[TNM-oriented therapy planning in bladder cancer].

The standard diagnostic procedures for bladder carcinoma include transurethral resection, bimanual palpation of the bladder under general anaesthesia, ultrasound examination and CT scan of the abdominal and pelvic regions. An IVP is additionally carried out for the detection of concomitant upper urinary tract tumours. Preoperative urinary cytology is also performed, as in 10% of all patients the degree of malignancy of bladder cancer according to the TNM system is revealed more exactly by cytology than by histology. New computerized examination techniques have been designed in attempts to improve the accuracy of the diagnostic process. Therapeutic modalities according to the classification of bladder carcinoma in the TNM system have to be based on the different efficacies of the diagnostic procedure: In superficial low-grade tumours the diagnostic efficacy is quite high, so that transurethral resection remains the treatment of choice. Superficial high-grade tumours are attended by reduced 5-year survival rates, and patients with such tumours must therefore be regarded as a high-risk group. In muscle-invasive bladder carcinoma reduced diagnostic efficacy and unsatisfactory treatment results are seen, so that in this condition cystectomy at an early stage is indicated.

Post-surgical necrobiotic granulomas of urinary bladder.

Palisading necrobiotic granulomas of the prostate are an uncommon but well-recognized sequela of transurethral surgery. Similar granulomas occurring in 3 patients after transurethral surgery of the urinary bladder are reported. In each case, the lesions were discovered as a result of biopsies done subsequent to transurethral resections of bladder tumors. An idiosyncratic hypersensitivity reaction to a diathermically induced change in the antigenicity of the subepithelial connective tissue is suggested as the cause of these granulomas.

[Clinical investigation on the effect of intravesical instillation of antifibrinolytic agents with bacillus Calmette-Guerin on preventing bladder cancer recurrence].

OBJECTIVE: To investigate the effect of intravesical instillation of antifibrinolytic agents with bacillus Calmette-Guerin (BCG) on preventing recurrence of superficial bladder transitional cell carcinoma (BTCC) after surgical management. METHODS: A total of 326 cases of superficial BTCC undergoing transurethral resection of bladder tumor (TURBT) or partial cystectomy were divided into 5 groups. Then the different dosage BCG with or without antifibrinolytic agents was regular instilled into bladders (once a week, then once a month after 6 times). Group A including 66 cases received intravesical instillation of 100-120 mg BCG plus 100 mg para-aminomethyl benzoic acid (PAMBA). Group B including 64 cases: instillation of 50-60 mg BCG plus 100 mg PAMBA; Group C including 65 cases: 100-120 mg BCG plus 2.0 g epsilon-aminocaproic acid (EACA); Group D including 64 cases: 50-60 mg BCG plus 2.0 g EACA; Group E (control group) including 67 cases: 100-120 mg BCG. All the cases had been followed up for 4 to 69 months (mean, 28.5 months). Not only was cystoscopy performed every 3 months, but also biopsy was carried out to identify recurrence when necessary. Side effect was recorded after instillation. RESULTS: The rate of tumor recurrence of Group A, Group B, Group C and Group D was 12%, 10%, 9%, 9% respectively, which was significantly lower than that of Group E (30%) (chi(2) = 5.699, 6.818, 7.380, 7.867, P = 0.017, 0.009, 0.007, 0.005). And there was no significant difference of tumor recurrence rate between Group A and Group B or between Group C and Group D (Group A and Group C: high dosage BCG plus antifibrinolytic agents, while Group B and Group D: low dosage BCG plus antifibrinolytic agents) (P > 0.05). But the side effects developing in Group B and Group D after BCG instillation were less than those in Group A and Group C. CONCLUSIONS: The efficacy of BCG on prevention the recurrence of superficial BTCC can be enhanced when combined with antifibrinolytic agents. Even if the dosage of BCG was reduced by half, the efficacy didn't changed. A new approach of low dosage BCG plus antifibrinolytic agents is recommended in the prophylaxis of recurrence of bladder cancer.

[Results of combined surgical and radiotherapeutic treatment of the carcinoma of the urinary bladder of stage P2/P3].

The authors studied the survival rates of the P2 and P3 patients within a group of 287 patients suffering from carcinomas of the urinary bladder who had been treated by urologic operation and radiotherapy. A differentiation was made between the surgical methods transurethral resection (TUR), bladder wall resection and cystectomy. P2 patients presented significantly higher three-year survival rates after bladder wall resection or cystectomy (10/17) than after TUR (7/23). In stage P3, no differences were found with regard to the surgical method. The results are discussed and compared to those communicated in literature.

[Clinical experience with three cases of haemorrhagic tendency--bladder carcinoma with hemophilia A, a bladder carcinoma with von Willebrand disease and a benign prostatic hypertrophy with idiopathic thrombocytopenic purpura (ITP)].

Recent understanding of the pathogenicity of haemorrhagic tendency as well as advance of management of patients with this disease makes its surgical treatment possible. Since 1979 we have experienced three male patients with haemorrhagic tendency. The first case was a bladder carcinoma with hemophilia A. He had been given 2500 unit of Conco-eight (coagulation factor VIII) before transurethral resection of bladder tumor (TUR-Bt) and was given 1000 unit of Conco-eight every eight to twelve hours for seven days postoperatively. The second case was a bladder carcinoma with von Willebrand disease. This case was also pretreated by Conco-eight before operation and we had difficulty in controlling his coagulopathy for one month postoperatively. The third case was a benign prostatic hypertrophy with idiopathic thrombocytopenic purpura (ITP). He had been given a huge dose of gamma globulin for six days before operation and on the operation day his platelet count was normalized. Since his prostate was estimated to weight over 70 g, suprapubic prostatectomy was selected. All these three cases were operated uneventfully. From this experience we believe that with proper preoperative and postoperative management, patients with haemorrhagic tendency can be operated safely.

[Value of the photodynamic method in the diagnosis of superficial bladder tumors].

Photodynamic diagnosis (PDD) is one of most precise method of neoplasms recognition. The aim of the study was to estimate the value of 5-amino levulinic acid in diagnosis of superficial bladder tumours. Since 1996 to March 2004, 597 cystoscopies were performed with 5-amino levulinic acid in 151 patients with superficial bladder tumours (102 men and 49 women) aged 19-87 years old. 132 pts were treated by PDD after transurethral resection of bladder tumours (TURB). 749 specimens were taken from mucosa during PDD. Before PDD 50 ml of of 5-amino levulinic acid solution was instilled into bladder. As a source of ultraviolet light (UV) D--Light Storz xenon lamp (405 nm) was used. In 16 pts single PDD was performed in 38 double and in 97 PDD was performed three or more times (3 to 7). All pts were observed 12 months before and after PDD. In 102 from 151 pts in white light cystoscopy (WL) no changes in the bladder mucosa were found. In both kind of light (WL and UV) changes were not observed in 44 pts (29.1%). In 58 pts in WL no changes were observed but in UV places of neoplastic changes in the mucosa were found. In these cases the presence of superficial bladder tumours was confirmed. These changes were pathologically confirmed only in UV in 26 pts. Ta (G1 and G2) changes were confirmed in 52 pts (21 in WL and additionally 31 in UV). T1 changes were confirmed in UV in 26 pts but in WL in 25 pts. In 3 pts T2 changes were confirmed in both UV and WL. Cystoscopy in ultraviolet light (photodynamic diagnosis--PDD) with 5 amino levulinic acid gives more precise diagnosis of superficial bladder tumours then the one in standard white light. Cystoscopy performed to diagnose bladder tumours and early recurrence and new focus of tumours recognition without PDD is less effective. Introduction of superficial bladder tumours photodynamic diagnosis into health care system reduced recurrences and improves results of treatment.

[Usefulness of magnetic resonance imaging for evaluation of therapeutic efficacy of bone metastases from bladder cancer: a case report].

A 70-year-old man had a prostate-invading bladder cancer with multiple bone metastases. The bladder cancer seemed to have metastasized via the vertebral vein system because there was no metastasis in other organs. Transurethral resection of bladder tumor was performed followed by one course of M-VAC (methotrexate, vinblastine, doxorubicin) therapy and MEP (methotrexate, etoposide and cisplatin) therapy. After chemotherapy, magnetic resonance imaging (MRI) of the spine revealed further progression of disease, although bone scintigraphy did not, and the patient died of disease.

Transurethral resection and partial cystectomy for invasive bladder cancer.

Bladder-preserving modalities for patients with invasive bladder cancer have become increasingly popular in recent years. Surgical-only approaches, such as transurethral resection (TUR) or partial cystectomy, are unique among a variety of bladder-preserving modalities, most of which involve combination with radiation and chemotherapy. TUR and partial cystectomy remain incompletely evaluated due to relatively small series in the literature and the lack of standardized selection criteria. The outcome as measured by long-term bladder preservation and overall survival is not dissimilar to concurrent radical cystectomy series, possibly because of positive selection of patients.

Minimal transurethral prostatectomy plus bladder neck incision versus standard transurethral prostatectomy in patients with benign prostatic hyperplasia: a randomised prospective study.

OBJECTIVE: To evaluate the effectiveness and complications of the combination of minimal transurethral resection of the prostate and bladder neck incision in comparison with those of the standard transurethral resection of the prostate (TURP). PATIENTS AND METHODS: Forty patients with prostates of <or=25 ml were randomized to two equal groups (20 cases in each). In group I minimal transurethral resection of the prostate followed by bladder neck incision at the 6 o'clock position and in group II standard TURP were performed. Patients were evaluated in terms of the change in symptom severity, maximal flow rate and sexual function in addition to operating time, length of hospital stay, problems of catheter positioning at the end of the operation and the rate of bladder neck contracture using chi(2) and independent t tests. RESULTS: Age, prostate volume postoperative follow-up time, improvement in international prostate symptom score and maximal flow rate, length of hospital stay and erectile dysfunction rates were not statistically different between the 2 groups. Operation time was shorter in group I (p = 0.001). Although the urethral catheter was easily positioned at the end of the operation in group I, it was not possible in group II for 4 patients (p = 0.053). Postoperative retrograde ejaculation for groups I and II were determined in 7 and 16 patients, respectively (p = 0.005). Bladder neck contracture was not found in group I, while it was seen in 2 patients in group II (p = 0.487). CONCLUSION: The combination of minimal transurethral resection and bladder neck incision in patients with small prostates appears to provide a satisfactory clinical outcome, comparable with standard TURP, while preventing the difficulty of urethral catheter positioning at the end of the operation, decreasing bladder neck contracture and lowering the incidence of retrograde ejaculation.

[A case of anaphylactic shock induced by pirarubicin hydrochloride].

A 75-year-old man was admitted to our hospital for treatment of superficial bladder tumor. Transurethral resection (TUR) was performed and histopathological examination revealed a transitional cell carcinoma (G2). Despite one course of post-TUR bladder instillation therapy using pirarubicin hydrochloride, carcinoma in situ (CIS) was found 4 months later. CIS disappeared after another course of bladder instillation therapy using BCG; but, it recurred a month later. BCG bladder instillation therapy was performed again, and no malignant cells were detected in the urinary tract thereafter. Four months later, lung metastasis was diagnosed and an MVAC regimen (cisplatin, methotrexate, vinblastin adriamycin) was administered. However, anaphylactic shock was induced by intravenous injection of pirarubicin hydrochloride, so this therapy was stopped in the middle of the second course. Even though the lung metastasis disappeared once after the same MVAC treatment, it recurred the following year. At that time, 3 courses of a cisplatin-methotrexate-vinblastin regimen were administered, and a complete response was achieved.

Treatment of patients with superficial bladder cancer stratified by risk groups treated with lyophilized Moreau-Rio de Janeiro BCG strain.

INTRODUCTION: The Moreau-Rio de Janeiro BCG strain is considered the most effective to stimulate immunologic activity in mice. The objective of this prospective study was to evaluate BCG results for patients with superficial bladder cancer stratified by risk groups. MATERIAL AND METHODS: From April 1988 to May 2000, 100 patients were treated by transurethral resection for bladder tumor, followed by intravesical instillation of 40 mg BCG, with induction and maintenance cycles. Fisher exact test and Chi-square test, with 95% significance, were used to evaluate possible associations among variables. The Kaplan-Meier method was used to evaluate the disease-free interval and patients' survival, while log-rank test was used to compare the curves among the groups. RESULTS: The median follow-up was 69.3 months and varied from 10 to 153 months. Overall recurrence and progression rates were 55% and 13%, respectively. The medium time to recurrence was 9.4 months and to progression was 24.4 months. The cancer specific survive was 90%. Univariate analysis revealed that tumor recurrence was significantly associated with weekly BCG failure (p=0.011), multifocality (p=0.001), number of recurrences after primary therapy (p=0.001) and the need to Mitomycin C instillation (p=0.001). However, no variable was significantly associated with recurrence in multivariate analysis. There were significant associations, in univariate analysis, between disease progression and the following variables: tumor grade, weekly and 15-days BCG failure, both as first line and second line therapy, recurrence and need of Mitomycin C therapy. Independent variables to progression were 6.7 relative risk to weekly BCG failure, tumor grade and 15-days BCG (p= 0.08; CI=0.79-56.7), 2.4 (p= 0.11; CI=0.80-7.15) and 1.5 (p=0.23; CI=1.05-2.13), respectively. Patient stratification by risk groups were able to predict progression (p=0.045), but not recurrence (p=0.311). Disease progression rates were 3.2%, 12.2% e 25%, in low, intermediate and high risk groups, respectively. The BCG administration was well tolerated, and 21 patients (21%) didn't present any side effects. CONCLUSIONS: Intravesical instillation of BCG was overall well tolerated. Adjuvant BCG didn't decrease significantly recurrence rates, and 16% of the patients underwent alternative therapy with intravesical Mitomycin to prevent new recurrences. The risk group classification was able to select patients with high risk to progression. Tumor grade, BCG failure as first and second line therapies, were predictive factors of poor prognosis. BCG of Moreau-Rio de Janeiro strain was well tolerated, similar to other strains used in literature.

First clinical experience with new transurethral bipolar prostate electrosurgery resection system: controlled tissue ablation (coblation technology).

BACKGROUND AND PURPOSE: Bipolar transurethral resection of the prostate (TURP) can allow transurethral debulking of the adenoma with a lower potential for complications from systemic absorption of hypotonic irrigant. We evaluated a new bipolar double-loop resection system with a dedicated controlled ablation (Coblation) generator in this context. PATIENTS AND METHODS: In our series of 36 patients, 32 underwent transurethral bipolar loop resection, of whom 12 had prostates >50 cc (range for entire series 30-126 cc) on transrectal ultrasonography. Ten men had indwelling catheters because of urinary retention. The four other patients underwent transurethral resection of bladder tumors. After an initial learning curve of eight TURPs, some minor technical modifications were made, both to the bipolar-loop resection technique and to the hardware. The modified Vista Coblation system was subsequently evaluated by 18 other urologists in 17 other centers throughout the U.S. RESULTS: The median resected dry weight was 22 g, and the median operating-room time was 48 minutes, giving a median resection rate of approximately 1 g every 2 minutes. Two of the eight men in our learning curve required treatment for delayed bleeding problems (clot obstruction of catheter), and one of these received a blood transfusion. Transfusion was not required subsequently regardless of prostate size, even in men with larger glands. One of the patients treated for bladder tumor had a minor perforation of the posterior bladder wall, which healed with conservative management. In the multicenter experience in the U.S., there were 59 evaluable patients. The median resected weight was 23 g, and the median resection time was 55 minutes. Two men required conversion to monopolar roller coagulation for intraoperative bleeding. Most patients (80%) were discharged from hospital within 24 hours. No adverse neuromuscular stimulation occurred. CONCLUSION: This first single-center and international multicenter experience with Coblation technology for bipolar double-loop saline TURP confirms the feasibility and safety of this procedure. Subjective evaluation showed the four most important perceived benefits of bipolar over monopolar TURP to be smoother cutting action, elimination of TUR syndrome, less tissue burning (and no smell), and reduced bleeding. The results of randomized studies with this technology are awaited, while its role in bladder tumor resection remains unclear.

Paraganglioma of the urinary bladder: a lesion that may be misdiagnosed as urothelial carcinoma in transurethral resection specimens.

Paraganglioma of the urinary bladder is a rare tumor with characteristic histologic and immunohistochemical features. However, in our experience, it may be misdiagnosed as urothelial cancer because of 1) its frequent involvement of the muscularis propria; 2) morphology that may suggest urothelial cancer in transurethral resection specimens, particularly if there are artifactual changes induced by that procedure; 3) failure of pathologists to include it in their differential diagnosis when evaluating a bladder tumor; and 4) only a minority of the cases are associated with symptoms that might prompt consideration of the diagnosis. Distinction between paraganglioma and urothelial cancer is important because of likely different therapeutic options. In this report, we describe our experience with the histopathology of paragangliomas of the urinary bladder with emphasis on the histologic features that have led to their being misdiagnosed as conventional urothelial cancer and, most importantly, those that will help pathologists recognize this rare tumor of the bladder. Fifteen cases of paraganglioma of the urinary bladder were studied, 11 of them consult cases. They affected patients (8 male, 7 female) with a mean age of 49.5 years; only two had symptoms suggestive of the diagnosis, including hypertension during cystoscopy and episodic headache. Three consult cases were submitted with a diagnosis of "transitional cell carcinoma" and 4 with a diagnosis only of "bladder tumor." Histologically, "zellballen" and diffuse patterns were present in 12 (80%) and 3 (20%) of the cases. A delicate fibrovascular stroma was obvious in 14 (93%) cases. Other patterns included irregular nests and pseudorosette formation. Tumor necrosis, significant cautery artifact, and muscularis propria invasion were present in 1 (7%), 3 (20%) cases, and 10 (67%) cases, respectively. All 15 tumors were composed of large polygonal cells with abundant granular cytoplasm. Focal clear cells were present in 3 (20%). The nuclei were mostly uniform, although occasional pleomorphic nuclei were seen in 6 (40%) cases, and 2 (13%) had frequent pleomorphic nuclei. Mitoses were rare overall, and no abnormal mitotic figures were found. The major histologic features that led to misdiagnosis included a diffuse growth pattern, focal clear cells, necrosis, and muscularis propria invasion, with significant cautery artifact compounding the diagnostic problems. Immunohistochemically, 2 of 2 tumors were positive for neuron-specific enolase, 9 of 10 tumors for chromogranin, and 2 of 3 tumors for synaptophysin; 3 of 3 tumors were negative for cytokeratin and 1 of 1 tumor negative for HMB-45. Paraganglioma of the urinary bladder may be misdiagnosed as urothelial cancer, but a careful search for the characteristic histologic features and, if necessary, supportive immunohistochemical studies, should lead to a correct diagnosis.

Cytochrome P450 isoenzyme mRNA expression pattern in human urinary bladder malignancies and normal urothelium.

This study was designed to analyze the cytochrome P450 isoenzyme mRNA expression pattern of transitional cell carcinomas of the bladder (N = 19) and normal urothelium (N = 10). In addition, biopsies from normal urothelium (N = 32) taken at the time of transurethral resection of bladder cancer in eight patients from surrounding histologically normal urothelium also were characterized concerning their specific cytochrome P450 mRNA expression pattern. A total of 13 of 19 of the analyzed tumor specimens (68%) revealed expression of cytochrome P450 1B1. Cytochrome P450 4B1 and 1A1 mRNA expression were detected in 79% (15 of 19) and 53% (10 of 19) of the tumor specimens, with no correlation between tumor stage and grade of the neoplasm. Biopsies from macroscopically and histologically normal urothelium from tumor-invaded bladders also showed expression of cytochrome P450 1B1 in 75% (24 of 32), 4B1 in 62.5% (20 of 32), and 1A1 in 50% (16 of 32). Furthermore, a 75% homology concerning cytochrome P450 1B1 and 4B1 mRNA expression was observed between the bladder tumor and the biopsies from this bladder. The polymerase chain reaction analysis of normal urothelium from normal bladders that do not harbor a neoplasm revealed CYP450 mRNA expression for CYP450 1A1 in 6 of 10; 1B1 in 5 of 10; 4B1 in 6 of 10; 2D6 in 2 of 10; and 2E1 in 2 of 10. According to our data, CYP450 1B1 mRNA expression is not tumor-specific. The present findings are the first to compare CYP450 expression in bladder cancer with biopsies from the same tumor-bearing bladder, and they indicate that, from the enzymatic point of view, bladder cancer also is a panurothelial field disease present in even normal urothelium.

[Initial conservative treatment for grade 3 Ta-1 superficial bladder cancer].

We retrospectively investigated the therapeutic outcomes of our series of 7 Ta and 62 T1 bladder cancers with grade 3 (G3) malignancy in 61 men and 8 women having a mean age of 66.2 years. Following transurethral resection of bladder tumor (TURBT), 35 and 6 patients received intravesical instillations of bacillus Calmette-Guerin (BCG) and anthracycline-derivants, respectively, whereas 15 received no adjuvant therapy. Five and 2 patients received systemic and local chemotherapy with irradiation, respectively, and six underwent radical cystectomy for invasive potential. The 5-year nonrecurrence, progression-free, and overall (cancer-specific) survival rates were 66, 82%, and 76 (88%), respectively, after a median follow-up of 52 months. The 5-year non-recurrence rates were 24% in non-adjuvant, 85% in BCG, 0% in anthracycline-derivants, 65% in systemic and local chemoradiation therapy, and 68% in cystectomy. The 5-year progression-free and overall (cancer-specific) survival rates of the patients treated with BCG instillation were 91% and 94 (100)%. There were no significant differences in the 5-year non-recurrence and progression-free rates between 12 patients with carcinoma in situ (CIS) and 23 patients without CIS. Complete TUR of all visible tumors and a reliable histopathological diagnosis of appropriate specimens bearing the muscle layer are mandatory for assessment of recurrence. G3 Ta-1 bladder cancers and CIS showed a high risk of recurrence, and required aggressive treatment. Since BCG therapy following TURBT significantly reduced the risk of recurrence and progression, adjuvant BCG therapy is considered to be the most promising initial conservative treatment for G3 Ta-1 bladder cancers.
