Long-term effect of incubation temperatures on production parameters and changes in luteinizing hormone and gonadal steroids during the onset of lay in the hen.

Earlier studies demonstrated that different incubation temperatures (33.8 or 37.8 C) from Day 17 of incubation until hatching resulted in differences in postnatal growth rate and hormonal parameters. These studies were repeated and extended to the reproduction period. The onset of egg laying was significantly delayed by 5 days in the 33.8 C group compared to the 37.8 C group. No differences in percent egg production were found between the groups after 3 or 5 months of production: however, mean egg weight was significantly reduced in the 33.8 C group by 1.9 and 1.8 g over 3 and 5 months of production, respectively. Serum concentrations of luteinizing hormone (LH), estradiol (E2), estrone (E1), progesterone (P4), and testosterone (T) were measured weekly at the same time of day approximately 4 weeks before to 4 weeks after the onset of lay. The changes in levels of all hormones over the period studied were identical for both experimental groups. Luteinizing hormone, T, and E2, but not E1 and P4, were different before and after onset of lay in both groups. There were no significant correlations between hormonal levels before onset of lay and production traits for individual hens. In the 33.8 C group, after onset of lay, significant positive correlations were observed between serum E2 and egg production and serum LH and egg production; however, these correlation coefficients were not significant in the 37.8 C group. Significant negative correlation coefficients were observed in both groups between serum E2 after onset of lay and age at first egg.

Effect of breeder quail age and egg weight on chick weight.

An experiment was conducted with 546 Japanese breeder quail to determine the effect of age and egg weight on day-old chick weight over a 16-wk reproduction period. Weight of chicks hatched from the same or different size eggs was found to be affected by parental age. The largest increase in chick weight occurred after 10 wk of age. It was found that egg weight did not affect the hatching weight if egg weight was above 11 g.

Maternal body weight and feed allowance of breeders affect performance of dwarf broiler breeders and tibial ossification of their progeny.

Four hundred and eight dwarf broiler breeder hens were raised collectively in a floor pen to 21 wk of age. At this age they were classified into four groups with reference to their individual BW as heavy (1.95 +/- .1 kg), medium (1.80 +/- .1 kg), light (1.69 +/- .1 kg), and ultralight (1.57 +/- .1 kg). All groups were individually caged at 23 wk of age. During the reproductive period, each group was divided into three subgroups fed on liberal, intermediate, or severe feed restriction (reaching up to 135, 125, and 115 g of daily feed allowance at 29 wk of age, respectively). Intergroup differences in BW were maintained throughout the experiment (21 to 61 wk) but tended to decrease with age. Hen-day egg production was depressed by the lower feed allowance. Fertility and hatchability were impaired when hens received the largest quantities of food. Hen size influenced female breeder performance only slightly. Shell quality and albumen quality were affected by the level of feed consumption. Egg weights as well as BW of the progeny at hatching were enhanced by increased maternal BW and feed allowance. This positive maternal effect was still present at 40 days of age. Despite better overall BW performances of the male versus female broilers, the abdominal fat pad of female broilers was heavier than that of males and tended to increase with breeder size and breeder feed allowance. Accordingly, tibial breaking strength and percentage ash of the progeny at hatching were markedly improved in proportion to the breeders' BW and to their feed allowance. The effect of breeder size on broiler tibial quality was maintained up to 40 days of age but the effect of breeder feed intake tended to disappear with increasing age of the broilers. Tibial strength and mineralization were higher in male than in female broilers at 40 days of age. Dyschondroplasia was higher in broilers hatched from heavier breeder hens, but was not influenced by breeder feed intake. The incidence of varus and valgus in progeny was similar whatever the breeders' treatment. It is concluded that performance of dwarf breeders in a given flock depends mainly upon breeder feed allowance but that broiler performance and, especially, tibial ossification of broilers is greatly influenced by maternal size and, to a lesser extent, by maternal feed intake.

Overview of exercise stress testing.

Exercise stress testing is a non-invasive, safe and affordable screening test for coronary artery disease (CAD), provided there is careful patient selection for better predictive value. Patients at moderate risk for CAD are best served with this kind of screening, with the exception of females during their reproductive period, when a high incidence of false positive results has been reported. Patients with a high pretest probability for CAD should undergo stress testing combined with cardiac imaging or cardiac catheterization directly. Data from the test, other than ECG changes, should be taken into consideration when interpreting the exercise stress test since it has a strong prognostic value, i.e. workload, heart rate rise and recovery and blood pressure changes. Only a low-level exercise stress test can be performed early post myocardial infarction (first week), and a full exercise test should be delayed 4 to 6 weeks post uncomplicated myocardial infarction. The ECG interpretation with myocardial perfusion imaging follows the same criteria, but the sensitivity is much lower and the specificity is high enough to overrule the imaging part.

Localized retroperitoneal lymphangioleiomyomatosis mimicking malignancy. A case report and review of the literature.

Lymphangioleiomyomatosis (lymphangiomyomatosis [LAM]), a rare disease of unknown etiology that is seen only in women usually in the reproductive period, generally presents with features of pulmonary involvement. Extrapulmonary involvement, such as angiomyolipomas and retroperitoneal adenopathy, can occur in up to 75% of cases. It is very rare, however, for patients to present with features of extrapulmonary LAM. We present an unusual, localized case of LAM presenting with neurologic symptoms related to a retroperitoneal mass in a 51-year-old woman. Magnetic resonance imaging showed that the mass involved retroperitoneal lymph nodes, and a clinical diagnosis of atypical sarcoma (possibly from a uterine primary) was made. The mass was resected, and a total abdominal hysterectomy was performed. On pathologic examination, the mass showed classic histologic features of LAM with spread along lymphatic channels in the lymph nodes. Intralymphatic projections simulated lymphatic metastasis; however, the cytologic features were benign. Immunostains revealed the tumor to be positive for smooth muscle actin and desmin, but negative for HMB-45. The uterus was unremarkable, except for a subserosal leiomyoma. Although intratumoral variability for HMB-45 has recently been described, to the best of our knowledge, this is the first documented case of HMB-45-negative, histologically classic LAM. Because of the presence of several atypical features in this case, such as age, location, compressive neurologic presentation, radiologic impression of atypical sarcoma, and HMB-45 negativity, we feel that this case may represent a distinct, as yet uncharacterized variant of LAM.

Testicular tumors in carp-funa hybrid: annual cycle and effect on a wild population.

This study documents the occurrence of testicular tumors in a wild population of carp-funa hybrids. The most prevalent tumor was a dysgerminoma. There were lower prevalences of seminomas, leiomyomas, Sertoli cell tumors, and spermatocytic seminomas. Sex-ratio, gonadosomic index (Gi) and the prevalence of tumors in the monthly catch was analysed for five consecutive reproduction periods (1980 to 1984). Gonadal tumors were found only in males. This may be important to the population dynamics of the carp-funa hybrid, since a high percentage of the fish that die during spawning have these tumors. A schematic model for the reservoir's population is suggested.

Do intestinal helminths affect condition and fecundity of adult mountain hares?

The abundance of the intestinal helminth Trichostrongylus retortaeformis in adult mountain hares (Lepus timidus) from a moor in northeastern Scotland was assessed monthly. Weight and fatness of each hare was measured and the reproductive output of females was estimated by sectioning ovaries. Abundance of the parasite was lower in December and January than at other times of year, and there was no difference in abundance between male and female hares. There was no correlation between intensity of infection with T. retortaeformis and weight or fatness of hares either at the onset of the reproductive period (February and March) or at its termination (August and September). We found no relationship between fecundity of female hares and parasite abundance.

[Iscador QuS and human recombinant interferon alpha (Intron A) in cervical intraepithelial neoplasia (CIN)].

For several years there has been the association between the persistent HPV infection (especially with high oncogenic potency i.e. 16, 18) and the cervical intraepithelial neoplasia. The pathomechanism is probably considered with spread of the early virus gene E1, E2 and the suppressor protein p53 complexes. Further on these complexes cause the neoplastic cell transformation. There has also been described the role of impaired immune response in these cases. The abnormalities cover malformation of antigen presenting system APC, decrease of MHC-I and MHC-II heavy chains rate, decrease of the Langer-hans cells and decrease of count and cytotoxic activities of lymphocytes B and NK cells. The invasive and destructive techniques of HPV associated CIN treatment do not respect its pathogenesis. Therefore the new non surgical methods of treatment would play a major role in treatment and prevention of women especially in their reproductive period. The aim of this work was the evaluation of the Iscador QuS and Intron A role in the management of HPV associated CIN. The 60 patients with CIN and HPV have been diagnosed and treated in our clinic for 12 months. Early results present increase of regression and significant decrease of progression rates in both groups of examined women, comparing to the control group. The stationery state rates in this groups of women were similar to the control group.

[Depression in women].

The woman is twice more likely than man to suffer from depression. The reasons can be biological, psychological and social. The woman is particularly sensitive to depression during her reproduction period depending on the psychological modifications she faces and the hormone variations she has to go through. The general practitioner can encounter various depressive disorders with the following specific clinical and therapeutic aspects: premenstrual syndrome, gravidic depression, postpartum disorders, menopause depression.

[Age-related changes of vertebral bone mineral density in Russian population].

Vertebral trabecular bone mineral density (BMD) was measured by quantitative computed tomography (QCT) in 1061 subjects (610 females and 451 males aged from 7 to 91 and from 12 to 89, respectively) with known history of diseases or taking medicines affecting bone metabolism. Peak BMD values in our patients were observed at the age of 20-29 years with further gradual decrement in men and more steep in women. Negative relationship between BMD and age was r = -0.991 for men and r = -0.968 for women. Analyzing BMD changes by decades we observed the largest decrement in men after 60 (13.1% for 60-69 and 14.1% for 70-79 years of age) and in women after 50 (22.5% for 50-59, 22.1% for 60-69 and 20.8% for 70-79 years) which was most probably due to decline in sex hormones production that is known to significantly influence bone metabolism. This was confirmed by BMD values three-phase approximation in women showing the lowest rate of calcium loss by trabecular bone in reproductive period (1.9 mg/cm3/yr) and the highest in perimenopause (3.98 mg/cm3/yr). Annual calcium loss in postmenopause was 2.22 mg/cm3.

[Malignant transformation of thyroid tissue in an ovarian dermoid cyst: case report].

INTRODUCTION: Mature cystic teratoma is the most common type of ovarian teratomas and also the most frequent tumor originating from germ cells. Usually unilateral, it is, by definition, composed of well-differentiated derivates which originate from all three layers. It is most frequently diagnosed in the female reproductive period; in its pure form, it is generally benign, but occasionally it may undergo malignant transformation in one of its elements. CASE HISTORY: The authors report a case of a 43-year-old woman who underwent a thyroid regulation substitutive therapy 12 years after thyroidectomy which had been performed because of Basedow's disease. On the occasion of her fourth delivery by cesarean section, bilateral cystical formations in both ovaries were found. Upon ending the delivery, the right ovary with the cyst was surgically removed, as well as part of the cystically changed left ovary. The pathohistological analysis of the samples taken from the left ovary cyst pointed to the following diagnosis: dermoid cyst of the ovary a mature cystic teratoma, whereas the samples from the cystic tumor of the right ovary were diagnosed as: ovarian teratoma malignant a papillary carcinoma of the thyroid tissue and cystadenoma papillare serosum ovarii. The histological type of the malignant thyroid tissue neoplasm in the teratoma was a papillary carcinoma consisting of papillary proliferations lined by cuboidal, cylindrical and pseudostratified epithelium with charasteristic "ground glass" nuclei, but without invasion into the blood vessels. DISCUSSION: Malignant transformation of the ectopic thyroid tissue, which is one of the elements of the ovarian cystic teratoma, is extremely rare: it is diagnosed on basis of the histological picture and criteria which are the same as for the tumor of the thyroid gland. One should not exclude a pssibility that the identical or different pathological alterations characteristic of the thyroid gland may also be detected in the ectopic thyroid tissue of a dermoid cyst. CONCLUSION: The reported case was chosen due to its complex and rich clinical and pathohistological picture characterized by the occurrence of a bilateral dermoid cyst with the malignant transformation of the thyroid tissue. The malignant neoplasm in the mature ovarian cystic teratoma was papillary carcinoma of the thyroid tissue with all histological properties typical for the thyroid gland carcinoma. The possibility of the malignant transformation of the endodermal thyroid tissue in the mature ovarian cystic teratoma and changes in the thyroid gland of the Basedow's disease type diagnosed before are without clear causation.

[The age-related structure of the morbidity of background and precancerous cervix uteri pathology].

For all breakthroughs in the treatment of cancer of the uterine cervix diagnosis of the incipient stages of malignancy still poses unique problems. The studies made on the basis of the clinical hospital N 16 and the Oncologic Centre of the Ministry of Health of Ukraine, histological investigations conducted on the basis of the clinical hospital N 25 and the Ukraine MH Oncological Centre permitted the examination of 209 women who had applied for medical advice for background and pretumour disorders of the neck of the womb. On having done the statistical processing of data, analysis of the secured findings was performed. The choice of the relevant method and theoretical preparation for the regression analysis were put through as recommended by Lakin G. F. Thus, the uterine cervix background and pretumour morbidity has two age peaks of different degree: the first--the main peak--falls on age 25, the second peak--that of a weaker degree--falls on age 38 years. The above two peaks fall on the active reproductive period of life (25 to 40 years of age)--just that time in women's life during which high case rates are recordable with background and pretumour pathology.

[Characteristics of oogenesis in the Barents sea sponge Leucosolenia complicata].

Ovogenesis of the Barents sea sponge Leucosolenia complicata Mont. having asconoid canal system was studied at light (using histochemical methods) and ultrastructural levels. It has been established that the reproduction period "arranges" for autumn-winter season. The ovogenesis lasts for approximately two months. Grand accroissement is the longest stage of ovogenesis, lastiing in L. complicata from the end of Sptember to the middle of November. At this stage ovocytes lose their amebic movement, which coincides with the beginning of nurse cell formation. The nurse cells are derived from choanocytes. The choanocytes contacting with the ovocyte surface lose the collar and flagellum, grow in size and transform into nurse cells. Any cell of nurse-cell complex is capable of seizing a spermium and being transformed in situ into a carrier-cell. The specific role of nurse-cell complex is to synthesize sudanophilic (lipid) granules for the carrier cell and ovocyte. In the whole, a weak phagocytic activity of ovocytes, is characteristic of the ovogenesis of L. complicata. During vitellogenesis the ovocyte synthesises endogenic mucopolysaccharide granules from oligosaccharides originated from the mesohyl. Lipid granules pass to the ovocyte from nurse cells. Ribosomal RNAs are accumulated as a result of the own activity of nucleolus-nucleus apparatus of the ovocyte.

[Proteolytic activity of viper venoms].

Proteolytic activities of venoms of vipers kept in a serpentarium for three years or captured in various environmental regions were estimated. Gurza venom contained considerable amounts of protein (830-930 micrograms/mg venom) and displayed a high proteolytic activity by tyrosine (80-140 micrograms/min mg protein). The proteolytic activity did not depend on season, as well as age or physiological state of snakes in the reproductive period. The proteolytic activity of venom in gurza offspring was similar to that in parent specimens. Proteolytic activities (by tyrosine) of venoms produced by Radde's vipers and common vipers were 77-90 and 18-36 micrograms/min mg protein, respectively. The proteolytic activity of venom in common vipers native to the north European part of Russia was 20-30% higher than that in common vipers inhabiting southern European Russia. An inhibition assay found various ratios of metalloendopeptidase and serine endopeptidase activities in venoms of gurza, Radde's viper, and common viper.

Pregnancy associated with hypergonadotropic hypogonadism.

A case of a 30-year-old female with secondary amenorrhea and relative hypergonadotropic hypogonadism is presented. The patient demonstrated persistently elevated levels of gonadotropins in spite of clinical and laboratory evidence of estrogen production. Laparoscopic directed biopsy revealed a total absence of ova, but in spite of this finding, conception ensued. Variations in ovarian sensitivity throughout the female reproductive period and in certain pathologic states are discussed, as well as the apparent limitation of single laparoscopic directed biopsies for confirming the diagnosis of premature menopause. Finally, the theoretical use of exogenous estrogen to induce ovulation in patients with the ovarian insensitivity syndrome is discussed.

[Therapy of inflammatory changes in Bartholin's glands].

INTRODUCTION: Bartholin's glands are bilateral structures whose function begins after puberty, so the diseases of these glands are most common during the reproductive period. Management of the inflammatory changes--abscess and cyst can be invasive and conservative, and quite often the changes become chronic, due to the failure of techniques. The aim of the study was to assess efficacy of various therapeutic approaches in the treatment of the inflammatory changes of Bartholin's glands. MATERIAL AND METHODS: The study was done at the Department of Obstetrics and Gynecology, Clinical Centre in Novi Sad, during the period 1993-1996, at the Dept. of Inflammatory Gynaecological Diseases. The total of 129 patients was treated. The first two years were analyzed retrospectively while during the years 1995 and 1996 a prospective study was done. The analysis of the therapeutic results was done according to the immediate efficacy of the therapeutic approach, incidence of recurrence, length of hospitalization and use of antibiotics. RESULTS: A total of 129 patients was treated, the mean age was 35.4 years. Cysts were treated in 14.7% and abscess in 85.3% of cases. Therapeutic approaches were different during different years. During 1993 and 1994 in majority of cases incision and complete extirpation of the glands were done, and rarely marsupialization, alone or in combination with silver nitrate (AgNO3). In 1995 and 1996 in a majority of cases incision of the gland was performed, together with the AgNO3 application (94.3% in 1995, 64% in 1996). In 1995 total gland extirpation was performed (5.7%) while in 1996 in 9 cases (36%) incision was done. The length of hospitalization was longer in earlier years, the shortest being when incision and AgNO3 application was performed (3.1/2.4 days). The use of antibiotics was significantly lower in the years when incision and AgNO3 were used. DISCUSSION: Our results are in concordance with the results of similar studies except for the fact that in our group the changes were seen more often in the age group 30-39 years. The parity was of no significance. The recurrence of the disease when this method is applied is the same as in other studies--around 3%, whereas the complications were rare. The length of hospitalization and the cost of the antibiotic therapy is of great significance as well, and they can be brought to minimum by employment of this method. CONCLUSION: The therapeutic approach of incision and AgNO3 application in therapy of inflammatory changes of Bartholin's glands is an efficient, safe, simple and cheap method, which can easily be applied in ambulatory conditions.

Seroprevalence of toxoplasmosis in women with recurrent abortions/neonatal deaths and its treatment outcome.

Toxoplasmosis, a parasitic infection, varies in its prevalence in various countries. Some studies have suggested its role in the causation of abortions. We reviewed the records of Microbiology Department at Sher-i-Kashmir Institute of Medical Sciences, Srinagar (Kashmir) and found that out of 2371 women with recurrent abortions and 310 women with neonatal deaths tested for IgM antibody against toxoplasma, 1260 (53.14%) and 215 (69.35%) tested positive respectively. One hundred and twenty-two women with recurrent abortions and 55 women with neonatal deaths who had tested positive for IgM antibody were followed during subsequent pregnancy and were treated with spiramycin; 115 94.26%) in current abortion group and 35 (63.64%) in neonatal death group delivered normal babies. We discuss the role of seropositivity for toxoplasma in women during reproductive period.

The levonorgestrel intrauterine system: more than a contraceptive.

Levonorgestrel can be released directly into the uterine cavity, where it causes pronounced endometrial suppression, although the dose of hormone is so low that the effect on ovarian function is negligible. The levonorgestrel-releasing intrauterine system (LNG-IUS) consists of a plain Nova-T device with a silastic reservoir attached to the vertical arm. The silastic reservoir is impregnated with levonorgestrel and is covered with a rate-limiting silastic membrane. The release rate oflevonorgestrel is approximately 20 microg/24 h for at least 5 years. The contraceptive efficacy of the LNG-IUS has been studied in a randomized, comparative trial over 5 years. A total of 1,821 women were fitted with the LNG-IUS and were compared with 937 women who were using the copper-releasing device, Nova-T. The continuation rates were 46.9 for the LNG-IUS and 44.5 for the Nova-T. The cumulative gross pregnancy rates were 0.5 for the LNG-IUS and 5.9 for the Nova-T. The Pearl index after 5 years was 0.09/100 woman-years for the LNG-IUS and the ectopic pregnancy rate was 0.02/100 woman-years. There were less withdrawals because of bleeding problems and pelvic inflammatory disease in the LNG-IUS group compared with the Nova-T group, but there were more withdrawals because of hormonal side-effects and absence of bleeding. There were no differences in the return of fertility after removal of the LNG-IUS and the Nova-T. All women will notice a change in their bleeding pattern after the LNG-IUS has been inserted and some will initially experience many days of spotting. It is extremely important to counsel women about the changes in bleeding pattern that will occur with the LNG-IUS before the system is fitted. One of the advantages of the LNG-IUS is that menstrual blood loss will decrease (and therefore hemoglobin levels will increase), which means that the LNG-IUS is one of the most effective reversible long-term treatments of idiopathic menorrhagia. The LNG-IUS can be used successfully throughout the reproductive period for effective contraception and treatment of menorrhagia. In addition, this system provides endometrial protection. Women who suffer from climacteric symptoms while they are using the LNG-IUS can be given estrogen to relieve their symptoms.
