Detection of immunologically sensitized isocyanate workers by RAST and intracutaneous skin tests.

Antibody estimations by RAST were performed on 621 partially selected isocyanate workers whose case histories were well-documented. Two hundred forty-seven of the workers were clinically suspected to be sensitized to isocyanates since they had suffered from occupationally related recurrent asthmatic attacks, chronic obstructive lung diseases, bronchitis, rhinitis, conjunctivitis, cutaneous hypersensitivity reactions, and/or fever. RAST studies with seven different isocyanates conjugated to human serum albumin (HSA) demonstrated that 14% of these symptomatic subjects had significant levels of specific-IgE antibodies. On the other hand, only one of the 374 asymptomatic workers had such antibodies. In contrast to toluene diisocyanate (80:20% mixture of 2, 4 and 2, 6-isomers, if not otherwise mentioned)-HSA, a toluene diisocyanate-ovalbumin conjugate exhibited no antigenicity in nearly all of the cases. Intracutaneous skin testing with five different isocyanate-HSA conjugates performed in 203 workers produced immediate-type wheel-and-flare reactions in 14 of the 53 symptomatic persons but in none of the 150 asymptomatic probands. There was a good overall correlation between RAST and skin test results. Our findings provide strong evidence for IgE-mediated sensitization to isocyanates in a subgroup of the subjects with isocyanate-induced diseases. By means of RAST and/or skin testing these immunologically sensitized persons can be identified.

Comparative estimation of general and local humoral immunity indicators in connection with local immunotherapeutic treatment.

All classes of immunoglobulins in blood and in the nasal secretions of 75 patients were studied in different phases of disease. There were 20 healthy persons in the control group. Forty patients were treated by preseasonal local immunotherapy through the application of water-salt extracts of pollen allergens intranasally, with the help of drops and aerosol during 2 years. It was found that the patients had a high level of total IgE in the nasal secretion, both in the phase of remission and especially in the phase of exacerbation, though the concentration of IgE in the nasal secretion, on the whole, was several times lower than in serum. Quantification of the level of IgE in the nasal secretion can be recommended to diagnosis allergy, "in vitro". The level of sIgA in the nasal secretion of the patients with pollen allergy was lower in comparison with the test group of healthy persons, being most remarkable in the phase of exacerbation. Patients with pollen allergy had a heightened level of IgD in blood with a marked reliability in the exacerbation phase. IgD was not detected in the nasal secretion. Local immunotherapy was found to have a high effectiveness of 80% after one course of treatment, and nearly 95% success with two courses. Therefore, we consider that local immunotherapy should be widely used. With the background of the local immunotherapy there were changes in the concentration of immunoglobulins, mainly in the nasal secretion of the immunoglobulins. IgE in the nasal secretion was lowered to a great extent after two courses of treatment. sIgA after local immunotherapy had a tendency to increase considerably after two courses of treatment. The increase of sIgA level and the lowering of IgE in the nasal secretion can be considered as immunologic criteria of local immunotherapy effectiveness.

Cellular and humoral responses following one year of hyposensitization with aqueous and tyrosine adsorbed Dermatophagoides pteronyssinus extracts.

The present study compared the influence of hyposensitization treatment with a tyrosine adsorbed house dust mite solution (Migen) and an aqueous house dust mite (HDM) solution on the immunological parameters in 41 adult asthma and rhinitis patients. The immunological parameters included total IgE, spec. IgE and spec. IgG antibody determination, circulating immune complexes (CIC), lymphocyte response to Dermatophagoides pteronyssinus and to candidin and streptokinase-streptodornase (SKD). The two groups were compared to a control group of 123 non-allergic subjects. An increase in spec. IgG antibodies was found, with no change in the total and spec. IgE antibodies. The one year hyposensitization treatments had no influence on the appearance and level of CIC. A positive lymphocyte test to HDM antigen was found in 62% of the controls and in 70% of the treated patients. The response was significantly greater in the allergic patients than in the controls. Under the influence of hyposensitization, an early increase of the lymphocyte response to HDM, followed by a decrease, was noticed, whereas the response to candidin and to SKD remained unchanged. The present study demonstrated that the tyrosine adsorbed HDM extract had the same immunological consequences as the aqueous solution.

Effect of immunotherapy on allergen-specific production of sulphidoleukotriene and histamine.

Numerous controlled trials have demonstrated the efficacy of specific immunotherapy, although its mechanism is not completely understood. Few studies have addressed the effects of immunotherapy on the release of mediators. We measured in vitro sulphidoleukotriene (sLT) and histamine release after specific stimulus (Dermatophagoides pteronyssinus or Lollium perenne) in a group of patients under immunotherapy (n = 35) and compared the results with those obtained in a group of allergic patients without immunotherapy (n = 57). SLT quantification was carried out by cellular stimulation allergen test (CAST)-ELISA and we measured the amount of histamine release using a fluorometric method. We found a significant (p < 0.05) reduction of allergen-specific mediator release on the group of patients under immunotherapy treatment. When we studied the group of patients sensitive to D. pteronyssinus we also observed a significant reduction in sLT release after the in vitro stimulus with anti-IgE. In vitro sLT production could be a good marker for follow-up immunotherapy. This study provides more evidence to support the immunological and cellular changes induced by immunotherapy.

[Immunologic mechanisms of specific hyposensitization in allergic diseases of an infectious nature. III. Immunologic processes in hyposensitization with microbial allergens].

The work contains the results obtained in the study of immunological processes in the dynamics of specific hyposensitization with microbial allergens, prepared from Proteus and staphylococci, in 22 infectious allergic bronchial asthma patients sensitized to these microorganisms. The therapeutic effect of this treatment was accompanied by the activation of the processes of cellular and humoral immunity, while the aggravation of the main disease was observed in patients with a pronounced decrease in the activity of immunological processes which were unaffected by the stimulating influence of specific hyposensitization.
