Taichung Veterans General Hospital has been developing a hospital-wide picture archiving and communication system (PACS) since 1993. A personal computer-based environment was implemented to reduce costs (only $2,500 for each view station) and take advantage of distributed system techniques. Other features of the PACS are automatic image acquisition, hierarchic storage management, efficient image transmission, robust fault tolerance, and user-friendly image manipulation. The system is integrated with the hospital information system so that Chinese-language patient data can be automatically transferred. A four-tier storage hierarchy and a multipath search strategy are used to improve reliability and efficiency. Image compression and efficient image transmission techniques (autorouting and prefetching) are used to reduce the response time. Robust fault tolerance is achieved with fault-tolerant hardware, image replication, and a system watchdog. User-friendly image manipulation features include easy adjustment of the brightness, contrast, or quality of the displayed image; several windows for image display; and image measurement capability. The PACS currently supports computed tomography, ultrasound, magnetic resonance imaging, computed radiography, and digital fluoroscopy; almost all appropriate personal computers in the hospital can be used as view stations. Users are satisfied with the quality, reliability, and performance of the system.

Over the past few years, voice mail has spread rapidly through the corporate environment. Healthcare facilities were slow to adopt this form of telecommunication until it became clear that healthcare was indeed a business, and this innovative technology was becoming a permanent fixture in the business world. Unfortunately, basic two-way communication with compassion, which is so critical to the success of healthcare, became peripheral to computer-mediated technology. While there is debate over the voice mail appropriateness in healthcare, one point of agreement is that voice mail is here to stay. The author examines one hospital's experience with computer-mediated communication, its initiative for improvement without monetary options, and its evolution toward a more compassionate, humanistic system for its internal and external customers.

PROBLEM: Store-and-forward clinical messaging system was cumbersome and inadequate to support modern technology. SOLUTION: Installation of a secure Web-based system that automates communication among physicians, hospitals, clinical laboratories and other departments. RESULTS: Time savings for office personnel and physicians; access by physicians to clinical information anytime, anywhere; consolidation of lab results into one report; customized form--filing capability. KEY TO SUCCESS: Comprehensive and ongoing training to ensure user acceptance.

The performance of a medical imaging department (MID) can be judged by the timing and availability of an image and its report to the treating physician, as this can impact on the patient's treatment, as well as the length and cost of a patient's hospitalization. The use of digital imaging has dramatically improved report turnaround time. In October 2001, The Townsville Hospital (TTH) was opened as a 460 bed greenfield site and as part of the installation a picture archiving and communication system, including web distribution of images and reports to wards and clinics, was included. This retrospective analysis of the MID at TTH is the first data on departmental productivity and individual workload and how these have changed since the hospital's opening 2 years ago, with some ideas for further improvement.
