Tissue engineering skin: a paradigm shift in wound care.

Tissue-engineered skin for the treatment of burns and ulcers is a clinical success, but making it commercially viable is more problematic. This article examines the industry, its techniques and suggests the way forward.

Adjuvant and neoadjuvant chemotherapy in NSCLC: a paradigm shift.

The role of adjuvant chemotherapy in patients with early-stage non-small cell lung cancer (NSCLC) remained controversial for many years. Although a positive impact on overall survival was long suggested for cisplatin-based chemotherapy, early randomized clinical trials failed to confirm a clear survival benefit. Recently, the results from 4 large randomized trials, IALT, JBR.10, CALGB 9633, and ANITA indicate a significant survival benefit with adjuvant chemotherapy for patients with IB through IIIA NSCLC, and the results of these trials support the adoption of chemotherapy in addition to surgery as a new standard of care. Neoadjuvant chemotherapy is potentially better tolerated compared to post-surgical adjuvant chemotherapy, though definitive survival benefits with this approach have yet to be shown. The current status of adjuvant and neoadjuvant chemotherapy for patients with early-stage NSCLC will be discussed.

["Modality shift" paradigms and subjective basic symptoms in schizophrenic patients with various developmental biological risk anamnesis].

Clinical experimental research on schizophrenia does not consider maturation processes of brain function in the majority of cases. In the course of neuronal organization and the differentiation phases of cerebral systems, early traumas in functionally important regions of the brain are accompanied by a facultative impairment of complex systems of information processing. Against the background of the comprehensively coherent "integrative psychobiological model of schizophrenia" (Ciompi) with high construct validity, (experimental) psychology investigations are reported on in the context of the "high-risk" concept in 44 young (20-37 years old) and mainly chronic schizophrenic patients with formation of a specific hypothesis for the developmental biology of the condition. In two homogeneous groups with the same structure, it is checked whether a discriminative appraisal concerning a different organization principle of central information processing systems is possible with "crossmodal" choice-reaction times (cCRT) and certain subscales of the Frankfurt symptom questionnaire (FBF) with regard to alternatively distributed and specially operationalized anamnestic biological risk factors. It can be objectively established in terms of experimental psychology that postpsychotically de-actualized "risk-free" schizophrenics differ from the reference group with a high-risk anamnesis in terms of significantly prolonged cCRT, but not from healthy subjects and an "MBD" control group. In addition, the FBF subscores "weakness of selective attention" and "loss of automatized skills" correlate with poor cCRT.(ABSTRACT TRUNCATED AT 250 WORDS)

Do nonlinear dynamics in economics amount to a Kuhnian paradigm shift?

Much empirical analysis and econometric work recognizes that there are nonlinearities, regime shifts or structural breaks, asymmetric adjustment costs, irreversibilities and lagged dependencies. Hence, empirical work has already transcended neoclassical economics. Some progress has also been made in modeling endogenously generated cyclical growth and fluctuations. All this is inconsistent with neoclassical general equilibrium. Hence there is growing evidence of Kuhnian anomalies. It therefore follows that there is a Kuhnian crisis in economics and further research in nonlinear dynamics and complexity can only increase the Kuhnian anomalies. This crisis can only deepen. However, there is an ideological commitment to general equilibrium that justifies "free enterprise" with only minimal state intervention that may still sustain neoclassical economics despite the growing evidence of Kuhnian anomalies. Thus, orthodox textbook theory continues to ignore this fact and static neoclassical theory remains a dogma with no apparent reformulation to replace it.

Diabetes care in Nigeria: time for a paradigm shift.

Diabetes mellitus is becoming a major public health problem in Africa and its burden is expected to increase. Persons with diabetes mellitus require continuing medical care and self-management education to prevent complications. In both developed and some resource-poor countries, the management of persons with diabetes has been undergoing rapid changes in order to improve standards of care, through restructuring of clinics or through the establishment of diabetes centres with a multidisciplinary team approach to care. There has been a progressive increase in the prevalence of diabetes mellitus in Nigeria and the burden is expected to increase even further. In view of the looming burden of diabetes in Nigeria, there is an urgent need to examine existing healthcare structures, revise the delivery process of healthcare programmes for persons with diabetes and effectively implement a process that facilitates accessibility to such programmes. Well-structured community-based care, appropriate to the local situation and resources, would provide for this, making it more accessible and realistic to the needs of persons with diabetes living in urban and rural areas of Nigeria. Various models have been adopted for the delivery of diabetes care. This article aims to highlight some of these various models of diabetes care. It concludes with a proposed model for the care of persons with diabetes mellitus in Nigeria.
