A simple diagnostic method of peritonitis in patients on CAPD.

The major complication of continuous ambulatory peritoneal dialysis (CAPD) still is peritonitis, therefore, early and simple methods for detection of infection are important. Peritoneal effluent cellularity is a predictor of peritonitis and can be detected by Cytur-Test, a test-strip developed for detection of leucocytes in urine. The purpose of the study was to evaluate the usefulness of Cytur-Test as a diagnostic test in peritonitis in patients on CAPD. A total of more than 5 000 tests were performed. During the period we diagnosed 46 cases of peritonitis. The Cytur-Test was positive in all cases. There were no false negative tests. In two cases only, the test appeared positive for 48 hours without clinical signs of peritonitis.

[Laparoscopy as a clinical diagnostic method at a hospital center in Peru].

Laparoscopy is an invasive endoscopic procedure, that permits the vision of the abdominal cavity, through a little hole in the abdominal wall. This method permits a almost complete vision of abnormalities. It also helps to define diagnosis and classify or visualize the progress of some diseases. In this study done in 141 patients with liver diseases detected by laparoscopy and biopsy in the Arzobispo Loayza General Hospital in Lima, Perú, we verified the diagnostic efficacy of laparoscopy in hepato-biliary diseases like cirrhosis and hepatocarcinoma etc., and also to establish the etiology of peritoneal diseases. Emphasis is made in the importance of biopsies taken under visual control.

[Substernal goiter: new diagnostic methods and treatment].

The authors report 47 cases of substernal goiters operated between 1971 and 1990. Forty-five patients had mediastinal-cervical-goiters (g.c.m.) and 2 had ectopic goiter (g.e.). Cervicotomy was performed in the 45 g.c.m., in 3 cases associated with sternal "split". The g.e. were treated with thoracotomy. Computed Tomography (TC) has been performed since 1984 in 26 patients; Magnetic Resonance (RM) since 1987 in 6 patients. The diagnostic accuracy for TC was 84.6% and 100% for MR on anatomical-topographic-definition, and 80.8% and 83.3% for presumptive diagnosis of nature, respectively. Considering the high diagnostic accuracy, the possibility to reconstruct the images in sagittal and coronal planes, the RM seems to be the elective diagnostic procedure for all substernal goiters.

Update on diagnostic methods, natural history and outcome variables in Alzheimer's disease.

The diagnosis of Alzheimer's disease (AD) currently relies on history obtained from family or friends and on mental status assessment matched to National Institute of Neurological and Communicative Disorders and Stroke criteria. Progression over time may or may not be typical, suggesting alternate diagnoses such as Lewy body or frontotemporal dementias. Apolipoprotein E genotype does not appear to be useful as a diagnostic marker. The usefulness of brain imaging in AD must be reexamined. Critical events in the natural history of AD, such as institutionalization and loss of ability for self-care, could be used as end points. Loss of ability for instrumental tasks, such as driving, traveling alone, or managing finances, would be preferable for early-stage stabilization studies. Different symptomatic domains of AD (mood, cognition, functional autonomy, behavior, motoricity) can be quantified using specific outcome measures. Although cognitive loss has been considered a core symptom of AD from a regulatory perspective, loss of functional autonomy and behavioral disinhibition are considered more important by clinicians and families. Recently, the availability of new scales has led to an interest in all of these domains. Results from symptomatic drug studies suggest a differential effect of cholinesterase inhibitors on cognition versus muscarinic agonists on functional autonomy and behavior. Hence there is a need to measure these domains separately and, eventually, to attempt combination therapy. Quality of life is a difficult but important dimension of AD therapeutic research, and it requires further methodological research.

Laparoscopy as a diagnostic method in internal medicine: experiences with 168 procedures at Kilimanjaro Christian Medical Centre, Moshi, Tanzania.

One hundred and sixty-eight laparoscopies, performed for various indications between 1985 and 1989, were studied retrospectively and the findings and experiences are reported. The most frequent diagnoses were hepatocellular carcinoma (20%) and liver cirrhosis (18%). Correlation of macroscopic and histopathological diagnosis was satisfactory in hepatocellular carcinoma (74%) and poor in liver cirrhosis (38%). Laparoscopy was judged diagnostic in 45%, diagnostically helpful in 20% and non-contributory in 35% of all cases. The highest yield was obtained in hepatocellular carcinoma and liver cirrhosis, the poorest results in obstructive jaundice. It is concluded that for optimum diagnostic outcome the indication for laparoscopy should be strict and that it should be practised only in those centres where experienced clinicians and competent histopathologists are available.

[Possibilities of various diagnostic methods in the assessment of the state of axillary lymph nodes in breast cancer].

The incidence of axillary lymph nodal metastases in breast cancer is proportional to the size of a focus: To is 25 +/- 25%, T1-33 +/- 17%, T2-55 +/- 7%, T3-65 +/- 11%, the choice of the optimum treatment regimen requires more accurate preoperative detection of metastatic lymph nodes. Ultrasound technique (its accuracy is 80%, sensitivity-88%) is of the most diagnostic value in the detection of metastatic lymph nodes. The absence of echographic signs supports the nonspecific nature of changes. Palpation is preferable in nonspecific axillary lymph nodal lesions. The status of axillary lymph nodes in breast cancer may be correctly assessed by comprehensive clinical and ultrasonic studies of the axillary area wherein, supplementing each other, each technique yield the maximum information.

Primary ureteral carcinoma. Report of 15 cases and comparison of the effectiveness of diagnostic methods.

Fifteen cases of primary urothelial carcinomas of the ureter are reported (14 transitional cell carcinoma and 1 mixed transitional cell carcinoma and squamous cell carcinoma). There was a clear male predominance (11/4); the peak incidence was in the sixth decade. Most tumors originated from the distal third of the ureter. Multicentricity and high recurrence rate after partial ureterectomy were noted. The clinical signs were hematuria and flank pain. Urine cytology and IVU were diagnostic in a limited number of cases. Retrograde pyelography was very helpful. CT showed to be the image modality of choice for diagnosis and preoperative staging.

Diagnostic methods in lung cancer.

We feel that the guidelines described here reflect the state of the art at the time of writing (April 1982). The role of CT scanning is likely to evolve further and nuclear magnetic resonance (NMR) will probably play a significant role in the future. Experience, expertise, and equipment will vary from one hospital to another and thus rigid rules cannot reasonably be applied for the workup of suspected lung cancer. Ideally the diagnostic approach to an individual patient will be highly tailored, not only in regard to the nature of the pulmonary lesion, but also to his or her overall medical and social situation. We have found that a policy of early consultation between clinician and imaging specialist most readily facilitates effective use of available diagnostic resources.

Use of various diagnostic methods in a patient with Gaucher disease type I.

A series of plain radiographs, bone scans, bone marrow scans, and MRIs is reported in a patient with Gaucher disease type I, in whom two episodes of acute bone crisis developed during a 6-year period of follow-up. Acute bone crisis and global indolent bone marrow displacement could both be assessed by bone marrow scintigraphy, whereas MRI could better clarify the corti-comedullary alteration after bone infarction. Thus, MRI and bone marrow scintigraphy could be used as complementary imaging methods in the management of patients with Gaucher disease.

Occult gastrointestinal bleeding. An evaluation of available diagnostic methods.

Occult gastrointestinal bleeding was defined as continued bleeding in spite of a normal series of roentgenorgrams of the upper part of the gastrointestinal tract, barium enema, and sigmoidoscopy. Twenty-six such patients were treated. A thorough systematic evaluation, including gastroscopy, colonoscopy, visceral angiography, and isotopic scanning, was done preoperatively. Using colonoscopy and arteriography, nearly 60% of the bleeding sites were identified. Seventy-six percent of the lesions identified were in the terminal part of the ileum or the ascending colon. Exploratory laparotomy should be performed for life-threatening hemorrhage or as a diagnostic test only after a thorough preoperative evaluation. If results of a complete preoperative evaluation including arteriography were normal, then the likelihood of finding a discrete cause of bleeding at laparotomy was high (80%). A systematic evaluation and diligence of both physcian and patient in localizing the site of bleeding are essential.

Development of DNA diagnostic methods for the detection of new fish iridoviral diseases.

A new disease of epidemic proportions caused by fish viruses within the Iridoviridae family inflicts serious damage on red sea breams (Pagrus major) and striped jack (Caranx delicatissimus) populations grown in aquacultures in Japan. A partial segment of the fish iridoviral DNA was directly amplified using the polymerase chain reaction (PCR) with synthetic primers designed from well conserved nucleotide sequences between the frog virus 3 (Ranavirus) and the silkworm iridescent virus type 6. The deduced amino acid sequence from the nucleotide sequence of the PCR fragment demonstrates a high correlation with a partial sequence from the frog virus 3. Using the PCR method with specific primers, we could detect three of four different known types of fish iridoviruses in diseased fishes. To construct more reliable detection methods specific for this viral family, DNA fragments which can specifically hybridize with all of the four known iridoviridae viral DNAs were screened from the genomic library of one iridoviridae strain. The hybridization assay, using a specific fragment which contains regions which are highly homologous with a characterized partial sequence from the frog virus 3, proved to be a reliable diagnostic tool for fish iridoviral diseases.

[Comparative characteristics of diagnostic methods of primary hyperparathyroidism in patients with urolithiasis].

Nephrolithiasis (NL) is a frequent complication of hyperparathyroidism (HPT). Such patients account for 5% of those with nephrolithogenesis. The study included 89 HPT patients (age 34.8 +/- 1.9 years) with NL, 22 patients (age 38 +/- 1.4 years) with NL and 10 healthy controls (age 30 +/- 1.2 years). Hypercalciemia was seen in 27-20% of patients with HPT and NL, 9-10% of patients with nephrolithiasis. Evaluation of parathyroid hormone (PTH) levels in the blood of the examinees showed its high diagnostic value: in 89% of HPT patients this level was high. It was also elevated in 10% of NL patients. cAMP in HPT and NL was elevated in 68.48 and 12% of patients, respectively, serum concentration of Ca2+ was high in 81.4 and 24.5%, Ca load test was positive in 80.9 and 10% of the patients, respectively. It is important for diagnosis that in HPT bone mass is low.

[Correlation between total serum IgE and the remaining allergologic-diagnostic methods in childhood].

The authors found in studing of 60 allergic children on Dermatophagoides Pteronyssinus (D.P.) that besides all clinical allergological tests is extraordinary radioimmunological determinations of serum IgE in allergic patients. In small group of healthy children they found the mean value for serum IgE 499 U/ml (+/- SD - 242). In allergic children with bronchial asthma and vasomotoric allergic rhinitis with positive skin reaction +++ on D.P. and with positive provocation inhalation and nasal test found statistically significantly higher values for serum IgE in chidren with above mentioned diagnostic tests. Detail analyses of determination of total serum IgE showed that is necessary to start to determine also specific IgE with Phadebas IgE test for different inhalative allergens. The greatest advantage of determination of serum IgE in vitro is undengereous technique whereas other allergological diagnostical test especially in childhood are dangerous for live.

[More efficient investigation in suspected acute coronary syndrome possible. New diagnostic methods can improve the quality of care and save resources].

The immediate evaluation of patients with suspected acute coronary syndrome (ACS) in the emergency department (ED) has remained almost unchanged for decades. At the same time, therapy for established ACS has undergone a remarkable and successful change towards early active intervention. Studies show that 7 out of 10 patients admitted with a suspicion of ACS do not have it, and that 2-5% of the patients with ACS are incorrectly sent home from the ED. With new diagnostic strategies, including e.g. risk prediction algorithms, new blood samples for plaque instability, special investigations like echocardiography, myocardial perfusion imaging and magnetic resonance imaging, as well as the Chest Pain Unit concept, improvements should definitely be possible. With the structured and evidence-based use of such strategies, it is our belief that more patients can be managed as outpatients, that length of stay can be shortened for those admitted, and that some patients with ACS can get an earlier adequate intervention.

Occlusal caries detection: a comparison of DIAGNOdent and two conventional diagnostic methods.

OBJECTIVES: The aims of this study were: 1) to compare the DIAGNOdent readings for the detection of caries on three different macroscopically sound and intact occlusal surfaces for the baseline measurement; and 2) to compare this laser device with visual and radiographic inspection. METHODOLOGY: Forty-five extracted premolars and molar human teeth were cleaned with a pumice slurry. Then 74 sites were defined and radiographs were taken from each one. Three examiners performed the examinations and took the DIAGNOdent measurements. Sensitivity and specificity were calculated for each diagnostic system, as well as intra- and inter-examiner reproducibility using Kappa tests. The cut-off point for visual and radiographic inspection was 1-2, and for the DIAGNOdent the manufacturer's instructions were followed. RESULTS: No statistical difference was found for the three predefined sound sites used for baseline measurements with DIAGNOdent. The visual inspection showed high sensitivity; radiographic inspection and DIAGNOdent measurements showed low sensitivity and high specificity. The highest intra- and inter-reproducibility was found with the DIAGNOdent. CONCLUSION: The thickness of the enamel did not have any effect on the readings with DIAGNOdent. It was concluded that in a low prevalence sample, the visual inspection provided the highest proportion of true disease identified correctly, and DIAGNOdent provided the highest proportion of non-disease identified correctly.

[Comparison of diagnostic methods in rotavirus infection: immunofluorescence, ELISA and the latex agglutination test].

To examine the sensitivity and reliability of a latex agglutination test for detecting rotavirus in faecal samples, we have tested 145 stool specimens from pediatric patients with or without diarrhea, comparing such latex test to immunofluorescent antibody technique (IF) and ELISA. Among all samples positive by IF and ELISA, 52.9% were positive by latex test. This percentage was raised to 85.7% when only positive samples from patients with diarrhea were considered. We conclude that the latex test is an useful diagnostic tool in acute diarrhea because of its simplicity and of its sufficient sensitivity.

[A novel diagnostic method to study the residual pelvic cavity after Miles abdomino-pelvic resection].

Local pelvic perineal recurrence represents the most frequent site of failure following abdominoperineal resection (APR) for rectal cancer. Patients con be studied at this level by Computerized Tomography (CT) scan, Magnetic Resonance Imaging (MRI), suprapubic or, in women, endovaginal ultrasound (US). CT scan and MRI show sensitivity and specificity in excess, respectively, of 70 and 90%, but the high cost and the invasiveness of CT scan controindicate their frequent use. Suprapubic US has no value in terms of diagnostic accuracy, whereas endovaginal US shows a good specificity, but it is quite refused by the patients since the discomfort of the examination. 34 patients from the Rehabilitation Unit of Colostomy Patients at the University of Rome, "La Sapienza", have been blindly followed by transperineal US. The results have been compared with those obtained by CT scan or MRI. Specimens have been obtained of any suspicious mass by needle biopsy. The diagnostic accuracy of transperineal US has resulted comparable to CT scan and MRI. The authors also describe the morphology and US pattern of the pelvic hollow as demonstrated by transperineal US and the distinctive features of abnormality seen.
