[Integrated approach to the oral health of drug-addicted undergoing rehabilitation].

Considering the current context of increasing and early utilization of licit or illicit drugs (habit-forming substances), the aim of this study was to evaluate the oral health conditions of 102 males from the Esquadrão da Vida rehabilitation center, in Bauru, São Paulo, Brazil. The evaluation was carried out by means of the assessment of caries experience (DMFT), dental plaque (PHP), gingival index and salivary flow. This study also aimed at instituting an educational-preventive program and evaluating its efficacy. The subjects' mean age was 29.17 years (sd = 11.40; n = 95). From all subjects (n = 102), 72.54% had the habit of eating between meals, and 69.60% did not utilize dental floss. The observed DMFT was 14.88 (sd = 6.38; n = 86). The initial PHP was 2.34 (sd = 0.69; n = 14) and, after 3 months, it was reduced to 1.91 (sd = 0.74; n = 14). The initial salivary flow was 1.31 ml/min (sd = 0.7; n = 97). Due to the fluctuation of this group, only 13 of the original subjects were reevaluated after 3 months, when a salivary flow of 1.13 ml/min (sd = 0.49) was registered. The initial gingival index and PHP presented r = 0.30 (p = 0.002 - statistically significant). The objective need for treatment and the subjective importance given to oral health were negatively correlated (r = -0.26; p = 0.019 - statistically significant), which indicates that the value attributed to oral health influences health care and affects the oral health-sickness process.

[The condition of the 2d milk molars and the 6 year molars in a Copenhagen suburban community].

The increase in fillings, i. e. in caries, of the second deciduous and first permanent molars was recorded for a period of 18 months, using the dmfs/DMFS index, in 117 children, aged from 7 to 9 years, of the Copenhagen suburb of Brøndby. According to the findings concerning the second deciduous molars, the children were subdivided into three groups of caries incidence. The three groups differed but little in caries progression, which may be explained by the fact that the school dental service ensures the regular treatment of the children and achieves fairly stable results.

[Motivational sensitivity of pregnant women for oral hygiene and use of fluoride tablets].

200 pregnant women (group 1) attending a four-week cure in a home, 50 pregnant dental office patients (group 2), and 50 non-pregnant patients of the same age were included in this study. The preventive care program (information on oral hygiene, fluoridation and healthy nutrition, instructions and training) showed its greatest effect in the women of group 1, reflected in a reduction in OHI and GPM index and in an improved health awareness. All of the probands of group 1 who had planned during pregnancy to use fluoride tablets for their babies, put this plan into effect. In group 2 this percentage was only 46%. Pregnancy is a phase of increased motivational sensitivity and this should be exploited by doctors and dentists in terms of preventive programs for individuals and groups.

Spatial analysis to identify differentials in dental needs by area-based measures.

OBJECTIVES: To examine the association between tooth decay and dental treatment needs in 5-12-year-old schoolchildren in São Paulo with area-level indicators of social development. METHODS: The present study refers to a representative sample of children from the city of São Paulo, Brazil, comprising 2491 girls and boys attending public and private schools in different areas of the city. The assessment of caries and treatment needs followed the international methodological standards prescribed by the World Health Organization. We used spatial data analysis to describe epidemiological measures distributed by small areas, and to explore hypotheses of ecological association between caries indexes and indicators of social development. RESULTS: Schoolchildren in central districts were less affected by tooth decay and presented fewer dental treatment needs when compared to those in peripheral deprived areas. At the spatial level, average family income, unemployment rate, household overcrowding, and an index of inequality of income distribution were significantly correlated with variables measuring children's caries experience. CONCLUSIONS: The heterogeneous distribution of caries indexes by areas of the city indicates higher levels of dental decay in areas of social deprivation. The delimitation of areas with increased risk of caries and greater dental treatment needs should be helpful to public health services for the formulation of policies and the targeting of resources to address these problems.

[Dental caries experience in children at public and private schools from a city with fluoridated water].

The aim of this study was to verify the relationship between type of school as a measure of socioeconomic conditions and caries prevalence among preschoolers and schoolchildren in Rio Claro, São Paulo State, Brazil, a city with fluoridated water supply. The data were secondary, from a sample of 888 children 5 to 12 years old enrolled in private and public schools. Caries was measured by the dmft and DMFT indices as well as the Care index. Qui-square and Mann-Whitney tests were utilized with 5% significance. In 5-year-old children, mean dmft was 2.50, and 42.20% were caries-free. At age 12, mean DMFT was 2.70 and 28.90% were caries-free. Caries prevalence rates in public schoolchildren as compared to private were 74.50% and 61.20%, respectively (p < 0.0001), and the dmft and DMFT scores were the highest in public schoolchildren (p < 0.05). The Care Index was higher in private schoolchildren (71.20%) as compared to public (52.80%). Highest caries rates were found among public schoolchildren, so the variable type of school proved sensitive for discriminating different oral health conditions; however limitations need to be recognized, suggesting that other variables should be assessed.

Dental caries in players belonging to a Swedish soccer team.

Since dental caries is associated with frequent use of sugar-containing products and since there is a potential risk for such a consumption among sportsmen, we conducted a clinical study of a group (n = 30) of 17-30-year old elite soccer players, including DFS and various caries-related factors, such as plaque index, numbers of cariogenic microorganisms in saliva and sugar intake frequency. Each player was asked to identify one friend of the same age, sex and social background but not active in any sport (n = 28). The mean values for DFS, plaque index, mutans streptococci and lactobacilli were less favourable for the players, but none of the differences were statistically significant, except for the dietary score (p less than 0.05). Thus, soccer players on an elite level seem to have a somewhat higher risk to develop dental caries than nonplayers. However, their regular use of fluoride-containing toothpaste probably to some part compensates for this increased risk.

Prevalence of dental caries in handicapped children of Calcutta.

A total of 1042, 3-14-year-old children with different types of handicapping conditions when recorded for dental caries using WHO 1987 caries recording index revealed that dental caries experience was higher in handicapped children than normal children. The prevalence of dental caries was highest in mentally retarded children followed by cerebral palsied, blind, epileptic, physically handicapped, children with Down's syndrome and deaf and dumb. Higher deft+DMFT was recorded in mandibular teeth compared to maxillary teeth.

Impact of diagnostic criteria on the prevalence of dental caries in Norwegian children aged 5, 12 and 18 years.

The study aimed to investigate caries prevalence in children using a severity grading diagnostic system, and to evaluate the influence of different diagnostic thresholds on the caries data. A group of 513 children, aged 5, 12 and 18 years, were examined clinically, and with available bitewing radiographs, by four calibrated examiners. The inter- and intra-examiner reliability, assessed by a weighted kappa, varied between 0.80 and 0.95. The mean dmft/DMFT values were 3.8, 5.8 and 11.0 for the three age groups respectively, and the corresponding dmfs/DMFS values were 5.4, 9.9 and 22.6. The d/D-component constituted the major part of the dmf/DMF index in all age groups, and enamel lesions accounted for 59%, 89% and 86% of the d/D-component in the three age groups respectively. It is concluded that enamel or initial caries lesions contributed substantially to the total caries prevalence, illustrating the importance of using diagnostic criteria that include all stages of clinical caries if a total picture of the caries situation is needed.

Estimation of optimal concentration of fluoride in drinking water under conditions prevailing in Chile.

The purpose of this comparative study of caries and dental fluorosis experience in Chilean children was to estimate the optimal range of fluoride concentration in tap water under conditions currently prevailing in Chile. The sample included 2431 schoolchildren 7, 12 and 15 years old, life-long residents of five communities with fluoride concentrations in their tap water in the range 0.07-1.1 mg/L. The study population received an oral clinical examination including caries experience and an enamel fluorosis evaluation of the permanent dentition (Dean's scoring system). For 15-year-old children, the DMFT index changed from 5.06 to 2.60, and for 12-year-olds it changed from 3.10 to 1.36 when fluoride water concentration changed from 0.07 to 1.10 mg/L. For 7-year-old children the dmft index correspondingly changed from 3.67 to 1.59. The relationship between DMFT for 12-year-olds and water fluoride concentration was best fitted by a logarithmic function (r2=0.98). The Community Fluorosis Index (CFI) was used to assess enamel fluorosis in the study population, and it showed a linear relationship (r2=0.983) with increasing fluoride concentration of water for the 12-year-old group. Results obtained suggest that under current Chilean conditions, the optimal range of fluoride concentration in potable water should lie in the 0.5-0.6 mg/L range.

Dental status and oral health quality of life in economically disadvantaged French adults.

Expansion of French health insurance coverage has increased funding for dental care for economically disadvantaged adults. This study aimed to measure clinical and self-perceived oral health, behaviors, and use of dental services by adults who were eligible for such coverage. The regional agency that gives administrative services for the health insurance funds provided a sample of 900 adults aged 35-44 years, insured through this program. We reached 805 of these adults by mail; of these 18% were surveyed and clinically examined. Self-perceived oral health was measured by the Global Oral Health Assessment Index (GOHAI) and participants' attitudes to dental health, by questionnaire. Decayed and Missing teeth constituted 40% of the DMFT. Participants reported poor oral health (63%), and 79% perceived a need for care, although they used dental services infrequently and had poor knowledge of available services. Cost of care and number of carious teeth were important predictors of the GOHAI.

[Oral status of drug addicted patients. Study of 66 cases].

We studied 66 heroin-addicted patients from Valencia Community, comparing them with a control group of 30 healthy subjects. In all cases the CAO index and gingival state were analysed, including possible pathology of oral mucous. It was revealed that the major oral deterioration was in the experimental group. This was directly related to oral hygiene deficiency, which was brought about in the period of active addiction. At the same time the difference in oral mucous pathology between the two groups was not significant.

[Examination of the pH of saliva in children and juveniles in relation to caries, gingivitis and oral hygiene].

The pH rate of unstimulated fasting saliva in the morning in vivo and the following DMF/T, DMF/S, OH indexes and the PIe had been measured or ascertained in 70 subjects aged 10 to 17 years. The analyses stated that there does not exist a direct relationship between incidence of caries and the degree of inflammation of the gingiva and the concentration of saliva hydrogen ions as well. The pH rate of saliva is stated to be also a favourable factor for plaque formation.

Dental caries and restorations in teenagers. I. Index and score system for radiographic studies of proximal surfaces.

An index and a score system for radiographical longitudinal studies of caries progression on proximal tooth surfaces are presented and compared with the DF-surface system. Generally, a good correlation between DF-surface differences and scores was found in a three-year study on teenagers. However, the score system seems to forward better means for a detailed study of the individual.

Caries pattern in primary molars in Dutch 5-year-old children.

AIM: The aim of this study was to investigate the difference in caries prevalence based on quadrant dmfs data between first and second primary molars in 5-year-old Dutch children. STUDY DESIGN: Cross-sectional observational study. METHODS AND STATISTICS: For this study 692 children, all insured by a "Health Insurance Fund", living in one of four selected cities in The Netherlands were asked to participate in the study. From the original cohort 435 children (49% girls) participated. Clinical examinations were performed and only carious lesions with involvement of the dentine were reported. Lesions on the occlusal, buccal, palatinal/lingual, mesial and distal surfaces as well as lesions in buccal and palatinal pits and fissures were reported separately. No radiographs were taken. Systematic differences in dmfs between first and second molars in the same quadrant of each primary dentition were tested with the Wilcoxon signed rank test. RESULTS: Second primary molars, even after correction for caries in pits and buccal/palatinal fissures, had a statistically significant higher total dmfs than the first primary molars. The differences were mainly found on the occlusal surfaces. On proximal surfaces, the first primary molars had significant more caries than the second primary molars. The d-component constituted the major part of the caries index. CONCLUSIONS: Second primary molars, corrected for decay in the pits and buccal/palatinal fissures of this molar, are more affected by caries than first primary molars and that the differences in caries prevalence are the largest on the occlusal surface. The specific site of the caries found suggests that developmental disturbances in second primary molars may attribute to their prevalence.

Oral health condition of 12-year-old handicapped children in Flanders (Belgium).

The dental condition of 626 12-year-old handicapped children with mild mental or moderate to severe mental retardation or learning impairment, being 25% of the population of each of these groups, was examined in Flanders. An evaluation of oral cleanliness showed poor oral hygiene in 31.8% of the children. No significant differences were found in oral cleanliness among types of handicapping conditions. The mean DMFT score was 2.9 (s: 2.6) and DMFS score was 5.4 (s: 5.6). Almost 21% of the children were free of caries or fillings. No significant differences were found among groups of handicapped children. Handicapped children presented a low level of restorative care (restorative index score: 48.7%). Mildly mentally retarded children demonstrated the lowest restorative index (43.9%). The caries experience of first permanent molars represented the largest part of the DMFT score (64.1%). Sealants were present in 7.9% of children examined. A considerable percentage of mildly mentally retarded children and learning impaired children did not brush daily (22.1% and 20.9%) and did not receive help with toothbrushing from their parents or carers (91.0% and 94.7%, respectively).

[Epidemiologucal studies on the health condition of the masticatora organ as related to sugar consumption].

In the Republic of Cuba, the authors compared, using the method of dietary interviewing, the oral findings (DMF/T, OHI-S, PDI and plaque pH) of children with primarily caries-free dentitions (25 children), with a DMF/T index up to 2.0 (50 children) and with a DMF/T index of more than 6.0 (50 children) with their eating habits. The nutrition index determined for the sugar consumption in the last-mentioned group was by 38% (meals) and by 48% (between-meal snacks) higher than that found in the group of primarily caries-free children. The other indices were also significantly higher. The group with a DMF/T index up to 2.0 holds an intermediate place. Since all the children originated from the same region and since there were no differences in dental and oral care between the groups, the individual differences in oral hygiene and in diseases of the dental hard structures and of the periodontium must be attributed to the individual sugar consumption, the between-meal snacks seeming to play a dominating role.

[Caries morbidity in the odonto-stomatologic services in Yaounde, Cameroon].

In our regions, it is believed that dental decay is an illness without prestige or danger for the life. It's the reason of teeth loss and many complications. The objective of this study was to valuate the morbidity of this pathology in odonto-stomatology services. With an average of 4.5 teeth decays per person and a morbidity index of 0.15; dental carie concern all socio-professional levels, without distinction of age or sex. Women seem more vulnerable to teeth decay, and they consult services more than men. We are potential edentulous because of dental decay, if preventive and training measures in mouth and tooth healths are not taken in our country.
