Immediate reconstruction using a scalp-forehead flap for the entire upper lip defect with the application of lyophilized porcine skin to surgical wounds. A case report of a malignant melanoma in the upper lip and oral mucosa.

A rare extensive malignant melanoma involving the upper lip and maxillary alveolar mucosa was removed by en bloc resection after chemotherapy and radiotherapy. Simultaneous immediate reconstruction of the entire upper lip defect was carried out with a bitemporal pedicle flap which included the scalp and forehead after applying lyophilized porcine skin to the wound where the alveolar bone had been resected, and the bilateral scalp-forehead and thigh donor sites. A partial denture was inserted six months postoperatively. Now, one and a half years after operation, the patient is quite satisfied with his cosmetic appearance and masticatory recovery.

Collagen corneal shields enhance penetration of topical prednisolone acetate.

We used an animal model to compare the corneal and aqueous penetration of topical prednisolone acetate 1% with and without the presence of a commercially available collagen corneal shield. A significant (P less than .02) increase in penetration into cornea and aqueous humor at 30 minutes and 120 minutes was found in eyes with a collagen corneal shield in place. Increased concentrations were achieved by soaking the shield in prednisolone acetate for 15 minutes prior to application.

Chest wall resection---alloplastic replacement.

In a retrospective study of 302 patients, who were operated on in our centre between 12/85 and 9/87, 25 patients required a chest wall resection. The most frequent indication (11 times) was an infiltration of the thoracic wall by bronchial carcinoma. 8 times we resected a primary rib tumour, in rarer cases a thoracic wall resection was required for mesothelioma, echinococcus alveolaris and tuberculoma. In 10 patients the defect was too small to require a patch. Initially we applied Marlex-mesh and Vicryl-mesh to cover the defects. In our last 10 cases GORE-TEX-2 mm-patch was used. The technical ease of the method, the mechanical stability and durability, the tightness for air and fluid and the positive tissue response speaks for the continued application of this material. Additional methods such as myocutaneous flaps are not necessary and should only be used in complex defects.

[Long-term experiences with lyophilized soft dura (Lyodura S) as a connective tissue substitute in pediatric surgery].

Between 1979 and 1985 plastic closures with lyophilised softened dura mater were performed in 45 children with the following indications: gastrochisis: 8; omphaloceles: 2; meningomyeloceles: 29; meningoceles: 3; diaphragmatic aplasia: 1; oesophageal atresia with tracheo-oesophageal fistula: 2. Infection as well as rejection of the implants were never observed. Mortality came up to 11%. Complications and causes of death are described. A follow-up was carried out postoperatively at semi-annual intervals. The scars showed good functional and cosmetic results.

Unusual complication with use of lyophylized dural graft.

A patient, who had undergone removal of a meningioma and the involved dura, developed Jacksonian fits in the postoperative period. There had been precocious resorption of the patch of lyophilized dura mater used to repair the dural defect and this was followed by development of a pseudomembrane adherent to the cortex. After its removal, the patient recovered completely. Activation of reactive reparative processes, proliferative action by fibroblasts, reabsorptive phenomena, new vessel formation, are the main pathological events involved in this as in similar cases, but their sequence and the precipitating cause are not easily recognizable.

Fundamentals of acute burn care and physical therapy management.

The purpose of this paper is to discuss general treatment guidelines for treating patients with acute burns and to review pathophysiology of acute burn injuries. I will discuss skin composition briefly, give a review of acute burn pathophysiology and medical treatment, outline the psychological factors physical therapists should consider when working with these patients, and discuss physical therapy for the patient with acute burns. Physical therapy for patients with burns has become a specialty as knowledge of burn injuries increases. I have attempted to review physical therapy burn knowledge for inexperienced clinicians beginning to treat patients with acute burns.

Porcine xenografts in the treatment of pressure ulcers.

Porcine heterografts are effective in the treatment of cutaneous ulcers as biological bandages that promote healing by: speeding the processes of granulation and epithelialization; maintaining a clean covering over the wound; reducing loss of fluid and protein; and preventing bacterial encroachment.

Decrease in rate of wound contraction with the temporary skin substitute biobrane.

In burn patients normal wound healing contraction may lead to contracture deformity requiring secondary reconstruction. Full-thickness skin grafts are observed to inhibit both contraction and myofibroblasts by an unknown mechanism. The temporary synthetic skin substitute Biobrane has become an acceptable alternative to cadaver allografts for many burn and reconstruction wounds. We have postulated that this synthetic membrane might also share the ability to inhibit wound contraction. By comparing open and Biobrane closed wounds in 20 rats, we have demonstrated a significant (p less than 0.005) difference in wound size during the time that the Biobrane is adherent to the wound. Linear regression curves are used to express the rate of contraction and demonstrate a threefold decrease. Alternative mechanisms for this inhibition are postulated, including mechanical stenting versus direct inhibition of contractile myofibroblasts.

Silver-impregnated porcine xenografts for treatment of meshed autografts.

Meshing of skin grafts has gained wide acceptance as a means of conserving donor sites in cases of extensive skin loss. In smaller injuries, meshed grafts have the advantage of providing for more efficient wound drainage than unmeshed grafts. Covering widely meshed skin grafts with meshed allografts has been found to result in reductions in infection-related rejection of the allografts. Porcine xenografts have been widely used as temporary wound coverings as they protect the wounds and provide a hospitable environment for reepithelialization. We have used silver-impregnated porcine xenografts to cover meshed skin grafts with excellent results. The porcine xenograft provides a protective wound covering that promotes epithelialization in the interstices of the meshed graft, and the silver ion in the collagen of the xenograft provides an improved means of controlling infection. Three cases are described.
