Oxyphil cell adenomas. Three case reports.

The clinical and histopathologic features of three cases of oxyphil cell adenomas (oncocytomas) include two cases involving the lacrimal sac and one involving the caruncle. These lesions generally occur in the caruncle of elderly women, although they have been reported to involve the conjunctiva, lacrimal sac, and rarely the lacrimal gland. They are usually controlled by wide local excision.

Intravenous pyogenic granuloma of the ocular adnexa. Report of two cases and review of the literature.

Intravenous pyogenic granuloma is a recently described form of pyogenic granuloma (PG) in which the angiomatous proliferation is confined entirely within the lumen of a vein. To our knowledge, only four cases involving the ocular adnexa, including two that we encountered, have been described. Histologically, this benign lesion is similar to PG of other locations and is characterized by lobular congeries of capillaries embedded in a fibromyxoid matrix containing scattered chronic inflammatory cells. The whole lesion appears as a single polypoid mass projecting into the lumen of a dilated vein. The histogenesis of this lesion remains obscure. Complete local excision is the treatment of choice. Intravenous PG can be differentiated from other intravascular fibroangiomatous proliferations, including intravascular papillary endothelial hyperplasia, intravenous atypical vascular proliferation, intravascular fasciitis, and organized thrombus.

Probing and irrigation for congenital nasolacrimal duct obstruction.

I reviewed the results of probing for congenital nasolacrimal duct obstruction in a series of 107 patients, with special reference to age at the time of probing. Relief of tearing and discharge was achieved in 90% of patients with the first probing, and an additional 6% were cured after a second probing. Altered nasolacrimal duct anatomy seemed to account for probing failures rather than any delay in probing. Primary probing continued to be an effective treatment well after 2 years of age and was successful in two 5-year-old patients. Unsuccessful probings were usually apparent at the time of the initial probing and were characterized by difficulty passing the probe and subsequent inability to irrigate saline through the nasolacrimal system into the nose. Dacryocystorhinostomy was an effective secondary procedure in the few patients in whom probing was unsuccessful.

Lacrimal drainage system injury in functional endoscopic sinus surgery. Incidence, analysis, and prevention.

Injury to the lacrimal drainage system with resultant epiphora is a complication of functional endoscopic sinus surgery. A study of 24 patients was conducted to determine the incidence and significance of lacrimal drainage system injuries during functional endoscopic sinus surgery. Intraoperative testing demonstrated seven occult injuries (15%) in 46 endoscopic procedures. Postoperative testing demonstrated diversion of lacrimal drainage into the middle meatus in two patients and spontaneous healing of the injured lacrimal drainage system with restoration of normal drainage in three of the patients available for follow-up testing. In no patient did epiphora develop. It is concluded that the nasolacrimal drainage system is in close proximity to the dissection cavity during functional endoscopic sinus surgery and, hence, is at risk for inadvertent injury. Occult damage to the nasolacrimal drainage system would appear to be a common event during functional endoscopic sinus surgery; however, the complication of postoperative epiphora is rare.

Tissue-engineered tear secretory system: functional lacrimal gland acinar cells cultured on matrix protein-coated substrata.

Dry eye is a general term that refers to a myriad of ophthalmic disorders resulting in the inadequate wetting of the corneal surface by the tear film. Dry eyes are typically treated by the application of artificial tears. However, patients with lacrimal insufficiencies such as Stevens-Johnson syndrome, chemical and thermal injuries, or ocular cicatricial pemphigoid have very limited options because of the short duration and action of lubricating agents. As a therapeutic strategy, we are working to develop a bioengineered tear secretory system for such patients. This article describes the growth and physiological properties of purified rabbit lacrimal gland acinar cells (pLGACs) on several matrix protein-coated polymers such as silicone, collagen I, copolymers of poly-D,L-lactide-co-glycolide (PLGA; 85:15 and 50:50), poly-L-lactic acid (PLLA), and Thermanox plastic cell culture coverslips. Monolayers of acinar cells were established on all of the polymeric substrata. An assay of beta-hexosaminidase activity in the supernatant medium showed significant increases in protein secretion, following stimulation with 100 microM carbachol on matrix protein-coated and uncoated polymers such as silicone, PLGA 85:15, and PLLA. Our study demonstrates that PLLA supported the morphological and physiological properties of purified rabbit lacrimal gland epithelial cells more successfully than the others.

Malignant mixed tumor of the lacrimal gland. A clinicopathologic report of two unusual cases.

Two cases of malignant transformation in an originally benign mixed tumor of the lacrimal gland (pleomorphic adenoma) are presented. In both patients the unusual histopathologic appearance of the malignant component gave rise to considerable problems in clinical and histopathologic diagnosis. In one case of the malignant tumor had a sarcomatous appearance, while in the other there was sebaceous differentiation mimicking an adenocarcinoma of the Meibomian glands.

Metastatic thyroid carcinoma masquerading as lacrimal gland tumor.

A 56-year-old woman presented with orbital signs and symptoms suggestive of lacrimal gland tumor. An excised biopsy specimen was obtained and showed glandular tissue, which could be confused with lacrimal gland acini. Closer microscopic examination and immunohistochemical studies revealed a metastatic tumor of thyroid gland origin. Initially, there was no identifiable nodule in the thyroid, but 3 months later a thyroid nodule was found by ultrasound and radioisotope scan. The histopathologic appearance of the thyroid nodule was similar to that found in the orbit. This case demonstrates the usefulness of immunohistochemistry in establishing a diagnosis when the microscopic appearance is inconclusive.

Clinical and histocytopathological study of chronic dacryoadenitis.

Chronic dacryoadenitis occurs mainly in males over 40 years of age and is characterized by mild inflammatory signs, such as swelling and redness of the eyelid without tenderness, bulbar conjunctival stasis and chemosis. The patient has slight exophthalmos, good visual acuity and almost normal fundi. The clinical findings of chronic dacryoadenitis differ from those of benign mixed tumor. CT scan showed tumor shadows in the orbit of all our patients with chronic dacryoadenitis, but failed to differentiate this disease from benign mixed tumor. Antibiotics and steroid hormone did not improve chronic dacryoadenitis, but extirpation was effective, and there was no recurrence. When examined by light microscopy, chronic dacryoadenitis showed prominent proliferation of connective tissues and local infiltration of monocytes, lymphocytes and eosinophils. On electron microscopy, the basal lamina of myoepithelial cells of some acini was thickened. In some acini, intercellular spaces between epithelial cells or epithelial and myoepithelial cells were wide, but the intercellular junction looked intact.

Immunohistochemical characterization of epithelial cells in human lacrimal glands. II. Inflammatory and neoplastic lesions of lacrimal glands.

The distribution of cytokeratins (CK), actin, lactoferrin (Lf), lysozyme (Ly), vimentin and S-100 protein was immunohistochemically investigated in paraffin-embedded specimens of five inflammatory and five neoplastic lesions of lacrimal glands (LGs). Atrophic acini in dacryoadenitis reacted with antibodies (ABs) KL1 and Pkk1 (CK 7, 8, 17, 18) in a manner similar to ducts. Apart from myoepithelial cells and some luminal-duct cells, the remaining epithelia in dacryoadenitis were negative with AB 34 beta E12 (CK 5). The number of AB HHF35 (actin)-positive myoepithelial cells was not altered in dacryoadenitis. Epithelia in dacryoadenitis reacted weakly but consistently with Lf while revealing weak and inconsistent staining for Ly. Vimentin was negative in epithelial cells in dacryoadenitis except in one case. S-100 protein was detected only in epithelia of inflammatory major LGs. Epimyoepithelial islands in lymphoepithelial proliferation reacted variably for CKs, Lf, Ly and vimentin and remained negative for actin and S-100. In pleomorphic adenomas, neoplastic cells showing duct-like differentiation (luminal) reacted consistently with CK 7, 8, 17, 18 and S-100 protein and inconsistently with CK 5, Lf and Ly but remained negative for actin and vimentin. Other neoplastic cells (ovoid/peripheral cells) stained consistently for CK 5, vimentin and S-100 protein and focally for CK 7, 8, 17, 18, actin, Lf and Ly. Spindle-form neoplastic cells found in the stroma exhibited vimentin and S-100 protein and, less frequently, actin. Determination of these antigens in pleomorphic LG adenomas may help to evaluate their prognosis.

[Early childhood angiofibroma in an unusual site].

Angiofibromas are usually characterized by their occurrences in adolescent males with a typical localization and involvement of the posterior nasal cavity and nasopharynx. Even the suspicion of an extranasopharyngeal angiofibroma or an angiofibroma in young children must be viewed in general with skepticism, although reports of angiofibromas with atypical localizations and manifestations in young children have appeared in the literature. Three cases of this fibro-vascular neoplasm with manifestations in the first decade of life and atypical localizations are presented: (1) an angiofibroma medial to the left lacrimal sac in a 15-month old boy, (2) a right paranasal localization in a 9-year-old boy and (3) an angiofibroma limited to the right sphenoid sinus in a 6-year old boy. In all three cases the neoplasm was resected via an endonasal, micro-endoscopic approach that avoided an external incision. According to the literature and based on our own experiences with typical histological findings in all three cases, the clinician has to be aware that the rare angiofibroma can occur in preadolescent children with atypical localizations. The endonasal surgical approach is without any doubt the least traumatic one and in selected cases allows resection of a circumscribed tumor considering functional and aesthetic aspects.

High-resolution conjunctival contrast-enhanced MRI dacryocystography.

We assessed high-resolution MRI of the lacrimal system with conjunctival contrast enhancement in patients with suspected stenosis. We studied 18 patients with epiphora affecting 22 eyes, using a surface coil, before and after conjunctival and intravenous Gd-DTPA. Stenosis or obstruction of the ducts was found in 18 of 22 cases: at canalicular level in 3 cases, at the sac in 7 cases, and in the nasolacrimal duct in 8 cases. Periductal pathology following surgery to the maxillary sinus or dacryocystorhinostomy was seen in three cases. In conjunction with the findings on syringing and/or conventional dacryocystography, the narrowing was classified as stenosis or obstruction.

[Progressive peripunctal nevus in an adult].

Nevi arise from migration of melanocytes into the epithelium. Over the years they experience a typical maturation, with the possibility of an increased thickness due to accompanying inflammation or fibrosis but actual growth of nevi is rare in adults. A 70-year-old patient presented with a growing tumour around the punctum lacrimale. The histological examination revealed a conjunctival nevus without any transformation. Peripunctal nevi are rare. The localisation of a nevus in the area of the punctum lacrimale was first published in 1931 and only 10 other patients with such a nevus have been reported. If at all, an enlargement of a peripunctal nevus has been described in younger patients. The patient presented here showed significant growth of the nevus at an atypically high age without any histological signs of malignancy or proliferation. The reason for this uncommon nevus enlargement remains unclear.

[Diseases of the lacrimal gland].

Experience with the differential diagnosis of lacrimal gland illnesses is limited in daily practice due to their rarity. It is therefore of particular importance to have guidelines for the practitioner by which he can decide on the application of conservative or surgical therapy, and whether it is necessary to take a biopsy to clarify the diagnosis. There is a wide spectrum of possible illnesses which include systemic diseases, inflammatory disorders, neoplastic, benign and malignant tumors. When a pleomorphic adenoma cannot be excluded, biopsy is contraindicated and complete tumor excision with its capsule is necessary to prevent the possibility of malignant transformation and a negative effect on long-term prognosis. In this overview, special interest is placed on the typical clinical aspects and imaging features of lesions of the lacrimal gland fossa, their differential diagnosis and management.
