[Local immunotherapy. Alternative therapy for malignant cutaneous lesions (demonstration on 30 patients with malignant melanoma) (author's transl)].

Experimental data in animal models and clinical experience with a systemic immunotherapeutic approach in patients with tumours were the determining factors in the decision to test the local effect of this form of therapy on primary and secondary malignant lesion of the skin. The antitumour effect of local immunotherapy seems to be based on the induction of a cell-mediated immune challenge reaction in close contact with the malignant neoplasm. A trial of local immunotherapy was undertaken in 30 patients with melanoma with local recurrence of the primary tumour or multiple metastases involving the skin. The therapeutic indications and clinical response are discussed.

A comparative study of the Papanicolaou staining method and the fat stain technique in malignant and non-malignant lesions of the urinary tract.

We have collected urine specimens from cases of malignant and nonmalignant lesions of the urinary system and have proceeded to a comparative study of these cases with their clinicopathologic findings after staining them with the Papanicolaou method and the fat stain method. It was found that the fat stain is not specific for renal cell carcinoma only. However, a combination of fat stain and Papanicolaou stain as a second check up of potential malignancy would be the best way of examination.

A malignant neoplasm with features of both squamous cell carcinoma and malignant melanoma.

A case of a malignant neoplasm with features suggestive of both malignant melanoma and squamous cell carcinoma is reported. The neoplastic cells were positive for both S-100 protein and keratin when stained by the indirect peroxidase-antiperoxidase method. The literature on other instances of bi- or multidirectional differentiation of neoplastic cells is reviewed. The significance of these findings for terminology of neoplasms is discussed.

Influence of interferon on C19-steroids in urine of malignant skin melanoma and malignant breast neoplasm patients.

The paper evaluates the effect of human leukocyte interferon (HLI) in patients with malignant skin melanoma and malignant neoplasm of the breast. HLI was administered intramuscularly in a daily dose of 2 X 10(5) IU 7 days at 8-week intervals over a period of 12 months. The chosen patients were in the surgical stage of the disease, i.e. they had undergone surgical operation on the primary tumor or on the regional lymph nodes T1-3, N0-1, M0. During the HLI treatment the values of C19-steroid metabolites and particularly of Dehydroepiandrosterone (DHA) were established and the following conclusions have been drawn: the application of HLI in a dose of 2 X 10(5) by the described method has an impact on the change of C19 steroid metabolism in terms of normalization.

Coincidence of primary malignant melanoma of skin and eye. Report of a case without B-K mole syndrome but with a further malignancy.

Although primary malignant melanomas of the skin or eye alone are not too rare, their coincidence is most uncommon. The case of a 41-year-old Caucasian male is reported who developed superficially spreading melanoma on the skin of his trunk 9 years after the enucleation of his right eye because of a fusiform melanoma of the choroidal membrane. Staging revealed a further malignancy: hemangiopericytoma inside and adjacent to the left psoas muscle. Both theoretical and practical conclusions to be drawn from the case are discussed.

[Local application of interferon beta to skin lesions of malignant melanoma and malignant lymphoma--clinical and histopathological analysis].

Interferon (IFN) beta was applied locally to skin lesions of malignant melanoma (MM: 3 cases in stage IV and 1 case in stage II) and malignant lymphoma (ML: 3 cases of mycosis fungoides in stages II-III and 1 case of primary cutaneous lymphoma in stage III). It was found that IFN beta was remarkably effective for ML (3 in CR and 1 in PR), but far less effective for MM (3 in PD and 1 in NC). Biopsy specimens of clinically regressed areas revealed histopathology entirely different from each other, skin lesion of MM being infiltrated by numerous inflammatory cells such as lymphocytes and macrophages, whereas that of ML infiltrated Least. Local application of IFN beta appears to provide a new modality for treatment of cutaneous ML, which has the primary lesions on skin and has been hardly controlled by systemic chemoimmunotherapy.

Differentiation between patients with malignant diseases and non-malignant diseases or healthy donors by changes of fluorescence polarization in the cytoplams of circulating lymphocytes.

Changes in the degree of fluorescence polarization in the fluorescent cytoplasm of lymphocytes from patients with malignant diseases, patients with nonmalignant diseases, and normal donors were investigated by the fluorescence polarization technique. In order to confirm Cercek's cancer test, two kinds of parameters, the degree of fluorescence polarization of lymphocytes stimulated with phytohemagglutinin (PHA) and cancer basic protein, were obtained for each person. Patients with malignant diseases were easily differentiated from the others without exception by an index, the response ratio of the degree of fluorescence polarization. Usefulness of this technique was confirmed for the detection of cancer-bearing patients.

[Eye and hair pigmentation in malignant melanomas and other malignant skin tumors].

With the intent to determine whether the phenotypic appearance of individuals with skin malignancies could be differentiated from individuals without such tumors, an objective standardized analysis of 183 patients with malignant melanoma and 111 patients with basal cell epithelioma, squamous cell epithelioma and premalignant skin lesions was undertaken. Eye color and hair color were studied in addition to birthplace of the patients. 527 patients without history of tumors were utilized as controls. High risk persons are characterized by light iris colors (for the melanoma group significance of 0.05%, for other tumors significance of 0.001%) and blond or reddish blond hair (for the melanoma group significance of 0.001%). The melanoma patients originated to a greater part from regions north from their present residence (significance of 0.01%) than the group of non-melanoma patients, who originated more often from the vicinity of Maniz (50 degrees N latitude).

[Second malignant disease in patients under treatment for malignant lymphoma].

Between 1978 and 1993, 1013 patients 529 with Hodgkin's disease and 484 with non-Hodgkin's lymphomas--were treated in our department. Out of 1013 patients secondary neoplasms developed in 23 cases: 3 acute nonlymphocytic leukemia, 19 solid tumors and 1 secondary non-Hodgkin's lymphoma. The median time from diagnosis of malignant lymphoma was 7 years and their median age was 49 years. 11 patients with secondary tumor were treated with chemotherapy and 12 received combined (radio- and chemo-) therapy. Since alkylating agents increase the risk of leukemia and radiation contributes mainly to other cancer, future treatment protocols should attempt to reduce the most serious consequences of therapy without compromising the survival. Careful lifelong observation is indicated for patients with malignant lymphomas, with special attention given to new clinical signs or symptoms.

[Genetic analysis and elaboration of principles for predicting malignant tumors in families of patients with stomach cancer and primary multiple malignant neoplasms].

The results of familial population analysis of stomach cancer and multiple primary malignant tumors (MPMTs) are presented. The data obtained provide evidence for the multifactorial nature of stomach cancer. Hereditary factors accounted for 32% of the general predisposition of individuals to stomach cancer. The age-related character of stomach cancer was established. Genetic heterogeneity of this tumor is suggested by the data obtained. Significant genetic commonness in the inheritance of stomach, colon, Genetic Analysis and Prognosis for Malignant Tumors in breast, endometrial, and ovarian cancer in families of MPMT patients was demonstrated on the basis of the obtained genetic correlations between tumor types in MPMT patients and solitary tumors in members of their families. The greatest genetic load was shown for families of MPMT patients, as compared with families of patients with solitary tumors, the coefficient of MPMT inheritance being equal to 77.4%. The data on the genetic character of stomach cancer and MPMT formed the basis for the identification of criteria for developing methodological approaches to the screening of individuals from risk groups to facilitate early diagnosis and prevention of cancer.

[Large-cell anaplastic malignant CD30/Ki-1-positive T-type lymphoma, associated with STLV-1 virus (baboon malignant lymphoma model].

Large-cell anaplastic malignant lymphoma of T-type (T-LCAL) as an original form among human non-Hodgkin's malignant lymphomas in Updated Kiel Classification (1988) was defined. In human pathology T-LCAL presents a considerable diagnostic difficulties and is not associated with HTLV except some cutaneous forms. Comparative examination was carried out in 5 cases of baboon T-LCAL as a part of long-term investigation of the lymphoma outbreak in Sukhumi monkey stock. In tumor-bearing baboons routine morphological methods, immuno-morphological phenotyping of lymphoma cells with panel of MoAT and polyclonal antibodies to T- and B-lymphoid cell antigens and antigens of cell activation, including CD30 antigen, in cell suspension and on cryostate sections were used. STLV-provirus integration in DNA of lymphoma cells in PCR was determined. Diagnostically important morphological features of baboon and human T-LCAL include pronounced anaplasia and anisocytosis of CD30-positive large blast cells, tumour cell tendency to infiltrate sinuses and vascular walls. The presence of T-cell markers (CD2, CD3, CD4, CD8 in our cases) and antigens of cell activation (CD25, HLA-DR) were found. In DNA of baboon lymph node lymphomas (also in the cases without skin involvement) env- and tax-fragments of STLV-1 provirus were detected.

[Multiple cooperative study of UFT-adjuvant chemotherapy for malignant tumor in the jaw and oral cavities. The Oral Surgery Malignant Tumor Research Association in Kanto Kohshinetsu District].

We studied the usefulness and safety of long-term administration of uracil and tegafur (UFT) after primary therapy of the malignant tumor in jaw and oral cavity regions randomized controlled trial. 112 cases were totallized 6 institutes belonging to the Oral Surgery Malignant Tumor Research Association in Kanto Kohshinetsu District during 2 years and 10 months, beginning in September 1986. After completing the primary therapy, treatment was not performed in group A and 400 mg/day of UFT was orally administered in group B for 1 year. A variation was observed in the stages of background factors such as sex and age (more than 30 years old and less than 80 years old), while no such discrepancy was observed in other stages. No significant difference of 1-year-survival ratio and non-recurrence ratio was noted in either group, while the non-recurrence ratio was more favorable in group B than group A, and the usefulness of UFT for adjuvant chemotherapy was suggested. The incidence of side effects in group B was 42.6%, and no serious side effect was observed.

Comparison of cytogenetic abnormalities and deoxyribonucleic acid ploidy of benign, borderline malignant, and different grades of malignant soft tissue tumors.

BACKGROUND: Both DNA flow cytometry and cytogenetic analysis have been used to study soft tissue tumors. With flow cytometry, the DNA content of a relatively large number of cells can be examined, but cytogenetic analysis gives more detailed information about genomic changes. EXPERIMENTAL DESIGN: In order to compare the advantages and drawbacks of DNA flow cytometry versus chromosomal analysis, 92 primary or recurrent malignant, 16 borderline malignant, and 13 benign soft tissue tumors were karyotyped after short-term culture. DNA ploidy was determined by flow cytometry of suspensions prepared from frozen or paraffin-embedded samples. From 97 patients, 121 samples were analyzed. RESULTS: On the basis of the results, four groups were distinguished: DNA-euploid tumors with normal diploid karyotypes (group a) or with abnormal (group b) karyotypes, and DNA-aneuploid tumors with normal (group c) or abnormal (group d) karyotypes. The findings in group b show that structural chromosomal abnormalities or minor numerical aberrations of chromosomes are not detected by DNA flow cytometry. In group c, the finding of tumors with an aneuploid DNA-profile and cells with normal karyotypes is most likely due to overgrowth of fibroblasts during culture and subsequent karyotyping of normal cells. CONCLUSIONS: The findings show that a) DNA flow cytometry has a higher success rate than karyotyping, b) both techniques are complementary, such that DNA flow cytometry gives an "overview", whereas karyotyping gives more detailed information; comparison of both techniques in individual cases leads to a better understanding of the chromosomal events that occurred during oncogenesis, c) histologically low grade tumors are generally DNA-diploid, but may have an abnormal karyotype, and d) histologically high-grade sarcomas tend to have an aneuploid DNA-profile; they are generally more difficult to karyotype.

[Detection of residual malignant cells in patients with hematopoietic malignancy].

Current progressive chemotherapy and transplantation induce not only remission but complete cure. By this reason, the detecting system for MRD must be established in complete remission phase. In this study, we showed the detecting for MRD by molecular biology techniques in the patients with non-Hodgkin's lymphoma, CML, B-ALL and AML. 1) Malignant cells could be detected in peripheral blood or bone marrow cells by Southern blot analysis in 15 of 30 patients with non-Hodgkin's lymphoma. 2) In CML patients, BCR/ABL fusion gene was positive by RT-PCR for 6 months after BMT, 4 patients became undetectable for 7 months after BMT. 3) Rearrangement of Ig H has been disappeared in 12 months after achieved complete remission the patients with B-ALL. In conclusion, the detection for MRD remain an important goal for therapy including chemotherapy and bone marrow transplantation in hematopoetic malignancy.

A case of malignant mastocytosis with circulating mast cell precursors: biologic and phenotypic characterization of the malignant clone.

The phenotypic and biologic properties of malignant cells in a case of aggressive mastocytosis with multi-organ involvement, circulating mast cell precursors and absence of skin infiltrates were analyzed. Circulating mast cell precursors were detected by immunostaining using antibodies against mast cell tryptase as well as by electron microscopy. These progenitors were tryptase+/chymase- (MCT) and accounted for 10 to 20% of nucleated mononuclear blood cells (MNC). A subset of them contained metachromatic granules. As assessed by combined toluidine blue/immunofluorescence staining, the granulated mast cell precursors were found to express CD9 (P24), CD33 (gp67) and CD44 (Pgp-1), but not basophil-related markers (CD11b (C3biR), CDw17 (lactosylceramide), CD123 (il-3R alpha))or monocyte-related antigens (CD14, CD15). Expression of the mast cell growth factor (MGF) receptor, c-kit(CD117), was also demonstrable, whereas the skin mast cell marker C5aR (CD88) could not be detected on mast cell precursors. The ligand of c-kit, recombinant human (rh) stem cell factor (SCF = MGF), induced histamine release from circulating mast cell progenitors, whereas rhC5a, a potent skin mast cell-/basophil-agonist, was ineffective over the dose-range (10(-9) to 10(-7(M)) tested. Analysis of mast cell antigens in malignant mastocytosis or mast cell leukemias may be helpful to establish a diagnosis and to determine the phenotype of the clone.
