Myositis ossificans of the chest wall.

We report the unusual case of a young man with progressive pain in the thoracic wall. The radionuclide bone scan revealed an increased uptake, and the bone roentgenogram, a calcified soft-tissue mass. Based on computed tomography findings, biopsy was avoided, and evolution was favourable for myositis ossificans. Although rare, myositis ossificans is one of the potential causes of thoracic pain, not to be mistaken for a malignant or infectious lesion.

Autoimmune pancreatitis associated with idiopathic retroperitoneal fibrosis: a case report.

A 69-year-old man presented with obstructive jaundice and dark urine. Contrast-enhanced computed tomography revealed an enlarged pancreas with homogenous enhancement. Endoscopic retrograde pancreatography demonstrated short-segmental, irregular narrowing of the main pancreatic duct. The patient underwent exploratory laparotomy and needle biopsies of the pancreas, which showed marked fibrotic change with lymphocyte infiltration. These clinicopathologic findings suggested autoimmune pancreatitis. Four years later, computed tomography demonstrated marked periaortic soft tissue surrounding a calcified infrarenal abdominal aorta compatible with retroperitoneal fibrosis. We diagnosed retroperitoneal fibrosis with noncontiguous pancreatic fibrosis. This patient responded well to corticosteroid treatment. Autoimmune pancreatitis associated with idiopathic retroperitoneal fibrosis seems to be extremely rare, and to our knowledge, only a few cases have been reported.

[A case of neurinoma of the nasal septum].

Neurinoma grows in the modulated nerve with Schwann cells, and is observed for all positions of the body. It is comparatively rare for neurinoma to grow in the nasal cavity and paranasal sinuses, however. We report a case of neurinoma of the nasal septum. A 24-year-old man seen for nasal obstruction was found in anterior rhinoscopy to have a tumor of the bilateral nasal cavity. Computed tomography (CT) showed a soft tissue density mass in the anterior part of the nasal cavity. Because we suspected neurinoma from biopsy, we resected the tumor by Denker's operation under general anesthesia. We found that it originated from the nasal septum. Histopathological findings of the tumor indicated an Antoni A type neurinoma. After surgery, cartilago costalis was transplanted the plastic surgery to correct the defect in the nasal septum. The post operative course was good and no signs of recurrence were seen. This is the 19th case found in the literature.

Toothpick foreign body in the foot. A case report.

A case report of a wooden toothpick as a foreign body in the foot was presented. Such lesions are visualized poorly, if at all, on standard radiographs, and localization of the foreign body may not be possible. Computed tomography, with its superior soft tissue imaging capabilities, can detect this otherwise radiolucent wood material and make accurate localization of the foreign body possible.

[Aggressive angiomyxoma in the scrotum: a case report].

A case of aggressive angiomyxoma occurring in the scrotum is reported. A 57-year-old Japanese man presented with a 2-year history of non-tender soft mass in the scrotum. Computed tomography and magnetic resonance imaging revealed a well-defined mass 8 cm in diameter extending from the medial aspect of the left ischium. Angiography revealed faint vascularity fed by the left internal pudendal artery. Excision was performed by a scrotal approach, and histological examination confirmed an aggressive angiomyxoma. The patient showed no recurrence at 3 months follow-up.

[Extensive Fournier's gangrene: a case report].

We report a case of extensive Fournier's gangrene that could not be rescued despite emergent debridement. A 51-year-old man presented at another hospital with cough, diarrhea and abdominal pain. He was diagnosed with acute enteritis and hospitalized. The next morning, he became severely hypotensive and his scrotum was swollen and black. The perineal skin also was black. Septic shock and disseminated intravascular coagulation were suspected. He was transferred to our emergency room, and was immediately diagnosed with Fournier's gangrene and acute peritonitis. Computed tomographic scan revealed soft-tissue gas in the scrotum, the retroperitoneal cavity and the abdominal wall. Emergent debridement and laparotomy was performed. Gangrene was also seen at the intestinal wall and the peritoneum, however, resection of intestine was not done because of his poor performance status. Although potent antibiotics and catecholamine were administered, he died of multiple organ failure 29 hours after the operation. This is the first case of Fournier's gangrene extending into the abdominal cavity reported in the Japanese literature.

Extraparotid Warthin's tumor presented as a neck mass.

Warthin's tumor is one of the benign tumors of major salivary glands. It was mostly found in parotid glands of male patients in their sixties. Warthin's tumor with extraparotid location is rare. We here report a 65-year-old male patient with a movable mass about 25 x 20 x 15 mm over his leftjugular-digastric area. The fibroscopic examination found no malignancy over the head and neck region. Computed tomographic scan showed a soft tissue mass about 18 mm in diameter in the left upper neck. The mass was not in connection with the parotid gland or the submandibular gland and was excised completely through a transverse neck incision. Histopathologic examination revealed Warthin's tumor. We present an original report of a case of Warthin's tumor with its unusual extraparotid location.

[Thymolipoma in association with myasthenia gravis; report of a case].

An 18-year-old man was admitted with easy fatigue and muscle weakness. A diagnosis of myasthenia gravis was made. Chest X-ray showed no mediastinal abnormality, however, chest computed tomography (CT) showed a soft tissue mass in the thymus. The patient underwent extended thymectomy and small tumor (2 x 2 x 2 cm) was resected. On histological examination the tumor proved to be a thymolipoma composed of mature adipose elements containing cords and nests of thymic tissue. Symptoms of myasthenia gravis were dramatically improved after the operation. This case is the 12th in the world literature in which a thymolipoma is associated with symptoms of myasthenia gravis.

[Idiopathic retroperitoneal fibrosis causing obstructive uropathy: a case report].

Idiopathic retroperitoneal fibrosis leading to obstructive uropathy is a rare disease in our country, so a typical case deserves presentation. The patient is a 56-year-old female to have a clinical manifestation of obstructive uropathy. Retrograde pyelogram permitted 5 Fr. ureteral catheter to pass the stenotic segment. At the mean-time, there was hydronephrosis, medial deviation and gradual thinning and pointing of the middle segment of the ureter (classic triad). Computed tomography revealed that retroperitoneal soft tissue plaque enveloped aorta, inferior vena cava, and bilateral ureters. The patient received ureterolysis with ureter intraperitonealization and lateralization. The plaque was pathologically proved fibrosis. Renal function became normal postoperatively.

Use of magnetic resonance imaging in the diagnosis of cortical blindness in pregnancy.

Magnetic resonance imaging (MRI) may be helpful in the assessment of eclampsia and preeclampsia with central nervous system symptomatology such as cortical blindness. We describe a rare case of complete binocular blindness postpartum with no other neurologic deficits, in which MRI abnormalities were undetected on computed tomography. The better soft-tissue discrimination of MRI may visualize important but subtle lesions which ultimately may help to explain the underlying pathophysiologic mechanism in such cases.

[The indirect calorimeter, "do it yourself"].

An original metabolic calculator is realized with common and fiable transducers: a "microgaz Roche Kontron 7640" gives the PiO2-PeO2 and PeCO2 (Clark and Stow Severinghaus electrods), a pneumotachograph Gould 17212 (or the reliable integrated flow meter of the respirator) gives the expiratory minute volume. These values are inserted in an micro computer (Sharp PC 1261). The soft is wrote so that results are not altered when high FiO2 are used. Consumption of lipids and glucids is displayed in percentage when diuresis and urea values are entered. This device is compared with a well known expenditure monitor: Deltatrac (Datex): simultaneous measures are carried out in patients with miscellaneous diseases (15 series of 10 measures). Regression curves appear to be very reliable, as well for VO2(r = 0.98), VCO2 (r = 0.94) energy expenditure (0.97) as for R.Q (r = 0.77).

[Contraception and informatics. SELF, a system to aid medical decisions applied to the prescription of contraceptive methods].

SELF (systeme en logique floue = system in soft discware), developed by the Computer Sciences Laboratory of the Faculté de Médecine in Nancy, is, by comparison to many systems of assistance in medical decisions already existing, a comprehensive and simple unit enabling the construction and modification of basic knowledge as well as its questioning. The first application of SELF concerned the field of gynecology, by producing a system of assistance applied to the prescription of a contraceptive method. The systematic use of this computer program along with the traditional prescription of a contraceptive method, was tested for three months in the Regional Maternity Hospital by several physicians in the Gynecology Clinic. The system was extremely well accepted and the responses have generated numerous discussions between physicians leading to an improvement of SELF and a better rationalization of the prescription. The medical and paramedical personnel in training at the hospital found SELF to be a teaching tool which, on the basis of simulated cases, permits an easier and more concrete explanation of the practical management in advising or prescribing a contraceptive method.

Clinics in diagnostic imaging (18). Multilocular cystic nephroma.

A male Chinese infant presented with the incidental finding of a left loin mass. Plain abdominal radiograph showed a large left-sided soft tissue shadow. Ultrasound and computed tomography scans demonstrated a multiloculated cystic lesion arising from the upper pole of the left kidney. Histological examination of the nephrectomy specimen confirmed the diagnosis of multilocular cystic nephroma. The differential diagnoses of infantile renal masses, the role of imaging in assessment of these masses and the surgical management of multilocular cystic nephroma are discussed.

Critical self assessment--is it too much for a surgeon?

Self-assessment can be performed by each individual surgeon or by a group of surgeons covering a sub-specialty at a hospital. The individual self-assessment programme can be performed by using appropriate soft-ware and a personal computer. In some countries such self-evaluation is an excellent preparation for re-certification examinations. Other parts of the self-assessment programmes should include indications for vascular surgery perhaps in the form of so-called indication conferences. Also morbidity and mortality conferences are of great importance to minimise similar problems after a broad multi-disciplinary discussion of complications and deaths which have occurred in the vascular service. By using vascular registers each individual surgeon can compare his own results with a national average or with the results of his colleagues at the actual hospital. Self-assessment programmes in vascular surgery should also include the results of diagnostic work performed in vascular laboratories. in this way we can get an impression of the sensitivity, the specificity, and overall accuracy for the procedures performed by the laboratory. A self-assessment programme should in conclusion be an integrated part of the practice of every surgeon.

CT and MRI in tuberculous sternal osteomyelitis: a case report.

We report a 58-year-old male patient presenting with a 1-year history of presternal swelling and pain. Plain radiography revealed increased soft tissue density anterior to the body of the sternum, which showed cortical sclerosis. Computed tomography (CT) demonstrated ring-enhancing hypodense soft tissue masses surrounding the sternum, whose anterior and posterior cortices were markedly thickened. On three-phase technetium bone scintigraphy, the left side of the sternum showed increased radiotracer uptake and the central part of the bone was photopenic. The bone marrow of the sternum and peristernal soft tissue lesions were hypo- and hyperintense on T1- and T2-weighted magnetic resonance (MR) images, respectively, and showed marked enhancement postgadolinium. Treatment included both surgical intervention and medical therapy.

Computerized axial tomography (CAT): the value in the diagnosis of bone and soft tissue tumours.

Computerized axial tomography (CAT) was used in the diagnosis of thirty-five cases of bone and soft tissue tumours. Using CAT and related computations for each tumour, the authors have constructed an atlas of tumour density distributions. CAT densities are useful in the pre-operative diagnosis of bone and soft tissue tumours. Also, CAT cross-sectional images can assess the anatomical location, in relation to the cortex, the medullary canal, and the neurovascular bundle. This is of crucial importance in planning the surgical treatment.

Image analysis and superimposition of 3-dimensional cone-beam computed tomography models.

Three-dimensional (3D) imaging techniques can provide valuable information to clinicians and researchers. But as we move from traditional 2-dimensional (2D) cephalometric analysis to new 3D techniques, it is often necessary to compare 2D with 3D data. Cone-beam computed tomography (CBCT) provides simulation tools that can help bridge the gap between image types. CBCT acquisitions can be made to simulate panoramic, lateral, and posteroanterior cephalometric radioagraphs so that they can be compared with preexisting cephalometric databases. Applications of 3D imaging in orthodontics include initial diagnosis and superimpositions for assessing growth, treatment changes, and stability. Three-dimensional CBCT images show dental root inclination and torque, impacted and supernumerary tooth positions, thickness and morphology of bone at sites of mini-implants for anchorage, and osteotomy sites in surgical planning. Findings such as resorption, hyperplasic growth, displacement, shape anomalies of mandibular condyles, and morphological differences between the right and left sides emphasize the diagnostic value of computed tomography acquisitions. Furthermore, relationships of soft tissues and the airway can be assessed in 3 dimensions.
