A study was made of the action of various doses of thyroidin in its prolonged administration to guinea pigs on the functional and morphological changes of the thyroid gland and the adenohypophysis. A high sensitivity of guinea pigs to the excess of thyroid hormone in comparison with such in dogs was shown. Already with the less thyroidin doses guinea pigs displayed an increased oxygen consumption under conditions of the basic metabolism, a marked increase of the content of protein-bound iodine in the blood serum and of the corticosteroid urinary excretion. There were revealed no significant differences in the thyroidin action depending on the method of its administration, i.e. in case of a gradual increase of the doses or in the administration of high doses from the very beginning. The presence of significant individual differences in guinea pigs in the resistance to the excess of the thyroid hormones was demonstrated.
This is a report on a patient 57 years old, hospitalized because of expressed fatigue, nausea, vomiting and epigastrium pain. Hyperpigmentation of skin and mucous and hypotension were present; and hypocorticism was suspected while ex iuvantibus substitutional therapy of corticosteroides was introduced. By testing thyroid gland function, hypothyroidism was established, so we suspected the presence of united pluriglandular insufficiency--Schmidt Syndrome. The patient reacted well to the substitutional therapy of hydrocortisone and levothyroxine. The diagnosis has been confirmed at the Clinic for Endocrinology in Novi Sad and today the patient has regular checks at the outpatients clinic for endocrinology in Sombor. The discussion contains important diagnostic and therapeutic problems.

Thirteen patients aged 24 to 65 with Cushing's disease have been treated by transphenoidal hypophysectomy. There was one incident of meningitis and one late death from myocarditis. No radiotherapy was given. In 12 patients pituitary histology showed Crooke's changes and an adenoma was present in seven. On review of the 12 survivors two to 11 years later, 11 are in complete remission. One patient still showed some clinical features of Cushing's syndrome but steroid levels were normal. In five of the seven premenopausal women normal menstruation has returned and pregnancy has occurred in three. In four patients replacement hormone therapy is no longer required and a further four take only partial treatment. We conclude that transphenoidal hypophysectomy is an effective treatment for Cushing's disease.
