[The possibilities for organ-preserving operations on the spleen].

Experience had with application of spleen salvaging operations in adult patients is shared. They are used traumatic lesions of the spleen in eleven patients, employing the method of Delany, as modified with the Bulgarian polyamide meshwork Ampoxen. In two of them the hemostasis fails, and splenectomy is resorted to. Preservation of the spleen is accomplished in one patient with chronic pancreatitis where left pancreatectomy with spleen preservation according to the technique of Pradere et al is done. In another female patient, presenting echinococcus of the spleen, echinococcectomy with preservation of the organ is performed. Based on updated knowledge of the role played by the spleen in promoting protection against infection, the strive for its salvage whenever possible is fully justified.

[Malignant tumors of the liver in children].

The treatment results of 65 children with malignant tumors of liver for the 1972-1992 yrs period were analyzed. Hepatocellular cancer was diagnosed in 31, hepatoblastoma--in 23, poorly differentiated sarcoma, angiosarcoma, cancer-sarcoma--in 11. Operations were conducted in 38 (58.5%) patients, of whom radical--in 22; the right-sided hemihepatectomy--in 14 (in expanded and expanded-combined), left-sided--in 2, nonanatomical resection--in 6. Laparothoracophrenotomy was done in 8 children with massive tumor for the right-sided hemihepatectomy conduction (in 2--with diaphragm resection and vast defect plasty with the help of a capron mesh). Of 22 radically operated children 4 (18.2%) died: after the right-sided expanded hemihepatectomy conduction--3, nonanatomical liver resection--1. One more child with a massive tumor died during mobilization of the liver right lobe. Of 18 discharged children 12 died during following 18 mos, 6 survived. Five-year survival was 23.5%.

Nylon-6 cylinders and a sponge-like derivative as supports for immobilizing trypsin.

1. Two types of nylon-6 supports (small cylinders and a sponge-like derivative) were prepared for immobilizing enzymes. Nylon-6 beads were solubilized by immersion in 80% formic acid and then reprecipitated using two different types of non-solvent solutions (distilled water or a 1:1 acetone:water solution) giving rise to a sponge-like derivative and to a colloidal suspension, respectively. The latter was molded into a thin thread which was cut into small cylinders. 2. Trypsin (EC 3.4.21.4) was covalently bound to glutaraldehyde-activated nylon-6 cylinders as well as to the sponge-like derivative. The maximum (100%) apparent initial enzymatic activity was found for the trypsin bound to small cylinders, while the initial activity of trypsin bound to the sponge-like material was 61% in comparison with that of trypsin-small cylinders, under the same conditions of enzyme immobilization reaction (1 g of nylon support and 5 ml of 1.3 mg/ml trypsin in 0.1 M sodium phosphate buffer, pH 8.5, at 10 degrees C for 18 h) and of enzymatic reaction (1 g of trypsin-nylon in a batch reactor, 2 ml of 0.7% w/v azocasein solution in 50 mM borate buffer, pH 8.5, at 37 degrees C, with shaking, for 1 h). However, the decrease of activity after enzyme immobilization was more conspicuous for the trypsin-small cylinders than for the trypsin-sponge. The former retained approximately 25% of its initial activity, while the latter retained approximately 67% of its initial activity, after seven cycles of utilization for 1 h, pH 8.5, at 37 degrees C and 8 days of storage, pH 8.5, at 4 degrees C in the presence of azocasein. 3. Scanning electron microscopy was performed to visualize the surface of the support after each step of the immobilization process. The electron micrographs show that the two types of nylon supports had a rough surface, which became rougher and full of craters after treatment with 5 N HCl. On the other hand, the partially hydrolyzed nylon surface acquired the appearance of Swiss cheese after treatment with 2.5% glutaraldehyde. After reaction with the enzyme molecules the surface became rougher again.

[The use of Kapromed A and Kaproiod sutures in surgery of the extrahepatic bile ducts].

The experimental substantiation and clinical approbation of the absorbable antimicrobial threads "Capromed A" and "Caproiod" in the surgery of the extrahepatic bile-ducts were carried out. The complications caused by the use of suture material were not revealed. A high effectiveness of the "Caproiod" thread in the emergency surgery of the extrahepatic bile ducts was noted.

On the basis of the use of anomal effects of X-radiation interaction with the media containing ultradispersed particles of some metals and their oxides with the sizes under 1 mm, the authors have suggested radiopaque material.

The Lichtenstein open tension-free inguinal hernia repair using a new prosthetic mesh-Bulgarian irresorbable ampoxen.

The routine use of mesh for repair of inguinal hernia has been popularized by Lichtenstein and coworkers. We adopted this technique and performed it widely using a mesh unknown in the Western world. Ampoxen [multifilamented polycaproamide, impregnated with 5-Nitro-8-Hydroxyquinolinum (Nitroxolinum, DCI), MEDICA, SA, Sandanski, Bulgaria] was discovered in 1975 and proved to be an excellent prosthetic material for replacement of attenuated or destroyed abdominal wall; furthermore, this mesh is very cheap and became widely applicable in our country. This report describes our experience with the first 846 adult inguinal hernia repairs under local anesthesia using Ampoxen. All 846 patients had excellent results, without recurrence. There were nine wound infections (1.1%), 16 testicular oedemas (1.9%), no seromata and no deaths. In no patient was the prosthetic mesh removed. There were no complications related to the use of Ampoxen; this mesh is permanent, has a great degree of fibrous tissue reaction, and wide spectrum antimicrobic activity. We advocate the Lichtenstein technique using irresorbable mesh (particularly Ampoxen) for all adult groin hernias except Types 1 and 2 (according to Nyhus' classification), and for all adult recurrent groin hernias.

Reaction of the periosteum to subperiosteal implantation of a foreign body.

Early cellular reactions of tibial periosteum following its mechanical ablation from the bone surface were studied in white female rabbits. Sterile capron nets were implanted subperiosteally thus obtaining a model of aseptic inflammation. Seven stages in the morphological picture of the reactive process were differentiated. THe chronology, cytomorphology and cytophysiology of the stages were established by studying the cellular reactions in the reactive exfoliation field. Using this experimental setting we show the peculiarities of periosteal reaction in an intact and a broken bone.

[Primary abdominal parietorrhaphy and the cure of eventrations with the relon monofilament suture (N.I. 2402/76)].

Primary repair of the abdominal wall and the cure of incisional hernias using the relon mono-filamentous fibres (N.I. 2402/76) Confronted with long lasting parietal suppurations which are entertained by the classic nylon poly-filamentous fibres, suppurations that cannot be avoided unless the above fibres are removed on one hand assuming the risk of a possible subsequent incisional hernias and on the other hand because of the relative elevated price of the mono-filamentous fibres brought from abroad, within the last years (from 1982) we have utilised the relon mono-filamentous fibre (N.I. 2402/76) in abdominal wall reconstruction (initially for the surgical cure of the incisional hernias and there after in primary abdominal wall reconstruction when there were factors predisposing to a possible subsequent parietal for septic contamination, in overweight patients, immune-compromised patients, in patients following steroid therapy, chemotherapy, etc. Our present experience extends over more than 1000 patients who have undertook a surgical procedure.
