Presence of psychiatric morbidity in prison population in Kuwait.

This paper surveys the prevalence of psychiatric morbidity in a Kuwaiti prison. The rate of major psychosis was considerably lower than the rate reported in other prison studies. Twenty-five percent of the sample had generalized anxiety disorder of a reactive nature which was most probably related to imprisonment itself. There was a preponderance of individuals suffering from personality disorder, alcoholism, and drug abuse problems. Prisoners with a diagnosis of major psychosis had very little exposure to psychiatrists and the large number of recidivists reflects the lack of a probation service. Financial embezzlement and drug/liquor smuggling remain the crimes of choice in Kuwait. Some underlying sociocultural factors are discussed and the need for an alternative prison-based therapeutic community service is emphasized.

PC-based research-oriented clinical information system: a case study of Kuwait.

This paper describes the design and application of a clinical information system (CIS) in a tertiary care facility (Kuwait Center for Cancer Control) using low-cost microcomputers (personal computers). The facility focuses on the collection and use of clinical information for prospective application in clinical trials, individual therapy and research. In order to meet this requirement the patient treatment and follow-up system (PTFS) was developed, based on recent database management technology. The exercise demonstrates that low-cost microcomputer systems can be used effectively in meeting the clinical management information and research needs of care facilities. It is anticipated that the application of the system will be replicated on other health care facilities.

Studies on cercariae from the Kuwait Bay. VI. Description and surface topography of Cercaria kuwaitae VI sp. n. (Trematoda: Haplosplanchnidae).

A new haplosphlanchnid cercaria, Cercaria kuwaitae VI sp. n., was found in the prosobranch snail Cerithidea cingulata in the Kuwait Bay. Details are presented on the morphology and behavior of the cercaria and the encystment process. The new cercaria is a biocellate, distome, with a prominent single sac-like intestinal cecum extending well posterior to the ventral sucker and develops in simple sporocysts. It differ from known haplosplanchnid cercariae in the absence of finger-like processes on the tail, and the presence of V-shaped excretory vesicle extending beyond ventral sucker and the presence of cervical glands. The surface topography of the cercaria and its sporocyst is examined by scanning electron microscopy. This is the first haplosplanchnid cercaria to be described from a Cerithidea species.

Spatial patterns of exogenous mortality in Kuwait.

"Exogenous causes [of death]...represent a serious threat to health in Kuwait, where they accounted for over 40 percent of the total mortality in 1985. This paper is concerned with the spatial distribution of these diseases. It will also help to reflect the influence of various social, economic, and demographic factors on the patterns of distribution. Mortality rates are calculated for 100,000 persons of the country's two communities: Kuwaitis and non-Kuwaitis.... In some parts of the country, and the Capital governorate in particular, nearly half of the deaths were caused by parasitic diseases. Other major causes are tuberculosis and intestinal infections.... Death tolls among Kuwaiti nationals are more than twice those of non-Kuwaitis." (SUMMARY IN JPN)

Invasive amebiasis: challenges in diagnosis in a non-endemic country (Kuwait).

Invasive zymodemes of the enteric protozoan Entamoeba histolytica infect the large intestine and cause extra-intestinal lesions such as amebic liver abscess (ALA). The clinical manifestations of ALA are protean, particularly in patients presenting in a non-endemic, desert country such as Kuwait, and diagnosis becomes problematic. In this study, we present cases of ALA to illustrate the clinical and diagnostic challenges. For serodiagnosis of ALA, we compared the sensitivity and specificity of the indirect hemagglutination assay (IHA) with the ImmunoTab assay and an enzyme-linked immunosorbent assay (ELISA) for this geographic region. We tested sera of 110 patients with ALA, 1,224 patients suspected of having invasive amebic infection, and 50 Europeans with no travel history to an amebic-endemic area. The IHA was simple, rapid, easy to perform, and reliable (sensitivity = 99%, specificity > 95%). The performance of the IHA in detecting ALA in suspected cases was significantly better than that of the ELISA and the ImmunoTab test. Compared with the IHA, both the ELISA and ImmunoTab assay detected relatively higher numbers of false-positive cases (4.7% and 3.6%, respectively). With the availability of ultrasound and computed tomography scans, the serology correlates excellently with the clinical presentation. In chronic cases where fibrosis may be present around the abscess, the IHA has limitations, as in the follow-up of treated patients. Pitfalls in diagnosis are highlighted by discussing the differential diagnosis of ALA from bacterial hepatic abscesses and infected hydatid cysts. Most importantly, the IHA in such cases was invariably at a titer that is considered not significant.

Reference levels of the tumor markers carcinoembryonic antigen, the carbohydrate antigens 19-9 and 72-4, and cytokeratin fragment 19 using the Elecsys Relecsys 1010 analyzer in a normal population in Kuwait. The importance of the determination of local reference levels.

The tumor markers CEA, CA 19-9, CA 72-4 and CYFRA 21-1 were analyzed in a group of apparently healthy subjects (n=232) in Kuwait using the Elecsys Relecsys 1010 analyzer. The distribution of the tumour marker levels was analyzed separately in Kuwaitis (n=103), non-Kuwaitis (n=129), smokers (n=68), non-smokers (n=164), males (n=138) and females (n=94). The distribution of CEA was significantly different in Kuwaitis vs. non-Kuwaitis in the total population (p=0.033) and in non-smokers (p=0.049); in males vs. females in the total population (p<0.0001) and in non-smokers (p=0.0002); and in smokers vs. non-smokers in the total population (p<0.0001) using the non-parametric Mann-Whitney U test. None of the other tumour markers showed significant differences in the subgroups. The upper reference level was defined as the 95th percentile of the normal values in each group. A higher reference level of CEA was observed in smokers (vs. non-smokers) in the total population. Also higher reference levels of CEA were observed in males (vs. females) both in the total population and in non-smokers. In the total population the respective reference levels were: CEA: 4.4 microg/L, CA 19-9: 35 kU/L, CA 72.4: 2.4 kU/L, and CYFRA 21.1: 2.1 microg/L. These results were compared with data in the kit inserts and literature data. The impact of 95th percentiles in a local heterogeneous population is discussed.

Challenges to military working dog management and care in the Kuwait theater of operation.

In a combat environment, troop safety and installation security are paramount. Documentation of the frequent use of explosive devices by insurgents in Southwest Asia has increased the need for augmentation of defensive capabilities. Military working dogs (MWDs) are trained and certified to perform missions supporting security and detection of explosives. Challenges arise in theater because the number of certified dogs available can be limited and the standards of housing and care that are available in the continental United States are not usually available in a combat theater. Planning, preparation, and training of MWD handlers, animal care specialists, and Veterinary Corps officers will maximize mission performance and MWD health. Some of th echallenges and needs associated with management and care of the MWDs in the Kuwait theater of operation are discussed. Suggested improvements to the current deployment readiness paradigm are offered from the perspective of a staff veterinarian who was deployed in support of Operation Iraqi Freedom in 2004.

Studies on Cercariae from Kuwait Bay. X. Description and surface topography of Cercaria kuwaitae X sp. n. (Digenea: Renicolidae).

A new renicolid cercaria, Cercaria kuwaitae X sp. n., from the prosobranch gastropod Cerithium scabridum from Kuwait Bay is described. The new cercaria is nonstyleted gymnocephalous with voluminous Y-shaped excretory vesicle, flame cell formula 2[(3 + 3 + 3) + (3 + 3 + 3)] = 36, and without caudal fins. Surface topography of the new cercaria is studied by scanning electron microscopy. Domed papillae are abundant on the anterior and lateral aspects of the body, ciliated papillae are sparse dorsally. The ventral sucker bears nine domed papillae. Domed papillae form two circles around the oral sucker with three circles of spines in between. This is the first renicolid cercaria to be recorded in a gastropod from the Arabian Gulf region.

A profile of tumor markers in the population of Kuwait.

The tumour markers CEA, AFP, CA 125 and CA 199 were analyzed in a group of apparently healthy subjects in Kuwait. The sample (n = 394) included both genders in the population with a mean age of 38 (S.d. 12.0) years. The distribution of CEA levels values was significant different (Mann-Whitney U test) between Kuwaiti and non-Kuwaiti. The distribution of AFP levels was found to be the same in all groups. The distribution of CA 125 levels was significantly higher in females than in males, both in Kuwaiti and non-Kuwaiti. The distribution of CA 19-9 values was found to be significantly higher in the Kuwaiti female group when compared to the males. The upper reference level was defined as the 95 percentile of the normal values in each group. In the total population the reference level of AFP was 5.6 micrograms/l and of CA 19.9 43 kU/l. The reference level of CA 125 was 16 U/l in males and 24 kU/l in females, respectively. The CEA reference level in Kuwaitis was 6.9 micrograms/l and in non-Kuwaitis 4.4 micrograms/l. The results indicated the importance of determining the reference levels of tumour markers for each individual laboratory. It was also emphasized that care should be taken on the impact of 95 percentiles of normal and benign disease groups.

Kuwait's total diet study: dietary intake of organochlorine, carbamate, benzimidazole and phenylurea pesticide residues.

The State of Kuwait in cooperation with the U.S. Food and Drug Administration (FDA) conducted a Total Diet Study (TDS) to estimate intakes of pesticide residues by the population. The levels of organochlorine (OC) pesticides, carbamates, benzimidazoles, and phenylureas in the TDS core list are reported here. The TDS core list was established through a national food consumption survey. All food items (140 for the Kuwaiti adult) were prepared as eaten and analyzed for the pesticides mentioned above. The FDA's multiresidue methods in Volume I of the Pesticide Analytical Manual were used in gas, liquid, and gel permeation chromatographic analyses. Only vegetable and fruit samples contained pesticide residues (mg/kg), including the carbamates 1-naphthol (1.4) and 3H-carbofuran (0.94) in carrots; the OC pesticide vinclozolin (0.47), 3H-carbofuran (0.66), and fenuron (0.6) in kiwi fruit; the OC pesticide procymidone (0.32) and carbendazim (0.5) in grapes; 3H-carbofuran (5.0) in apricots; the OC pesticides captan (0.013) and thiabendazole (0.63) in pears; captan (0.035) in plums; and carbendazim (0.4) in mandarin oranges. The levels of 3H-carbofuran found in both apricots and kiwi fruit exceeded the maximum residue limits (MRLs) of the Food and Agriculture Organization/World Health Organization (FAO/WHO) of the United Nations. The daily intakes of pesticides by the different population groups are discussed in light of the FAO/WHO acceptable daily intakes.

[New methodological guidelines for the prevention and treatment of venous thromboses during pregnancy and in the puerperal period in Kuwait. Thrombophilia Clinic, Al-Adan Hospital].

The high incidence of venous thromboembolic problems among women during pregnancy and puerperium in Kuwait require a new guidelines to cope with the increased number of thrombotic events and complications in this group. Pregnant ladies should be refer red in very early trimester to a Thrombophilia Clinic where they will be checked routinely for general and special laboratory tests accordingly. They will be classified to high risk category or low risk category. The high risk category group should receive a prophylactic measures throughout pregnancy and puerperium, while the low risk group do not need further action. Those who develop venous thrombosis from any categories should be classified in the high risk category during next pregnancies.

Temporal variations in the infection of a population of Cerithidea cingulata by larval trematodes in Kuwait Bay.

The prosobranch gastropod Cerithidea cingulata (Gastropoda: Potamididae) in Kuwait Bay was examined for larval trematode infections over a 17-month period. A total of 2537 snails were examined and 1265 (49.9%) found to be infected with one or more species of trematodes. The component community in the snail comprised 12 species representing the families Cyathocotylidae (2), Echinostomatidae (2), Haplosplanchnidae (1), Heterophyidae (2), Microphallidae (1), Philophthalmidae (2), Plagiorchiidae (1) and Schistosomatidae (1). Cyathocotylid II (41.6%) was by far the most prevalent species followed by the microphallid (3.9%), the two species comprised 90% of the total trematode fauna. The prevalence of infection increased with shell size and was significantly higher in male (47%) than female (33%) snails. Multiple infections were observed in only 15 (1.2%) of the infected snails; cyathocotylid I and cyathocotylid II combination occurred 14 times and heterophyid I and the microphallid occurred once. Trematode species were more diverse and prevalent in winter, and cercarial shedding peaked in summer. Behaviour of the definitive host and snail population dynamics were probably the major contributors to the detected temporal pattern in the infections.

Determinants of job satisfaction among selected care providers in Kuwait.

The study reports job satisfaction among a sample of 370 physicians, nurses, pharmacists, and medical laboratory technologists in the Ministry of Health hospitals in Kuwait. Job satisfaction was measured using Dunnette's scale. The respondents in all four categories were satisfied with all aspects of their jobs except salary. The medical laboratory technologists were, however, dissatisfied with professional advancement as well. The physicians were found to be most satisfied, closely followed by nurses and pharmacists, while medical laboratory technologists were least satisfied. Multivariate analysis was used to assess the net effects of background and work environment characteristics on overall job satisfaction. Introduction to job, in-service training, and monthly income had significant positive effects on overall job satisfaction. These findings may have serious implications for health care delivery systems that recruit expatriate care providers.

Health sciences libraries in Kuwait: a study of their resources, facilities, and services.

The purpose of this study was to examine the current status of health sciences libraries in Kuwait in terms of their staff, collections, facilities, use of information technology, information services, and cooperation. Seventeen libraries participated in the study. Results show that the majority of health sciences libraries were established during the 1980s. Their collections are relatively small. The majority of their staff is nonprofessional. The majority of libraries provide only basic information services. Cooperation among libraries is limited. Survey results also indicate that a significant number of health sciences libraries are not automated. Some recommendations for the improvement of existing resources, facilities, and services are made.

Hypertension and its determinants among primary-school children in Kuwait: an epidemiological study.

A multistage, stratified random sample of 1312 students (aged 6-10 years) was selected from the five regions in Kuwait. Parents were interviewed and weight, height and urine analysis of the children were taken. Blood pressure was measured on at least three separate occasions. The overall prevalence of hypertension (average systolic and/or diastolic blood pressure > 95th percentile for age and sex) was 5.1%. In multivariate logistic regression analysis, certain groups of Kuwaiti schoolchildren were much more likely to develop hypertension. They included children whose parents were consanguineous, children with a family history of hypertension and obese children.

Conservative treatment of developmental dysplasia of the hip in Kuwait.

This paper evaluates the results of conservative treatment for developmental dysplasia of the hip after an average follow-up of 3 years in 69 patients and 99 hips: 31 bilateral, 23 left side, and 14 right. They were treated at the Al-Razi Hospital, Kuwait, between January 1995 and December 1996. There were 59 girls and 10 boys aged between 5 months and 24 months (average age 12 months). The treatment consisted of traction for 1 or 2 weeks, followed by manipulation in 36 hips and manipulation with adductor longus tenotomy in 45 hips and with adductor longus and psoas tenotomy in 18 hips. Avascular necrosis of the femoral head developed in 16 hips (16%): 14 hips on the affected side and 2 hips on the unaffected side. Persistent acetabular dysplasia was present in 13 hips, subluxation in 9 hips and dislocation in 13 hips. Open reduction was later required in 5 hips, open reduction and innominate osteotomy in 8 hips, and hip extracapsular innominate osteotomy in 1 hip. The results were evaluated according to the age groups with highest rate of complications after the age of 15 months. The treatment protocol was changed to limit conservative treatment until the age of 14 months.

Mortality levels, trends and differentials in Kuwait, 1957-1983.

"This paper attempts to discuss briefly the trends in the level of infant and general mortality and the differentials by age, sex and nationality [for Kuwait for the years 1957-1983]. It also examines socio-economic differentials in mortality. The leading causes of death are briefly highlighted as a mixture of diseases associated with developing countries and diseases associated with developed countries." The authors find that "the factors associated with the decline in mortality have been linked with socio-economic development (as for instance, that reflected in the standard of living, nutrition, education and income) and with the expansion of health care and medical facilities."
