour, 2 hours 30 minutes, 4 hours 30 minutes and 6 hours 30 minutes, respectively. In clinical trials, DQ-2556 (2.0 g b.i.d. for daily dose) was given in 5 patients with gynecological infections such as pyometra (1 case), salpingitis (1), retroperitoneal space infection (2), pelvic peritonitis (1). The clinical results were evaluated as good in 3 cases and poor in 2 cases including a case with salpingitis infected by Pseudomonas aeruginosa and the other with pelvic peritonitis caused by Methicillin-resistant Staphylococcus aureus. Bacteriologically, 11 organisms were isolated from patients, and eradication rate was 54.5%. Neither side effect nor abnormal laboratory test result was observed. Thus, DQ-2556 appears to be effective for gynecological infections, and the good results were supported by good penetration of the compound into tissues of internal genital organs and retroperitoneal exudate after i.v. or d.i.v.

The oral administration of megestrol acetate to postpone oestrus in cats.

During 1974, megestrol acetate tablets were distributed to 397 cat owners in the Oslo district for use as an oestrus suppressing agent in cats. A weekly dose of 2.5 mg megestrol acetate was given for at least 30 weeks. Treatment was carried out according to plan in 244 cats. Results are shown in Table I. The course of birth was abnormal in two cats which were pregnant before the trial began. Pregnancy was not registered in any of the remainder. Heat was observed during the period of treatment in 21 of the 244 cats treated according to plan without pregnancy being registered. As regards side effects of the preparation, increased appetite, weight increase and changes of temperament were the most pronounced. One cat developed pyometra after three years of treatment with the preparation.

[Plasma viscosity in the dog].

Plasma viscosity (PV) was examined in healthy (n = 122) and sick (n = 58) dogs. The mean value of PV in healthy adult dogs is 1.22 mPas. PV is remarkably increased in case of illness. In dogs with chronic heart failure the mean value was 1.46 mPas. Patients suffering from uraemia or pyometra revealed mean values of 1.90 mPas and 1.96 mPas respectively. An elevation of the PV is correlated with an increase of the "acute-phase-proteins". The examination of PV is useful for a better assessment of microcirculation. The PV is a parameter, which is important in diagnostics as well as in follow-up during special therapeutical procedures.

Pathological effects of dietary zearalenone and/or tamoxifen on female mink reproductive organs.

The efficacy of dietary tamoxifen (TAM) to alleviate the hyperestrogenic effects of the mycotoxin zearalenone (ZEN) was assessed by pathologic examination of the reproductive organs of female mink (Mustela vison). Mink were fed 20 mg/kg ZEN, 10 mg/kg TAM, or 20 mg/kg ZEN + 10 mg/kg TAM from about 2 mo prior to breeding until the kits reached 3 w of age. All female mink fed ZEN mated, but only 25% whelped. No mink fed TAM or TAM + ZEN mated. Postmortem examination revealed moderate to severely distended uteri filled with caseated necrotic substances in the TAM, ZEN and ZEN + TAM fed mink. Histologic examination revealed mild to severe endometrial hyperplasia to uterine atrophy, endometritis, metritis and pyometra. Ovarian follicles were atrophied and degenerated. TAM was not effective in alleviating the hyperestrogenic effects of ZEN but was a potent estrogen agonist in mink.
