The champagne angle.

A patient's observation led us to investigate whether drinking from a champagne flute required more cranio-cervical extension than drinking from other types of wine glasses. We measured the cranio-cervical extension required by normal volunteers to drink from four different types of glass. The mean [95% confidence intervals] extension from the neutral position required to drain each glass was: narrow flute 40 degrees [35-44]; wide flute 22 degrees [19-25]; wine glass 26 degrees [24-29]; champagne saucer 0 degree [-1-2]. Drinking from the narrow rimmed champagne flute required significantly more extension than the other types of glass (p < 0.001), and 73% of the total available cranio-cervical extension.

Removal of a mis-swallowed long spoon via gastrotomy--a case report.

Foreign body ingestions are a frequent occurrence, a common presentation at emergency departments, but long, pointed-end metallic spoon swallowing is an uncommon occurrence. Unlike most cases of foreign-body ingestion, there have been no cases of spontaneous passage reported. Consequently, prompt removal is recommended before complications develop. We report a case of accidental ingestion of a long, pointed metallic spoon which failed to pass through the gastrointestinal tract and be excreted. This patient was managed successfully with gastrotomy to remove the object. We recommend this approach for the removal of long pointed foreign bodies to avoid complications developing.

[Lead poisoning--a case report].

Lead poisoning may cause irreversible health defects, including anaemia, central nervous system problems and various organ defects. We describe a patient with lead poisoning. A 54-year-old woman was admitted to hospital with anaemia and unspecific gastrointestinal symptoms. Peripheral blood smear and bone marrow aspirate showed basophilic stippling of erythrocytes suggestive of lead poisoning, which was confirmed by high concentrations of lead in her blood. The lead source was the glazing of a ceramic wine jug. Chelating therapy was started. Haemoglobin was normalised; the patient returned to work after nine months. Correct diagnosis and treatment can prevent serious health problems caused by lead poisoning.

Migration and material tests of some food-contact plastic wares made in Thailand.

Migration and material tests of food-contact plastic wares were carried out in compliance with the Thai Food Act. Sample materials studied were melamine resin, polyethylene, polypropylene, polystyrene, and polyvinyl chloride. Migration levels of phenol, formaldehyde, colors and heavy metals were determined. A test for vinyl chloride monomer in polyvinyl chloride was carried out. Migrant and residual levels in all samples were in compliance with the Act.

[Migration test of lead and cadmium from plastic wares in contact with food].

Release of lead and cadmium from plastic tableware was examined. Samples were treated with 2 ml of 4% acetic acid or 0.1N HCl per cm2 of the surface area at 60 degrees C or 95 degrees C for 30 min. No migration of lead and cadmium from samples containing up to 391 ppm lead or 6863 ppm cadmium was observed.

[Determination of volatile substances and leachable components in polystyrene food contact wares].

Material and migration tests of food-contact plastic wares made of polystyrene were carried out. The average concentration of volatile substances (sum of toluene, ethylbenzene, isopropylbenzene, n -propylbenzene, and styrene) in materials was 861 +/- 692 ppm. Styrene was observed in 95% of 19 samples. alpha-Methylstyrene, which is one of the material compounds of polystyrene, was detected in one sample. Release of volatile substances into n-heptane was not observed at the detection limits of 0.1 ppm.

Burns associated with fondues.

OBJECTIVE: To describe the causes of burns associated with fondues. DESIGN: Descriptive case series. PATIENTS: All 17 patients admitted to a burn centre between Apr. 1, 1985, and Mar. 31, 1990, whose burns were associated with fondue. Eleven agreed to complete a telephone interview. RESULTS: The age of the 17 patients varied from 2 to 56 (mean 27) years. Two causes were identified: spilling of the contents of the fondue pot and explosion of the fondue fuel when added to the burner during a meal. The telephone interview revealed that eight people other than the respondents were burned during the same accidents. CONCLUSION: Although we identified only badly burned patients the problem may be more extensive. The knowledge of specific causes of burns from handling fondue equipment indicates that preventive action should be undertaken. More epidemiologic information is needed to obtain a precise estimate of the magnitude of this public health problem.

Factors affecting plasma aluminum concentrations in nonexposed workers.

In this study, the distribution and determinants of plasma aluminum concentrations were examined in 71 office employees not occupationally exposed to aluminum. The samples were analyzed by Zeeman graphite furnace atomic absorption spectroscopy and were found to be log normally distributed. After using the International Federation of Clinical Chemistry (IFCC) recommended procedure for removal of likely aberrant values, the 95th percentile value was 198 nmol/L (90% CI:165-238); when those using antacids were also excluded, the 95th percentile value fell to 175 nmol/L (90% CI:147-208). Multiple regression analysis indicated that the factors most predictive of log plasma aluminum were the batch in which the sample was analyzed and the use of antacids containing aluminum. The statistical significance of the batch variable likely indicates the well-recognized problem of contamination in sampling and analyzing aluminum.

Effects of shift changes on female workers at a dish factory.

This study investigated the effects of working night shifts on social and family life by examining changes in workers' daily life before and after a change in their shifts. Subjects were 40 women aged 27-59 years, working at a dish factory. During the health examination of night workers in autumn of 2000, the subjects were directly interviewed about changes in their lives induced by the shift change. Question parameters consisted of 8 items including 30 sub-items related to social and family life, such as sleep, rest, meals, sports, family time, hobbies, neighborhood association and social activities. The subjects selected one of four response categories: "becoming worse", "no change", "becoming better" and "difficult to determine." With regard to the percentage of "becoming worse", meal-related items ranked high in all of the shift types. "Family time" and "hobbies" showed high percentages in the subjects transferring from day shifts to night shifts, and in those transferring from early-morning shifts to night shifts. "Rest", "sports" and "hobbies" showed high percentages in the subjects transferring from night shifts to midnight shifts. Decreased sleeping hours were confirmed in all of the shift types, while the subjects tended to sleep more soundly. As the workers transfer to shifts at earlier hours, they were obliged to make sacrifices in various aspects of their social and family life. Therefore, much assistance in this regard should be given to them.

Impacted woodstick needles in the antrum: a unique foreign body.

Three cases of impaction and penetration of a unique type of foreign body in the gastric antrum are reported. The foreign body-woodstick needles-used to staple leaves together to produce disposable leaf-plates (khalli-patra) was accidentally swallowed with food and presented as a case of acute abdomen. Awareness of this possibility is essential for a timely diagnosis in regions where the practice of using khalli-patra is in vogue, particularly eastern India.
